Impact Assessment of the Allergy Fact Checker, a Clinical Decision Support Tool for Noninvasive Beta-Lactam Antibiotic Allergy Label Delabeling: Protocol for a Multicenter Crossover Cluster-Controlled Study.

BACKGROUND: Beta-lactam allergy labels (BLALs), especially penicillin allergy labels, are frequently recorded in hospitalized patients and are associated with increased use of broad-spectrum and second-line antibiotics. Most BLALs are incorrect, but current allergy workups require invasive testing and specialized resources. We recently developed a strictly noninvasive, electronic patient record-embedded clinical decision support tool, the Allergy Fact Checker (AFC), which proactively identifies potentially incorrect BLALs by detecting uneventful re-exposures to the culprit or other beta-lactam

Restoration of meropenem efficacy against multidrug-resistant Acinetobacter baumannii by green tea polyphenol EGCG: Dual targeting of efflux and porin pathways.

BACKGROUND: Acinetobacter baumannii is a Gram-negative bacterial pathogen responsible for severe nosocomial infections. Carbapenems, including meropenem and imipenem, are β-lactam antibiotics commonly used as a last-resort treatment for infections caused by A. baumannii. While it is recognized that green tea polyphenols can enhance the efficacy of carbapenems against A. baumannii, the underlying mechanisms of this synergy remain undefined. PURPOSE: This study aims to investigate the synergistic actions of green tea aqueous extract and its key bioactive component, epigallocatechin gallate (EGC

Isolation, characterisation and potential applications of a novel bacteriophage targeting beta-lactam-resistant Staphylococcus saprophyticus.

Staphylococcus saprophyticus is a gram-positive commensal characteristically resistant to novobiocin. Clinically, it is associated with urinary tract and other opportunistic infections. But reports on phages against them are scanty, which underlines the importance of the study. We isolated a Staphylococcus saprophyticus strain from soiled socks. The strain was characterised through biochemical tests and 16S rDNA ribotyping. Furthermore, the antibiogram revealed that the strain is resistant to penicillinase-resistant beta-lactam antibiotics. A double-stranded DNA temperate phage, ØPh_SS01, was

Genetic basis of cefiderocol resistance in Acinetobacter baumannii: insights from functional genomics and clinical isolates.

Cefiderocol (CFDC) is a siderophore-conjugated cephalosporin that hijacks bacterial iron uptake pathways to traverse the outer membrane, offering potent activity against carbapenem-resistant Acinetobacter baumannii (CRAB). Although mutations in pirA, a TonB-dependent siderophore receptor, have been linked to CFDC resistance, the broader genetic basis remains poorly defined. Using Himar1 transposon mutagenesis in a CFDC-susceptible sequence type 2 (ST2) CRAB strain, we identified ten genes whose disruption reduced CFDC susceptibility, spanning siderophore-mediated uptake (pirA and puiA), oxidat

Accurate and Sensitive UHPLC-Tandem Mass Spectrometry Sequential Methods for Therapeutic Drug Monitoring of Aztreonam/Avibactam in Human Plasma.

Background/Objectives: The aztreonam/avibactam combination represents a promising therapeutic option for severe infections caused by multidrug-resistant Gram-negative pathogens, particularly in critically ill patients. Due to marked pharmacokinetic variability and the need to achieve joint pharmacokinetic/pharmacodynamic (PK/PD) targets of both agents, therapeutic drug monitoring (TDM) may play a pivotal role in optimizing treatment. This study aimed to develop and validate two rapid, accurate, and sensitive UHPLC-qTOF MS/MS sequential methods for quantifying aztreonam and avibactam in human p

Exploration of a benzothiophene scaffold for use as adjuvants with β-lactam antibiotics against methicillin-resistant Staphylococcus aureus.

Rising antibiotic resistance coupled with diminishing investment in antibiotic development has led to limited treatment options for multi drug-resistant bacterial infections. Penicillin binding protein 2a (PBP2a) is a key determinant of resistance to many β-lactam antibiotics in methicillin-resistant Staphylococcus aureus (MRSA), while over-expression of PBP4 mediates resistance to fifth-generation cephalosporins. We previously identified benzothiophene NDM-335 as an inhibitor of PBP4 that overcomes PBP4 mediated resistance to ceftobiprole, and have shown that other PBP4 inhibitor scaffolds ca

Background/Objectives: Therapeutic drug monitoring (TDM) of β-lactams (BL), BL/β-lactamase inhibitor (BLI) combinations (BL/BLIc), and of fosfomycin may play a key role in optimizing antimicrobial therapy and in preventing resistance development, especially when used by continuous infusion in critically ill or immunocompromised patients. Unfortunately, analytical methods for simultaneously quantifying multiple BL/BLIc in plasma are still lacking. Methods: The aim of this study was to develop and validate two rapid, sensitive, and accurate UPLC-qTOF-MS/MS methods for the simultaneous quantifica

Case Commentary: When one target is not enough-PBP3 insertions and target redundancy in Escherichia coli.

This challenging clinical case highlights the impact of PBP3 insertions in Escherichia coli, which reduce susceptibility to key β-lactam agents and complicate treatment, particularly in the setting of NDM production (C. Fabrizio, F. Valzano, S. Giuliano, E. Morelli, et al., Antimicrob Agents Chemother 70:e00887-25, 2026, https://doi.org/10.1128/aac.00887-25). Clinical improvement was achieved with imipenem-relebactam plus aztreonam, supporting the idea that targeting multiple penicillin-binding proteins can overcome functional redundancy. These findings emphasize the clinical relevance of PBP3

Systematic analysis of CHAP domain-containing genes deletion mutants links cell wall architecture to antibiotic susceptibility and pathogenesis in S. aureus.

Staphylococcus aureus is a major human pathogen whose resilience is largely attributed to a robust and dynamically remodeled cell wall. CHAP (cysteine, histidine-dependent amidohydrolase/peptidase) domain-containing proteins constitute a large family of peptidoglycan hydrolases implicated in cell wall turnover, but their precise physiological roles remain poorly defined due to functional redundancy and limitations of prior mutagenesis approaches. Here, we performed a systematic functional analysis of CHAP proteins in the epidemic methicillin-resistant S. aureus USA300 JE2 by constructing clean

Treatment strategies for imipenemase-producing Gram-negative infections: lessons from Japan.

Carbapenems remain essential for treating serious infections caused by drug-resistant Gram-negative bacteria because of their broad-spectrum activities and favourable safety profiles. However, the emergence of carbapenemase-producing Enterobacterales, which produce enzymes that efficiently hydrolyse β-lactams including carbapenems, continues to undermine their clinical utility. Although new antibiotics such as ceftazidime-avibactam, imipenem-relebactam, meropenem-vaborbactam, aztreonam-avibactam, and cefiderocol have expanded therapeutic options, their effectiveness varies substantially across

Genomic analysis of IncH plasmids reveals their role as drivers of antimicrobial resistance and adaptive traits in enterobacterales.

Plasmids of the incompatibility group H (IncH) are large mobile elements that confer multidrug resistance and are prevalent in Enterobacterales from clinical and environmental sources. We analyzed 1308 globally distributed IncH plasmid sequences to assess their genomic features and functional potential. IncH plasmids were classified into IncHI1 and IncHI2, with IncHI1 subdivided into IncHI1B and IncHI1AB based on co-occurring replication proteins. These subtypes exhibited distinct host preferences and genomic patterns. IncH plasmids carried antimicrobial resistance genes and other adaptive det

From Susceptible to Resistant: The Emergence of Carbapenemase-Producing Escherichia coli.

Carbapenemase-producing Escherichia coli (E. coli) has emerged as a critical contributor to antimicrobial resistance (AMR), significantly compromising the efficacy of last-resort carbapenem antibiotics. Carbapenemase-producing E. coli has significantly reduced the effectiveness of carbapenems, which were previously considered last-resort antibiotics for treating severe infections caused by extended-spectrum β-lactamase (ESBL)-producing organisms. Numerous β-lactam antibiotics, including carbapenems, are hydrolyzed by these enzymes, which results in fewer therapy choices, greater rates of treat
