RNase-mediated defence and its association with SCCmec in Staphylococcus aureus.

The rise of antibiotic resistance in Staphylococcus aureus is a major threat to global health. Resistance often emerges when bacteria acquire foreign DNA elements, such as the SCCmec cassette that makes strains resistant to nearly all β-lactam antibiotics. Yet, not all S. aureus lineages take up these elements equally, raising the question of what prevents some strains from becoming drug-resistant. Here we identify a previously unknown defence factor that explains this mystery in a common skin lineage (ST188), frequently isolated from patients with atopic dermatitis. Such defence factor is enc

Essential roles of lytic transglycosylases in l,d-transpeptidase-mediated peptidoglycan cross-linking in Escherichia coli.

UNLABELLED: Bacteria resist the turgor pressure of the cytoplasm thanks to peptidoglycan, a mesh-like macromolecule that completely surrounds the cytoplasmic membrane and consists of glycan chains cross-linked to each other by short peptides. Protecting the cell from swelling and lysing during the entire cell cycle, while enabling cell expansion and division, requires the coordinated action of enzymes that form (glycosyltransferases and transpeptidases) and cleave (lytic transglycosylases, endopeptidases, and amidases) peptidoglycan chemical bonds. In Escherichia coli, cleavage of glycosidic b

AmpC β-Lactamases in SPICE (Serratia, Pseudomonas, Indole-Positive Proteus, Citrobacter, Enterobacter) Organisms: A Rising Threat to Antimicrobial Therapy.

The global rise of antimicrobial resistance (AMR) among Gram-negative bacteria is a major public health threat. AmpC β-lactamases, particularly in SPICE (Serratia, Pseudomonas, indole-positive Proteus, Citrobacter, Enterobacter) organisms, contribute significantly to multidrug resistance by hydrolyzing cephamycins and other β-lactam antibiotics. Unlike extended-spectrum β-lactamases (ESBLs), AmpC β-lactamases are generally not inhibited by classical β-lactamase inhibitors such as clavulanic acid, sulbactam, and tazobactam. However, newer non-β-lactam β-lactamase inhibitors, including avibactam

The efficacy and safety of omadacycline plus β-lactam versus moxifloxacin plus β-lactam for the treatment of severe community-acquired pneumonia: study protocol of a multicenter, prospective, randomized controlled non-inferiority trial.

BACKGROUND: Severe community-acquired pneumonia (sCAP) is associated with high morbidity and mortality. Current empirical antimicrobial regimens are increasingly challenged by rising antimicrobial resistance and safety concerns (e.g., fluoroquinolone-related adverse events). Omadacycline, a novel aminomethylcycline antibiotic, has demonstrated broad-spectrum activity against common respiratory pathogens (including drug-resistant strains) and a favorable safety profile. However, direct comparative evidence to support its use in combination with β-lactams in this setting is lacking. Therefore, t

Shewanella oneidensis employs both β-lactam resistance and tolerance controlled by the PghKR system.

Antibiotic tolerance enables bacterial survival during antibiotic exposure and can promote the evolution of resistance, thereby threatening therapeutic efficacy. However, the molecular mechanisms underlying tolerance and its relationship with resistance remain incompletely understood. In this study, we show that a class D β-lactamase-deficient strain of Shewanella oneidensis (∆blaA) is highly susceptible to β-lactams, but retains the ability to survive lethal antibiotic concentrations. Under β-lactam stress, ∆blaA cells form reversible spheroplasts that revert to rod-shaped morphology and resu

Amoxicillin and penicillin allergy direct oral challenge in a campus health clinic.

Introduction: Although 9.4% of patients globally report a penicillin allergy, the vast majority are not truly allergic and needlessly avoid penicillin and other beta-lactam antibiotics when these drugs are clinically indicated. Such patients are more likely to be exposed to broad-spectrum antibiotics, which can negatively impact their quality of care and contribute to antibiotic resistance. Direct amoxicillin oral challenge (DOC) in low-risk patients reporting penicillin or amoxicillin allergy is safe and effective in the outpatient and inpatient setting. Methods: Direct oral challenge for pen

Circumnavigating Antibiotic Mechanisms of Action and Resistance Research.

Antibiotics have revolutionized human health by significantly reducing morbidity and mortality associated with bacterial infections. Antibiotics exert bactericidal or bacteriostatic effects through inhibition of cell wall synthesis and disruption of cell membrane integrity, inhibition of protein, nucleic acid synthesis, and other metabolic pathways. Despite their remarkable success since the mid-20th century, antimicrobial resistance (AMR) has emerged as a major global health concern, undermining current treatments and complicating infection management. Key drivers of AMR include the overuse a

First report of OXA-484 carbapenemase harbouring clinical isolate of Escherichia coli ST2099 in India: Genome characterization using whole genome sequencing.

The global spread of carbapenemase-producing Escherichia coli, particularly OXA-48-like variants, poses serious public health concern due to their low-level carbapenem hydrolytic activity and phenotypic detection challenges. Among OXA-48-like variants, OXA-484 is rarely reported, differing from common variants (blaOXA-181,232) by a single amino acid substitution (214G), resulting in reduced carbapenem hydrolysis. This is probably the first report of OXA-484-producing E. coli (ST2099) from India with genomic characterization of the strain. Identification, antibiotic susceptibility and whole-gen

Uncovering antimicrobial resistance structures in Staphylococcus spp. from companion animals: latent class analysis of isolates from dogs.

Antimicrobial resistance (AMR) in companion animals poses therapeutic challenges and potential zoonotic risks. Staphylococcus spp., a leading cause of skin and ear infections in dogs and cats, shows high levels of resistance to different families of antibiotics. This study aimed to describe AMR patterns in Staphylococcus spp. isolated from companion animals in Lima, Peru, and to identify latent resistance phenotypes among canine isolates using latent class analysis (LCA). We analyzed 2,159 isolates (2,035 from dogs and 124 from cats) collected between 2021 and 2024, assessing resistance to sev

Genotypic and phenotypic landscape of carbapenem-resistant Pseudomonas aeruginosa isolated from respiratory and non-respiratory samples in a tertiary hospital.

BACKGROUND: The World Health Organization lists carbapenem-resistant Pseudomonas aeruginosa (CRPA) as a critical priority pathogen. However, the links between resistance phenotypes, virulence factors, and clonal spread remain incompletely understood. We aimed to characterize the genotypic and phenotypic landscape of clinical CRPA isolates and evaluate whether specimen source or type III secretion effectors (exoT/exoY) serve as predictors of antibiotic resistance. METHODS: Fifty-eight consecutive CRPA isolates from respiratory and non-respiratory specimens were analyzed. Susceptibility to 10 a

Landscape of resistance to ceftolozane/tazobactam and ceftazidime/avibactam in strains of Pseudomonas aeruginosa: a genomic approach in strains without selection pressure in a tertiary-care paediatric hospital in Mexico.

BACKGROUND: Carbapenems are reserved as a last-line therapy for infections caused by Pseudomonas aeruginosa. However, the resistance to these antibiotics exceeds 24.2%, and in paediatric patients, infections caused by carbapenem-resistant P. aeruginosa (CR-PA) have a mortality between 25 and 38.2%. Therefore, antimicrobial agents such as ceftazidime/avibactam (CZA) and ceftolozane/tazobactam (CT) have been developed. This study aimed to characterize CT- and CZA-non-susceptible P. aeruginosa isolates from paediatric patients in a hospital where these therapeutic agents are not yet available. ME

Penicillin allergy documentation in the UAE: 10-year trends and stewardship implications.

INTRODUCTION: Penicillin allergy labels are frequently documented in electronic medical records (EMRs), although most patients are not truly allergic upon formal evaluation. Inaccurate allergy labels result in unnecessary avoidance of first-line beta-lactam antibiotics, increased use of broad-spectrum alternatives, and contribute to antimicrobial resistance and healthcare-associated infections. Data describing allergy-label documentation and de-labelling practices in the United Arab Emirates remain limited. METHODS: This retrospective study examined temporal trends, documentation quality, and

The Direct and Potentiating Antimicrobial Activities of Zafirlukast.

Background: Antimicrobial resistance is at alarming levels. Methicillin-resistant Staphylococcus aureus is a clinical concern and has been classified as one of the so-called ESKAPE pathogens. The increase in resistance coupled with the lack of new agents in development requires innovative ways of developing new antimicrobials. Methods: Here, we investigate an approved drug (Zafirlukast), which has been previously reported to have activity against Mycobacterium, for activities against other bacterial strains. Antibacterial activity was assessed using standard MIC testing along with checkerboard

Investigating the resistome, taxonomic composition, and mobilome of bacterial communities in hospital wastewaters of Metro Manila using a shotgun metagenomics approach.

We profiled antibiotic resistance genes, bacterial communities, and mobile genetic elements in untreated hospital wastewater from three tertiary hospitals in Metro Manila using shotgun metagenomic sequencing. The resistome analysis revealed high abundances of genes known to confer resistance against sulfonamides (sul1, sul2), aminoglycosides (aadS), and macrolides/streptogramins (msrE, mphE). High-risk resistance genes were also detected, including those known to confer resistance to β-lactams (blaOXA, blaTEM, blaGES, blaNDM, blaKPC), colistins (mcr-5), and tetracyclines [tet(C), tet(A), tet(L

Genomic characterization of mobile genetic elements associated with antimicrobial resistance in Streptococcus pneumoniae from Australia.

The emergence and spread of antimicrobial resistance (AMR) in Streptococcus pneumoniae threatens current antibiotic treatment strategies. While β-lactams remain the first-line therapy for pneumococcal infections in Australia, resistance to macrolides, tetracyclines and other antibiotics, driven by resistance genes carried on mobile genetic elements (MGEs), is increasingly reported. In this study, we conducted a comprehensive analysis of 573 S. pneumoniae genomes from South Australia, Queensland and Victoria to investigate the distribution of MGEs and their association with acquired AMR genes.

Intrapartum group B streptococcus antibiotic prophylaxis in beta-lactams allergic women and risk of neonatal infection.

OBJECTIVE: In women with Group B Streptococcus (GBS) vaginal colonization who report a beta-lactams allergy, the prevention of neonatal GBS early-onset disease (EOD) mainly relies on maternal antibiotic prophylaxis with lincosamides during labor. With approximately 30% of GBS being resistant to macrolides or lincosamides, this antibiotic prophylaxis may be inappropriate, leading to a risk of EOD. We aim to measure the proportion of inappropriate antibiotic prophylaxis despite the existence of an antibiotic susceptibility testing, and its possible impact on the newborn. METHODS: Single-center

Whole Genome Sequencing Reveals Genetic Variability of Escherichia coli Across Dairy Farm Environments.

Background/Objectives: Antimicrobial agents have revolutionized disease management in humans and animals; however, their misuse and overuse have accelerated the emergence and spread of antimicrobial resistance (AMR) and antimicrobial resistance genes (ARGs). Dairy farms are recognized as potential hotspots for ARG dissemination, particularly through Escherichia coli, which acts as a reservoir and vector of ARGs, enabling their horizontal transfer via plasmids and other mobile genetic elements. This study aimed to characterize the genomic diversity, ARG profiles, plasmid content, and phylogenet
