Longitudinal genomic insights into resistance and virulence dynamics of Klebsiella pneumoniae clinical isolates from Pakistan.

Multidrug resistant K. pneumoniae is a public health treat in Pakistan, yet longitudinal genomic data on the evolution of resistance and virulence remain limited. This study integrates phenotypic and whole-genome analyses to explore the evolution of resistance and virulence in clinical isolates. In this study, 39 clinical K. pneumoniae isolates from a single center in Lahore, Pakistan were characterized. Comparative genomic analysis of six representative isolates was performed against 45 historical Pakistani isolates (2010 vs. 2019). Pan-genome analysis, resistance and virulence gene profiling

Deciphering the genomic landscape of hypervirulent Klebsiella pneumoniae in South African tertiary hospitals.

BACKGROUND: Hypervirulent Klebsiella pneumoniae (hvKp) is an emerging pathogen causing severe infections globally, yet genomic data from inland South African regions remain limited, hindering understanding of local hvKp diversity and virulence. METHODS: Elevenhypermucoviscous Klebsiella pneumoniae isolates collected in 2021 and 2024 were subjected to WGS. Genomes were assembled and analysed for sequence types, capsular loci, virulence, and antimicrobial resistance genes using established bioinformatics pipelines, with phylogenomic relationships inferred by core genome analysis against represen

Whole-genome sequencing of MDR K. pneumoniae circulating in Moroccan river ecosystems: a comprehensive genomic analysis of environmental isolates.

BACKGROUND: Klebsiella pneumoniae is a major opportunistic pathogen and a recognized contributor to the global burden of antimicrobial resistance (AMR). Although traditionally associated with healthcare settings, it is increasingly detected in environmental compartments influenced by anthropogenic activities. Aquatic ecosystems may act as reservoirs and dissemination hubs for multidrug-resistant (MDR) lineages and resistance genes. However, genomic data on environmental K. pneumoniae in Morocco remain scarce. This pilot study aimed to characterize a targeted subset of MDR K. pneumoniae isolate

Utilization of Cefiderocol in a Case of Extensively Drug-Resistant Klebsiella pneumoniae Prostatitis and Recurrent Urinary Tract Infection.

Treatment of extensively drug-resistant (XDR) organisms is complicated by pharmacodynamic and pharmacokinetic (PK) limitations of current antimicrobial agents. Prostatitis is an infection that is historically difficult to treat with beta-lactams given poor tissue penetration as compared to fluoroquinolones and sulfamethoxazole-trimethoprim. Cefiderocol is a novel cephalosporin that is often used to treat a wide variety of gram-negative bacteria that have extensive resistance to other antibiotics. Cefiderocol remains stable against many β-lactamases and shows enhanced bacterial cell entry as a

Emergence of Klebsiella pneumoniae Carbapenemase-Producing K. pneumoniae with Penicillin-Binding Protein 3 Insertions, Taiwan, 2021.

Carbapenem-resistant Enterobacterales (CRE) are a major global health threat with limited treatment options. Aztreonam/avibactam is a promising therapy against metallo-β-lactamase (MBL)-producing and other CRE, but emerging resistance threatens its effectiveness. Insertions in penicillin-binding protein 3 (PBP3), which are well described in Escherichia coli, are linked to reduced aztreonam/avibactam susceptibility but remain poorly characterized in Klebsiella pneumoniae. We report clinical K. pneumoniae carbapenemase-producing K. pneumoniae sequence type 11 isolates carrying a novel PBP3 YRIT

Silent Synergy: Is the Presence of Concurrent β-Lactamase Mechanisms Undermining Antimicrobial Effectiveness?

Background The worldwide escalation of antimicrobial resistance poses a substantial challenge to the effective management of both community-acquired and hospital-associated infections. Among Gram-negative bacteria, resistance to β-lactam antibiotics is predominantly mediated by enzymes such as extended-spectrum β-lactamases, AmpC β-lactamases, and metallo-β-lactamases. Aim This study aimed to identify the concurrent production of these β-lactamase mechanisms in Gram-negative clinical isolates. Materials and methods A cross-sectional laboratory-based study was performed on 185 non-duplicate Ent

Burden of Difficult-to-Treat Resistant Organisms in Gram-Negative Bloodstream Infections: A Retrospective Observational Study From Eastern India.

Background Gram-negative bacteria (GNB) are responsible for about one-third to nearly half of adult bloodstream infections, resulting in higher rates of morbidity and mortality. Difficult-to-treat resistance (DTR), among GNB, is characterised as being intermediate or resistant to all β-lactam classes (including carbapenems) and fluoroquinolones, thus constituting a clinically significant resistant phenotype. This study aims to ascertain the prevalence and antibiotic susceptibility pattern of DTR organisms isolated from Gram-negative bloodstream infections (GNBSIs) at a tertiary-level hospital

Isolation of a Methicillin-Resistant Staphylococcus pseudintermedius Strain from a Domestic Dog with Vulvovaginitis: A Potential Public Health Concern?

Background:Staphylococcus pseudintermedius is a major opportunistic pathogen of dogs and the primary cause of canine pyoderma and other infections. The global emergence of methicillin-resistant S. pseudintermedius (MRSP) represents a significant challenge in veterinary medicine due to its frequent multidrug-resistant phenotype and limited therapeutic options. Methods: We describe the phenotypic and genomic characterization of an MRSP isolate recovered from a vaginal swab of an 11-year-old dog diagnosed with vulvovaginitis in southern Italy. Antimicrobial susceptibility testing was performed us

Trends in primary care antibiotic prescribing during the implementation of national stewardship policies: Türkiye, 2011-2019.

BACKGROUND: Antimicrobial resistance is driven by inappropriate antibiotic use, and Türkiye has long shown one of the highest outpatient consumption rates in the WHO European Region. In response, the Ministry of Health implemented stewardship measures from 2014 onward, including the National Action Plan for Rational Drug Use, banning over-the-counter antibiotic sales, integrating e-prescribing with mandatory diagnostic coding, educational initiatives, and audit-feedback systems. This study evaluated temporal trends in primary care antibiotic prescribing in Türkiye between 2011 and 2019 and ass

Continuous Infusion of Aztreonam-Avibactam After High Loading Dose for an Infection Caused by an OXA-48- and NDM-1-Co-producing ST147 Klebsiella pneumoniae.

INTRODUCTION: Infections caused by metallo-β-lactamase-producing Enterobacterales offer limited therapeutic options. Aztreonam-avibactam (ATM-AVI) provides a promising alternative, but its approved intermittent regimen is complex and can lead to substantial drug waste. METHODS: We describe a case of mastoiditis with a retrotympanic abscess due to OXA-48- and NDM-1-producing Klebsiella pneumoniae, managed with continuous infusion (CI) of ATM-AVI after a full-vial loading dose, supported by therapeutic drug monitoring (TDM) and whole-genome sequencing (EPISEQ CS V2.0, bioMérieux). RESULTS: A 3

Distinctive Lacritin Cleavage-Potentiated Bactericidal Alteration of the P. aeruginosa Transcriptome.

Lacritin is a tear, saliva, plasma and cerebrospinal fluid glycoprotein with broad polypharmacology. Its protease generated N-104 proteoform endogenous in tears (and likely elsewhere) is bactericidal and synergizes with the tear thrombin peptide GKY20. In the pathogenic and multidrug resistant PA14 strain of P. aeruginosa, we recently discovered that N-104 at subinhibitory and inhibitory concentrations binds to the outer-membrane lipoprotein YaiW to gain access to the periplasm where it targets and inhibits the inner-membrane ferrous iron transporter FeoB (of FeoABC) as well as PotH, a subunit

Two Rare Cases of Neonatal Meningitis Caused by Globicatella sanguinis at the Marrakech University Hospital: Diagnosis and Therapeutic Implications.

Two infants, aged 3 and 5 months, were admitted to the neurosurgery ward with febrile meningeal signs and underwent lumbar punctures, which revealed turbid cerebrospinal fluid (CSF) with a predominantly neutrophilic pleocytosis, decreased CSF glucose levels, and elevated CSF protein levels. CSF cultures revealed alpha-hemolytic colonies. Identification by time-of-flight mass spectrometry with matrix-assisted laser desorption and ionization (MALDI-TOF) confirmed Globicatella sanguinis (G. sanguinis) in both cases. G. sanguinis is a pathogenic bacterium that is rarely isolated in clinical practi

Phenotypic Detection of Antimicrobial Resistance Patterns in Microorganisms Isolated From the Primary Molars of Pediatric Dental Patients Undergoing Pulp Therapy.

Background Pulp necrosis is a prevalent complication following dental caries in primary dentition. Certain bacteria exhibit increased adaptability to adverse conditions and possess virulence factors that enable more effective colonization of dental tissues. The invasion of the root canal system (RCS) by pathogenic microorganisms, compounded by the rising concern of antimicrobial resistance (AMR), emphasizes the necessity for a culture-driven and evidence-based approach to antibiotic selection in pediatric endodontics, wherein the therapeutic efficacy of root canal treatment is inherently depen

Systematic evaluation and meta-analysis of prevalence and trends for antibiotic resistance in Canine Pseudomonas infections.

A limited number of antibiotics are available to treat pathogenic Pseudomonas spp. infections in animals and among currently available antipseudomonal options, only fluoroquinolones can be administered topically, orally and parenterally. We hypothesised that canine Pseudomonas spp. isolates showed higher rates of resistance against fluoroquinolones compared to other antipseudomonal antibiotics. A systematic review and meta-analysis were conducted to determine prevalence and distribution patterns of antimicrobial resistance in Pseudomonas isolates from canine infections. Extracted data were str

Difficult-to-treat resistance (DTR), treatment, and outcomes of carbapenem-resistant Enterobacterales infections in the setting of IMP-type carbapenemase predominance in Japan.

Carbapenem-resistant Enterobacterales (CRE) pose a global threat due to limited treatment options and high mortality. Difficult-to-treat resistance (DTR), defined as non-susceptibility to all conventional β-lactams and fluoroquinolones, has primarily been applied to Pseudomonas aeruginosa. However, its relevance for Enterobacterales remains unclear, particularly in regions with a distinct carbapenemase landscape, such as Japan where IMP-type metallo-β-lactamases predominate. We analyzed the MultiDrug-Resistant organisms clinical research network (MDRnet) cohort, a multicenter prospective study

Structure-based virtual screening, molecular dynamics and MM-PBSA/GBSA analysis for the identification of novel CDK6 inhibitors against prostate cancer.

BACKGROUND: Necrotizing fasciitis (NF) of the head and neck is a rare but rapidly progressive and life-threatening soft tissue infection that constitutes a true surgical emergency. Due to the complex anatomy of the cervicofacial region and the proximity to the upper airway, early diagnosis and management are particularly challenging, and delayed recognition is associated with high morbidity and mortality. This article aims to provide a concise and clinically oriented overview of the presentation, diagnostic pitfalls, and current management strategies for cervicofacial necrotizing fasciitis. M

Fatal multi-drug-resistant gram-negative bacterial pneumonia among adults hospitalized àt Mulago National Referral Hospital, Uganda: an autopsy study.

BACKGROUND: Pneumonia, responsible for three million deaths annually worldwide, remains a leading cause of death from hospital-acquired infections. Unfortunately, treatment in most cases is empirical and not based on microbiological data because of difficulty in obtaining reliable lower respiratory tract specimens as they are frequently contaminated by upper respiratory tract bacterial flora. We examined aseptically collected lung specimens from decedents with fatal pneumonia to (a) isolate the multi-drug-resistant gram-negative bacteria (MDR GNB) and analyze their antimicrobial susceptibility

A prognostic model based on seven cell adhesion molecule-related genes to predict the prognosis of colorectal cancer.

BACKGROUND: Magnetic resonance cholangiopancreatography (MRCP) is useful to assess exocrine pancreatic function by combining rapid imaging acquisition with the administration of secretin, a gastrointestinal hormone that stimulates the secretion of bile and pancreatic juice. However, extensive data on this method are lacking. This study aimed to determine whether MRCP with secretin administration is able to simultaneously detect alterations of both the pancreatic ducts and exocrine pancreatic function. METHODS: All subjects older than 18 years who underwent magnetic resonance imaging (MRI) and
