Low necessity, high overuse: revisiting empirical vancomycin practices for children in low MRSA prevalence settings.

BACKGROUND: The overuse of vancomycin as empirical therapy ranged from 62% to 66% worldwide, while the prevalence of proven infections requiring vancomycin was only 11% to 27%. Evidence from Thailand where the prevalence of methicillin-resistant Staphylococcus aureus (MRSA) infection is low and remains limited. This study aimed to determine the incidence of confirmed infections requiring vancomycin as definitive therapy among children aged 1 month-18 years receiving vancomycin as empirical therapy, and to identify factors associated with vancomycin necessity. METHODS: A single-centre retrospec

Pharmacokinetics and Customized Dosing of Vancomycin in Adult Patients With Hematological Malignancies: Status, Challenges, and Opportunities.

Vancomycin (VAN) remains the first-line treatment for methicillin-resistant, multidrug-resistant gram-positive bacterial infections, even among cancer patients. Patients with hematological malignancies (HMs) represent a unique population, whose physiological and pathological changes often differ from those of the general population. These variations may cause significant alterations in the in vivo pharmacokinetics (PK) of VAN, causing unpredictable changes in the safety, efficacy, and bacterial resistance. Customized dosing of VAN (CDvan) offers potential solutions to these challenges. However

Dietary ellagic acid inhibiting gastrointestinal pathogens by modulation of microbiome-metabolite-immune axis.

Antibiotic-induced depletion of the gut microbiota facilitated the colonization of vancomycin-resistant Enterococci (VRE) in the gastrointestinal tract, and then increased patients' susceptibility to secondary infections. Ellagic acid, a major constituent of fruits and nuts, showed various bioactivities except for antibacterial. Interestingly, it promoted the recovery of gut microbiota, enhanced microbial diversity and stimulated the proliferation of probiotic gut microbes, and then ameliorated the overgrowth of pathogens in vivo in our experiment. Moreover, ellagic acid activated Gpr41 and Gp

High-resolution molecular typing of vancomycin-resistant Enterococcus faecium from Romania and Bavaria: combining enhanced DNA microarray and next generation sequencing.

Antimicrobial resistance poses a significant challenge for infection control, requiring the development of accurate and high-throughput diagnostic techniques. We expanded and optimized an existing DNA microarray platform for the molecular characterization of vancomycin-resistant Enterococcus (VRE) by incorporating resistance, virulence, species-specific, and typing markers. The enhanced microarray allows for the simultaneous analysis of up to 96 strains, providing detailed genetic profiles of clinical isolates. VRE strains from Romania and Bavaria, Germany, were analyzed, and the results were

Design and Synthesis of Membrane-Targeting Poly-Ruthenium Complexes Containing Quaternary Ammonium Cations with Potential Antibacterial Activity against Methicillin-Resistant Staphylococcus aureus (MRSA).

The escalating threat of Methicillin-resistant Staphylococcus aureus (MRSA) infections seriously endangers human health, so facing the immense threat of drug-resistant bacteria, discovering new and potent antibacterial drugs less prone to inducing resistance is urgently needed. In this study, four amphiphilic ruthenium polypyridyl complexes were synthesized, namely [Ru(II)(bpy)2(DIPPB)] (PF6)3 (Ru-1), [Ru(II)(dmb)2(DIPPB)] (PF6)3 (Ru-2), [Ru(II)(dmob)2(DIPPB)] (PF6)3 (Ru-3) and [Ru(II)(bpy)2(DIPPB)] (PF6)3 (Ru-4). Among these complexes, complex Ru-4 incorporates N-methylimidazole and quaternar

The Nexus of Heavy Metals and Antibiotic Resistance: Investigating Industrial Effluent-Associated Bacterial Strains.

Industrial effluents are reservoirs for multidrug-resistant (MDR) bacteria due to co-selection pressure from heavy metals and residual antibiotics. This study assessed the prevalence of heavy metal- and antibiotic-resistant bacteria in wastewater from four industrial sectors in the Hayatabad Industrial Zone, Peshawar, Pakistan. A total of 23 bacterial isolates were obtained from 16 composite samples; while this sample size is appropriate for an initial characterization in this understudied region, it highlights the need for expanded surveillance. Isolates included Pseudomonas aeruginosa, Bacil

Vancomycin-resistant Enterococci Colonization Without Clinical Infection in a High-acuity Pediatric Intensive Care Unit: Challenging the Rationale for Universal Surveillance.

BACKGROUND: Routine surveillance for vancomycin-resistant enterococci (VRE) colonization remains the standard practice in many intensive care units. However, in high-acuity pediatric intensive care units (PICUs), where clinical VRE infections are uncommon, universal screening may impose substantial financial and operational burdens with uncertain clinical benefits. This study evaluates the necessity and value of universal VRE surveillance by describing the clinical characteristics and risk profiles of asymptomatic VRE-colonized patients in a newly established, high-acuity, 54-bed PICU. METHOD

Vibrational spectroscopy for rapid profiling of vancomycin susceptibility in Enterococci.

Fast diagnosis of antimicrobial resistance (AMR), especially in high-priority pathogens, is essential to maintain the best-possible patient outcomes. In this communication, we present a new approach for rapid AMR assessment that is able to distinguish between vancomycin-resistant and vancomycin-susceptible enterococci (VRE and VSE, respectively) within 2 hours using ATR-FTIR spectroscopy, chemometrics and chemometrics-based modelling. This study outlined how early spectral markers of effective vancomycin action were used to accurately predict resistance profiles in both known and blinded Enter

Vancomycin variable Enterococci in the Netherlands (2018-2023) and the mechanism of resistance induction.

Enterococci are common human commensals but can cause severe infections in immunocompromised patients. Enterococcus faecium is a notable example, capable of acquiring resistance to multiple antibiotics, including the critically important drug vancomycin. Such strains, known as vancomycin-resistant E. faecium (VRE), are routinely detected in clinical laboratories using phenotypic assays. However, some isolates carry vancomycin resistance genes yet remain phenotypically susceptible; these are termed vancomycin variable enterococci (VVE). Because phenotypic assays may fail to identify VVE, patien

Protocols for decolonisation of carbapenem-resistant Enterobacterales and vancomycin-resistant enterococci: a systematic review and meta-analysis.

BACKGROUND: Rises in the prevalence of multi-drug-resistant organisms threaten patient safety globally. Vancomycin-resistant enterococci (VREs) and carbapenem-resistant Enterobacterales (CRE) are linked with prolonged hospitalisation, treatment failure, and increased mortality. Decolonisation strategies could reduce transmission and improve outcomes, but their efficacy and safety remain uncertain. This study systematically evaluates decolonisation protocols for VRE and CRE through a meta-analysis. METHODS: Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines
