Usnic acid chitosan-coated liposomes synergistic activity with polymyxin B against multidrug-resistant Pseudomonas aeruginosa and Acinetobacter baumannii clinical in vitro.

BACKGROUND: The rise of multidrug-resistant (MDR) Gram-negative bacterial infections, particularly those caused by Pseudomonas aeruginosa and Acinetobacter baumannii, necessitates innovative treatment strategies. Usnic acid (UA), a natural compound with antimicrobial properties, has limited clinical use due to poor solubility and toxicity. This study evaluated the antibacterial, antibiofilm, and hemocompatibility profiles of UA encapsulated in chitosan-coated liposomes (Lipo-UA-Chi), alone and in combination with polymyxin B (PMB). METHODS: UA, PMB, Lipo-UA-Chi, and their combinations were te

Vacuolar iron export alters the synergy between doxycycline and fluconazole by affecting cidal ROS levels in Candida albicans.

Fungal infections are combatted using three main classes of antifungals, of which the azoles, considered to be fungistatic, are the most widely used. Slow growth of Candida albicans at supra-minimal inhibitory concentrations (MIC) of fluconazole (FLC), termed tolerance, is routinely observed. A combination therapy resulting in the eradication of this fungistatic character would be a valid therapeutic strategy, and indeed, the synergistic combination of the antibiotic doxycycline and FLC has such an effect. We hypothesized that iron-requiring mitochondrial functions may be the targets of the sy

Naturally Occurring Coumarins as Promising Antimicrobial Agents: Latest Progress.

One of the twenty-first century's most significant threats to human health is the rise of microbial diseases. Infectious diseases are caused mainly by microorganisms, including bacteria, viruses, fungi, and protozoa. These organisms are responsible for significant increases in morbidity and mortality worldwide. Since the discovery of penicillin, antibiotics have been used to treat microbial illnesses. Antibiotic abuse and overuse in humans, animals, and plants, as well as in the treatment of non-microbial disorders, have led to the fast development of multidrug-resistant and high-virulence pat

Can voriconazole gradient diffusion testing results be extrapolated to isavuconazole and posaconazole in Aspergillus spp.? Comparative analysis with CLSI broth microdilution and cyp51A gene sequencing.

This study assessed whether voriconazole (VRC) gradient diffusion results can be extrapolated to isavuconazole (ISA) and posaconazole (PSC) in Aspergillus fumigatus and Aspergillus terreus by comparison with the Clinical and Laboratory Standards Institute (CLSI) broth microdilution (BMD) and cyp51A sequencing. Agreement with CLSI BMD was highest at 48 h, but remained limited for some azole-species combinations. VRC-PSC extrapolation was reliable in both species, whereas VRC-ISA extrapolation was species-dependent. Gradient diffusion underestimated resistance detection in some cyp51A-mutant iso

High azole resistance among non-albicans Candida species causing vulvovaginal candidiasis in Western Uganda: a cross-sectional study.

BACKGROUND: Non-albicans Candida (NAC) species are increasingly implicated in vulvovaginal candidiasis (VVC) and are frequently associated with reduced susceptibility to commonly used azole antifungals. In sub-Saharan Africa, empirical azole therapy remains widespread despite limited local resistance data. This study aimed to determine the species distribution and antifungal susceptibility patterns of NAC isolates among women presenting with abnormal vaginal discharge in Western Uganda. METHODS: we conducted a hospital-based cross-sectional study among 314 women aged 18-49 years attending Mub

Tackling a global threat: a clinical scenario-based framework for preventing and managing Candidozyma auris infections.

Candidozyma auris (formerly Candida auris) has emerged as a formidable global health threat, characterized by its multidrug resistance (MDR), high transmissibility in healthcare settings, and significant mortality. The World Health Organization classifies it as a 'critical priority' fungal pathogen. Conventional infection control paradigms, often applying a 'one-size-fits-all' approach, have struggled to contain C. auris due to its unique environmental resilience and transmission dynamics. This review departs from traditional linear analyses and proposes a novel, scenario-based framework to de

Candida auris: a multidimensional focus on its identification, epidemiology, pathogenesis, and therapeutic options.

Humans have encountered many epidemics caused by pathogenic microorganisms since the turn of the century. The prevalence of fungal infections has significantly increased on a global scale, adversely impacting human health. Candida species remain among the most prevalent and widely spread opportunistic fungi, with Candida auris emerging as a notorious hospital-acquired pathogen. This fungal pathogen causes nosocomial bloodstream infections (BSI), coupled with a high in-hospital death rate and significant multidrug-resistance (MDR). Standard quaternary ammonium compounds (Quats), are often ineff

[Candidozyma auris : an emerging fungal pathogen with epidemic potential].

Candidozyma auris, formerly Candida auris, is an emerging yeast that is now recognized as a major healthcare-associated pathogen worldwide. Its near-simultaneous emergence across multiple continents over the past decade, its high capacity to colonize skin, its prolonged environmental persistence, and its multidrug resistance to antifungal agents underscore its clinical and epidemiological significance. In Switzerland, the first case was documented in 2017. Since then, imported cases have been reported sporadically each year, almost exclusively among patients who were previously hospitalized in

Drug-resistant clinical Candida responded to 5-aminolevulinic acid photodynamic therapy (ALA-PDT) in vitro via enhanced ROS release.

Photodynamic therapy (PDT) has emerged as a potential strategy to combat drug-resistant fungal infections. This study aimed to evaluate the therapeutic efficacy of 5-aminolevulinic acid PDT (ALA-PDT) against drug-resistant Candida strains isolated from patients with candidiasis and to elucidate its underlying mechanisms. Drug-resistant strains were identified by their reduced antifungal susceptibility to azoles, micafungin, amphotericin B, and terbinafine. RNA sequencing (RNA-seq) was used to assess gene expression related to mycelial formation and fungal growth. Scanning electron microscopy (

Functional characterization of Candida auris DHA1 transporters conferring flucytosine resistance.

Candida (Candidozyma) auris has recently been designated a critical priority pathogen by the World Health Organization (WHO) in its Fungal Priority Pathogens List (FPPL). With resistance now widespread against frontline antifungals and limited availability of alternative therapeutics, combination regimens incorporating the nucleoside analog flucytosine (5-fluorocytosine; 5-FC) have gained renewed attention. While transporter-mediated resistance is well established for azole antifungals, knowledge on the molecular basis underlying reduced susceptibility to 5-FC in C. auris remains limited. Here
