Grand SLAM study protocol: a prospective randomised multicentre study of shortened versus standard duration adjuvant immune checkpoint inhibition for stage IIB-C, III and IV cutaneous melanoma.

BACKGROUND: Adjuvant treatment with PD-1 inhibitors for 12 months has been the established standard of care for patients with resected stage IIB-IV cutaneous melanoma. In other solid tumours (e.g. breast and colorectal), a shorter duration of adjuvant chemotherapy has been shown to be non-inferior with improved toxicity profile. More recently, neoadjuvant immunotherapy with immune checkpoint inhibitors for clinically detectable stage III and stage IV disease has been introduced. There is no clear biological rationale for the chosen duration, and no studies have investigated duration of adjuvan

Clinical Utility of Copy Number Abnormality Analysis in the Evaluation of Melanocytic Lesions for Diagnosis and Prognosis: An Evidence-Based Review from the Cancer Genomics Consortium Working Group for Melanocytic Lesions.

Background/Objective: Although most melanocytic lesions are diagnosed as benign or malignant by histopathologic evaluation, with or without the aid of immunohistochemistry, diagnosis may remain uncertain in a minority of cases. Assessment of copy number abnormalities (CNAs) may provide sufficient additional evidence to favor either a benign or malignant diagnosis in both pediatric and adult cases and in melanocytic lesions of various subtypes, including Spitzoid, mucosal, and acral. CNAs are common in melanomas, while they are rare, with few exceptions, in benign lesions. Detection of CNAs by

Rhenium-188 resin therapy for non-melanoma skin cancer.

Non-melanoma skin cancer (NMSC), which includes basal cell carcinoma and cutaneous squamous cell carcinoma, is the most common malignant neoplasm worldwide, with an increasing incidence in Europe and Spain. Although surgery remains the treatment of choice, there are patients in whom this option is not feasible or does not ensure a satisfactory functional and esthetic outcome. Rhenium-188 (Re-188) resin represents a minimally invasive, painless, and outpatient therapeutic alternative, allowing the delivery of high doses of localized beta radiation to lesions up to 3 mm in depth while preserving

Autophagy meets tissue-resident memory: Emerging crosstalk and therapeutic opportunities in cutaneous melanoma.

Melanoma is a highly immunogenic human cancer characterized by complex immune responses within the tumor microenvironment (TME). Among these, tumor-infiltrating lymphocytes (TILs) and tissue-resident memory T cells (TRM) are strongly associated with favorable prognosis and response to immune checkpoint blockade, reflecting their central role in sustaining anti-tumor immunity. Emerging evidence suggests that the activity and persistence of these T cell subsets are critically influenced by autophagy, a cellular process that maintains metabolic fitness, prevents exhaustion, and supports effector

Early detection of metastatic risk in primary cutaneous melanoma using weakly supervised learning.

Early identification of melanomas with high metastatic potential is crucial for treatment planning and survival prediction. We investigated whether weakly supervised WSI-based and multimodal learning approaches can identify metastatic risk in primary cutaneous melanoma beyond current clinicopathological predictors. A total of 426 routinely stained whole-slide images (WSIs), from primary melanomas (249 metastatic and 177 non-metastatic) together with corresponding clinicopathological features, were collected and divided to training and validation, and a hold-out test set. WSIs were divided into

Non-Operable Squamous Cell Carcinoma of the Infraorbital Region Treated With Electrochemotherapy: A Case Report.

Cutaneous squamous cell carcinoma (cSCC) is a malignant epithelial tumor of the skin, classified as a nonmelanoma skin cancer. It may develop from precancerous conditions such as chronic inflammatory dermatoses, actinic keratosis, or actinic cheilitis, but can also arise de novo. The aim of this case report is to describe the clinical management and therapeutic outcome of a patient with infraorbital squamous cell carcinoma who was deemed inoperable due to poor general health conditions, and was therefore treated with electrochemotherapy (ECT) as an alternative local therapy. A 73-year-old male

Integrative bioinformatic and experimental analysis reveals prognostic and immunological roles of psychological stress-related genes in skin cutaneous melanoma.

BACKGROUND: Skin cutaneous melanoma (SKCM) represents the most aggressive type of skin cancer. Accumulating evidence underscores the critical involvement of chronic psychological stress and depressive states in cancer development. Genes linked to psychological stress pathways are known to regulate tumor dynamics and may serve as prognostic indicators. However, their precise implications for SKCM patient outcomes and the efficacy of immunotherapeutic interventions remain to be fully elucidated. METHODS: Prognostically significant psychological stress-related genes (PSRGs) were first screened v

Clinical and Histopathological Predictors of Disease Progression in Stage II Cutaneous Melanoma.

INTRODUCTION: The recent approval of pembrolizumab and nivolumab for the adjuvant treatment of stage IIB-IIC cutaneous melanoma (CM) significantly improved the relapse-free survival probability of this patient population. Real-life data suggest that not all stage IIB and IIC CM patients will invariably undergo disease progression. There is a growing need for accurate patients' selection for whom adjuvant therapy is expected to be beneficial. The aim of this study was to investigate the baseline clinical and histopathological features associated with tumor recurrence in stage II CM patients in

Circulating Tumor DNA as a Predictor of Recurrence and Survival in Cutaneous Melanoma Treated With Dermatologic Surgery: A Review.

BACKGROUND: Circulating tumor DNA (ctDNA) measures cancer-specific genetic material migrated from a tumor to a patient's bloodstream. Limited research exists aggregating data on ctDNA as a measure of recurrence, treatment success, and survival in patients with melanoma. OBJECTIVE: The authors aim to evaluate evidence for ctDNA as a measurement of recurrence and survival, and how those results differ by tumor grade and measurement time point in cutaneous melanoma treated with dermatologic surgery. METHODS: An OVID Medline search and manual filtering resulted in six studies meeting the inclusi

Translating ctDNA into cutaneous melanoma care: An international expert survey.

BACKGROUND: Circulating tumor DNA (ctDNA) is a promising biomarker in melanoma, with higher sensitivity for tumor burden detection than conventional diagnostics. While well established in research, clinical routine implementation remains pending. Key global questions concern optimal clinical applications and barriers to adoption. METHODS: A web-based survey of 116 members of the Melanoma World Society Study Group assessed international expert opinions on ctDNA utility across predefined clinical scenarios. The questionnaire included 18 general questions on ctDNA use and 5 clinical vignettes wi

Development and Validation of Novel Senescence-TIME Biomarkers for Predicting the Prognosis and Immunotherapy Responsiveness of SKCM Patients.

BACKGROUND: Increasing evidence indicates that tumor cellular senescence can impair antitumor immunity and promote skin cutaneous melanoma (SKCM) progression. However, effective methods for assessing tumor cellular senescent status and tumor immune microenvironment (TIME) status remain lacking. This study intends to establish a novel Senescence-TIME Risk Score (STIRS) based on senescence and TIME related genes to predict prognosis and immunotherapy responsiveness in SKCM patients, thereby providing new strategies for current clinical personalized treatment. METHODS: We identified distinct sen

Dermoscopic features of amelanotic cutaneous melanoma metastases: evidence from four cases.

Cutaneous metastases of malignant melanoma (MM) are relatively common, occurring in 2-20% of cases. These metastases typically present as pigmented papules or nodules but can occasionally manifest in unusual forms, such as amelanotic cutaneous melanoma metastases (ACMMs). Due to their relative rarity (less than one-third of cutaneous metastases of MM), definitive dermoscopic characterization of ACMMs remains elusive. This case series investigates the dermoscopic features of four ACMM cases, all of which exhibited vascular patterns. Consolidating our findings with existing literature, polymorph

Second primary invasive cutaneous melanomas in Queensland over 4 decades.

Second primary invasive cutaneous melanomas (SPICMs) are common and pose an increased risk of death compared with single invasive melanoma only. Using data from the Queensland Oncology Repository, we investigated changes in the cumulative incidence of SPICMs for people diagnosed with a first primary invasive cutaneous melanoma between 1982 and 2022, with follow-up until December 31, 2023. Death due to any cause was treated as a competing risk. Among the 101,035 people in the study cohort, 9% (n = 9224) were diagnosed with a metachronous SPICM. Estimated 40-year cumulative incidence of SPICMs w

Skin cancer and vitamins: In theory and practice.

Skin cancers are common malignant tumors with an increasing incidence worldwide. The major types are cutaneous malignant melanoma and keratinocytic tumors, such as basal cell carcinoma and cutaneous squamous cell carcinoma. The first treatment of choice is complete surgical removal, but nonsurgical treatment options and primary prevention may be requested. Vitamins may play a role in skin cancer prevention, because they are capable of reducing the deteriorating effects of sunlight (UV and visible light and infrared radiation) by scavenging reactive oxygen species. Patients with skin cancer are

Evaluation of the Contribution of Repeat Total Body Photography for the Early Diagnosis of Melanoma.

UNLABELLED: <p>Introduction: Early detection of cutaneous melanoma is crucial, as prognosis is strongly determined by Breslow thickness. Total body photography (TBP) is increasingly used in high-risk populations, yet optimal surveillance strategies remain undefined. In our study, we aimed to evaluate how different follow-up approaches, including those incorporating TBP at varying frequencies, influence the early diagnosis of melanoma, as measured by Breslow thickness. METHODS: This retrospective cohort study included adult patients diagnosed with primary cutaneous melanoma at Rabin Medical Ce
