Identification of NTRK3 fusions in plaque-like CD34-positive dermal fibroma.

AIMS: Plaque-like CD34-positive dermal fibroma (PDF), previously termed medallion-like dermal dendrocyte hamartoma, is a rare CD34-positive superficial spindle cell fibroblastic tumour that may closely mimic dermatofibrosarcoma protuberans. Recent studies have identified recurrent kinase gene fusions in a subset of congenital and paediatric CD34-positive plaque-like superficial spindle cell tumours with overlapping clinicopathologic features, highlighting an emerging molecular framework for this group. However, the molecular spectrum of lesions meeting classic histopathologic criteria for PDF

Untangling the Complexity of Childhood Atopic Dermatitis: Phenotypes and Endotypes.

Atopic dermatitis (AD) is a highly prevalent, chronic inflammatory skin disease that predominantly begins in early childhood. Increasing evidence supports that pediatric AD is not a single entity, but rather a spectrum of overlapping phenotypes and endotypes, defined by clinical presentation, immune profile, genetics, ethnicity, and comorbid atopic conditions. This review provides a comprehensive synthesis of current knowledge on childhood AD heterogeneity, emphasizing severity, age of onset, allergen sensitization, allergic comorbidities, ethnicity, and disease trajectory. Spitz Nevi With Novel BRAF Fusions: A Report of Two Cases With Striking Morphologic Features.

Spitz neoplasms comprise a spectrum of melanocytic lesions with overlapping clinical and histologic features. BRAF fusions account for only 5%-6% of Spitz neoplasms with a limited number of fusion partners being characterized. We report 2 Spitz nevi with novel BRAF fusions and reproducible morphologic findings. The first case, a 35-year-old man with a finger mass, morphologically demonstrated intraepidermal and dermal proliferation of enlarged epithelioid melanocytes without maturation, and a striking pattern of neurotropism and angiotropism. Immunohistochemistry revealed <30% PRAME expression

Anti-Myeloperoxidase (MPO)-Positive Granulomatosis With Polyangiitis Presenting With Pulmonary and Cutaneous Vasculitic Flares in End-Stage Renal Disease: A Diagnostic and Therapeutic Challenge.

Granulomatosis with polyangiitis (GPA) is a systemic small-vessel vasculitis typically associated with anti-proteinase-3 antibodies. Anti-myeloperoxidase (MPO) positivity is uncommon in GPA and often creates clinical overlap with microscopic polyangiitis. Managing atypical flares in patients with end-stage renal disease (ESRD) is particularly complex, as traditional goals such as renal recovery are absent, shifting the focus to life-threatening extrarenal manifestations. We present the case of a 50-year-old woman with known anti-MPO-positive GPA and ESRD on peritoneal dialysis who presented wi

Pseudomyogenic hemangioendothelioma of the tibia: a case report.

INTRODUCTION AND IMPORTANCE: Pseudomyogenic hemangioendothelioma (PMHE) is a rarely metastasizing intermediate endothelial neoplasm. It typically presents as multiple, discontinuous nodules in the lower extremities of young adult males. Most cases present skin or soft-tissue tumors, but intraosseous lesions without soft-tissue involvement are extremely rare. We report a case of PMHE of the tibia, diagnosed with FOSB staining. PRESENTATION OF CASE: A 36-year-old man presented with pain in the lower left leg after exercise and at night. X-ray, SPECT/CT, and MRI revealed two discontinuous lytic

A type 2 diabetes patient with three years of persistent abdominal pain: the culprit was variegate porphyria-a case report.

INTRODUCTION: Variegate porphyria (VP) is a rare metabolic disorder. Its diagnosis is challenging when cutaneous features are absent and symptoms overlap with common conditions like diabetic neuropathy. CASE PRESENTATION: We report a 71-year-old female with a 30-year history of type 2 diabetes and a 3-year history of mild chronic abdominal pain and psychiatric symptoms. Extensive workup for common abdominal pathologies was negative. A positive urine sun exposure test prompted genetic analysis, which identified a heterozygous pathogenic variant in the PPOX gene (c.567A>C, p.Gln189His), confirm

When Foot Drop Tells a Bigger Story: POEMS (Polyneuropathy, Organomegaly, Endocrinopathy, Monoclonal Plasma Cell Disorder, and Skin Changes) Syndrome Revealed by Femoral Plasmacytoma.

POEMS (polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, and skin changes) syndrome is a rare multisystem disorder caused by a monoclonal plasma cell dyscrasia, often presenting with a constellation of neuropathy, organomegaly, endocrinopathy, monoclonal protein, and skin changes. Its initial manifestations frequently overlap with more common causes of chronic progressive neuropathy, such as chronic inflammatory demyelinating polyneuropathy (CIDP) and diabetic or paraproteinemic neuropathies, which can obscure the diagnosis and delay definitive treatment. We report

Cytomorphological Features of Scalp Cylindroma: Insights From Fine Needle Aspiration Cytology.

Cylindroma is a rare benign cutaneous adnexal tumour that commonly occurs on the scalp. Accurate cytological diagnosis can be difficult due to overlap with other basaloid neoplasms. We report a 71-year-old woman presenting with two slowly enlarging scalp nodules. FNAC revealed moderately cellular smears with clusters and acini of basaloid cells with hyperchromatic nuclei and scant cytoplasm, along with a second population of larger cells with vesicular nuclei and moderate cytoplasm. Tumour cell groups were surrounded by abundant hyaline basement membrane-like material. No atypia, mitoses or ne

The Role of Imaging Techniques in Pigmented Bowen Disease and Lentigo Maligna of the Head and Neck: A Comparative Dermoscopic and Reflectance Confocal Microscopy Study.

Pigmented Bowen disease (pBD) may closely mimic lentigo maligna (LM) both clinically and dermoscopically. Accurate differentiation between these entities is clinically crucial, as misclassification may lead to overtreatment or undertreatment. Noninvasive imaging techniques, including dermoscopy and reflectance confocal microscopy (RCM), have improved diagnostic accuracy, but direct comparative data between pBD and LM remain limited. This observational cohort study included 10 consecutive cases of pBD and 10 consecutive cases of LM diagnosed between January and July 2024 at a tertiary referral

Case Report: Two neonatal cases of genetically confirmed junctional epidermolysis bullosa in a tertiary care center.

BACKGROUND: Epidermolysis bullosa (EB) comprises a group of genetically heterogeneous disorders caused by defects in proteins responsible for dermoepidermal adhesion. Severe forms frequently present in the neonatal period and may be associated with early extracutaneous involvement, reflecting the widespread expression of these structural proteins beyond the skin. CASE PRESENTATION: We report two term neonates presenting at birth with extensive mucocutaneous blistering. The first neonate demonstrated congenital skin fragility associated with radiologic evidence of pyloric atresia. Surgical cor

A Diagnostic Conundrum in Fumarase Deficiency: Expanding the Clinical and Genetic Spectrum in a Cohort of Pediatric and Adult Patients.

Fumarase deficiency (FMRD) is a rare autosomal recessive disorder caused by fumarate hydratase (FH) deficiency, with variable presentation from severe early-onset encephalopathy to mild cognitive impairment. Heterozygous carriers of some variants are predisposed to hereditary leiomyomatosis and renal cell carcinoma (HLRCC). We retrospectively reviewed clinical, biochemical, and molecular data from patients with confirmed or suspected FMRD across three metabolic centers in Northern England. Ten patients (seven females, three males) from seven families were identified, with a median diagnostic a

Tinea Incognito Imitating Pustular Psoriasis in a Psoriatic Patient Following Immunosuppressive Therapy: Case Report and Narrative Review.

Tinea incognito is an atypical form of dermatophytosis caused by previous use of topical or systemic immunosuppressive therapy, most often corticosteroids. Modification of the clinical presentation frequently leads to diagnostic delay and misdiagnosis, especially in patients with concomitant chronic inflammatory skin diseases such as psoriasis. We present a narrative review of the literature on tinea incognito in patients with psoriasis during immunosuppressive therapy. We screened 386 abstracts and included 16 comparable case reports focusing on tinea incognito occurring in patients with psor

SKIN AS A POTENTIAL ENTRY POINT FOR SARS-COV-2.

The primary route of SARS-CoV-2 entry is via respiratory epithelium. However, many COVID-19 patients developed dermatological lesions, and SARS-CoV-2 RNA has been detected in the patients' skin. Inflammatory skin diseases, psoriasis and atopic dermatitis (AD), significantly increased the risk of COVID-19. To evaluate the potential role of skin in SARS-CoV-2 host interactions, we utilized 3D human skin organoids (HSO) generated from human epidermal keratinocytes, as well as neonatal skin explants. HSO were treated with cytokines involved in acute and chronic skin inflammation and cytokine storm

A Rare Hybrid Presentation: Coexistence of Necrotizing and Histiocytoid Variants of Sweet Syndrome (SS) in a Patient With Acute Myeloid Leukemia (AML).

Sweet syndrome (SS) is a rare, acute, febrile neutrophilic dermatosis marked by neutrophilic infiltration into the skin, blood, and various organ systems. It manifests as painful skin eruptions, which can range from violaceous papules and plaques to vesicles and ulcers, accompanied by peripheral blood neutrophilia and fever. Sweet syndrome has three subtypes: classical, drug-induced, and malignancy-associated. There are different clinical and pathological variants of SS. However, rare variants, such as necrotizing SS and histiocytoid SS, can complicate the clinical and pathological picture, pa

The Clinicopathologic and Genomic Features of Mature Versus Blastic Plasmacytoid Dendritic Cell Neoplasms Arising From Chronic Myeloid Neoplasms.

Clonal mature plasmacytoid dendritic cell proliferations (MPDCP) are a recently recognized entity in the WHO fifth edition, but their pathologic spectrum remains poorly characterized. Cases with extensive MPDCP in the bone marrow (BM) are rare and can exhibit overlapping features with blastic plasmacytoid dendritic cell neoplasm (BPDCN). We compared clinicopathologic and genomic features of 11 patients with myeloid neoplasm-associated MPDCP to 5 patients with secondary BPDCN arising from clonally-related myeloid neoplasms. MPDCP exhibited variable degrees of BM involvement (5% to 50%) and arch

Progressive Pseudorheumatoid Dysplasia Presenting with Cauda Equina Compression: A Case Report.

PURPOSE: Fresh vegetables are essential human dietary components but are increasingly implicated in foodborne infections caused by Escherichia coli. Contamination may occur during cultivation, irrigation, handling, and retail display, and the presence of pathogenic or antimicrobial-resistant E. coli poses significant public health risks. Despite widespread vegetable consumption in Ogun State, Nigeria, limited data exist on the resistance and virulence characteristics of E. coli in these foods. This study therefore assessed antimicrobial resistance profiles and virulence genes in E. coli isolat

Multi-class classification of autoimmune skin disease by efficient localization of overlapped lesion boundaries using SAM.

BACKGROUND: Detection of Autoimmune skin disease is found challenging due to overlapping features and irregular skin lesion boundaries. Although numerous deep learning models have been proposed for skin disease classification, most are primarily designed for general skin lesions and do not capture the complex and irregular visual characteristics specific to autoimmune skin conditions. RESEARCH DESIGN AND METHODS: The proposed study develops a hybrid deep learning model, AutoImmune-HybridNet, that uses the Segment Anything Model for intelligent isolation of lesion regions, and ResNet50 with a

Morphology-guided attention networks for explainable skin cancer detection under clinical uncertainty.

Accurate and reliable skin cancer detection from dermoscopic images remains challenging due to large visual variability, overlapping lesion appearances, and inherent clinical uncertainty. To address these issues, this work proposes a morphology-guided attention framework for explainable and uncertainty-aware skin lesion classification. The system integrates lesion segmentation to preserve clinically meaningful morphological structures, followed by an attention-based classification network that emphasizes diagnostically relevant regions while suppressing background artifacts. Visual attention a

Platelet-Rich Plasma Injections and Scalp Cutaneous Malignancies: Clinical Implications and Knowledge Gaps.

Platelet-rich plasma (PRP) injections are an effective and increasingly utilized treatment for androgenetic alopecia; however, the delivery of concentrated growth factors raises theoretical concerns regarding cutaneous malignancy risk in chronically ultraviolet (UV)-exposed, hair-thinning scalps. In this article, the current hypotheses linking PRP to scalp skin cancer risk are reviewed, including potential oncogenic mechanisms, such as platelet-derived growth factor (PDGF)-mediated signaling, and protective effects, such as improved hair-mediated photoprotection and antiproliferative findings
