Chronic myelomonocytic leukemia (CMML) is an MDS/MPN overlap neoplasm defined by monocytosis, clonal hematopoiesis, and marrow dysplasia, with outcomes ranging from indolent cytopenic disease to aggressive proliferative phenotypes with early AML transformation. Extramedullary disease (EMD) refers to clonal myelomonocytic infiltration outside the bone marrow and is an underrecognized, consequential manifestation that can dominate presentation, drive morbidity, and identify adverse disease biology. While hepatosplenic involvement, often reflecting extramedullary hematopoiesis, is common, biopsy

RATIONALE: Ulcerative colitis (UC) and psoriasis are both chronic inflammatory diseases mediated by the immune system. Ixekizumab, used to treat psoriasis, carries a risk of inducing or exacerbating UC. Infliximab has also been reported to exacerbate psoriasis. Guselkumab (GUS) can be used to treat both UC and psoriasis. Currently, there are few case reports on the use of GUS for the treatment of UC-psoriasis comorbidity. Our observations support the rationale for IL-23-targeted agents in patients with overlapping psoriasis and inflammatory bowel disease, and they provide exploratory evidence

INTRODUCTION: Lentigo maligna (LM) is a melanoma subtype that develops predominantly on photodamaged skin. Classic criteria may miss early lesions due to subtle or partially present malignancy-associated features or their overlap with benign or premalignant conditions related to photodamage. The recently introduced inverse approach, which focuses on the absence of typical non-melanoma dermoscopic features, has shown promise in identifying these challenging cases.
OBJECTIVES: This study prospectively evaluated the real-life performance of the inverse approach for differentiating LM from its mi

BACKGROUND: A 60-year-old male patient presented to a senological clinic with a left axillary tumor. The histomorphological characteristics were ambiguous, initially pointing to apocrine carcinoma of mammary origin. This suspicion led to a delay in establishing the correct diagnosis. Only after complete tumor excision and comprehensive work-up in a multidisciplinary dermato-oncological clinic could a definitive diagnosis of primary cutaneous apocrine carcinoma (PCAC) be reached, allowing for appropriate therapy to be commenced.
CASE PRESENTATION: The patient had an unremarkable medical histor

BACKGROUND: Psoriasis is an immune-mediated inflammatory disease driven by dysregulated crosstalk between immune cells and skin-resident cell populations, particularly keratinocytes and fibroblasts. However, transcriptomic findings across studies remain heterogeneous and incompletely integrated.
OBJECTIVES: To integrate transcriptomic evidence across independent studies to define reproducible immune-gene expression patterns in plaque psoriasis and identify hypothesis-generating pathways beyond canonical immune programmes.
METHODS: We performed a systematic review and meta-analysis of transcr

Lupus panniculitis (LP) is a neglected, often treatment-refractory subtype of cutaneous lupus erythematosus. It is characterized by inflammation of subcutaneous adipose tissue, and the pathomechanisms are poorly understood. We sought to explore the cellular and molecular signatures of LP and their relationship to systemic lupus erythematosus. We performed imaging mass cytometry (IMC; LP n = 8, Healthy Control (HC) n = 6) and targeted transcript profiling by NanoString nCounter (Human Immunology Panel, 579 genes; LP n = 9, HC n = 9). LP lesions showed extensive septal leukocyte infiltration. Th

Mantle cell lymphoma (MCL) is a rare and heterogeneous subtype of B-cell non-Hodgkin lymphoma (NHL), defined by the t(11;14)(q13;q32) translocation leading to overexpression of cyclin D1. The blastoid variant represents a highly aggressive form, with a high mitotic index and unfavorable prognosis. Cutaneous involvement by mantle cell lymphoma is rare and may present with atypical clinical features, often leading to delayed diagnosis due to morphological overlap with other high-grade lymphomas and unusual extranodal presentations. We describe a 73-year-old male who presented with a solitary flu

Cutaneous T-cell lymphoma is a rare form of non-Hodgkin lymphoma, characterized by the clonal proliferation of neoplastic T cells in the skin. Its early diagnosis is often challenging owing to overlapping clinical features with inflammatory dermatoses. This study investigates TRBC1 as a marker for T-cell clonality in formalin-fixed, paraffin-embedded skin biopsies. We analyzed single-cell RNA-sequencing data from cutaneous T-cell lymphoma skin lesions to examine the expression of TRBC1 in T helper cells. To quantify the extent of TRBC1 polarization, we developed the TRBC1 clonality score (TRBC

BACKGROUND: VEXAS syndrome (Vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) is a recently described adult-onset autoinflammatory disease caused by somatic mutations in the UBA1 gene. Its heterogeneous clinical presentation frequently overlaps with inflammatory, autoimmune, and hematological disorders, resulting in diagnostic delay. We report the first genetically confirmed case of VEXAS syndrome in Tunisia and highlight its clinical complexity, with particular emphasis on cardiac and ocular involvement, as well as therapeutic management in light of current literature.

BACKGROUND: Psoriasis is a common chronic inflammatory skin disease characterized by recurrent flares at previously affected locations. Although biologics targeting IL-17 or IL-23 effectively suppress active disease, they rarely result in long-term remission, suggesting that clinically resolved skin retains molecular changes predisposing to relapse.
OBJECTIVE: The STEPIn main study indicated that early intervention with secukinumab, an anti-IL-17A monoclonal antibody, may modify the course of psoriasis. This mechanistic substudy sought to define cellular and molecular changes in psoriatic ski

Psoriasis, a chronic inflammatory skin disease affecting men and women equally, presents distinct gender-based differences in severity and treatment response. While molecular mechanisms underlying psoriasis are well-studied, sex-specific differences remain largely unexplored. To address this, we conducted a comprehensive analysis of transcriptomic data from lesional psoriasis skin and healthy controls, comparing male and female cohorts. Our findings reveal 2760 overlapping differentially expressed genes (DEGs) between sexes, highlighting shared pathways like IL-17 signaling and Th17 differenti

Background/Objective: Leishmaniasis is a neglected tropical disease caused by species of the genus Leishmania, with a broad clinical spectrum that can overlap with other infectious and non-infectious conditions. Accurate species identification is critical for appropriate treatment and prognosis; however, parasitological methods are limited by suboptimal sensitivity, specificity, and inability to reliably differentiate species. This study aimed to develop and validate a real-time PCR assay based on melting-curve analysis (Leish-qPCR) for the detection of Leishmania spp. and the differentiation

BACKGROUND: Psoriasis is a chronic immune-mediated inflammatory skin disease driven by dysregulation of the IL-23/IL-17 axis and influenced by genetic and environmental factors. The role and molecular mechanisms of the environmental pollutant dibenzo-p-dioxins, particularly 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD), in psoriasis remain unclear.
METHODS: An integrative strategy combining network toxicology, machine learning, bioinformatics analysis, molecular simulation, and in vitro keratinocyte qRT-PCR validation was employed to systematically investigate the potential toxicity and molecula

INTRODUCTION: Dermatological disorders are emerging as important indicators of systemic health and have drastic implications for cardiovascular disease (CVD). Skin and cardiovascular diseases share common pathophysiological mechanisms, including chronic inflammation, oxidative stress, and endothelial dysfunction. These overlapping pathways contribute to the increased cardiovascular (CV) risk observed in patients with inflammatory and genetic dermatological conditions, such as psoriasis, atopic dermatitis, and systemic lupus erythematosus.
METHODS: This narrative review was conducted by evalua
