Acral Subcorneal Hematoma Mimicking Melanoma: Dermoscopic Features and Exploratory Digital Color Analysis.

Acral pigmented lesions pose a diagnostic challenge due to the overlap between benign entities, such as subcorneal hematoma, and acral melanoma. Dermoscopy is a key noninvasive tool that aids in differentiation by revealing subsurface structures not visible to the naked eye. We report the case of a 54-year-old man presenting with an asymptomatic acral pigmented macule on the volar aspect of the finger. Clinically, the lesion appeared dark reddish-brown, raising concern for a melanocytic process; however, dermoscopic evaluation revealed a homogeneous, predominantly red, structureless pattern wi

Ligament-Focused Advanced Monopolar Radiofrequency: A New Approach to Face and Neck Skin Laxity.

Facial and neck skin laxity due to aging represents a major aesthetic concern among women. Despite this, most patients prefer non-invasive treatments that may offer natural results, minimal downtime, and low risk of complications. Advanced monopolar radiofrequency (RF) technology addresses these needs through continuous low-temperature epidermal cooling, enabling safe delivery of maximal energy (140 W) to superficial and mid-dermal layers and facial retaining ligaments without burns, lesions, or discomfort. This protocol presented a novel ligament-focused approach for adults aged 35 years or o

POEMS syndrome: A case report.

POEMS syndrome is a rare multisystem disorder caused by a monoclonal plasma cell neoplasm, characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, and skin changes. Due to its overlapping features with other neuropathies, diagnosis is often delayed, impacting prognosis. We report a 37-year-old male presenting with progressive limb weakness, inguinal lymphadenopathy, and spinal nerve root enhancement. Imaging revealed mixed lytic-sclerotic bony lesions, lymphadenopathies, hepatosplenomegaly, and spinal nerve root thickening and enhancement, which raised e

Topical Trichosanthis Radix Water Extract Attenuates Atopic Dermatitis-Like Skin Inflammation: Marker Standardization, Network Pharmacology, and Preclinical Validation.

Trichosanthis Radix (TR) is a pharmacopeial crude drug traditionally used to clear heat, reduce swelling, expel pus, and generate fluids, suggesting potential relevance to inflammatory skin disorders such as atopic dermatitis (AD). In this study, we evaluated the AD-related activity of a TR water extract using an integrated workflow combining network-based target analysis, marker compound analysis, and experimental validation in vivo and in vitro. The overlap between predicted TR targets and AD-associated genes was analyzed using a protein-protein interaction network, and extract was analyzed

Psoriasis-risk SNP rs610604 reveals nuclear IKKα-TNFAIP3 repressor axis driving autoinflammatory circuits.

Psoriasis is a chronic inflammatory skin disease characterized by keratinocyte hyperproliferation and immune activation. The SNP rs610604 within the TNFAIP3 locus is associated with increased psoriasis susceptibility and response to TNF-targeted therapies, yet its functional mechanism remains unclear. In this study, we identify, to our knowledge, a previously unreported regulatory axis linking IKKα to TNFAIP3 in keratinocytes and demonstrate how genetic variation at rs610604 modulates inflammatory signaling. Genome-wide chromatin profiling revealed that nuclear IKKα predominantly occupies tran

Lichen Sclerosus and Keloidal Morphea Overlap: A Case Report of an Unusual Nodular Presentation.

Lichen sclerosus (LS) and morphea are chronic inflammatory sclerosing dermatoses that occasionally exhibit overlap. While their coexistence within the same lesion is increasingly recognized, nodular presentations remain exceptionally rare. We report a 50-year-old woman with a strikingly distinctive presentation of a solitary, well-circumscribed, exophytic nodule on the left posterior thigh, characterized by an ivory-colored peripheral rim, and an erythematous central zone. Histopathological examination revealed features of both lichen sclerosus (epidermal atrophy, basal vacuolar alteration, an

Non-Melanocytic Histopathological Clues for Melanoma Diagnosis: A Practical Review of Solar Elastosis, Stromal Regression, and Epidermal Reaction Patterns. Do Old-School Clues Still Matter?

Histopathologic melanoma diagnosis extends beyond melanocytic cytology to encompass non-melanocytic features: solar elastosis patterns, stromal regression, adnexal relationships, epidermal reaction patterns, and the host inflammatory response. These "old school" low-power clues are particularly valuable on sun-damaged skin, where benign nevi, reactive melanocytic hyperplasia, and melanoma in situ share overlapping features. Quantitative data support two elastosis-based signs: the "umbrella sign" (reduced elastosis beneath the lesion's central third; PPV (Positive Predictive Value) for nevus, 9

Network Toxicology and Machine Learning Uncover BPA-Driven Molecular Mechanisms in Atopic Dermatitis.

Bisphenol A (BPA) is a common industrial chemical primarily used in the manufacture of plastics, and it has been found in more than 90% of people worldwide. As an endocrine disruptor, BPA can impair reproduction, development, immunity, metabolism, and cognition; it also disturbs immune balance and thus fosters chronic inflammation. A number of population-based studies have indicated a link between environmental BPA exposure and atopic dermatitis (AD). Nevertheless, the detailed molecular pathways connecting BPA to AD remain poorly understood. AD is the leading chronic recurrent inflammatory sk

Blastic plasmacytoid dendritic cell neoplasm: ontogeny, immunophenotype, genetics and epigenetics, immune dysregulation, skin tropism and therapeutic implications.

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare, highly aggressive hematologic malignancy characterized by frequent cutaneous involvement, rapid systemic dissemination, and poor clinical outcomes. Although initially misclassified due to overlapping morphologic and immunophenotypic features with other acute leukemias, lymphomas, and NK-cell neoplasms, BPDCN is now recognized as a distinct hematologic malignancy. Advances in immunophenotyping, transcriptional profiling, and genomic analysis have clarified the cellular origin of BPDCN and revealed that the disease commonly arises f

Artificial intelligence vs. dermoscopy for malignancy risk stratification of pigmented skin lesions: a systematic review and meta-analysis for public health.

BACKGROUND: Accurate risk stratification of pigmented skin lesions is essential for early melanoma detection and for reducing unnecessary excisions. Although artificial intelligence (AI) is increasingly applied to dermoscopic image analysis, its diagnostic performance compared to dermoscopy remains unclear. OBJECTIVE: This study aimed to compare the diagnostic performance of AI, dermoscopy, and AI-assisted clinicians for malignancy risk stratification of pigmented skin lesions. METHODS: PubMed, Embase, Web of Science, and the Cochrane Library were systematically searched for studies evaluati

Coexistence of Acantholytic Dyskeratosis and Epidermolytic Hyperkeratosis in a Single Cutaneous Lesion: A Case Report and Brief Review.

Acantholytic dyskeratosis and epidermolytic hyperkeratosis are distinct histopathologic patterns reflecting abnormalities in keratinocyte differentiation. Although typically considered separate histopathologic reaction patterns, their coexistence within a single lesion is uncommon and remains poorly characterized. We report the case of a 51-year-old woman with a lumbar skin lesion excised for suspected intradermal nevus or basal cell carcinoma. Histopathologic examination revealed a combination of Acantholytic dyskeratosis and epidermolytic hyperkeratosis features without evidence of malignanc

DIG-MambaNet: A Dual-Path Interactive Guided Mamba Network for Medical Image Segmentation.

Reliable medical image segmentation remains challenging because models must preserve fine boundary details while maintaining global semantic consistency. CNNs capture local structures effectively but have limited long-range modeling ability, whereas Transformer-based methods improve global context at high computational cost. Mamba-based state space models offer efficient long-range modeling, but may weaken high-frequency textures and boundary cues. To address these limitations, we propose DIG-MambaNet, a Dual-path Interactive Guided Mamba Network for medical image segmentation. The network int

Exome Sequencing Uncovers Phenotypic and Genotypic Heterogeneity in 196 Indian Families Evaluated for Autoinflammatory Disorders.

Autoinflammatory disorders (AIDs) are a clinically heterogeneous group of inborn errors of immunity primarily caused by dysregulation in the innate immune system. Clinical diagnosis is often challenging due to clinical heterogeneity and the overlapping phenotypes with other inborn errors of immunity and monogenic conditions that mimic AIDs. Worldwide, and especially in India, there is a lack of systematic large cohort studies employing genomic testing early in the course of evaluation for individuals with autoinflammatory disease. Herein, we describe our experience using exome sequencing as a

Squamous Cell Carcinoma Arising in a Mature Cystic Teratoma of the Ovary Presenting With a Ruptured Sister Mary Joseph Nodule: A Rare Case Report From Northern Tanzania.

Squamous cell carcinoma arising from malignant transformation of a mature cystic teratoma is an exceptionally rare ovarian malignancy, and metastatic umbilical involvement is particularly uncommon. Diagnosis and treatment planning are challenging due to overlapping presentation with epithelial ovarian cancer, particularly in settings with limited access to ancillary pathology, molecular testing, and specialist oncology services. We report a 54-year-old postmenopausal woman from northern Tanzania who presented with progressive abdominal distension and a spontaneously ruptured umbilical lesion w

Conservative tissue sparing management of pediatric pyogenic granuloma with preserved joint function: A narrative review with an illustrative case.

The pediatric population presents specific management challenges with pyogenic granuloma (PG), particularly when located near joints, as surgical treatment may compromise mobility. While surgical excision remains the gold standard treatment, emerging reports suggest that more conservative approaches may be suitable in select cases, offering less invasive treatment options. We report the preservation of joint function while successfully resolving pyogenic digital granuloma with topical steroid-antibiotic preparations. Basal Cell Carcinoma and Syringocystoadenoma Papilliferum Arising in Naevus Sebaceous-A Narrative Review.

Naevus sebaceous (NS) is a congenital cutaneous hamartoma characterised by epidermal, follicular, sebaceous and apocrine components, with a rare but significant potential for malignant transformation, most commonly into basal cell carcinoma (BCC). Syringocystoadenoma papilliferum (SCAP), a rare adnexal tumour, occasionally arises within NS lesions and may progress to malignancy. This study presents a comprehensive narrative review and a rare case of SCAP arising within NS and subsequently transforming into BCC, highlighting the interplay between these lesions. Hidradenitis suppurativa with systemic autoinflammatory features in patients of Moroccan origin: case report and implications for personalized medicine.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease primarily affecting intertriginous regions, and emerging evidence suggests that systemic autoinflammation ("metainflammation") contributes to its clinical heterogeneity. We report two cases of HS in patients of Moroccan origin with overlapping autoinflammatory features. The first case involves a 42-year-old man with HS since 2012, Hurley stage II, treated with adalimumab since 2019 and achieving an IHS4-70, who developed Behçet's disease-like systemic manifestations in 2025; genetic testing revealed a heterozygous MEFV varian

Multicentric Asynchronous Endocrine Mucin-Producing Sweat Gland Carcinoma and Mucinous Carcinoma of the Skin.

Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is an adnexal neoplasm which typically occurs on periorbital skin and demonstrates overlapping histopathologic features with primary mucinous carcinoma of the skin (MCS). Herein, we report a patient who developed five distinct lesions of EMPSGC and MCS over an eight-year period, some of which occurred on periorbital sites and others which occurred on the lower face. We further sought to better characterize these tumors on a molecular basis; targeted next generation sequencing analysis of two discrete tumors revealed identical missense va

ARAF mutated langerhans cell histiocytosis developed on the background of ARAF/JAK2 mutated polycythaemia vera: evidence of branching evolution from the same progenitor cell. A case report and literature review.

The case report presents the first documented instance of ARAF-mutated Langerhans cell histiocytosis (LCH) arising in a patient with ARAF/JAK2-mutated polycythaemia vera (PV), providing compelling evidence for a clonal relationship and branching evolution from a common progenitor precursor. The patient, a 63-year-old male, initially presented with classic PV symptoms and was diagnosed with JAK2 p.V617F mutated PV. Over the disease course, persistent monocytosis and progressive splenomegaly were noted, with subsequent development of a diffuse, non-itching maculopapular skin eruption in the sett
