BACKGROUND: The aim of this study was to assess clinical and histologic features of primary basal cell carcinomas of the periocular region and to calculate the rate of recurrence of these tumors after surgical excision without intraoperative frozen section control.

PURPOSE: To review the clinical features of pilomatrixomas of the eyelids and eyebrows in children and adolescents.
METHODS: The authors reviewed the medical records of 95 patients, all of whom were 20 years old or younger and had the histopathologic diagnosis of pilomatrixoma made from 1976 through 1995. Special attention was given to that subgroup of patients with tumors involving the eyelids and eyebrows. Data analyzed included age at diagnosis, patient sex, location and size of tumor, preoperative diagnosis, and clinical description of tumor.

An island transposition flap for use in lower eyelid reconstruction and the treatment of ectropion is described. The flap is based on a subcutaneous pedicle 1 to 2 cm lateral to the outer canthus. Its long axis extends inferolaterally from this point, and the donor site can be closed directly. Reconstruction of the orbit can at times be difficult when tumors of this region are resected. We describe a new surgical procedure to reconstruct the outer canthus of the orbital region, including lateral portions of the upper and lower lids. The techniques that were used until now are often limited in repairing this anatomical area, and the results we obtained with this case encourage us to utilize the superficial temporal fascia.

Of 30 consecutive patients with ocular adnexal basal cell carcinoma, 18 (60%) had at least one additional unsuspected foci of basal cell carcinoma. Thirty-one cases of trichilemmoma of the eyelid and eyebrow were studied. All lesions were solitary, small, and asymptomatic. Unlike basal cell carcinoma of the eyelid, trichilemmoma rarely involves the lid margin or the inner canthus. The age range of our patients was from 22 to 88 years old (mean, 56 years).
