In this 5-year follow-up study, 60 auricular non-melanoma skin cancers, selected at a skin tumour clinic, were treated by a thorough curettage with different sized curettes followed by cryosurgery in a double freeze-thaw cycle. Tumours of the external auditory canal were excluded. Forty-seven basal cell carcinomas (BCCs), nine squamous cell carcinomas (SCCs), three Bowen's disease and one basisquamous cancer were included. The mean diameter of the malignancies was 18 mm (range 5-70). Morphoeiform BCCs, recurrent BCCs with fibrotic component and most of the SCCs were selected for Mohs' microgra

Localized necrosis of the bone, cartilage, and soft tissue of the external auditory canal is an uncommon side effect of radiotherapy to the parotid region. Five patients developed late onset skin necrosis of a quadrant of the ear canal secondary to an underlying osteoradionecrosis of the tympanic ring. We report a one-stage procedure to excise the necrotic tissue and replace it with a local rotational flap derived from the post-auricular skin. Otological side effects of radiotherapy are discussed.

Surgical defects of the concha-helix part of the ear larger than 2 cm may pose a reconstructive challenge. Split- or full-thickness skin grafts or local flaps may be used, and a number of these have been described. Yet cosmetic results are often unsatisfactory. Our experience with a postauricular myocutaneous island flap is described. Eleven patients (12 ears), aged 48 to 89 years, underwent the procedure under local anesthesia following excision of conchal bowl malignant tumors that included the cartilage underlying the skin. The surgical technique is described in detail. Few complications we

Meatoplasty is designed to create a lasting external auditory canal and provide improvement in hearing acuity. In three patients with congenital atresia of the external auditory canal not requiring otoplasty, we carried out meatoplasty using an island pedicle flap obtained from the postauricular region. The island pedicle flap was made into a tubed pedicle when sutured with the epidermis facing inward, which was then made into a blind tube pedicle flap when sutured to a free skin graft. The external auditory canal was completed when the blind tubed pedicle flap was inserted into a newly create

An autologous free skin graft taken from the upper or the lower external ear canal or the auricle was implanted in the otic bulla of 28 guinea pigs. An epidermal cyst simulating middle ear cholesteatoma was successfully produced in 25 (89.3%) animals 8 weeks after grafting. Using this model, the histological reaction of middle ear granulation tissue to the debris was studied. Cysts originated from the auricular skin showed the strongest activity with regard to growth rate, amount of debris produced, and inflammatory reaction to the exposed debris. We conclude that the exposed debris plays an i

The authors report a retrospective series of 12 patients presenting periauricular tumors having invaded the external auditory canal and concha: 5 squamous cell carcinomas, 3 basal cell carcinomas, 2 Darrier-Ferrand sarcomas, 1 parotidean adenocarcinoma, 1 radionecrosis. The operation included an extralabyrinthic petrosectomy, an amputation of the pavilion, and a ganglionic evidement following a histological analysis. The reconstruction processes used 3 pedicle grafts and 9 free grafts. The carcinological extension modalities of the cancers invading the entire external auditory canal require th

Epithelial migration occurs in a lateral direction from the tympanic membrane to the external auditory canal. The underlying mechanism which determines the direction of this laterally directed movement has not been elucidated. This study attempts to analyze whether migration is stimulated at the chondro-osseous junction, the very place where squamous cells stop migrating and desquamate. Active movement of the junction is postulated as a stimulus for the migratory process. Such movement may be caused by contraction of the peri-auricular muscles which attach to the cartilaginous canal. In this s

The anatomic complexity of the pinna predisposes that structure to a variety of unique, site specific postoperative complications following management of skin cancer. We describe four unusual auricular complications: 1) radiochondronecrosis; 2) autonecrosis of skin during second intention healing; 3) hearing loss secondary to tragal retraction over the external auditory canal; and 4) extension of tumor through fenestrated cartilage. Well-known postoperative auricular complications are reviewed and anticipation and recognition of these unusual complications are emphasized.

Leukemic involvement of the temporal bone is not uncommon and may present in a variety of ways including auricular or external canal skin lesions, red or thick tympanic membrane, middle ear effusions, otitis media, hearing loss or mastoiditis. Symptomatic facial nerve involvement, on the other hand, is extremely unusual. We discuss a pediatric patient whose sudden onset facial nerve paresis was the presenting symptom that led to her diagnosis of leukemia. At the time of mastoidectomy, a granulocytic sarcoma or chloroma was noted to be overlying the VIIth nerve.

Three hundred ninety-seven patients with 407 cutaneous malignancies of the auricle, periauricular region, and cartilaginous external ear canal were reviewed. Tumors were most commonly located in the preauricular and postauricular regions, followed by the helix, concha, antihelix, and ear canal. All lesions were excised with Mohs microscopic control of margins. For lesions requiring lateral temporal bone resection, an adaptation of fresh-tissue microscopic control was used to analyze deep and anterior margins suspected of harboring residual tumor. Two-year minimum follow-up of 229 patients with

The rational treatment of severe microtia or anotia should be to restore not only the form but also the auditory function of the ear. To achieve this aim it is obvious that plastic surgeons must co-operate closely with an otologist: yet this degree of co-operation is seldom seen. This paper describes a two-stage procedure for total ear reconstruction with auditory function. In stage I, a tympanoplasty is performed before transplantation of the carved cartilage framework. Stage II consists of elevation of the reconstructed auricle and transplantation of a full-thickness skin graft to the post-a

The investigation used 2 methods to study the cutaneous innervation of the concave surface of the pinna of the dog: electrophysiologic and behavioral. The cutaneous innervation of the external ear canal was studied, using electrophysiologic methods. On one side of each dog, the area of remaining sensitivity for the 3 internal auricular branches of the facial nerve (InAuB-FN) was determined by sectioning the rostral auricular nerve, dorsal cutaneous branch of the second cervical nerve, and great auricular nerve and then examining the dogs 1 week later. The cutaneous area (CA) of each branch of

The bacterial flora of the external ear canals and posterior auricular skin surfaces were investigated in a group of 26 divers after 25 dry-suit dives in harbor water and 20 dry-suit dives in clear test tank test. A control group of 16 divers wore rubber hoods 19 times for a similar period (25 to 30 min) but did not dive. The protective effect of 2% acetic acid was tested by instilling it in the left ear of 14 divers and 8 nondivers. Staphylococcus epidermidis, Propionibacterium acnes, alpha-hemolytic streptococci, and enteric gram-negative rods were the predominant isolates from skin and ear

The recommendation of a strictly endaural approach to the tympanic cavity in congenital atresia is based on a series of 26 cases of severe malformations (grade II and III). Two bony ridges at the lower edge of the atresial plate and the deep auricular artery served as landmarks. A bony posterior canal wall was always conserved or reconstructed. There was no collision with the facial nerve. The reobliteration of the new external auditory canal was avoided by four means: sufficient resection of both overlying soft tissue and the parotid gland, insertion of auto- or homologous cartilage into the
