Cutaneous Involvement by Blastoid Mantle Cell Lymphoma Presenting as Cervical Abscess: A Rare Extranodal Diagnostic Challenge.

Mantle cell lymphoma (MCL) is a rare and heterogeneous subtype of B-cell non-Hodgkin lymphoma (NHL), defined by the t(11;14)(q13;q32) translocation leading to overexpression of cyclin D1. The blastoid variant represents a highly aggressive form, with a high mitotic index and unfavorable prognosis. Cutaneous involvement by mantle cell lymphoma is rare and may present with atypical clinical features, often leading to delayed diagnosis due to morphological overlap with other high-grade lymphomas and unusual extranodal presentations. We describe a 73-year-old male who presented with a solitary flu

Evaluating T-cell clonality in skin using a quantitative TRBC1 clonality score.

Cutaneous T-cell lymphoma is a rare form of non-Hodgkin lymphoma, characterized by the clonal proliferation of neoplastic T cells in the skin. Its early diagnosis is often challenging owing to overlapping clinical features with inflammatory dermatoses. This study investigates TRBC1 as a marker for T-cell clonality in formalin-fixed, paraffin-embedded skin biopsies. We analyzed single-cell RNA-sequencing data from cutaneous T-cell lymphoma skin lesions to examine the expression of TRBC1 in T helper cells. To quantify the extent of TRBC1 polarization, we developed the TRBC1 clonality score (TRBC

Early intervention with secukinumab prevents epigenetic scar development in new-onset psoriasis: STEPIn mechanistic substudy results.

BACKGROUND: Psoriasis is a common chronic inflammatory skin disease characterized by recurrent flares at previously affected locations. Although biologics targeting IL-17 or IL-23 effectively suppress active disease, they rarely result in long-term remission, suggesting that clinically resolved skin retains molecular changes predisposing to relapse. OBJECTIVE: The STEPIn main study indicated that early intervention with secukinumab, an anti-IL-17A monoclonal antibody, may modify the course of psoriasis. This mechanistic substudy sought to define cellular and molecular changes in psoriatic ski

Sex-Specific Transcriptomic Profiles in Psoriatic Lesions: A Large-Scale Integrative Study.

Psoriasis, a chronic inflammatory skin disease affecting men and women equally, presents distinct gender-based differences in severity and treatment response. While molecular mechanisms underlying psoriasis are well-studied, sex-specific differences remain largely unexplored. To address this, we conducted a comprehensive analysis of transcriptomic data from lesional psoriasis skin and healthy controls, comparing male and female cohorts. Our findings reveal 2760 overlapping differentially expressed genes (DEGs) between sexes, highlighting shared pathways like IL-17 signaling and Th17 differenti

Molecular Diagnosis of Leishmaniasis: Development of a qPCR Assay for Genus Detection and Differentiation of Leishmania (L.) amazonensis and Leishmania (V.) braziliensis.

Background/Objective: Leishmaniasis is a neglected tropical disease caused by species of the genus Leishmania, with a broad clinical spectrum that can overlap with other infectious and non-infectious conditions. Accurate species identification is critical for appropriate treatment and prognosis; however, parasitological methods are limited by suboptimal sensitivity, specificity, and inability to reliably differentiate species. This study aimed to develop and validate a real-time PCR assay based on melting-curve analysis (Leish-qPCR) for the detection of Leishmania spp. and the differentiation

Integrated systems toxicology identifies TCDD-responsive targets linked to immune dysregulation and treatment response in psoriasis.

BACKGROUND: Psoriasis is a chronic immune-mediated inflammatory skin disease driven by dysregulation of the IL-23/IL-17 axis and influenced by genetic and environmental factors. The role and molecular mechanisms of the environmental pollutant dibenzo-p-dioxins, particularly 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD), in psoriasis remain unclear. METHODS: An integrative strategy combining network toxicology, machine learning, bioinformatics analysis, molecular simulation, and in vitro keratinocyte qRT-PCR validation was employed to systematically investigate the potential toxicity and molecula

Bridging Dermatology and Cardiology: The Link between Skin Lesions and Cardiovascular Risk.

INTRODUCTION: Dermatological disorders are emerging as important indicators of systemic health and have drastic implications for cardiovascular disease (CVD). Skin and cardiovascular diseases share common pathophysiological mechanisms, including chronic inflammation, oxidative stress, and endothelial dysfunction. These overlapping pathways contribute to the increased cardiovascular (CV) risk observed in patients with inflammatory and genetic dermatological conditions, such as psoriasis, atopic dermatitis, and systemic lupus erythematosus. METHODS: This narrative review was conducted by evalua

Acral Subcorneal Hematoma Mimicking Melanoma: Dermoscopic Features and Exploratory Digital Color Analysis.

Acral pigmented lesions pose a diagnostic challenge due to the overlap between benign entities, such as subcorneal hematoma, and acral melanoma. Dermoscopy is a key noninvasive tool that aids in differentiation by revealing subsurface structures not visible to the naked eye. We report the case of a 54-year-old man presenting with an asymptomatic acral pigmented macule on the volar aspect of the finger. Clinically, the lesion appeared dark reddish-brown, raising concern for a melanocytic process; however, dermoscopic evaluation revealed a homogeneous, predominantly red, structureless pattern wi

Ligament-Focused Advanced Monopolar Radiofrequency: A New Approach to Face and Neck Skin Laxity.

Facial and neck skin laxity due to aging represents a major aesthetic concern among women. Despite this, most patients prefer non-invasive treatments that may offer natural results, minimal downtime, and low risk of complications. Advanced monopolar radiofrequency (RF) technology addresses these needs through continuous low-temperature epidermal cooling, enabling safe delivery of maximal energy (140 W) to superficial and mid-dermal layers and facial retaining ligaments without burns, lesions, or discomfort. This protocol presented a novel ligament-focused approach for adults aged 35 years or o

POEMS syndrome: A case report.

POEMS syndrome is a rare multisystem disorder caused by a monoclonal plasma cell neoplasm, characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, and skin changes. Due to its overlapping features with other neuropathies, diagnosis is often delayed, impacting prognosis. We report a 37-year-old male presenting with progressive limb weakness, inguinal lymphadenopathy, and spinal nerve root enhancement. Imaging revealed mixed lytic-sclerotic bony lesions, lymphadenopathies, hepatosplenomegaly, and spinal nerve root thickening and enhancement, which raised e

Topical Trichosanthis Radix Water Extract Attenuates Atopic Dermatitis-Like Skin Inflammation: Marker Standardization, Network Pharmacology, and Preclinical Validation.

Trichosanthis Radix (TR) is a pharmacopeial crude drug traditionally used to clear heat, reduce swelling, expel pus, and generate fluids, suggesting potential relevance to inflammatory skin disorders such as atopic dermatitis (AD). In this study, we evaluated the AD-related activity of a TR water extract using an integrated workflow combining network-based target analysis, marker compound analysis, and experimental validation in vivo and in vitro. The overlap between predicted TR targets and AD-associated genes was analyzed using a protein-protein interaction network, and extract was analyzed

Psoriasis-risk SNP rs610604 reveals nuclear IKKα-TNFAIP3 repressor axis driving autoinflammatory circuits.

Psoriasis is a chronic inflammatory skin disease characterized by keratinocyte hyperproliferation and immune activation. The SNP rs610604 within the TNFAIP3 locus is associated with increased psoriasis susceptibility and response to TNF-targeted therapies, yet its functional mechanism remains unclear. In this study, we identify, to our knowledge, a previously unreported regulatory axis linking IKKα to TNFAIP3 in keratinocytes and demonstrate how genetic variation at rs610604 modulates inflammatory signaling. Genome-wide chromatin profiling revealed that nuclear IKKα predominantly occupies tran

Lichen Sclerosus and Keloidal Morphea Overlap: A Case Report of an Unusual Nodular Presentation.

Lichen sclerosus (LS) and morphea are chronic inflammatory sclerosing dermatoses that occasionally exhibit overlap. While their coexistence within the same lesion is increasingly recognized, nodular presentations remain exceptionally rare. We report a 50-year-old woman with a strikingly distinctive presentation of a solitary, well-circumscribed, exophytic nodule on the left posterior thigh, characterized by an ivory-colored peripheral rim, and an erythematous central zone. Histopathological examination revealed features of both lichen sclerosus (epidermal atrophy, basal vacuolar alteration, an

Non-Melanocytic Histopathological Clues for Melanoma Diagnosis: A Practical Review of Solar Elastosis, Stromal Regression, and Epidermal Reaction Patterns. Do Old-School Clues Still Matter?

Histopathologic melanoma diagnosis extends beyond melanocytic cytology to encompass non-melanocytic features: solar elastosis patterns, stromal regression, adnexal relationships, epidermal reaction patterns, and the host inflammatory response. These "old school" low-power clues are particularly valuable on sun-damaged skin, where benign nevi, reactive melanocytic hyperplasia, and melanoma in situ share overlapping features. Quantitative data support two elastosis-based signs: the "umbrella sign" (reduced elastosis beneath the lesion's central third; PPV (Positive Predictive Value) for nevus, 9

Network Toxicology and Machine Learning Uncover BPA-Driven Molecular Mechanisms in Atopic Dermatitis.

Bisphenol A (BPA) is a common industrial chemical primarily used in the manufacture of plastics, and it has been found in more than 90% of people worldwide. As an endocrine disruptor, BPA can impair reproduction, development, immunity, metabolism, and cognition; it also disturbs immune balance and thus fosters chronic inflammation. A number of population-based studies have indicated a link between environmental BPA exposure and atopic dermatitis (AD). Nevertheless, the detailed molecular pathways connecting BPA to AD remain poorly understood. AD is the leading chronic recurrent inflammatory sk
