Revisiting the anatomy of the retroperitoneum and renal fascia. Aspects applied to radiology, laparoscopic and robotic surgery - a narrative review.

This review aims to provide a comprehensive analysis of the anatomy of the retroperitoneal and abdominal fascias. We analyzed papers published in the past 70 years in the databases of Pubmed, Embase and Scielo. We excluded case reports, editorials and opinions of specialists. Studies were excluded if they lacked clear anatomical descriptions of the fascias, if full-text access was unavailable, or if they were published in languages other than Portuguese, English, or Spanish. This narrative review addresses the anatomical integration, surgical techniques, and functional importance of the fascia

HMGB1 directly activates BAP1 transcription to sustain tumor growth in mesothelioma.

BAP1 and HMGB1 alterations are implicated in the pathogenesis of mesothelioma, an aggressive cancer of the membranes covering the thoracic and abdominal cavities. Genetic inactivating mutations of BAP1 cause cancer in ~100% of carriers. The mechanism/s that regulate BAP1 expression were unknown. We discovered that HMGB1 directly modulates BAP1 expression. Studying our Hmgb1 knock-out mouse mesothelioma models, primary fibroblasts obtained from carriers of pathogenic heterozygous germline BAP1 mutations, and mesothelioma cell lines, we discovered that HMGB1 loss caused a significant reduction i

Ultrasonic manifestations of diffuse pericardial mesothelioma masquerading as pericardial effusion: a case report.

BACKGROUND: Primary pericardial mesothelioma (PPM) is an exceedingly rare, highly invasive cardiac neoplasm with insidious onset and non-specific clinical symptoms. Due to the lack of typical early manifestations and effective routine diagnostic methods, PPM is often misdiagnosed and missed, resulting in delayed treatment and poor prognosis. CASE PRESENTATION: A 42-year-old male presented with 6 months of recurrent chest tightness and pain, worsening over 2 months, and two prior admissions for unexplained pericardial effusion. On admission, he had tachycardia, distant heart sounds, bilateral

Reprogramming the tumor microenvironment in mesothelioma: from immune suppression to therapeutic opportunity.

Pleural mesothelioma (PM) is a rare and aggressive cancer with limited treatment options and poor prognosis. Unlike other thoracic cancers responding well to immunotherapy, PM typically lacks key predictive biomarkers such as Programmed Death-Ligand 1 (PD-L1) tumor expression. Nonetheless, immune checkpoint inhibitors (ICIs) have shown clinical activity in PM, particularly in non-epithelioid subtypes. However, only a subset of patients benefits from this approach, and resistance remains a major challenge. A growing body of evidence suggests that tumor microenvironment (TME) plays a central rol

Ganglioneuroma of the sacral canal: an unusual location with MR imaging findings.

Ganglioneuroma is a rare, benign, slow-growing tumor arising from embryonic neural crest cells that differentiate into the sympathetic ganglia. Accordingly, it can occur anywhere along the sympathetic chain, from the clivus to the sacrum, although it is seen most commonly in the mediastinum, retroperitoneum, adrenal glands, and neck. Although involvement at the sacral level is uncommon, ganglioneuroma typically arises from the anterior surface of the sacrum, with presacral lesions occasionally extending through the sacral foramina into the sacral canal. Only two cases of ganglioneuroma confine

Case Report: Solitary fibrous tumor of the pelvis associated with Doege-Potter syndrome.

INTRODUCTION: Solitary fibrous tumor (SFT) is an uncommon mesenchymal spindle cell neoplasm (incidence ~1.4 per million). Although most SFT occur in the thoracic cavity, they can also arise in the retroperitoneum, pelvis, head, and neck. While the majority exhibit benign behavior, approximately 5% are associated with paraneoplastic syndromes, most notably Doege-Potter syndrome (DPS). DPS is characterized by non-islet cell tumor hypoglycemia (NICTH) resulting from tumor hypersecretion of high-molecular-weight insulin-like growth factor 2 (IGF2), which saturates insulin and IGF receptors causing

Awareness of Partial Loss is Essential in Interpreting Merlin Immunohistochemistry When Diagnosing Pleural Mesothelioma.

Ancillary assays, including BAP1 (BRCA-1-associated protein 1), MTAP (methylthioadenosine phosphorylase), and Merlin immunohistochemistry (IHC), are effective in discriminating pleural mesothelioma (PM) from reactive mesothelial proliferations (RMP). IHC of Merlin, a protein product of the neurofibromin 2 gene, was most recently developed. Our previous study suggested uneven occurrence of Merlin loss within PM tissues. This was further investigated here in a total of 41 tissues of PM obtained from seven institutions. Partial or diffuse Merlin loss was detected with excellent interobserver agre

Firmonertinib in advanced non-small cell lung cancer with epidermal growth factor receptor P-loop and αC-helix compressing mutations.

P-loop and αC-helix compressing (PACC) mutations comprise a structurally distinct subgroup of epidermal growth factor receptor (EGFR) mutations in non-small cell lung cancer (NSCLC) with limited treatment options. Firmonertinib is a once-daily oral, brain-penetrant, mutant-selective EGFR tyrosine kinase inhibitor with a unique trifluoroethoxy side chain. Firmonertinib had high anti-proliferative efficacy in EGFR PACC-mutant cells and xenografts. In the phase 1b FURTHER study (NCT05364073), patients with metastatic EGFR PACC-mutant NSCLC randomized to receive first-line firmonertinib 160 mg (n

Contributions of smoking behaviour to lung cancer subtypes: A two-sample Mendelian randomization study.

Smoking is an established cause of lung cancer (LC), but the relative causal contributions of whether a person ever smokes and how heavily they smoke across histological subtypes remain uncertain. This study investigated the causal effects of smoking status and smoking intensity on major LC subtypes. Two-sample Mendelian randomization (MR) was performed. Genetic instruments were drawn from the GWAS and Sequencing Consortium of Alcohol and Nicotine Use: 85 variants for smoking status, a binary trait contrasting ever with never smokers, and 23 variants for smoking intensity, a continuous trait m

Anatomic Variations of the Upper Extremity: Effects on Ultrasound-Guided Peripheral Nerve Blockade.

Peripheral nerve blockade provides many advantages in the perioperative period for surgery on the shoulder, elbow, wrist, or hand. Aberrant anatomic structures or anomalous courses of nerves may create challenging conditions for use of ultrasound to accurately guide needles for nerve blocks. This paper presents a review of such anomalies affecting the conduct of various peripheral nerve blocks in the upper extremity. A comprehensive literature review was performed, utilizing the PubMed and Embase databases. Variations of anatomy of the brachial plexus and associated structures are described, a

[IgG4-related lesion of the retroperitoneum, ureter, and kidney, mimicking infiltrating endometriosis].

Urinary tract endometriosis is rare, and ureteral involvement is particularly insidious due to its asymptomatic nature. The absence of symptoms can lead to ureteral stricture, hydronephrosis, chronic kidney disease up to end-stage renal failure, and irreversible loss of kidney function. Diagnosis requires imaging methods to assess the degree of obstruction, while treatment involves surgical release of the ureter from endometriotic infiltrates to preserve organ function. Our observation illustrates the difficulty of establishing the etiology of a retroperitoneal tumor-like infiltrate leading to

Beyond the pleura: National trends in mesothelioma mortality by anatomical site in Spain, 1999-2024.

OBJECTIVES: Mesothelioma mortality continues to reflect asbestos exposure because of its long latency. While previous Spanish studies focused primarily on pleural disease, this study assessed mesothelioma mortality across the ICD-10 C45 category in Spain from 1999 to 2024. MATERIALS AND METHODS: A population-based time-trend study was conducted using mortality data (ICD-10 C45) from the Spanish National Statistics Institute. Age-standardised mortality rates (ASMRs) were calculated using the 2013 European Standard Population. Temporal trends and sex-specific parallelism were evaluated using Jo

What happens in vagus.

The parasympathetic nervous system is phylogenetically ancient. In humans, the vagus nerve is the brain's only pathway for bidirectional parasympathetic signaling with the thorax, abdomen, and upper pelvis. Unsurprisingly, the vagus nerve has played a significant role in scientific discovery and therapeutic applications, attracting the attention of Galen, Darwin, and Pavlov, among others. For example, Otto Loewi used the vagus nerve to gather data that confirmed Santiago Ramón y Cajal's Neuron Doctrine. This intensive focus has led to a number of clinical advances related to vagus nerve functi

A Case of Primary Leiomyoma of the Chest Wall Showing High Uptake on FDG-PET.

INTRODUCTION: Primary leiomyoma of the chest wall is a rare entity. We report a case of primary chest wall leiomyoma with intense uptake on fluorodeoxyglucose PET (FDG-PET), requiring differentiation from malignancy. CASE PRESENTATION: An 82-year-old man with a history of asbestos exposure underwent annual chest CT surveillance. A 19-mm nodule was newly identified on the right chest wall and had not been present on CT the previous year. Although the lesion had well-defined margins on CT, FDG-PET demonstrated intense uptake with a maximum standardized uptake value of 11.8, raising suspicion fo

Primary borderline retroperitoneal mucinous cystic neoplasm and situs inversus totalis: a case report.

Primary retroperitoneal mucinous cystic neoplasms (PRMCNs) are rare tumors classified into cystadenoma, PRMCN with borderline malignancy (PRMCN-BM), and cystadenocarcinoma. We report a 60-year-old woman with left flank abdominal pain whose MRI revealed situs inversus totalis alongside a complex cystic lesion in the left retroperitoneum measuring 4.8 × 5.2 × 4.5 cm. She underwent laparotomy with complete resection of the lesion, and histopathology confirmed a primary retroperitoneal neoplasm with borderline malignancy, with an uneventful postoperative course and no recurrence after 8 months.
