Pan-cancer prognostic value of TGFBR1: Bioinformatics and experimental validation with a focus on gastric cancer.

TGF-β receptor 1 (TGFBR1) is a key component of TGF-β signaling and has been implicated in tumor progression. However, its expression patterns, prognostic relevance and associated molecular contexts vary among malignancies and remain incompletely characterized within a consistent pan-cancer framework. In the present study, TGFBR1 expression, clinicopathological associations, survival outcomes, immune and stromal infiltration, genomic features, co-expression profiles and transcriptional programs were systematically evaluated using The Cancer Genome Atlas (TCGA), TCGA-Genotype-Tissue Expression

FANCD2 knockdown suppresses malignant phenotypes and is associated with caspase/PARP-mediated apoptosis in NCI-H2052 pleural mesothelioma cells.

BACKGROUND: Malignant pleural mesothelioma (PM) is a highly aggressive thoracic malignancy with an extremely poor prognosis and limited effective therapeutic strategies, representing a critical unmet clinical need. Fanconi anemia complementation group D2 (FANCD2) is a critical effector of the cellular DNA damage response system and plays a central role in maintaining genomic integrity. While the genoprotective functions of FANCD2 have been extensively characterized, its specific biological roles and molecular regulatory mechanisms in the malignant progression of PM remain poorly understood. Primary malignant mesothelioma of the tunica vaginalis presenting as hydrocele: imaging pitfalls.

A man in his late 70s presented with progressive, painless scrotal swelling. The initial ultrasound was reported as a simple hydrocele, and he was scheduled for surgery. At operation, friable nodules on the tunica vaginalis were biopsied, confirming malignant epithelioid mesothelioma. Staging CT showed no pleural or peritoneal disease, supporting a primary tunica vaginalis origin. As the tunica vaginalis is an embryological continuation of the peritoneum, it is exceptionally rare for mesothelioma to arise here as a true primary rather than as a secondary spread from the peritoneum. This case h

Case Report: Observational use of mycophenolate mofetil in probable steroid-unresponsive immune checkpoint inhibitor-associated transverse myelitis.

Immune checkpoint inhibitor (ICI)-associated transverse myelitis (TM) is a rare and severe neurological immune-related adverse event (n-irAE) with an incidence of < 0.01%, and the clinical management of steroid-unresponsive cases remains extremely challenging. This article reports the case of a 37-year-old male patient with biphasic pleural mesothelioma who developed ICI-associated TM after treatment with nivolumab combined with ipilimumab. Initial treatment with high-dose glucocorticoids plus intravenous immunoglobulin (IVIg) was ineffective. After mycophenolate mofetil (MMF) was added in com

Endothelial neo-antigen expression initiates CD8⁺ T cell-driven CNS vasculitis amplified by peripheral immune challenge.

Primary central nervous system (CNS) vasculitis is a rare and devastating inflammatory cerebrovascular disease for which pathogenic mechanisms remain poorly defined, largely due to limited access to human tissue and the absence of validated experimental models. Here, we present a mouse model of CNS-restricted vasculitis based on inducible, endothelial-biased neoantigen expression, termed brain endothelial ovalbumin (BrEndO). In BrEndO mice, tamoxifen-induced ovalbumin (OVA) expression in cerebral endothelial cells (ECs) elicited a controlled endogenous CD8+ T cell response at the blood-brain b

Environmental Asbestos Exposure as a Possible Pathway to Early-Onset Pleural Mesothelioma: A Case From Afghanistan and Pakistan to London.

Malignant pleural mesothelioma (MPM) is strongly associated with occupational asbestos exposure and typically presents decades after exposure, in older men. The association of MPM with environmental exposure and genetic susceptibility is less recognised. We report a case of a 40-year-old man presenting in London with dyspnoea, weight loss and a persistent right-sided pleural effusion following hospitalisation for pneumonia. Imaging showed circumferential nodular thickening of the right pleura with lymph node involvement, and histopathology was in keeping with epithelioid-type MPM. With no know

Impact of interventions to prevent asbestos-related respiratory disease in an exposed worker registry using a simplified G-computation.

The Ontario Asbestos Workers Registry is a regulatory exposure registry obligating employers to report the number of work hours with asbestos-containing materials for each of their workers. Currently, each worker is notified of the need for a medical examination once they have accrued 2000 reported hours of work with asbestos. We sought to evaluate the impact on disease prevention of alternative policies limiting asbestos work hours among registry participants. A cohort of 26 164 asbestos workers were followed for cancer and nonmalignant disease diagnoses between 1986 and 2019. Analyses of the

Pharmacogenomic Characterization of a Large Cohort of Patient-Derived Cell Lines Identifies Therapeutic Strategies for Pleural Mesothelioma.

UNLABELLED: Pleural mesothelioma (PM) urgently requires effective treatments. This study aimed to identify potential therapies using a drug repurposing strategy in the context of the molecular heterogeneity of PM. We performed a multiomics study of a large cohort of patient-derived primary PM cell lines (n = 58) and conducted a multistep pharmacologic study starting with a large-scale drug screen with 1,327 compounds using 11 cell lines to select drugs of interest. Integrated multiomics analysis demonstrated that the molecular landscape of the cell line cohort recapitulates the main findings i

Intratumour, anatomical and temporal heterogeneity in mesothelioma.

BACKGROUND: Pleural mesothelioma (PM) is characterised by marked heterogeneity, both clinically in terms of survival and response to treatment, and in terms of histology and molecular status. Development of novel therapies, stratified treatment pathways and a better understanding of resistance mechanisms are urgently needed. This requires an in-depth understanding of the multiple sources of heterogeneity affecting tumour cells and the tumour microenvironment. METHODS AND RESULTS: This review, which synthesises the key studies available in the literature, provides a detailed description of the

Targeting uPARAP with an Antibody-Drug Conjugate Exhibits Efficacy against Mesothelioma and Synergizes with Cisplatin.

UNLABELLED: Urokinase plasminogen activator receptor-associated protein (uPARAP) is a collagen-internalizing receptor with emerging relevance as a therapeutic target. The involvement of uPARAP in extracellular matrix turnover and stromal remodeling within the tumor microenvironment (TME) makes it an especially promising target in tumors with a high stromal component. Mesothelioma, characterized by its unique complex TME, is a highly refractory cancer type for which no effective targeted therapy exists. uPARAP has been shown to be expressed across all subtypes of mesothelioma, especially in the

Influence of Subcellular Localization on the Cytotoxicity of Targeted α-Therapy.

Targeted α-therapy (TAT) is a promising approach for radiopharmaceutical therapy with various molecular targets, but uncertainty exists as to whether the subcellular location of a target influences its cytotoxicity. We used a model system that provides a subcellular "zip code" for TAT to determine whether localizing α-emitters closer to the DNA increases their cytotoxic effect. Methods: Human pleural mesothelioma (I45) and ovarian adenocarcinoma (SKOV3) cell lines were engineered to express a fusion Escherichia coli dihydrofolate reductase-yellow fluorescent protein localized to the DNA, nucle

Ascorbic Acid Analog 6-Deoxy-6-18F-Fluoro-l-Ascorbic Acid PET Imaging of 23 Various Cancer Types.

High-dose ascorbic acid (AA) has shown promise as an adjunctive treatment for various cancers because of its prooxidant cytotoxicity at high concentrations and its multitargeting effects. However, identifying patients who may benefit from AA treatment remains an unmet need, as the therapeutic efficacy of AA is controversial. In this study, we aimed to use an AA analog-6-deoxy-6-18F-fluoro-l-AA (18F-FAA)-as a PET tracer and evaluate its uptake in various cancers, exploring its potential clinical application. Methods: This prospective study enrolled 116 patients with histopathologically confirme

Journal of Thoracic Oncology 20th Anniversary Series Surgery.

In the last 20 years, major changes have occurred in the diagnosis and management of thoracic oncologic pathology. Furthermore, thoracic surgery has undergone considerable changes since the inception of the Journal of Thoracic Oncology in 2006. Regarding staging of lung cancer, pleural mesothelioma, and thymic epithelial tumors, several revisions were implemented in the TNM classification. The recently published ninth edition represents a tremendous effort of the Staging and Prognostic Factors Group of the International Association for the Study of Lung Cancer to analyze worldwide survival dat

Interaction between ENPP1 and homologous recombination deficiency defines distinct pan-cancer signatures: A retrospective observational study.

Ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1), 1st identified in breast cancer and subsequently in multiple other cancer types, is an innate immune checkpoint regulator that recently emerged as a promising biomarker and therapeutic target. Homologous recombination deficiency (HRD) has gained clinical relevance with therapeutic vulnerability, particularly in breast and ovarian cancers. Despite the increasing significance of ENPP1 and HRD in cancer biology and treatment, their potential relationships have not yet been comprehensively investigated. We analyzed the relationship betwee

Cross-Cancer Perspectives on CD40 Agonist Therapy: Modulating the Tumor Immune Microenvironment.

The CD40 pathway has become one of the most promising immunologic axes in oncology. As a central intermediary between innate and adaptive immunity, CD40 activation engages dendritic cells (DCs), amplifies antigen presentation, and stimulates potent cytotoxic T-cell (CTL) activity. Therapeutic CD40 agonists have been studied in a broad range of cancers over the past decade to reconfigure the tumor's immune microenvironment (TIME) and overcome immune refractoriness to standard immunotherapies. Clinical development has been most significant in pancreatic cancer, an exemplar cold tumor resistant t
