Imaging Feature Analysis of HHV8-Associated Multicentric Castleman Disease: A Comparative Study With HIV-Related Diseases and Idiopathic MCD.

OBJECTIVE: The purpose of this study was to evaluate differences in the imaging features of human immunodeficiency virus (HIV)-associated Kaposi sarcoma (KS), HIV-associated lymphoma (HAL), and human herpesvirus 8-associated multicentric Castleman disease (hMCD) and clarify the differences between idiopathic MCD-not otherwise specified (iMCD-NOS) and hMCD. METHODS: This single-center retrospective study included 39 episodes of KS, 13 of HAL, 11 of hMCD, and 25 of iMCD-NOS. Pulmonary lesions were divided into 5 patterns: pulmonary nodules/masses; perihilar opacity; centrilobular nodules with o

Association between KSHV genotype and immune response to Kaposi sarcoma.

Kaposi sarcoma (KS) is a locally aggressive endothelial tumor that can be classified into four clinical types: classic, AIDS-related, iatrogenic, and endemic. KS usually affects the skin and soft tissues, but it can develop into mucosal, lymph node, and multiple-organ lesions. KS is caused by Kaposi sarcoma-associated herpes virus (KSHV), which has six genotypes (A, B, C, D, E, and F). However, it remains unclear whether the KSHV genotype affects clinical presentation. In this study, we investigated clinical stage, cellular proliferation, NF-κB activity, and lymphocyte infiltration around the

Gastroduodenal Kaposi Sarcoma Without Cutaneous Lesions in an Immunosuppressed Patient.

Kaposi sarcoma (KS) is an angioproliferative neoplasm that develops as a result of infection with human herpesvirus 8 (HHV-8). The disease most commonly appears as painless, nonpruritic, violaceous papules, plaques, or nodules on the skin, but it can also affect lymph nodes, mucosal surfaces, and internal organs. A 53-year-old man from Guinea-Bissau with membranous nephropathy who was being treated with prednisolone and cyclophosphamide presented with abdominal pain, early satiety, anorexia, and fever. His symptoms emerged while he was taking antibiotics for a recent respiratory tract infectio

Extracavitary Primary Effusion Lymphoma: Diagnostic Challenges and Differential Diagnosis in a Resource-Constrained South African Setting.

AimsPrimary effusion lymphoma (PEL) is an aggressive, human herpesvirus 8 (HHV8)-associated large B-cell lymphoma characterised by malignant effusions in serous cavities (classic PEL). However, a subset of tumours may present exclusively as solid masses, referred to as extracavitary primary effusion lymphoma (EC-PEL). Despite the high regional prevalence of human immunodeficiency virus (HIV) and Kaposi sarcoma-associated virus /HHV8, PEL is likely under-reported in Sub-Saharan Africa. This study reports on the clinicopathologic features and differential diagnoses of EC-PEL.MethodsA retrospecti

Primary nasopharyngeal Kaposi's sarcoma without cutaneous manifestations: a case report.

BACKGROUND: Kaposi's sarcoma (KS) is a neoplastic lymphovascular disorder. It typically appears on the skin of the upper and lower extremities, but can also occur on the mucosal surfaces of the head and neck. The most common site reported for head and neck injuries is within the oral cavity, particularly on the palate, and other mucosal sites, such as the nasopharyngeal area. Moreover, involvement of the nasopharynx without skin involvement is even rarer. CASE PRESENTATION: Here, we present a rare case of HIV associated/epidemic KS in a 30-year-old man of African origin, where the primary man

An additional dermoscopic clue in basal cell carcinoma of the lower limb: the purpura-like pattern.

BACKGROUND: Dermoscopy is a critical tool for diagnosing basal cell carcinoma (BCC) and differentiating its histopathological subtypes. However, BCC on the lower limbs may present with atypical dermoscopic features, making its diagnosis challenging. OBJECTIVES: To identify specific dermoscopic patterns of BCC on the lower limbs, focusing on the newly observed purpura-like pattern, and to assess how these features differ from BCCs in other anatomical sites. METHODS: A retrospective, single-centre study reviewed standardized polarized dermoscopic images of 478 BCCs collected between January 20

Secondary changes in the peripheral liver due to kaposiform haemangioendothelioma in the hepatic hilum: a report of two transplant cases.

AIMS: Kaposiform haemangioendothelioma (KHE) is a rare vascular neoplasm of infancy and childhood that typically affects the soft tissues of the extremities; however, a variety of visceral sites have been reported. When KHE is located within the liver hilum, it may involve hilar structures and lead to secondary changes in the hepatic parenchyma. We report the histological changes in the explant livers of two patients transplanted for liver failure secondary to clinically unsuspected KHE at the hepatic hilum. METHODS AND RESULTS: Patient A initially presented with the signs and symptoms of liv

Challenges for the effectiveness of infectious disease interventions during global change.

BACKGROUND: Sequential treatment of ankylosing spondylitis (AS) that includes tumour necrosis factor-α antagonists (anti-TNF agents) has been applied in most of the Western countries. Existing cost-effectiveness (CE) models almost exclusively presented the incremental CE of anti-TNF agents using a closed cohort while budget impact studies are mainly lacking. Notwithstanding, information on impact on total population health and societal budget as well as on actual incremental CE for a given decision time span are important for decision makers. This study aimed at quantifying, for different deci

A Multimodal Radiation Approach to a Complex Case of Kaposi Sarcoma: Successful Treatment and Long-Term Follow-Up.

Kaposi sarcoma (KS) is an angiogenic tumor frequently seen in HIV-infected patients that involves skin, mucosa, and splanchnic organs. Here, we describe the case of a 57-year-old man with a history of HIV/AIDS on antiretroviral therapy (ART) who presented with KS of the lower extremity. The patient exhibited multiple exophytic lesions circumferentially involving his left leg, causing discomfort and difficulty in activities of daily living. External beam radiation therapy (EBRT) was pursued in lieu of amputation in order to achieve locoregional control while preserving ambulatory function. Due

Oncogenic Viruses in Organ Transplantation: Implications of Virus-Host Interactions for Cancer Development.

Organ transplantation significantly enhances the survival and quality of life for recipients. However, multiple dependent and independent variables can adversely affect life expectancy after transplantation. Cancer is one of the most common causes of morbidity and mortality for long-term organ transplant recipients. The incidence of cancer in transplanted tissues can be twice as high in approximately 32 distinct cancer types. Oncogenic viruses present in graft tissues may contribute to the etiology of various cancers in transplant recipients. Such oncogenic viruses include hepatitis viruses, p

A Unique Case of Metastatic Oral Kaposi Sarcoma in an HIV-Negative Patient.

Kaposi's sarcoma (KS) is a tumor involving blood vessels and lymphatic tissue. It is caused by human herpes virus-8, typically in HIV infection in individuals with AIDS. There are 4 major types of KS including classic, endemic, immunosuppression, and AIDS-related. Endemic KS is unique among the different types as it is associated with patients with HIV-seronegative and can present either as an indolent tumor or as a fast-growing mass. The authors discuss a unique presentation of metastatic, endemic KS in a patient with HIV-negative, non-immunosuppression, which has yet to be described in the U

Relative Tumor Density of Soft-Tissue Sarcoma in Korean Population: An Institutional Review.

BACKGROUND: Comprehensive studies on the tumor burden of soft-tissue sarcoma (STS) by anatomical site are lacking in Asian populations. OBJECTIVE: To investigate the anatomical distribution of STS via relative tumor density (RTD) in a Korean cohort. METHODS: The RTDs of patients with STS at a single-institution from 2007-2022 were retrospectively analyzed. To describe the STS locations, the body was divided into 4 anatomical sites, and the RTD of each was calculated to the compare topographic tumor burden. RESULTS: Fifty-nine cases in 58 individuals, 35 male (60.3%) and 23 female (39.7%), w

Structure of the Kaposi's sarcoma-associated herpesvirus gB in post-fusion conformation.

Discovered in 1994 in lesions of an AIDS patient, Kaposi's sarcoma-associated herpesvirus (KSHV) is a member of the gammaherpesvirus subfamily of the Herpesviridae family, which contains a total of nine that infect humans. These viruses all contain a large envelope glycoprotein, glycoprotein B (gB), that is required for viral fusion with host cell membrane to initial infection. Although the atomic structures of five other human herpesviruses in their postfusion conformation and one in its prefusion conformation are known, the atomic structure of KSHV gB has not been reported. Here, we report t

Electrochemotherapy for cutaneous Kaposi Sarcoma: A European register-based cohort study from the International Network for Sharing Practices of electrochemotherapy (InspECT).

INTRODUCTION: Kaposi sarcoma (KS) is a cancer of endothelial cells involving the skin and visceral organs. Electrochemotherapy (ECT) is an alternative treatment option for patients with cutaneous malignancies of skin and non-skin origin not suitable for conventional treatments. ECT exerts anti-tumour activity through enhanced drug delivery to tumour cells and cytotoxicity, inducing immunogenic tumour cell death and through a vascular-disrupting effect. MATERIALS AND METHODS: In this multi-institutional prospective, observational study, we aimed to evaluate the outcomes of using ECT as a treat
