Classic Kaposi's sarcoma is primarily a skin disease of the lower extremities affecting predominantly elderly men of Mediterranean origin. We report classic Kaposi's sarcoma first presenting in the third decade in two heterosexual, HIV-negative, males of Greek origin from Albania. Ten years after onset, the disease became aggressive with unusual clinical features that included exophytic tumors, extensive lesions on the hands as well as the legs, and prominent leg edema. One of the patients also presented lesions on the face, trunk and palate, and bubonic lymphadenopathy. In both cases, CD4 cou

The epidemic form of Kaposi's sarcoma is associated with human immunodeficiency virus infection. Cutaneous and mucosal manifestations are frequently reported in the ENT sphere, mostly involving the oral cavity. The external and middle ear are only rarely concerned with only one case of a mastoid lesion without extension to the external auditory canal (EAC) being reported to this day. The present article describes the first case of involvement of the EAC with extension to adjacent structures. This patient presented other Kaposi lesions and had been treated by systemic hormonal therapy. Thereaft

Kaposi's sarcoma (KS) is a neoplastic vascular disorder characterized by multiple reddish blue nodules which, according to the original description (Kaposi, 1872), usually present on the skin of the upper and lower extremities. It is not until the disease progresses that nodules appear on the skin of the head and neck, on mucosal membranes and as visceral lesions. However, with the large increase in the incidence of this tumour associated with the acquired immune deficiency syndrome (AIDS) epidemic, there have been increasing numbers of cases of Kaposi's sarcoma presenting primarily on the ski

This paper describes a case report of an HIV-infected patient with mucocutaneous Kaposi's sarcoma (KS) with oral involvement, which presented complete clinical resolution of lesions on antiretroviral treatment with ritonavir, an HIV-1 protease inhibitor. Although it has still not been demonstrated that ritonavir has a specific antiviral action against HHV-8, a gamma herpesvirus probably involved in KS aetiopathogenesis, it has been proven that it reduces the HIV load significantly. This affects certain growth factors of KS, such as Tat protein and cytokines, and favours recovery of immune func

The life span of HIV-infected patients has increased because of anti-retroviral therapy and improved means for treating opportunistic infections. However, HIV infection and subsequent immunosuppression result in an increased risk of high-grade B-cell lymphomas. The purpose of this study was to determine the incidence, localization, clinical features, therapy and survival time of patients with HIV-related lymphomas of the head and neck. Between 1988 and 1995, 280 HIV-infected patients presented with otolaryngologic symptoms. Seventy-two (25.7%) were found to have a neoplastic disease. Fifty-six

PURPOSE: External beam radiotherapy for Kaposi sarcoma of the palate in HIV positive patients has often severe acute side-effects. As alternative treatment, a high dose Microselectron brachytherapy technique was developed.
METHODS AND MATERIAL: A customised dental plate with afterloading tubes was made; from outlines, three dimensional coordinates of source positions and surface points were input to a Target 2 treatment planning computer. For the best dose-distribution on the mould surface, weighting of each source dwell position was adjusted, a plan produced, and a test run with TLD measurem

OBJECTIVE: This study reports the main clinical and histological features, and the treatment and outcomes of six HIV-associated oral lymphomas.
SUBJECTS: During the period 1991-1995, six cases of lymphoma presenting in the oral cavity were diagnosed in this clinic. All six patients were men ranging in age from 36 to 62 years with CD4 counts below 100 cells mm-3.
RESULTS: In five patients, oral lymphoma was the first AIDS defining disease. Four lesions occurred on the gingiva, one on the palate and one on the tongue. The duration of symptoms prior to presentation ranged from 2 weeks to 14 mon

A retrospective study was performed to assess the efficacy and low mucosal toxicity of intracavitary contact X-ray therapy (ICRT), a proposed treatment of small/medium sized lesions of oral HIV-associated Kaposi's sarcoma (HIV-KS). Twenty-six patients with histologically confirmed oral HIV-KS underwent ICRT in the period 1986-1995. No patient received antiblastic or interferon therapy during the radiotherapy or follow-up periods. ICRT was performed according to the usual technical modalities of contact X-ray therapy, but the end of the source of ionizing radiations was introduced into the oral

Recently, a new human herpesvirus (KSHV/HHV-8) has been identified in classic, transplant, endemic, and AIDS Kaposi's sarcoma that may be involved in the pathogenesis of Kaposi's sarcoma. The purpose of this study was to evaluate oral AIDS-Kaposi's sarcoma for detection of KSHV/HHV-8 DNA. DNA extracted from 54 oral AIDS-Kaposi's sarcoma lesions (47 initial, 7 postvinblastine treated), 5 non-Kaposi's sarcoma HIV-positive lesions, and 3 non-Kaposi's sarcoma HIV-negative lesions was evaluated by polymerase chain reaction (KS330(233bp)amplicon) for KSHV/HHV-8. The AIDS-Kaposi's sarcoma study popul

Hypoalbuminaemia and diarrhoea are common complications of HIV infection and substantial causes of morbidity, but the specific intestinal pathologies that cause enteric protein loss have not been clearly defined. Two hundred and twenty stool samples from patients with a variety of HIV related conditions were analysed for faecal alpha 1 antitrypsin. Patients with intestinal Kaposi's sarcoma had a significantly raised faecal alpha 1 antitrypsin value and hypoalbuminaemia. A faecal alpha 1 antitrypsin value of greater than 0.3 mg/g wet stool has a sensitivity of 94% and a specificity of 76% for t

While most cases of intraoral cancer arise in the cancer-prone horseshoe comprised of the floor of the mouth, the posterolateral borders and ventral surface of the tongue, and the retromolar region, no intraoral site is immune. This article reviews the clinical features of squamous cell carcinoma of the gingiva, buccal mucosa and palate, as well as the less common oral malignancies that can arise at these intraoral sites.

PURPOSE: This study evaluated the use of intralesional vinblastine injections for the treatment of the intraoral lesions of Kaposi's sarcoma.
PATIENTS AND METHODS: Eighteen patients were treated with up to three intralesional injections of 0.1 mg/cc vinblastine.
RESULTS: All lesions responded to the local injections; 40% required one injection; 31%, two injections; and 29%, three injections. Large, exophytic lesions usually required the multiple injections. No complications were encountered, and the patients tolerated the protocol well. Ten patients were followed for 24 months. Eight died of

We realized a clinical study in 65 patients with acquired immunodeficiency syndrome, aged from 3 to 62 years old (mean, 34 years) (34 males and 31 females) interned or showed in consultations at the hospital Gabriel Touré in Bamako, Mali (departments of ENT diseases and of internal medicine), in order to analyse and to state precisely different otorhinolaryngologic manifestations in AIDS. Oropharyngeal candidiasis was the most prevalent otorhinolaryngologic manifestation (57 %). Other less common lesions were suppurative otitis media (29 %), atrophic pharyngitis (18,5 %), sinusitis (11 %), Kap

The purpose of this clinical study was to determine the effect of intralesional vinblastine administration on intraoral Kaposi's sarcoma (KS) in AIDS patients. One hundred and forty-four KS lesions in 50 HIV-positive homosexual males (mean CD4 count 64/mm3) were treated periodically with intralesional vinblastine injection (0.1 mg/cm2) until lesion resolution or no further reduction in lesional area. The most common lesion sites were: palate 56% (hard palate 42%, soft palate 14%); gingiva 22% (maxillary 15%, mandibular 7%); and maxillary tuberosity 6%. The mean lesion area was 4.6 cm2 (range =

Seventeen male homosexual outpatients with limited but progressive AIDS-associated Kaposi's sarcoma (KS) of skin, lymph nodes, oral cavity, and gastrointestinum were consecutively enrolled in an open, nonrandomized, prospective study. Patients received low-dose IFN-alpha 2b (3 MIU t.i.w.) in combination with 250 mg ZDV b.i.d. After remission of KS, continuation of treatment as maintenance therapy was provided. Eleven (65%) of 17 patients had a complete (CR) or partial remission (PR) of KS lasting more than 24 months in four patients. Response to treatment was strongly dependent on pretreatment

BACKGROUND: Not every patient with Kaposi's sarcoma (KS) has acquired immunodeficiency syndrome (AIDS). The "classic" form of KS is rare, and is associated with an indolent course. It is very distinct from AIDS-associated KS in which oral involvement is common and may be the initial presenting feature of this disease. Two other types of KS are recognized, the African and renal transplant-associated, which also are rarely associated with oral involvement.
METHODS: We present the case of a 76-year-old man of Mediterranean ancestry who was found to have a biopsy-proven classical KS lesion of the

Syphilis in HIV-infected patients reveals some particular aspects which are clinically relevant and disturbing: it is more frequent and develops more quickly to late stages, namely neurosyphilis; this progression may happen even if classic therapy in the early stages has been undertaken; the diagnosis of neurosyphilis is rather complex, the serological tests being of no definite credit; high doses of intravenous penicillin should be administered for a long period. We report a case that exemplifies the aspects described above.
