We studied two cases of a recently recognized systemic lymphoproliferative disease with morphological features of Castleman's disease: multicentric giant lymph node hyperplasia. Both patients developed Kaposi's sarcoma and had laboratory evidence of immune abnormalities, including reversed T4 to T8 ratios. One patient's disease had a subacute course with fevers of unknown origin, mucocutaneous candidiasis, and progressive thrombocytopenia, while the other patient's disease had a fulminant course with anemia, thrombocytopenia, and splenic lymphoma. Both patients were elderly, and both died of c

Multicentric giant lymph node hyperplasia (MGLH) is a distinct lymphoproliferative disorder, which may terminate in malignant lymphoma. The clinical features and laboratory findings of seven cases are reported here. The histologic changes in lymph nodes were those of giant lymph node hyperplasia, plasma cell type. Immunoperoxidase staining showed intracytoplasmatic polyclonal immunoglobulins. Malignant lymphoma supervened in two cases, one of which was proved by autopsy; in the other there was transformation of a polyclonal gammopathy into a monoclonal one. Two of the patients also developed K

The authors present the results of a histopathological study on the lymph-nodes taken from 45 subjects suffering from either an AIDS or from a chronic adenopathy corresponding to the definition of AIDS related complex (ARC). The various aspects observed were classed as type I to type IV. The lymph-node modifications observed in the 29 patients with an ARC could be divided into three principle groups: an extensive follicular hyperplasia associated with other elementary lesions or type IA (25 lymph-nodes from 23 patients); changes resembling a multicentric Castleman syndrome or type IB (1 case);

An increased incidence of lymphoid neoplasias is associated with the states of immune deficiency, both congenital and acquired. Twenty-one cases of lymphoma in men at high risk for Acquired Immune Deficiency Syndrome (AIDS) were diagnosed in one community hospital in New York City within the last 2 years. The mean age of these patients was 39.6 years, 20 were homosexual, and 1 was an intravenous drug abuser. There were 3 Hodgkin's and 18 non-Hodgkin's lymphomas of various histologic types, but almost all of high-grade categories. The proportion of extranodal lymphomas, the involvement of the g

At endoscopy, a 30-year-old man with acquired immune deficiency syndrome (AIDS), Kaposi's sarcoma, diarrhea, and unexplained malabsorption showed erythematous macular duodenal lesions consistent with Whipple's disease by histology and electron microscopy. Symptoms did not respond to tetracycline. Subsequent cultures revealed systemic Mycobacterium avium (M. avium) infection. Tissue from this patient, from patients with Whipple's disease and from a macaque with M. avium were compared. All contained PAS-positive macrophages but M. avium could be distinguished by positive acid-fast stains and a d

By November 7, 1983, 24 cases of AIDS in the United Kingdom had been reported to the Communicable Disease Surveillance Centre. At the same time an increasing number of homosexual men with unexplained lymphadenopathy syndrome (LAS) have been seen in our department. Between December 1982 and July 1983, 14 homosexual men with LAS and 11 healthy homosexual men were studied. Patients with LAS had a high number of lifetime episodes of sexually transmitted diseases, a history of recent sexual activity in the United States (9 of 14), sexual contact with British AIDS patients or other persons with LAS

Eighty-nine lymph node biopsies were performed on 82 patients with lymphadenopathy, immunosuppression, and possible acquired immune deficiency syndrome. The 21 patients with diagnoses of lymphoma or Kaposi's sarcoma were older, had more sexual contacts, and were sexually active for more years than patients with benign diagnoses. Cytomegalovirus and Epstein-Barr viral titers were elevated in both groups but were not significantly different in the benign and malignant groups. Skin flora were cultured from lymph node tissue in 24.7% of the patients. Two patients (2.5%) had wound infections with t

In this second part, the dysimmunitary conditions which associate a polyclonal hypergammapathy and adenopathy are studied. The latter simulate a malignant lymphoma both clinically and histologically, hence the term sometimes used: "pseudo-lymphoma". Above all, these dysimmunitary states should be considered as real pretumoral states, and a malignant lymphoma or Kaposi sarcoma can arise at any time during the evolution of the illness. Finally, all these illnesses, even apart from the appearance of malignant tumors, have a severe evolution and death can occur due to consequences of immune disord

A total of 299 cases of Kaposi's sarcoma (KS) occurring in Zaire over a 21-year period have been reviewed in the Department of Pathology, Kinshasa University Hospital. Of these, 268 were localized in the skin (89,6%) with the majority (51.8%) in the lower limbs; 31 (10.4%) were located elsewhere, chiefly in the lymph nodes (71.0%). Adults were more severely affected (67.6%) than children (6.7%). The proportion of females (12.8%) was much less than that of males (79.6%) and the adjusted M:F ratio was 4.81:1. The highest frequency (24.7%) of KS was found in Equator Province. Only 1 KS-associated

The light-microscopical, ultrastructural and immunohistochemical findings in the first reported European case of ocular changes in the acquired immuno-deficiency syndrome (AIDS) are presented. The syndrome occurred in a 31-year-old homosexual man who died of Kaposi sarcoma in the skin and internal organs, cytomegalovirus (CMV) infection of the larynx, the lungs and the small intestine, and mycobacterial infection of the lungs, the lymph nodes and the spleen. A right-sided necrotizing retinitis was shown to be caused by CMV and was clinically and histopathologically similar to previously descri

Abdominal computed tomography (CT) was performed in 31 patients with Kaposi sarcoma (KS) related to acquired immunodeficiency syndrome (AIDS), three patients with classic KS, and 12 patients with the newly described lymphadenopathy syndrome (LNS). The frequency, distribution, and appearance of lymphadenopathy and splenomegaly were similar in the AIDS-related KS and LNS groups. Rectal and perirectal disease was identified in 86% of the homosexual men studied; rectal KS could not be distinguished from proctitis on CT criteria alone. No CT abnormalities were seen in patients with classic KS. The

This report describes two cases of multicentric Castleman's disease associated with Kaposi's sarcoma, and reviews the literature. With the recent delineation of a multicentric variant, Castleman's disease has evolved as a disease entity with a wide clinical spectrum ranging from localized, reversible disease as originally described by Castleman et al., to a generalized disease that is rapidly progressive and fatal. The latter represents the most severe form of the disease in which the associated immune derangement predisposes the patients to infections and Kaposi's sarcoma.

Ultrastructural changes (tubuloreticular structures and tube and ring shaped forms) previously described in patients with acquired immunodeficiency syndrome (AIDS) are described for the first time in the lymph nodes and circulating lymphocytes of patients with persistent lymphadenopathy. These observations support the view that the persistent lymphadenopathy syndrome and AIDS are caused by the same transmissible agent(s).

Kaposi's sarcoma developed in two lymphoma patients previously treated by chemotherapy and chemoirradiation. The histologic picture in the two cases showed a non-Hodgkin's lymphoma with multifocal epithelioid histiocytic reaction. Both patients had Hepatitis-B surface antigen and other infections due to herpes and streptococcus. The European Jewish origin of the two patients, the previous chemotherapy, and their infective state tend to support more the hypothesis of predisposing factors in the occurrence of Kaposi's sarcoma than a coincidental association.

Kaposi's sarcoma (KS) and angioimmunoblastic lymphadenopathy (AILD) are exceptionally rare neoplastic diseases, although the former has recently been more frequently reported among certain populations, and is lately a topic for extensive medical and investigational interest. This case is unique in that it describes an association between KS and AILD. Such an association raises the question of a common pathogenic mechanism. The authors believe that predisposition to each of both diseases may be related to acquired immune deficiency which in turn may be induced by specific viral infections.
