Malignancy in renal transplant recipients.

Immunosuppressed organ allograft recipients have a 3-4 fold increased risk of developing cancer, but the chance of developing certain malignancies is increased several hundredfold. With the exception of skin cancers, most of the common neoplasms seen in the general population are not increased in incidence in organ allograft recipients. Instead, there is a higher frequency of relatively rare tumors including lymphomas, Kaposi's sarcoma, other sarcomas, vulvar and perineal carcinomas, renal and hepatobiliary carcinomas. Tumors appear after a relatively short time post-transplantation. The earli

Malignancies in pediatric AIDS.

Patients with immunodeficiency disorders, including children infected with HIV type 1, are at increased risk to develop a malignancy. Although the exact incidence is not clear, an excess of non-Hodgkin's lymphomas, soft tissue tumors, and, in the case of adolescent girls, cervical carcinomas, has been reported in HIV-infected children. Kaposi's sarcoma is rare in children as compared with HIV-infected adults. To understand the pathogenesis of these disorders, one must take into account the multiple interactions between the immunodeficient host, the cytokine dysregulations, and the concurrent i

Development of lymphoid hyperplasia in transgenic mice expressing the HIV tat gene.

During HIV infection, individuals experience multiorgan disorders such as adenopathy, splenomegaly, and lung and brain diseases. There is an increasing body of evidence that the HIV trans-activating tat gene product possesses multiple activities. First, it can activate several cellular genes; second, in its extracellular soluble form, it plays the role of growth factor in some cells such as Kaposi's sarcoma cells. Thus, we introduced the HIV tat gene, under the control of the cellular proteolipoprotein promoter, into the germline of mice and demonstrate that, when expressed, the tat gene produ

Depressed immunity and the development of cancer.

Various disorders of immune competence are associated with increases in uncommon tumors, particularly non-Hodgkin's lymphomas (NHLs), which often exhibit unusual features: they are frequently extranodal, show a broad spectrum of lesions, ranging from benign polyclonal hyperplasia to frankly malignant monoclonal lymphomas, and are frequently localized to the brain. Of 7136 tumors in organ transplant recipients, the predominant lesions are NHLs, carcinomas of the skin and lips, carcinomas of the vulva/perineum, Kaposi's sarcoma (KS), and renal carcinomas. Skin cancers present unusual features: p

Neoplastic complications of transplantation.

Cancer incidence in transplant patients ranges from 4% to 18% (average, 6%). We present data on 7668 types of malignancy that occurred in 7192 patients. The predominant tumors are skin/lip carcinomas, lymphomas, vulvar/perineal carcinomas, in situ uterine cervical carcinomas, Kaposi's sarcoma (KS), renal carcinomas, hepatobiliary tumors, and soft tissue/visceral sarcomas. Tumors appear relatively early after transplantation. The earliest is KS, appearing an average of 22 months post-transplantation whereas the latest are vulva/perineal carcinomas, presenting an average of 113 months posttransp

Systemic expression of HIV-1 tat gene in transgenic mice induces endothelial proliferation and tumors of different histotypes.

The human immunodeficiency virus tat protein, a transactivator of viral and cellular genes, is suspected to be involved in the pathogenesis of acquired immunodeficiency syndrome-associated tumors. We report that transgenic mice carrying a recombinant DNA containing BK virus early region and the human immunodeficiency virus tat gene develop skin leiomyosarcomas, squamous cell papillomas and carcinomas, adenocarcinomas of skin adnexa, glands, and B-cell lymphomas. Although the incidence of hepatocellular carcinoma is low, most animals show a liver cell dysplasia of variable degree. These mice ar

AIDS-associated Kaposi's sarcoma in an Indian female.

We report the first case of AIDS-associated Kaposi's Sarcoma in a 35 year old Indian female, a commercial sex worker, with lesions involving skin and mucous membranes. Diagnosis was confirmed on histopathology, electron microscopy, seropositivity, (ELISA and Western Blot) as well as Pepti-LAV test and viral culture. Antibodies to human immunodeficiency virus, both HIV-1 & 2, were present. HLA DR5,7 was positive on oligotyping. Genetic predisposition, therefore, may be suspected.

Multiple pathogens may induce growth factor cascade resulting in KS.

While lipopolysaccharide endotoxin is the most prominent inducer of the kinecascade (TNF alpha, IL-1, 4, 6, 8) that leads to shock and multiple organ failure, bacterial exotoxins and products of certain gram positive bacteria can induce the same end results. We theorize that more than one pathogen can induce the sequence of protooncogene activation and growth factor release that results in the formation of KS. If KS has its own unique viral etiology, this virus has not as yet been isolated or identified but we continue to search for it. However, it is entirely possible that these lesions do no

[Kaposi's sarcoma in Tainan: report of five cases].

Kaposi's sarcoma (KS) is an uncommon cutaneous neoplasm with an increased incidence among Jews and Italians of Europe and North America and Negroes of equatory Africa. In recent years, several cases have been diagnosed in AIDS patients and in patients who have undergone organ transplantation or received immunosuppressant treatments. We have diagnosed five cases of KS over a 3-year period in our department. They include four men and one women, aged from 58 to 79 years, and who were born and had lived in Tainan. They all denied a history of homosexuality, multiple sexual partners, or drug abuse.

Surgical pathology of the liver in HIV infection.

In the 6-year period 1984-1989, 101 liver biopsies or 'needle necropsies' from human immunodeficiency virus positive patients were examined histologically. Of these, only nine showed no abnormality whatsoever. The commonest histological findings were either fatty change or changes related to co-existent chronic viral hepatitis. Granulomas were seen in 15 cases, four of which were positive for acid-fast bacilli. A range of organisms were recorded: cytomegalovirus (4); Histoplasma capsulatum (1); Pneumocystis carinii (2); Cryptococcus neoformans (1); and Leishmania donovani (1). There were two c

Cytomegalovirus vasculitis accompanied by an exuberant fibroblastic reaction in the intestine of an AIDS patient.

A case of cytomegalovirus (CMV) vasculitis in the intestine of a patient with acquired immune deficiency syndrome (AIDS) is reported. The distal jejunum and ileum had multiple well-demarcated mucosal ulcers and microscopic examination revealed an unusual, exuberant fibroblastic proliferation in the ulcer base. Amidst these fibroblasts, there were several small blood vessels of which the endothelial cells contained cytomegalic inclusion bodies. The lesion showed a Kaposi's sarcoma-like appearance, but spindle cells were negative in immunostaining for factor VIII-related antigen or Ulex europaeu

Extrapulmonary Pneumocystis carinii infections.

A case of disseminated infection with Pneumocystis carinii is presented, and the English-language literature is reviewed for cases of documented extrapulmonary infection with this organism. In this case--with P. carinii diffusely replacing the bone marrow and causing hepatic, adrenal, and glomerular tuft necrosis--the clinical illness and multiple-organ dysfunction attributed to disseminated P. carinii were more severe than had previously been described. Because the rate of extrapulmonary P. carinii infection found at autopsy in patients with AIDS is at least 2.5% at our institution, we cautio

Kaposi's sarcoma developed in a patient with a thymoma in the setting of excess numbers of CD8-positive cells in the peripheral blood.

We describe an autopsy case of Kaposi's sarcoma in a patient with an increased count of suppressor T lymphocytes. This case was distinct from the acquired immunodeficiency syndrome. First, it did not have any detectable viral antibody relating to the acquired immunodeficiency syndrome. Second, the patient's blood sample showed that the ratio of CD8+ cell (suppressor/cytotoxic T lymphocyte) to all lymphocyte population was increased to 0.61, as compared with the normal value of 0.12 to 0.16; however, absolute number of CD4+ T cell (helper/inducer T lymphocyte) was not decreased (1800/mm3). [Iatrogenic immunologic disorders and malignant tumors].

A severe or lengthy disturbance of immunity favors the development of malignant tumors. The increased incidence of lymphomas, leukemias and certain carcinomas in cases of congenital immunodeficiency, as well as of Kaposi's sarcoma and certain lymphomas in the acquired immunodeficiency syndrome (AIDS) are well known. In transplantees, patients undergoing immunosuppressive treatment for autoimmune disorders and cancer patients receiving chemotherapy, the occurrence of secondary neoplasias represents a phenomenon with a specific profile. We have seen 14 solid tumors in patients who were immunosup

Kaposi's sarcoma of lymph nodes associated with multicentric angiofollicular hyperplasia.

A case of multicentric angiofollicular hyperplasia (hyaline-vascular type) associated with Kaposi sarcoma of lymph nodes is reported. The patient was a 75-year-old man who suffered from edema, fever, maculopapular skin rashes and polyclonal hypergammaglo-bulinemia and died 10 days after admission to hospital for acute tubular necrosis and pulmonary edema. No other localizations of Kaposi's sarcoma were detected at autopsy; this is a very uncommon finding in Western countries and in adult people.

Culture-proved disseminated cat-scratch disease in acquired immunodeficiency syndrome.

A male homosexual (positive for the human immunodeficiency virus) with a recent cat scratch developed fever, epitrochlear and axillary lymphadenopathy, and retinitis. Subsequently, he developed skin (epitheloid hemangioma) and mucosal lesions (Kaposi's sarcoma), multiple liver abscesses, and pleural effusion. Warthin-Starry stains and/or electron micrographs of lymph nodes and skin lesions demonstrated bacilli characteristic of those associated with cat-scratch disease. Cultures of lymph node, pleural fluid, and liver abscess specimens yielded organisms believed to be the causative agent of ca
