Anatomic Variations of the Upper Extremity: Effects on Ultrasound-Guided Peripheral Nerve Blockade.

Peripheral nerve blockade provides many advantages in the perioperative period for surgery on the shoulder, elbow, wrist, or hand. Aberrant anatomic structures or anomalous courses of nerves may create challenging conditions for use of ultrasound to accurately guide needles for nerve blocks. This paper presents a review of such anomalies affecting the conduct of various peripheral nerve blocks in the upper extremity. A comprehensive literature review was performed, utilizing the PubMed and Embase databases. Variations of anatomy of the brachial plexus and associated structures are described, a

Applied surgical anatomical approach to pudendal nerve: Step-by-step key neurovascular structures for pelvic nerve surgery.

OBJECTIVE: To define and demonstrate a step-by-step surgical anatomical approach to pudendal nerve dissection using female cadavers, focusing on the identification of safe roadmap and key neurovascular structures to optimize pelvic nerve surgery. METHODS: A descriptive anatomical study was conducted on 60 hemipelvises from 30 female cadavers. Dissections were performed in three stages comprising nine procedural steps to expose the pudendal nerve and related pelvic nerves. Key neurovascular landmarks were documented using video recordings. RESULTS: The dissection was organized into three stag

What happens in vagus.

The parasympathetic nervous system is phylogenetically ancient. In humans, the vagus nerve is the brain's only pathway for bidirectional parasympathetic signaling with the thorax, abdomen, and upper pelvis. Unsurprisingly, the vagus nerve has played a significant role in scientific discovery and therapeutic applications, attracting the attention of Galen, Darwin, and Pavlov, among others. For example, Otto Loewi used the vagus nerve to gather data that confirmed Santiago Ramón y Cajal's Neuron Doctrine. This intensive focus has led to a number of clinical advances related to vagus nerve functi

Peripheral Nerve Stimulation for Chest Wall and Persistent Shoulder Pain After Reverse Total Shoulder Arthroplasty: A Case Report.

BACKGROUND: Persistent pain after reverse total shoulder arthroplasty (RTSA) warrants evaluation for surgical and infectious causes. Peripheral nerve stimulation (PNS) may be considered when these causes have been assessed, but evidence for its use in chest wall and shoulder pain after RTSA is limited. CASE REPORT: A 70-year-old man had persistent right anterior shoulder pain and new chest wall pain after revision RTSA. Diagnostic testing did not establish a surgical or infectious cause. A right pectoral nerve (PECS I) block provided temporary relief. A temporary PNS lead was placed near the

Biphasic Brachial Plexus Blockade Following Interscalene Administration of Liposomal Bupivacaine (Exparel): A Case Report.

This case report describes a 70-year-old male who experienced a biphasic blockade following an interscalene block with 10 mL of liposomal bupivacaine (Exparel) mixed with 5 mL of 0.25% bupivacaine for shoulder arthroscopy. The patient exhibited complete motor and almost complete sensory recovery by postoperative day (POD) 1, followed by a recurrence of motor and sensory deficits on POD2, which resolved spontaneously by POD3. To our knowledge, this represents one of the earliest documented recurrences of dense motor blockade after brachial plexus block with Exparel. Potential mechanisms, includ

Visceral branches of the inferior hypogastric plexus: a cadaveric and immunohistochemical study.

BACKGROUND: The inferior hypogastric plexus, also known as the pelvic plexus, represents an important autonomic structure of the lesser pelvis, providing sympathetic and parasympathetic innervation to the pelvic viscera. This study aimed to investigate the detailed morphology, topography, and neural composition of its branches and secondary plexuses, including the vesical, prostatic, middle rectal, and uterovaginal plexuses, with particular emphasis on their terminal visceral branches and their anatomical variability. MATERIALS AND METHODS: A total of 50 formalin-fixed human cadavers were exa

Thoracoabdominal nerves block through the perichondrial approach: a narrative review.

The thoracoabdominal nerves block using the perichondrial approach (TAPA) was first described by Tulgar et al. as a trunk nerve block technique designed to provide analgesia for abdominal surgery. Subsequently, the modified TAPA (M-TAPA) was proposed, in which a local anesthetic was injected only at the lower aspect of the costal cartilage. More recently, a re-modified TAPA was developed to achieve a more consistent blockade of the lateral cutaneous nerve branches. Despite increasing clinical adoption of the TAPA and its modifications, many important details remain insufficiently understood. I

Deltoid reanimation after axillary nerve injury: Comparing grafts and nerve transfers outcomes.

BACKGROUND: Loss of deltoid function following axillary nerve (AN) injury or upper brachial plexus injury (BPI) severely limits shoulder function. Both interposition nerve grafting and distal nerve transfer are established reconstructive strategies; however, comparative outcome data remain limited. METHODS: A retrospective single-center study included patients treated between 2006 and 2022 for isolated AN injury or C5-C6 BPI with deltoid paralysis. Exclusion criteria were graft repair performed >8 months after trauma, nerve gaps >7.5 cm, and nerve transfers to the posterior AN branch. Overall

Innovative intramedullary nail system with internally locking: A safe alternative fixation technique for humerus shaft fracture that reduces the risk of the iatrogenic radial nerve injury and surgical time.

PURPOSE: Humeral shaft fractures may be managed conservatively or operatively; however, increasing non-union and complications after non-operative treatment have renewed interest in intramedullary fixation. Innovative intramedullary nails with distal internally locking, requiring neither fluoroscopy nor external guidance, aim to reduce surgical time, simplify the procedure, and minimise the risk of iatrogenic radial nerve injury. This study evaluates the clinical and radiological outcomes of a humeral nail utilising distal internally locking. MATERIAL AND METHOD: A retrospective review includ

Nurturing a Sustainable Transplantation Journey: the best of ESOT Congress 2025.

BACKGROUND: Pudendal neuralgia (PN) is a chronic neuropathy that causes pain, numbness, and dysfunction in the pelvic region. The current state-of-the-art treatment is pulsed radiofrequency (PRF) in which a needle is supposed to be placed close to the pudendal nerve for neuromodulation. Given the effective range of PRF of 5 mm, the accuracy of needle placement is important. This study aimed to investigate the potential of augmented reality guidance for improving the accuracy of needle placement in pulsed radiofrequency treatment for pudendal neuralgia. METHODS: In this pilot study, eight subj

Integrated tear proteomics define the molecular blueprint of corneal epithelial repair.

Tears are easy to collect, repeatable, and reflect the state of the corneal surface-attributes that make them attractive for bedside monitoring after surgery or injury. We performed a cross-species meta-analysis of tear proteomes from patients undergoing photorefractive keratectomy (PRK) and from mice after mechanical epithelial abrasion to define molecular programs that are both conserved and clinically actionable. Roughly one-third of the injury response was shared between species, centering on innate immune activation (complement/acute phase), epithelial migration and cytoskeletal remodelin

Obturator-to-pelvic nerve rerouting to restore bladder function after radical pelvic nerve injury.

BACKGROUND: Damage to the pelvic nerve can occur at the time of a radical hysterectomy or during resection of a rectal cancer or sacrococcygeal teratoma and lead to a neurogenic bladder and an inability to volitionally void. We report on our experience with somatic-to-autonomic nerve regeneration by obturator-to-pelvic nerve rerouting and its results in terms of restoration of normal bladder function. METHODS: Eleven patients (2 male, 9 female; age range 4-58 years) with a neurogenic bladder following radical hysterectomy or resection of a sacrococcygeal teratoma underwent an obturator-to-pel

Comparison of the effectiveness of transcranial direct current stimulation and transcutaneous tibial nerve stimulation on the urgency and frequency of women with overactive bladder syndrome: A randomized controlled trial.

INTRODUCTION: Recent findings suggest that key areas in the brain, particularly the medial prefrontal cortex (mPFC), are important for regulating bladder function. This research explores the potential of transcranial direct current stimulation (tDCS) on the mPFC as an emerging approach to alleviate overactive bladder (OAB) symptoms, in comparison to transcutaneous tibial nerve stimulation (TTNS). METHODS: This single-blind, two-arm, parallel, randomized controlled clinical trial with a 1:1 allocation ratio was conducted at Firoozgar Hospital. A total of forty-four women diagnosed with overact

Parasympathetic nerves in the kidney pelvis contribute to blood pressure regulation.

INTRODUCTION: The existence of parasympathetic innervation in the kidney has long been controversial. Recent anatomical studies have provided evidence supporting such innervation by demonstrating the presence of acetylcholine (ACh) metabolic enzymes and tracing a brain-kidney vagal axis. Although these findings confirm the anatomical pathway, the functional capacity of these nerves to release neurotransmitters remains unverified, and the physiological role of this pathway is still unknown. METHODS: We defined the innervation of the kidney pelvis by co-labeling sympathetic, parasympathetic, an

Accuracy of blind approach and ultrasound-guided techniques for pudendal nerve block in cadaveric cats - a pilot study.

This blinded, randomized and prospective cadaveric pilot study compared the accuracy of a blind approach for pudendal nerve block (PNB–B) with that of the ultrasound–guided technique (PNB–US) in cats. Two male cat cadavers were used for a preliminary anatomical study to identify palpable bony landmarks, while eight additional cadavers were employed for the experimental phase. Each hemipelvis was randomly assigned to one of the two techniques: PNB–US, performed according to the deep dorsolateral method described by Adami et al. (J Feline Med Surg 15:901–907, 2013), or the PNB–B, developed based

Afferent innervation of the kidney: projections to and processing in the central nervous system.

FDA approval for catheter-based renal nerve ablation has sparked renewed interest in the role of renal nerves in hypertension and their potential contribution to other pathophysiologies. While the anti-hypertensive effects of catheter-based renal nerve ablation were thought to be due to the ablation of the sympathetic efferent nerves going to the kidney, preclinical hypertensive rodent studies and unexpected beneficial clinical outcomes have highlighted the renal afferent (sensory) nerves as a contributor to the pathogenesis of hypertension. Renal afferents are most abundant in the renal pelvi
