The Efficacy of Transcutaneous Tibial Nerve Stimulation as an Adjunct to Standard Rehabilitation for Postpartum Pelvic Floor Dyssynergic Defecation in Primiparous Women: A Randomized Controlled Trial.

INTRODUCTION AND HYPOTHESIS: Postpartum pelvic floor dyssynergic defecation (PFDD) is a common and distressing condition. We hypothesized that transcutaneous tibial nerve stimulation (TTNS) combined with standard rehabilitation would be superior to rehabilitation alone for alleviating symptoms in primiparous women with PFDD. METHODS: In this randomized controlled trial, 40 primiparous women with PFDD were allocated to an intervention group (TTNS + standard rehabilitation, n = 20) or a control group (sham stimulation + standard rehabilitation, n = 20). The intervention lasted 6 weeks. Primary

The Optimal Surgical Approach for Deep Infiltrating Endometriosis Involving Pelvic Nerves (type 1 and 2).

OBJECTIVE: Excision of deep infiltrating endometriosis in the parametrium involving pelvic nerves can be challenging and requires good anatomical knowledge and surgical strategy [1]. Based on our experience, we have identified two important types of nodule in this compartment, differing in location and the pelvic structures involved, and therefore propose this new classification. This video aims to show a standardized, reproducible method for each type. SETTING: The surgeries were performed with the DaVinci Xi system at the IFEMEndo center in Bordeaux, France. Illustrations were produced with

COL4A3 Promotes Invasion and act as Biomarker in Pituitary Neuroendocrine Tumor.

INTRODUCTION: The Transgluteal approach (TGa) to the hip, proposed by BAUER, and often used for total arthroplasties, could be responsible, according to some authors for bad clinical results, due to injury of the nervus gluteus superius (NGS). The aim of this study was to verify the nerve's anatomical condition and evaluate the risks of injury during TGa. MATERIAL AND METHODS: Thirty three dissections of fresh corpses in the lateral position (preceded by a TGa) permitted the estimation of the risk and the measurement of the distances between the nerve and the trochanter major (TM). The NGS wa

Anatomical study of the innervation of the posterior elbow joint capsule: implications for ultrasound-guided peripheral nerve block and radiofrequency ablation procedures.

INTRODUCTION: Ultrasound-guided peripheral nerve block and radiofrequency ablation have been developed for pain management in various joints including the hip, knee and shoulder, but not the elbow. Precise three-dimensional (3D) localization of the articular branches and landmarks visible on ultrasound are needed. The objectives of this anatomical study were to determine the presence, course, frequency, landmarks and areas innervated by the articular branches supplying the posterior elbow joint. METHODS: In 12 upper extremity specimens, articular branches to the posterior elbow joint were dis

Effect of superior laryngeal nerve stimulation on swallowing and respiratory coordination in dexmedetomidine-administered adult rats.

It is not clear whether swallowing, which is induced by electrical stimulation of the superior laryngeal nerve (SLN), dominantly suppresses respiration. We examined how the swallowing coordinates with respiration in adult rats under continuous infusion of dexmedetomidine, which possesses α2-adrenoceptor/imidazoline I1 receptor (α2AR/I1R) mediated sedative-analgesic profile with minimal direct GABAergic potentiation, thereby reducing interference with swallowing circuitry. Prior to the experiment, adult rats (203-322 g, n = 12) under isoflurane anesthesia had their tail vein cannulated for dexm

The impact of nerve management on the risk for persistent postoperative pain one year after open anterior mesh inguinal hernia repair.

PURPOSE: Persistent postoperative pain is a major challenge in inguinal hernia surgery. However, the impact of intraoperative nerve management on postoperative pain is poorly understood. The aim was to evaluate how management of the three inguinal nerves during anterior mesh repair of inguinal hernia affects the risk for persistent postoperative pain. METHODS: Cohort study based on data from the Swedish Hernia Register (SHR) concerning management of the three inguinal nerves. Adult patients with an open anterior mesh repair between 2012 and 2017 and who had responded to a patient-reported out

Abdominal ultrasound activates afferent vagus nerve fibers and induces anti-inflammatory effects.

Abdominal ultrasound has emerged as a noninvasive modality with immunomodulatory potential. Although its anti-inflammatory effects have been demonstrated in various disease models, the underlying mechanisms remain unclear. Previous studies suggest that ultrasound promotes anti-inflammatory macrophage polarization via α7 nicotinic acetylcholine receptor (α7nAChR) signaling in the spleen. However, the upstream events initiating this response have not been elucidated. Here, we demonstrate that abdominal ultrasound activates afferent vagal fibers and suppress systemic inflammation. In a murine mod

Mapping axillary sensory cutaneous nerves for enhanced analgesic approaches in axillary surgery: a cadaveric study.

BACKGROUND: Innervation of the breast includes branches of thoracic intercostal nerves, the superficial cervical plexus, the brachial plexus, and the intercostobrachial nerve (ICBN). Commonly used blocks for breast surgery provide incomplete analgesia of the axillary region. This cadaveric study aims to identify and map the axillary sensory cutaneous nerves. METHODS: We conducted nine axillary dissections on cadavers of both sexes with cadavers in supine position and upper limbs abducted. Incisions along the anterolateral thorax and superior clavicle created laterally reflecting skin flaps, a

Chronic Groin Pain in Female Cyclists: Consider Pudendal Neuroma as a Possible Cause.

A 58-year-old female cyclist is experiencing chronic right groin pain, adversely affecting her quality of life and athletic performance. Despite undergoing comprehensive diagnostic evaluations, including pelvic MRI and specialized physical examinations, the primary findings were chondrolabral separation and ischiofemoral impingement. Notably, a pudendal neuroma was identified through ultrasound imaging. The patient received ultrasound-guided corticosteroid injections (Depomedrone 20 mg) and local anesthetic (0.25% bupivacaine 1 mL, with 1% lidocaine for skin and subcutaneous tissues), substant

Minimally invasive extraperitoneal posterior neurectomy for anterior cutaneous nerve entrapment syndrome (ACNES): Primary description of the MEPONE technique and clinical outcomes.

BACKGROUND: Open anterior neurectomy for Anterior Cutaneous Nerve Entrapment Syndrome (ACNES) necessitates targeted dissection of the anterior rectus sheath, potentially compromising abdominal wall integrity. Utilizing established eTEP (enhanced-view totally extraperitoneal) principles, this study introduces a novel minimally invasive extraperitoneal posterior neurectomy (MEPONE) technique. By accessing the retrorectus space, direct proximal nerve visualization is achieved while avoiding direct transfascial dissection at the entrapment site. METHODS: A retrospective, single-center analysis ev

Rehabilitation strategies for abdominal wall reinnervation after intercostal nerve injury: A narrative review.

The abdominal wall musculature, including the rectus abdominis, transversus abdominis, and internal and external obliques, is a complex, multifunctional unit essential for trunk stability, postural control, and respiratory function. Injury to the intercostal nerves innervating these muscles, which may occur following rib fractures, thoracic surgery, trauma, or iatrogenic interventions, can lead to significant motor and respiratory deficits, including abdominal wall bulging, reduced trunk stability, impaired cough, and diminished forced expiration. Surgical interventions such as intercostal ner

Perioperative guidelines for peripheral nerve blocks.

Peripheral nerve blocks (PNBs) are a key component of multimodal analgesia, offering effective postoperative pain control with minimal adverse effects. Widely utilized in various surgical procedures involving the upper and lower extremities, thorax, abdomen, and spine, PNBs enhance perioperative care by reducing opioid consumption, improving pain management, and minimizing anesthesia-related complications. The use of ultrasound-guided techniques has significantly improved the efficacy and safety of PNBs, ensuring precise needle placement, reducing complication rates, and increasing patient sat

How I do it: The Hi-UP approach-single-port da Vinci SP-assisted mediastinoscopic subtotal esophagectomy with a transhiatal-dominant strategy in the abdominal and cervical phases.

Mediastinoscopic esophagectomy for thoracic esophageal cancer may reduce pulmonary morbidity; however, upper mediastinal lymphadenectomy, particularly along the left recurrent laryngeal nerve (station 106recL), remains technically challenging. We developed a transhiatal-dominant mediastinoscopic subtotal esophagectomy using the da Vinci SP system for both the abdominal and cervical phases. After completing lower and mid-mediastinal lymphadenectomy while preserving the mediastinal pleura, transhiatal dissection is advanced beyond the carina into the upper mediastinum to perform 106recL lymphade

Inner, close-to-body reachable workspace to quantify upper extremity mobility: a study on children with brachial plexus birth injury.

Brachial plexus birth injury (BPBI) results in individualized impairments in upper extremity (UE) mobility. A patient-specific understanding of these movement limitations is critical for optimizing decision-making and outcomes; however, current assessments may fail to capture the entirety of a patient's UE mobility. Reachable workspace quantifies an individual's global UE mobility by measuring the regions that can be reached by their hand. Despite most UE activities of daily living requiring adequate close-to-body function, current workspace approaches only assess far-from-body mobility. This
