Neural stem cell-loaded biohybrid hydrogel improves cochlear implants by electrode-neural coupling and neural regeneration.

Background: Contemporary cochlear implants (CIs) face unresolved dual challenges: biomechanical-electrochemical mismatch at the electrode-tissue interface and progressive spiral ganglion neuron (SGN) degeneration, severely limiting long-term auditory restoration. Integrating regenerative medicine with bioelectronic engineering offers promise to overcome these bottlenecks. Methods: A biohybrid neural interface was developed by embedding neural stem cells (NSCs) in photopolymerized poly(3,4-ethylenedioxythiophene):poly(styrenesulfonate) (PEDOT:PSS)/collagen hydrogel. Physicochemical properties w

EGFR activation sensitizes trigeminal NMDA receptors to promote pain and morphine analgesic tolerance in oral cancer.

Pain associated with oral cancer is debilitating. Opioids are the gold standard for cancer pain management, but tolerance and side effects limit their use. Epidermal growth factor receptor (EGFR) signaling is commonly amplified in oral tumors. Here, we found that EGFR activation contributed to both oral cancer pain and opioid tolerance by sensitizing trigeminal ganglion (TG) cells, the main sensory neurons innervating the face and mouth. EGFR ligands were secreted by oral squamous cell carcinoma (OSCC) cells and by peripheral glial cells cocultured with OSCC cells. In human OSCC and an orthoto

Dissection Report of Combined Variations of the Facial Nerve, Cervical Plexus, and Superficial Venous System in the Cervicofacial Region.

The cervicofacial region contains densely layered neural and venous structures with clinically significant anatomic variability. Variations involving the extratemporal facial nerve (FN), its communications with the cervical plexus (CP), and the superficial venous system may increase surgical risk during parotid, submandibular, and upper cervical procedures. A stepwise cervicofacial dissection was performed on an 82-year-old male donated cadaver. The branching pattern of the extratemporal FN, intrafacial interconnections, communications with CP branches, and relationships to the retromandibular

Facial reanimation in radical parotidectomy: The Milan proposal.

Surgery for advanced stage or recurrent parotid tumors often involves VII nerve sacrifice. Facial reanimation techniques aim to restore corneal protection and both static and dynamic facial symmetry, without compromising resection. Surgical strategies were based on tumor histology and intraoperative frozen section analysis. Masseteric to facial nerve branch (for the ZMM) neurorrhaphy was done in patients without perineural invasion, while temporalis muscle tendon lengthening was used when that nerve was resected. Facial symmetry and function were objectively assessed using the eFACE system pre

Duplication of Right Superior Cerebellar Artery Leading to Trigeminal Neuralgia: A Rare Neurovascular Conflict.

Trigeminal neuralgia (TN) is a chronic neuropathic pain condition characterized by brief, intense, and electrical shock-like facial pain. The most common causes of this are due to neurovascular compression of the superior cerebellar artery (SCA). However, as seen in this case report, duplication of the SCA resulting in TN is extremely uncommon. A 63-year-old woman presented with a 1-day history of paroxysmal, shock-like pain on the right side of her face affecting both the ophthalmic (V1) and maxillary (V2) branches of the trigeminal nerve. Each episode of pain would last 1-2 min, occur multip

ARID1B regulates sphingolipid metabolism and myelin development via STAG2: Mechanistic insights into ARID1B-related coffin-siris syndrome.

OBJECTIVES: Coffin-Siris syndrome (CSS) is predominantly attributed to variants in ARID1B gene, however, the molecular pathways connecting ARID1B to myelination and neural development are not well elucidated. METHODS: We employed ARID1B knockdown in SK-N-SH cells coupled with transcriptomic analysis to identify downstream targets. We conducted quantitative Real-Time PCR, Western blotting, chromatin immunoprecipitation (ChIP) and co-immunoprecipitation (Co-IP) assays to investigate the regulatory mechanisms of ARID1B. The clinical phenotypes of three CSS patients with pathogenic ARID1B variant

Protocol for a prospective single-arm feasibility study of suzetrigine for postoperative pain management in outpatient facial plastic surgery.

Despite relatively brief postoperative pain courses and well-established risks of adverse effects and dependency, opioid analgesics remain routinely prescribed following outpatient facial plastic surgery. Suzetrigine (Journavx) is a first-in-class, oral, non-opioid analgesic that selectively inhibits the voltage-gated sodium channel 1.8 (NaV1.8), a peripheral nociceptive pathway target that may avoid central nervous system adverse effects associated with opioids. While suzetrigine has demonstrated efficacy in phase III trials of acute postoperative pain, its use in facial plastic surgery has n

Sixteen years of cochlear implant surgery in cochleovestibular malformation and cochlear nerve deficiency: Insights from northern Malaysia.

INTRODUCTION: Congenital sensorineural hearing loss associated with cochleovestibular malformations (CVM) and/or cochlear nerve deficiency (CND) presents distinct surgical and audiological challenges. Patients with CVM face elevated risks of intraoperative cerebrospinal fluid (CSF) gushers and aberrant facial nerve courses, while those with CND have historically been viewed as poor candidates for cochlear implantation (CI) due to concerns regarding neural sufficiency. This study aims to bridge this gap by evaluating the surgical safety and audiological outcomes of CI in a focused cohort of pat

Predictors of Delayed Facial Nerve Palsy Following Acoustic Neuroma Surgery.

OBJECTIVE: Delayed facial nerve palsy (DFP) may occur after acoustic neuroma surgery despite normal immediate postoperative facial nerve function. The mechanisms and predictors of this phenomenon remain poorly defined. This study aimed to identify factors associated with the development of delayed facial nerve palsy and characterize recovery at 1 year. METHODS: A retrospective cohort study with prospective follow-up was conducted including patients who underwent acoustic neuroma resection and demonstrated normal immediate postoperative facial nerve function (House-Brackmann [HB] grade I). Facial nerve schwannoma of the accessory parotid gland region in a child managed through a transoral approach.

Facial nerve schwannomas arising in the accessory parotid gland region are extremely rare, and paediatric cases have not been reported previously. We present the case of a girl in middle childhood who developed a painless, gradually enlarging mass in the mid-cheek area. Radiologic evaluation revealed a well-defined lesion with imaging features suggestive of a benign neoplasm. A transoral surgical approach was chosen to avoid cutaneous scarring and minimise surgical morbidity. Complete excision was achieved without injury to the facial nerve or Stensen's duct, and histopathology revealed a beni

Functional outcomes of dual nerve transfer in patients with facial paralysis.

BACKGROUND: Facial paralysis, as a social-facing disorder, is associated with high levels of depression, anxiety, and social isolation. Dual nerve transfers allow for simultaneous coaptation of the hypoglossal and masseteric nerves to improve dynamic and static domains of facial function. In this study, we investigated clinician-graded long-term functional outcomes as well as their impact on disease-specific quality of life and communicative perception in patients with facial paralysis after dual nerve transfer surgery. METHODS: Retrospective data of 8 patients with facial paralysis who under

Comparative Analysis of End-to-End versus End-to-Side Neurorrhaphy for the upper crossface in the second surgical step of triple innervation technique for Facial Reanimation Surgery: A Retrospective Study.

Facial paralysis poses significant challenges including functional impairments and aesthetic anomalies. The management of facial palsy often involves many surgeries aimed at reanimating the facial muscles and restoring protective mechanisms such as the blink reflex. In recent years, the triple innervation technique has emerged as a promising approach for facial reanimation, combining multiple neural inputs to achieve functional restoration. This retrospective study compares the outcomes of two different neurorrhaphy techniques, namely end-to-end and end-to-side, in the context of the upper cro

Surgical approach to model facial nerve paralysis in an ovine model for in vivo testing of a novel intervention.

Facial nerve paralysis (FNP) disrupts neural inputs to the facial muscles, the most significant being those responsible for blinking. This loss often results in discomfort, corneal exposure, and if improperly treated, vision loss. FNP typically presents unilaterally, resulting in an asymmetric face, causing distress and social isolation. The Bionic Lid Implant for Natural Closure (BLINC) was developed to restore functional blinking in patients with FNP by mechanically actuating eyelid movement via an upper eyelid sling, with a focus on restoring a natural and symmetric appearance. To move this

A Deeper Dive into the Anatomy of the Frontal Branch of the Facial Nerve.

The frontal branch of the facial nerve innervates several muscles in the upper face and is essential for facial expression and eye closure. Injury to the frontal branch can result in functional and aesthetic issues; therefore, a thorough understanding of the anatomy and course of the frontal branch is essential for safe surgery in the lateral face and temporal region. The course of the frontal branch is variable, but certain anatomic landmarks can be used to locate the nerve. Preoperative planning, careful dissection, and intraoperative monitoring are critical to minimize nerve injury in tempo

Transcutaneous Facial Nerve Frontal Branch Stimulation to Restore Dynamic Elevation of the Paralyzed Eyebrow in Synkinetic Patients.

BACKGROUND: While patients commonly recover motion following facial nerve injury, aberrant neural regeneration can lead to synkinesis and chronic nonflaccid facial palsy (NFFP); many patients fail to recover volitional eyebrow elevation. OBJECTIVE: To assess if transcutaneous electrical stimulation of injured frontal branches can achieve eyebrow elevation. METHODS: We studied healthy volunteers (N = 10) and patients with unilateral NFFP (N = 36). We performed monopolar transcutaneous stimulation of healthy and paralyzed frontal branches with 4-10 mA pulse trains. We measured eyebrow elevatio

Buccal Fat Pad Modification in Deep Plane Facelift: Technical and Facial Nerve Considerations.

Buccal fat pad (BFP) modification can help aesthetic facial balancing. It can be approached through an intraoral incision, which is more standard, or through a deep-plane facelift approach. Here, we describe the unique technique of removal and repositioning of the BFP through a deep-plane facelift approach with clinical pearls for safe manipulation around the facial nerve and redistribution techniques for facial volume.

Dual Innervation of the Depressor Labii Inferioris and Implications for Deep Plane Facelift and Structural Neck Contouring: Insights from In Vivo Stimulation of the Facial Nerve Branches.

Injury to the depressor labii inferioris (DLI) is a frequent cause of lower lip asymmetry following deep plane facelift and structural neck contouring. This study characterizes the in vivo innervation of the DLI through intraoperative facial nerve mapping during selective facial neurectomy in 20 consecutive patients. In all cases, dual innervation of the DLI was observed: a superficial branch of the marginal mandibular nerve (MMN) supplies the lateral DLI, while an inferomedial cervical branch innervates the medial DLI. Both branches are located superficially and closely adherent to the deep s
