This case report describes a 70-year-old male who experienced a biphasic blockade following an interscalene block with 10 mL of liposomal bupivacaine (Exparel) mixed with 5 mL of 0.25% bupivacaine for shoulder arthroscopy. The patient exhibited complete motor and almost complete sensory recovery by postoperative day (POD) 1, followed by a recurrence of motor and sensory deficits on POD2, which resolved spontaneously by POD3. To our knowledge, this represents one of the earliest documented recurrences of dense motor blockade after brachial plexus block with Exparel. Potential mechanisms, includ

PURPOSE: Humeral shaft fractures may be managed conservatively or operatively; however, increasing non-union and complications after non-operative treatment have renewed interest in intramedullary fixation. Innovative intramedullary nails with distal internally locking, requiring neither fluoroscopy nor external guidance, aim to reduce surgical time, simplify the procedure, and minimise the risk of iatrogenic radial nerve injury. This study evaluates the clinical and radiological outcomes of a humeral nail utilising distal internally locking.

INTRODUCTION: Ultrasound-guided peripheral nerve block and radiofrequency ablation have been developed for pain management in various joints including the hip, knee and shoulder, but not the elbow. Precise three-dimensional (3D) localization of the articular branches and landmarks visible on ultrasound are needed. The objectives of this anatomical study were to determine the presence, course, frequency, landmarks and areas innervated by the articular branches supplying the posterior elbow joint.
METHODS: In 12 upper extremity specimens, articular branches to the posterior elbow joint were dis

OBJECTIVE: The spinal accessory nerve (SAN) is frequently used as a donor for reinnervation of the suprascapular nerve (SSN) in the reconstruction of brachial plexus injuries (BPIs). This procedure can be performed using either the anterior or posterior approach. However, no studies have compared the postoperative outcomes between these two approaches in adult patients with complete BPI. Thus, this study aimed to compare postoperative shoulder functional outcomes following the anterior and posterior approaches.
METHODS: Between 2017 and 2022, 48 adult patients with complete palsy following a

PURPOSE: The suprascapular (SSN) and axillary (AXN) nerves are vital for shoulder mobility and are at notable risk for injury during glenohumeral surgeries due to their proximity to key anatomical landmarks. This systematic review evaluates their anatomical positioning relative to the glenohumeral joint and highlights the clinical significance of these relationships in shoulder procedures.
METHODS: A thorough literature search across PubMed, MEDLINE, and Google Scholar (2018-2025) yielded 424 studies. After applying inclusion criteria, 21 articles-including clinical, cadaveric, and systematic

OBJECTIVE: A key residual deficit in brachial plexus birth injury (BPBI) is the lack of active glenohumeral external rotation (ER). Surgical options for reinnervating the suprascapular nerve (SSN) include nerve grafting from a proximal stump and nerve transfer using the spinal accessory nerve (XIN) as a donor. The optimal surgical strategy is unclear from the current literature. This study aimed to compare the outcomes of nerve grafting and nerve transfer for SSN reinnervation in upper trunk BPBI.
METHODS: The authors conducted a retrospective study (1990-2020) on prospectively collected data

INTRODUCTION: The axillary nerve is a crucial peripheral branch arising from the posterior cord of the brachial plexus. It provides both motor and sensory innervation to the shoulder girdle, enabling essential movements such as abduction and external rotation-functions that are crucial for daily activities and various athletic movements. Despite its significance, detailed documentation of the axillary nerve's branching patterns remains limited. This study aimed to examine the branching configurations of the axillary nerve in a South African cadaveric sample.

Background: Targeted muscle reinnervation (TMR) is an emerging surgical technique that reroutes severed peripheral nerves to nearby motor targets to prevent symptomatic neuroma formation and reduce amputation-related pain. This case series evaluates the effectiveness of TMR in managing symptomatic neuromas following digit and hand amputations and reports postoperative outcomes and complications. Methods: Seventeen patients with traumatic or surgical digit/hand amputations distal to the wrist crease presenting with symptomatic neuromas were prospectively enrolled between August 2021 and August

PURPOSE: Loss of independence is a major issue for quadriplegic patients. Restoring active wrist extension is essential to improving their autonomy. We aim to evaluate the feasibility of neurotization of the brachialis muscle nerve on branches destined to the extensor carpi radialis muscles.
METHODS: 30 upper limbs were dissected to analyze the anatomy and morphometry of the radial nerve, the musculocutaneous nerve, and the distance between the brachial nerve and the nerve branches of the radial extensor muscles of the carpus.
RESULTS: There was a variability in the positioning and number of

PURPOSE: Clinical management for brachial plexus birth injuries (BPBI) is often focused on childhood, leaving a gap in understanding of how this condition impacts individuals into young adulthood. Recent studies suggest that pain persists beyond pediatric care, although the long-term trajectory of pain and function remains poorly understood. This study compares pain interference in young adults with BPBI to matched controls. Secondary objectives were to compare pain intensity between groups, examine the relationship between upper limb function and pain, and identify common areas of pain using

BACKGROUND: Children with brachial plexus birth injury often rely on compensatory movements due to limited shoulder mobility. In clinical practice, the Modified Mallet Scale (MMS) is frequently used to assess shoulder movements. However, MMS scores can be influenced by trunk contributions, preventing an accurate evaluation of pure shoulder complex motion. This study aimed to quantify the relative contributions of the extremity and trunk during MMS movements using wearable inertial sensors.
METHODS: The study included 24 children (7.20 ± 1.41 years; range 4-10 years), and three-dimensional kin

PURPOSE: Current functional assessments after brachial plexus (BP) reconstruction, including surgeon-graded active range of motion and strength scores, and patient-reported outcomes (PROs), have limitations. This study evaluated the feasibility of wearable sensors to quantify recovery and functional outcomes following BP reconstruction. We hypothesized that sensor-derived measures of daily upper-extremity use would correlate with PROs and that real-world upper-extremity use would improve.
METHODS: Traumatic BP injury patients wore bilateral wrist accelerometers for 7-day periods at 3- to 6-mo

PURPOSE OF REVIEW: Shoulder arthroplasty is often associated with significant postoperative pain, making effective analgesia a crucial part of surgery day. The interscalene brachial plexus nerve block (ISB) is the gold standard for regional anesthesia in otherwise healthy patients. However, its use is limited by a high incidence of ipsilateral phrenic nerve paralysis, which may result in respiratory compromise, particularly in patients with chronic obstructive pulmonary disease (COPD), reduced pulmonary reserve, or preexisting diaphragmatic dysfunction. The incidence of hemidiaphragmatic paral

BACKGROUND: Effective analgesia and preservation of diaphragmatic function are key considerations in analgesia for shoulder surgery. The superior trunk block provides analgesia with reduced phrenic nerve involvement, while the erector spinae plane block offers minimal impact on diaphragm motion. This randomized controlled trial compared the analgesic efficacy, impact on diaphragmatic motion, and postoperative recovery between the two blocks.
METHODS: Sixty patients undergoing arthroscopic shoulder surgery were randomized to receive either erector spinae plane block or superior trunk block. Pr

Brachial plexus injuries are challenging conditions. Over the past decades, nerve transfer surgery has progressively evolved from proximal nerve reconstruction toward selective distal neurotization strategies, considerably expanding the possibilities for functional restoration. As the number of described donor-recipient combinations has increased, the literature has become increasingly fragmented, often focusing on isolated techniques or specific functional targets. The aim of the present study was to provide a comprehensive state-of-the-art overview of currently available motor nerve transfer

PURPOSE OF REVIEW: Phrenic-sparing regional anesthesia for shoulder surgery has expanded in response to concerns that interscalene block (ISB) frequently causes hemidiaphragmatic paresis (HDP). This review examines the balance between respiratory safety and block completeness, emphasizing patient selection, technique modification, and clinically meaningful outcomes.
RECENT FINDINGS: Contemporary evidence supports a spectrum of approaches. Low-volume, lower-concentration, and extrafascial ISB techniques can reduce but may not eliminate HDP and may not provide consistent analgesia. Superior tru

Accurate assessment of shoulder kinematics in Modified Mallet Scale (MMS) movements is essential for clinical evaluation and treatment planning in brachial plexus birth injury (BPBI). However, inconsistencies in reporting methods affect the reliability of kinematic data. This study aims to determine the optimal Euler sequence for reporting shoulder kinematics during MMS movements. Kinematic data from the unaffected arms of ten children with BPBI were collected using inertial measurement units and analyzed retrospectively. Three commonly adopted Euler sequences (ZXY, XZY, YXY) were analyzed. Th

Parsonage-Turner syndrome (PTS) can present with upper extremity weakness that overlaps clinically with traumatic upper-trunk brachial plexus injury, cervical radiculopathy, and shoulder pathology. Although many patients improve with conservative management, persistent paralysis of elbow flexion after a defined period of observation creates a difficult management decision. In selected patients with preserved donor nerve function and no evidence of meaningful reinnervation, distal nerve transfer may be considered to attempt restoration of elbow flexion. Brachial plexus injury causes severe arm weakness, forcing patients to rely on compensatory movements for daily activities. Most robotic solutions target isolated elbow or hand function, whereas multi-joint assistance remains limited. Here we show that a soft wearable exosuit providing shoulder and elbow assistance through pneumatic actuation and inertial sensing improves arm function in 14 adults with chronic traumatic brachial plexus injury, including six with complete lesions and ten after nerve transfer surgery. In single-session testing, the device increases active range of motion ( + 80-
