The evaluation of the test protocols on continuous lumbar plexus blockade using the 3-in-1 block with a lumbar plexus catheter showed the following results: The study included 104 patients. In 91.3% of cases, puncture of the fascial sheath of the femoral nerve proved successful. In 95.7% of cases, the plexus catheter could be positioned and left in place (Table 1). Four catheters had to be removed due to intravascular misplacement. Good analgesic results were achieved in 85.7% of patients. In 14.3% of cases, the judgement of pain reduction could not be sufficiently or definitely ascertained (T

The pyriformis syndrome is a rare entrapment neuropathy in which the sciatic nerve is compromised by the pyriformis muscle or other local structures. It usually presents with sciatic pain and hypoaesthesia and a limp caused by weakness of the gluteus maximus muscle. Peroneal nerve innervated muscles and hamstrings are also usually affected. The diagnosis relies on the clinical presentation and the EMG findings. Other causes of symptoms should be excluded by careful examination, detailed x-ray studies of the lumbosacral spine (including myelogram), sacro-iliac and hip joints. A case of this syn

A 21-year-old man was left with complete paraplegia and lower extremity sensory loss after a gunshot wound to the 4th lumbar vertebral body. Within three weeks of the injury he noted slight recovery of proximal left leg movements. With further motor recovery and rehabilitation, he was discharged ambulatory at nine months postinjury. The patient described late motor recovery developing 13 to 16 years after injury, particularly in right knee extensors and bilateral ankle plantar flexors, simultaneous with an aggressive home exercise program. Serial manual muscle tests revealed increased strength

To study the biomechanical effects of gastrocnemius-soleus dysfunction and its potential remediation, the gait patterns of six able-bodied young adults were analyzed before and after induced temporary tibial nerve paralysis. Ambulation with the tibial nerve block was performed with and without the assistance of an ankle-foot orthosis (AFO) with a rigid anterior stop adjusted to either 5 degrees plantarflexion or 5 degrees dorsiflexion. The gait abnormalities resulting from tibial nerve paralysis include delayed advancement of the center of pressure, delayed ipsilateral heeloff and early contra

Eighteen cases of iatrogenic femoral nerve injury are described. In two patients the damage was incurred as a result of an intra-abdominal operation. In three others the nerve was injured at the level of the inguinal ligament. In the majority femoral nerve palsy followed surgery to the hip joint and was attributable to a lesion-in-continuity. Only one of the patients in the series made a full recovery, but because only severe cases were included these disappointing results by no means represent the expected outcome. To avoid this serious and distressing complication of surgery, the surgeon mus
