USMSIT/NMUSIT Fundamental Guide on Diagnostic and Interventional Hip Ultrasonography.

Hip pain arises from a spectrum of intra-articular, periarticular, and neurogenic conditions. Ultrasound offers real-time, radiation-free visualization of the hip joint, adjacent soft tissues, and peripheral nerves - making it an essential tool in contemporary musculoskeletal evaluation. Guided by the EURO-MUSCULUS/USPRM framework, this guideline proposes a standardized, anatomy-based protocol for both diagnostic and interventional hip ultrasonography. Developed under the educational mission of the Ultrasound and Musculoskeletal/Neuromuscular Sonography Initiative in Taiwan - through its USMSI

Distinct fascicular and epineurial architecture of the obturator nerve: a comparative analysis with clinical and surgical relevance.

Peripheral nerves of the lumbar plexus share a common origin but exhibit distinct anatomical and clinical features. Although the gross anatomy of these nerves has been described, differences in cross-sectional structure remain unclear. This study compared the histological architecture of the obturator nerve (ON) with that of the femoral nerve (FN) and lateral femoral cutaneous nerve (LFCN). Eighteen nerves (ON, FN, and LFCN) from six cadavers were examined using Masson's trichrome-stained cross-sections. Morphometric analysis included the short-to-long axis ratio and fascicular area ratio. Epi

Dissection guide for en bloc brachial and lumbosacral plexuses and central nervous system.

Traditionally anatomy curricula often separate dissections of the central nervous system (CNS) and peripheral nerve plexuses, limiting opportunities for students to appreciate the nervous system as an integrated whole. Viewing the CNS and somatic components together offers the potential to enhance conceptual understanding, spatial reasoning, and clinical correlation. This guide details a step-by-step approach for the en bloc removal of the entire CNS with its associated brachial (C5-T1) and lumbosacral (L1-S4) plexuses. The dissection utilizes standard laboratory tools and follows a logical pr

Innervation of the anal canal revisited through micro-CT imaging.

Studies in anatomy are frequently performed through dissection and histology as investigative methods. These techniques are, however, destructive and struggle to capture the peripheral nerves over larger areas, such as the pelvis, and their trajectories thus remain elusive. This study aimed to explore the capabilities of non-destructive micro-CT in identifying small nerves in the periphery of the pelvic area and map them in human fetal development. Six human fetal pelvic samples were derived from the Dutch Fetal Biobank; samples were contrast enhanced with B-Lugol and scanned through micro-CT

Short-term sacral neuromodulation sustained long-term efficacy in the management of postoperative urinary retention after pelvic surgery: a case report.

BACKGROUND: Sacral neuromodulation (SNM) serves as an effective therapeutic option for lower urinary tract dysfunction, and long-term intervention is generally required to sustain clinical benefits. Reports regarding durable therapeutic outcomes achieved with short-term SNM remain uncommon. This case indicates that short-term SNM can bring about sustained improvement of refractory non-obstructive voiding dysfunction after resection of large pelvic teratoma. CASE PRESENTATION: A 19-year-old female developed refractory urinary retention following excision of a huge mature cystic teratoma of the

Mechanical‑immune feedback regulation in distraction osteogenesis: From mechanotransduction to immune microenvironment remodeling (Review).

BACKGROUND: Prostatic male genital schistosomiasis and prostate cancer co-existence cases are uncommon however, some studies have indicated that schistosomiasis may trigger development of prostate cancer regardless of age. Schistosomiasis is a public health problem in sub-Saharan Africa and may account for some undocumented cases of schistosomiasis prostatic cancer in schistosome endemic rural communities. It is against this background that we investigated the association between schistosomiasis and risk of prostate cancer development in residents of Murehwa Community, a schistosomiasis endemi

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Image-based evaluation of a commercial AI synthetic CT generator for brain and prostate MR-only radiotherapy.

BACKGROUND: Synthetic CTs (sCT) were developed to provide electron density information for MR-only treatment planning. Some AI-based sCT generators have received FDA-clearance and better capture tissue heterogeneity through voxel-wise mappings to improve image guidance and dose calculation. PURPOSE: To quantitatively characterize the image quality and geometric surrogate agreement of an FDA 510(k) cleared AI-based synthetic CT generator (TheraPanacea) for brain and prostate MR-only radiotherapy, using per-structure volumetric analysis, Hounsfield unit (HU) assessment, and multi-metric geometr

Neurogenic tumors of the abdomen and pelvis: imaging features, differential diagnosis, and diagnostic pitfalls.

Neurogenic tumors of the abdomen and pelvis are a diverse group of neoplasms arising from paraganglia, peripheral nerves, and autonomic ganglia. Their imaging appearances range from benign incidental lesions to aggressive neoplasms with metastatic potential, creating diagnostic challenges because of overlapping imaging features and broad differential diagnoses. Cross-sectional imaging, supplemented by functional imaging, plays a central role in lesion characterization, staging, and surgical planning. This review summarizes the imaging features of paraganglion cell-derived, nerve sheath-derived

Protocol for EHS Guideline on Treatment of Ventral and Incisional Hernias in Emergency.

BACKGROUND: Various interventions, including the superior hypogastric plexus block and ganglion impar block, are commonly used for the treatment of pelvic or perineal pain caused by cancer. The inferior hypogastric plexus block (performed using a trans-sacral approach under fluoroscopy and using a local anesthetics⁄steroid combination) for the diagnosis and treatment of chronic pain conditions involving the lower pelvic viscera was first described in 2007. Neurolysis of the inferior hypogastric plexus may be useful for the treatment of pelvic and perineal pain caused by cancer. OBJECTIVES: To

Combined laparoscopic and gluteal approach for resection of a sciatic notch dumbbell lipoma.

The greater sciatic, lesser sciatic and obturator foramina are openings in the pelvis through which pelvic lesions can herniate, leading to the formation of rare dumbbell tumours. We present the case of a man in his late 60s, who presented with obstructed defecation. A per rectal examination revealed a palpable right sided rectal mass. MRI pelvis identified a dumbbell lipoma traversing the right sciatic foramen, with a pelvic component causing displacement of the rectum and a gluteal component impinging on the sciatic nerve. The lipoma was successfully removed by a multidisciplinary team using

Comparison of the effects of transcranial direct current stimulation and transcutaneous tibial nerve stimulation on the urgency and frequency of women with overactive bladder syndrome: study protocol of a randomized clinical trial.

BACKGROUND: Overactive bladder syndrome is common, with a prevalence of 12-17% among adults. Posterior tibial nerve stimulation is the primary nonpharmacological and conservative treatment for overactive bladder syndrome. While several human brain imaging studies have shown the involvement of supraspinal centers in bladder control, a literature review has found that no research has specifically investigated cortical stimulation through transcranial direct current stimulation as a treatment for overactive bladder syndrome in women. Therefore, this study aims to assess the potential benefits of

Perineal and Rectal Nerve Recruitment Order Varies During Pudendal Neurostimulator Implant Surgery.

INTRODUCTION: Pudendal nerve stimulation (PNS) is an off-label therapy for patients experiencing pelvic pain and voiding dysfunction. The pudendal nerve has two efferent branches to the rectum and perineum. Only the rectal branch is monitored via external anal sphincter electromyography during the implant procedure to help determine the lead position. We examined intraoperative PNS-driven urethral pressures to infer nerve recruitment order and tracked patient reported outcomes. METHODS: Patients receiving PNS for pelvic pain and/or urinary symptoms were recruited. During the implant surgery,

Internal Hernia Beneath the Obturator Nerve After Robot-Assisted Radical Cystectomy and Pelvic Lymphadenectomy: A Case Report With Literature Review.

A man in his 40's presented with vomiting and numbness in the right thigh. Eight months earlier, he had undergone robot-assisted radical cystectomy and pelvic lymphadenectomy for bladder cancer. Computed tomography (CT) revealed a caliber change and a closed loop of the small intestine in the right pelvis, prompting emergency diagnostic laparoscopy. Intraoperative findings showed that the small intestine had herniated into the space between the right obturator nerve and pelvic wall, resulting in strangulated intestinal obstruction. The necrotic small intestine was resected and reconstructed, w

Effect of Transcutaneous Tibial Nerve Stimulation in Multiparous Egyptian Women with Vaginal Laxity: A Randomized Controlled Trial.

Vaginal laxity (VL) is a common symptom of pelvic floor disorder, an underreported condition, and has a substantial impact on women's sex lives and relationships. The study aimed to determine the effect of transcutaneous tibial nerve stimulation (TTNS) on women with VL. Fifty women with vaginal laxity were chosen from an outpatient clinic in Egypt. They were randomly split into two equal categories (A and B). Group A (n = 25) received pelvic floor exercises (PFE), while Group B (n = 25) received TTNS and PFE. Every week, both groups performed three sessions for three months. The outcomes were

Comparing the Anatomy of the Lateral Femoral Cutaneous Nerve in Patients With and Without Meralgia Paresthetica: A Systematic Review and Meta-Analysis.

The anatomical variations of the lateral femoral cutaneous nerve (LFCN) are well documented, with several studies hypothesizing that specific variants may increase the LFCN's susceptibility to meralgia paresthetica (MP). This systematic review and meta-analysis aimed to compare the prevalence of LFCN anatomical variations among limbs with and without MP. It also sought to determine if the LFCN's cross-sectional area (CSA) on ultrasound differed between limbs with and without MP. PubMed, SCOPUS, CINAHL, and Cochrane databases were used to query publications from inception to June 13, 2024. Incl

Impact of laparoscopic pudendal nerve decompression on quality of life in patients suffering from pudendal neuralgia caused by entrapment syndrome.

BACKGROUND: Pudendal neuralgia caused by entrapment syndrome (PNE) is an underdiagnosed condition with severe quality of life (QoL) impact. In cases unresponsive to medical treatment, a minimally invasive surgical approach, such as laparoscopic surgery, is preferred, particularly in patients presenting central sensitisation (CS) and myofascial syndrome (MS). Although the procedure has proven to be safe and feasible, limited studies examinate its impact on QoL. OBJECTIVES: This retrospective observational study was conducted at the Résilience Center, Axium Clinic, Aix-en-Provence, France, with

MR Neurography of the Lumbosacral Plexus: Technique and Disease Patterns.

The lumbosacral plexus (LSP) comprises a complex network of nerves supplying the pelvis and lower extremities and may be affected by a wide range of diseases. Lumbosacral plexopathy can be challenging to diagnose due to overlapping clinical presentations and difficulty performing electrodiagnostic testing of the deep pelvic structures. MRI-more specifically, MR neurography (MRN)-can readily depict most LSP segments. MRN techniques, preferably performed at 3.0 T, continue to evolve, with most protocols including two-dimensional and optionally three-dimensional, heavily T2-weighted fat-suppresse

Anatomical structures of the obturator foramen and its importance for pelvic interventıons.

BACKGROUND: In clinical scenarios such as pelvic fractures, damage to the obturator neurovascular bundle, obturator neuropraxia, and mesh placement in patients with incontinence, preserving the anatomical structures related to the obturator foramen (OF) is crucial. This study aims to investigate the anatomical variations of the OF as a guide for identifying failure criteria and improving success parameters in clinical practice. METHODS: The OFs of one hundred adult dry pelvic bones were examined. Measurements were evaluated using ImageJ software. The morphology of the OF and the presence of b
