Long Term Results of Proton Therapy in Adult Infradiaphragmatic Lymphoma.

BACKGROUND AND PURPOSE: The role of proton radiation (PT) for infradiaphragmatic targets in lymphoma patients is unknown. We report on radiation planning details, doses achieved to organs at risk (OARs), and outcomes for this population. PATIENT/MATERIAL AND METHODS: This is a single-institution retrospective study of patients with biopsy-proven lymphoma who received PT to infradiaphragmatic targets between 2011 and 2022. Comparison photon plans were generated for all patients. Toxicity was reported using CTCAE version 5.0. Dosimetric and clinical factors associated with toxicity and oncologi

Undifferentiated testicular pleomorphic sarcoma: A case report.

BACKGROUND: Genitourinary sarcomas include testicular sarcomas and are the most common subtype of sarcoma within the genitourinary system. Undifferentiated pleomorphic sarcoma is a subtype of soft tissue sarcomas that may affect the extremities and retroperitoneum. However, the presence within the testicle is rare. Here, we present a case of an undifferentiated testicular pleomorphic sarcoma, which will explore the presentation and treatment of a rare type of testicular cancer. CASE SUMMARY: Here we present a 56-year-old male who comes to the urology clinic for left testicular swelling. The p

Surgical treatment of gastrointestinal stromal tumors.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the gastrointestinal tract, though they represent a small proportion of overall gastrointestinal malignancies. Originating from the interstitial cells of Cajal, GISTs exhibit a broad spectrum of clinical behavior, ranging from indolent lesions to aggressive malignancies that can arise throughout the gastrointestinal tract, most frequently in the stomach and small intestine, and rarely in extra-gastrointestinal sites such as the omentum, mesentery, or retroperitoneum. Surgical management, particularly for gastric

Spontaneous Massive Hemorrhage Caused by Inflammatory Myofibroblastic Tumor (IMT) of the Greater Omentum: A Case Report.

Inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal soft tissue neoplasm. Its histologic features include proliferating fibroblastic/myofibroblastic spindle cells and a dense inflammatory infiltrate rich in plasma cells, lymphocytes, and eosinophils. However, it can exhibit aggressive behavior, including local invasion, malignant transformation, recurrence, and even metastasis. We report a case of a 27-year-old man who experienced a spontaneous massive hemorrhage caused by IMT of the greater omentum and who had previously undergone right upper lobectomy for the IMT of the lung. Furt

Dedifferentiated liposarcoma originating in the prostate: a rare case report.

Dedifferentiated liposarcoma (DDLPS) is an aggressive subtype of liposarcoma, associated with high rates of recurrence and metastasis, that most commonly occurs in the extremities and retroperitoneum. Here we report a 41-year-old Chinese male who was found to have a prostatic mass on imaging evaluation. Following radical prostatectomy, histopathological examination revealed that the tumor predominantly exhibited low-grade spindle cell morphology, with focal areas showing well-differentiated liposarcoma components. Immunohistochemical analysis demonstrated positivity for MDM2 and CDK4 in the tu

Gastric Mucosal Biopsy Revealing Immunoglobulin G4 (IgG4)-Related Disease With Subtle Macroscopic Changes: A Case Report.

Immunoglobulin G4 (IgG4)-related disease affects multiple organs throughout the body, including the pancreas, bile ducts, lacrimal glands, salivary glands, lungs, kidneys, retroperitoneum, and lymph nodes; however, reports of gastrointestinal involvement are scarce. We report the case of a 40-year-old Japanese woman who presented with bilateral eyelid edema and enlargement of the submandibular and parotid glands. Upper gastrointestinal endoscopy was performed due to epigastric discomfort. Although the endoscopy revealed minimal mucosal abnormalities, the gastric mucosal biopsy demonstrated mar

Retrograde Duodenojejunal Intussusception Caused by Ladd's Bands in an Adult With Intestinal Malrotation: A Rare Variant of Waugh's Syndrome.

We report a rare case of retrograde duodenojejunal intussusception caused by Ladd's bands in an adult with intestinal malrotation. A 27-year-old man with a history of cerebral palsy and gastrostomy presented with frequent vomiting. Abdominal contrast-enhanced computed tomography showed marked distention of the stomach and duodenum, along with retrograde intussusception of the jejunum into the duodenum at the horizontal segment. No neoplastic lesions were identified as a lead point. A diagnosis of intestinal obstruction due to retrograde intussusception was made, and emergency laparoscopic surg

Abscessed Lumbar Hypodermatitis Revealing Pyonephrosis: A Diagnosis to Keep in Mind.

Pyonephrosis resulting from an obstructing calculus commonly presents with symptoms such as loin pain, fever, and signs indicative of a urinary tract infection. In some cases, significant thinning of the renal parenchyma in pyonephrosis may lead to direct rupture into the retroperitoneum, and exceptionally rarely, into the lumbar abdominal wall, potentially mimicking an isolated abdominal wall abscess, which can be mistaken for a complication of hypodermatitis. Our case is a good illustration of a condition that is not well known among practitioners, particularly family physicians and dermatol

Diagnostic Accuracy of Circulating Tumor DNA to Predict Retroperitoneal Histology in Patients Treated With Retroperitoneal Lymph Node Dissection for Testicular Germ Tumor.

Testicular germ cell tumor (GCT) has survival rates exceeding 90% and thus contemporary research has focused on reducing morbidity. While chemotherapy is efficacious, long-term effects are significant. Primary retroperitoneal lymphadenectomy (P-RPLND) has been offered, and postchemotherapy lymphadenectomy (PC-RPLND) is considered, based on residual node size, to reduce overtreatment. A significant number of patients have necrosis in the retroperitoneum and are thus overtreated. Circulating tumor DNA (ctDNA) may be used to determine which patients would benefit from RPLND. This retrospective an

Surgical Determinants of Outcomes in Partial Nephrectomy: a Contemporary Review.

PURPOSE OF REVIEW: Partial nephrectomy has become the preferred treatment for localized renal masses, particularly in the minimally invasive surgery (MIS) era. However, outcomes depend not only on the surgical approach but also on how each step of the procedure is performed. This review examines the key technical steps and clinical determinants that influence oncologic control, renal functional preservation, and perioperative morbidity in contemporary minimally invasive partial nephrectomy. RECENT FINDINGS: Recent evidence highlights the importance of assessing tumor complexity, tailoring isch

Retroperitoneal Masses Decoded: A Practical Multidisciplinary Diagnostic and Surgical Strategy Integrating Radiology and Pathology.

Retroperitoneal masses comprise a heterogeneous group of benign and malignant entities originating from mesenchymal, lymphoid, neurogenic, germ cell, and metastatic tissues. Because of the large potential space of the retroperitoneum, tumors frequently attain significant size before detection and therefore present diagnostic and therapeutic challenges. Management varies widely depending on tumor biology; consequently, inappropriate surgical intervention may lead to major morbidity without therapeutic benefit. This narrative review provides a clinically oriented diagnostic and management framew

Paraneoplastic leukocytosis in peritoneal dialysis.

Liposarcoma (LPS) typically occurs in the extremities and retroperitoneum, but rarely occurs in visceral organs such as pancreas. Only few cases of primary pancreatic LPS have been reported in literature. Here we describe the first case of primary pancreatic LPS in a patient receiving continuous ambulatory peritoneal dialysis (CAPD). This patient was a 60-year-old female who had been on CAPD for 3 years due to kidney failure from type 2 diabetes mellitus. She presented with a 2-week history of intermittent fever measuring up to 38 degrees Celsius. The septic workup including peripheral blood,

Primary small cell neuroendocrine carcinoma of the liver: a case report.

This study presents a case of primary hepatic small cell neuroendocrine carcinoma (PHNEC). The patient was a 74-year-old male with a history of hepatitis C virus (HCV) infection, who presented with persistent right upper quadrant and lower back pain for two weeks. Imaging studies revealed multiple nodules and masses in the left hepatic lobe, accompanied by enlargement of lymph nodes at the hepatic hilum and in the retroperitoneum. Subsequent liver biopsy was performed, and to rule out potential origins from the gastrointestinal tract or lungs, gastrointestinal endoscopy and chest computed tomo

Malignant Peripheral Nerve Sheath Tumor of the Pancreas: A Case Report and Updated Review of Related Literature.

Malignant peripheral nerve sheath tumors (MPNSTs) are rare soft tissue sarcomas with poor prognosis due to their high recurrence rates. The prevalence of MPNST in the general population is 0.001%, with tumors arising from the retroperitoneum accounting for only 1% of all MPNSTs. In this report, we present a case of a 59-year-old male with pancreatic MPNST. To the authors' knowledge, this is the first documented case of pancreatic MPNST in the Philippines. The patient initially presented with a 3-month history of abdominal pain, weight loss, and anorexia. On abdominal computed tomography (CT) s

Communication skill training in surgical residency: insights from Y-SICO (Young-Italian Society of Surgical Oncology).

Surgical training programs primarily emphasize technical skills for diagnosis and treatment. However, communication and interpersonal skills are equally essential for residents. Trainees often face sensitive situations, such as delivering bad news and discussing end-of-life decisions and lack of communications skills could result in a negative impact on patients and malpractice claims. Despite their importance, communication skills training is often underrepresented in surgical education. This study aimed to explore the extent to which such training is offered during surgical residency in Ital

A global clinicopathologic and molecular portrait of spindle cell/sclerosing rhabdomyosarcoma with emphasis on retroperitoneal cases.

INTRODUCTION: Spindle cell and sclerosing rhabdomyosarcoma (Sc/SRMS) are rare histologic subtypes of rhabdomyosarcoma, increasingly recognized for their distinct molecular profiles and aggressive clinical behavior. Retroperitoneal involvement is exceptionally uncommon and poorly characterized. To characterize the clinical, pathological, and molecular features of retroperitoneal Sc/SRMS through a combined institutional case series and individual patient data (IPD) meta-analysis. METHODS: We retrospectively analyzed 12 adult patients with retroperitoneal Sc/SRMS treated at Peking University Int

Meckel's Diverticulum as a Rare Etiology of Small-Bowel Obstruction in an Otherwise Healthy Adult: A Case Report.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract and is often asymptomatic in adults. When symptomatic, adult presentations frequently involve obstruction or inflammation and can be difficult to diagnose preoperatively due to nonspecific imaging findings. We report a case of a healthy 31-year-old male with progressive abdominal pain, distention, and emesis, ultimately found to have a high-grade small-bowel obstruction refractory to conservative management. Diagnostic laparoscopy identified a Meckel's diverticulum tethered to the retroperitoneum at the t

Occult Small Bowel Injury With Delayed Retroperitoneal Abscess Formation After Blunt Trauma: A Case Report.

Blunt abdominal trauma can result in bowel injury, but diagnosis is often delayed because early clinical and radiological findings are non-specific. We report a 64-year-old man with pelvic fractures following a road traffic collision who was initially haemodynamically stable, with no definite gastrointestinal injury on computed tomography. Subsequent imaging demonstrated retroperitoneal gas adjacent to a psoas haematoma, without clear evidence of hollow viscus perforation. Despite remaining clinically well, he developed sepsis on day seven post-injury. Repeat imaging revealed progression of re

Radiofrequency Ablation in the Treatment of Chronic Radiation Enteritis.

INTRODUCTION: Radiation enteritis is the most common complication during radiotherapy for malignant tumors in the abdominal cavity, retroperitoneum, and pelvis. Bleeding is one of the common and severe complications in patients with radiation enteritis (Eur J Med Res 30(1):23, 2025). Endoscopic radiofrequency ablation (RFA) has emerged as a potential treatment, but data on its efficacy and safety are limited. This study conducted a retrospective analysis of the clinical data of patients with radiation enteritis whom we treated, aiming to evaluate the efficacy and safety of radiofrequency ablat
