Enteric fever remains a public health challenge. We analyzed data from a cluster-randomized Vi-tetanus toxoid conjugate vaccine trial to compare the epidemiology between Salmonella enterica serovars Paratyphi A, which causes paratyphoid fever, and Typhi, which causes typhoid fever. The overall incidence rate of paratyphoid fever was 27 (95% CI 23-32)/100,000 person-years (PY) and of typhoid fever was 216 (95% CI 198-236)/100,000 PY. We observed the highest incidence for both diseases in children 2-4 years of age: 72 (95% CI 41-117)/100,000 PY for paratyphoid and 887 (95% CI 715-1,088)/100,000

Typhoid intestinal perforation (TIP) is a life-threatening, late complication of typhoid fever that disproportionately impacts children in low resource settings and continues to have devastating consequences worldwide. Despite elimination of typhoid fever in most high income countries, typhoid fever and TIP remain endemic in many countries around the world as a result of inadequate investments in water, sanitation, and hygiene (WASH) and lack of access to vaccines. A first National Typhoid Conference was held in Niamey, Niger on July 22, 2023, where surgeons and other medical and health profes

Typhoid fever remains a major public health threat in low- and middle-income countries (LMICs), where inadequate access to clean water and sanitation drives recurrent outbreaks. With antimicrobial resistance on the rise, the urgency of deploying preventive strategies such as typhoid conjugate vaccines (TCVs) have grown. In this study, we developed a dynamic compartmental model calibrated to the 2015 typhoid outbreak in Kampala, Uganda, to assess the health and economic outcomes of various outbreak response immunization (ORI) strategies using TCVs. We aimed to identify optimal ORI strategies th

In sub-Saharan Africa, typhoidal and non-typhoidal Salmonella (NTS) are leading causes of invasive disease among young children. Trivalent Salmonella conjugate vaccine (TSCV) consists of Vi capsule polysaccharide conjugated to tetanus toxoid and core-plus-O-polysaccharides from the two most prevalent invasive NTS serovars conjugated to flagellin subunits. We conducted a first-in-human, randomized, placebo-controlled, phase 1 trial evaluating the safety and immunogenicity of TSCV. A total of 22 healthy adults aged 18-45 years were randomly allocated to 6.25-µg TSCV (n = 8), 12.5-µg TSCV (n = 10

Typhoid fever, a type of enteric fever transmitted through consumption of food or water contaminated with Salmonella enterica subspecies Typhi, continues to cause illness in Asia. Since the World Health Organization recommended use of typhoid conjugate vaccines in 2018, several countries have introduced or begun the decision-making process to use the vaccine in campaigns or by introduction into the routine immunization schedule. This paper describes the disease burden and vaccine implementation policy setting in the region, based on presentations and discussions at the second Asia Regional Mee

INTRODUCTION: Typhoid fever, caused by Salmonellaenterica serovar Typhi (S. Typhi), can be transmitted via faecal-oral route. Although significant reduction in incidence from 11 million in 2017 to 7 million in 2021, typhoid fever remains a global health problem. The World Health Organization (WHO) recommended implementation of typhoid conjugate vaccine (TCV) in endemic countries. However, only ten countries have introduced TCV into their national immunisation programs to date. This study aims to estimate the real-world effectiveness of TCV to inform evidence-based decision making and support i

During Napoleon's retreat from Russia in 1812,1 countless soldiers of the French army succumbed to infectious diseases, but the responsible pathogen or pathogens remain debated.2,3,4,5 We recovered and sequenced ancient DNA from the teeth of 13 soldiers who, based on historical records, likely died from infectious diseases, aiming to identify the pathogens responsible for their deaths.6 Our results confirmed the presence of Salmonella enterica subsp. enterica belonging to the lineage Paratyphi C, the causative agent of paratyphoid fever7; and Borrelia recurrentis, responsible for relapsing fev

BACKGROUND: Salmonella enterica serovar Paratyphi A (also known as S. Paratyphi A) is responsible for more than 2 million cases of enteric fever annually. There are no licensed vaccines against S. Paratyphi A.
METHODS: In a double-blind, randomized, placebo-controlled trial, we evaluated an orally administered live, attenuated S. Paratyphi A vaccine (CVD 1902) using a controlled human infection model. Healthy U.K. adults were assigned in a 1:1 ratio to receive two doses of CVD 1902 or placebo 14 days apart. Twenty-eight days after the second dose, participants were challenged orally with S. P

Splenic infarction is an uncommon diagnosis, often overlooked due to its non-specific clinical presentation. While a splenic abscess is common, infarction due to infective processes is rare. Among infections, Salmonella is infrequently implicated, and splenic infarct secondary to typhoid or paratyphoid fever is exceptionally rare. We report the case of a young adult man who presented with acute fever and pain in the abdomen and was subsequently diagnosed with splenic infarction. A meticulous diagnostic workup excluded common causes such as infective endocarditis, hematological malignancies, hy

Salmonella enterica serovar Paratyphi A is a significant but under-characterised cause of enteric fever in South Asia. In Pakistan, where the typhoid conjugate vaccine has been introduced to combat S. Typhi, S. Paratyphi A remains a prominent cause of bacteraemia, raising concerns about shifts in disease burden and antimicrobial resistance (AMR). Here, we provide a comprehensive genomic and phylogenetic analysis of 354 S. Paratyphi A isolates collected from three provinces in Pakistan between 2017 and early 2022. Whole-genome sequencing revealed the dominance of genotypes 2.3.3 and 2.4.5, indi

Salmonella Paratyphi B (SPB) has become endemic in the city of Salta, Argentina, with a staggering burden of bloodstream infections requiring hospitalization. This study unravels the underlying drivers of this endemicity through an integrated clinical-environmental approach. A novel duplex qPCR system was developed to accurately distinguish SPB from other Salmonella serotypes, enabling precise diagnosis and treatment. Clinical analysis revealed SPB in 98 % of positive blood cultures, confirming its strong association with severe infections. Contrary to initial hypotheses, the public drinking w

BACKGROUND: Updated estimates of the prevalence of complications and case-fatality ratio (CFR) among patients with typhoid fever are needed to inform typhoid fever prevention and control. To support country-level decisions on typhoid prevention and control with contemporary estimates of morbidity and mortality, we updated our 2020 review.
METHODS: We performed a systematic review and meta-analysis of non-surgical (typhoid fever) and surgical (typhoid intestinal perforation [TIP]) observational studies and control groups of vaccine trials that reported on typhoid fever complications or mortali

Antimicrobial resistance (AMR) is considered one of the top 10 threats to global public health and development. Opportunistic bacteria such as Enterobacter cloacae have been reported to acquire resistance determinants, making them pathogenic reservoirs and a threat to health and most are on the path of becoming superbugs. These bacteria are commonly isolated along with pathogens from the stool and urine of patients diagnosed with typhoid fever, paratyphoid fever, gastroenteritis, urinary tract infection, and bloodstream infection or sepsis. The E. cloacae strain EC78 studied here is a metageno

Enteric fever, a systematic bacterial infection caused by Salmonella Typhi and Paratyphi, continues to impose a significant public health burden in low and middle-income countries, yet our understanding of the serum antibody dynamics following infection remains incomplete. Although previous work has characterized the longitudinal seroresponses following acute typhoid infection, gaps persist in deciphering how repeated exposures influence antibody decay and protection. In our longitudinal cohort study of blood culture-confirmed enteric fever cases enrolled in Bangladesh, Nepal, and Pakistan, we

The global health burden from enteric fever, caused by Salmonella enterica serovars Typhi and Paratyphi A, is exacerbated by the emergence of drug resistance. This growing resistance undermines the effectiveness of empirical antibiotic treatments and narrows the options for definitive antimicrobial therapy. Presence of vaccines against typhoidal strains has caused an evolutionary selection of paratyphoid strains, causing rising incidence of paratyphoid fever, particularly in the Southeast Asian countries. This calls for a bivalent vaccine against both the serotypes to provide a broad-spectrum

Nepal has a high burden of enteric fevers. An observational study conducted in a tertiary-level hospital in Nepal over 11 years (2014-2024), reviewing hospital records of all blood culture-positive cases of Salmonella Typhi and Paratyphi in children, showed a gradual decline in blood culture-positive enteric fever cases with year-to-year variability. Typhoid conjugate vaccine (TCV) was introduced into the national immunisation schedule of Nepal in 2022. We observed that the proportion of Salmonella Paratyphi cases has increased in the post-typhoid conjugate vaccine years compared with the pre-
