Epidemiology, clinical manifestation, diagnosis, and treatment of bursitis iliopectinea: A systematic review.

BACKGROUND: Bursitis iliopectinea (BI) is a condition that is characterized by swelling of the iliopsoas bursa, leading to compression of inguinal neurovascular structures, causing swelling, pain, paresthesia, or thrombosis of the leg. QUESTIONS: Due to the rare occurrence of BI, the available literature consists of case reports. Our study aims to systematically review the literature for a comprehensive analysis of the etiology, treatment modalities, and clinical outcomes of patients with BI. METHODS: We systematically analyzed 217 studies with 502 cases of BI and extracted information about

Staged Correction of Hip Contractures, Severe Knee Flexion, and Clubfoot in Arthrogryposis: Enabling Assisted Ambulation.

CASE REPORT: A 10-year-old boy with arthrogryposis multiplex congenita (AMC) presented in a "Buddha position" with severely limited mobility, including bilateral hip, severe knee flexion, and clubfoot deformities. Surgical correction involved (1) bilateral femoral derotation and shortening osteotomies, knee capsulotomies, and soft-tissue releases; (2) gradual left knee extension using an external fixator; and (3) distal femoral extension osteotomies and Verebelyi-Ogston procedures for clubfoot correction. Postoperative recovery involved intensive rehabilitation and orthotic support. Improved l

Orthopedic surgical treatment of osteomalacia induced by culprit soft tissue tumor in the hip region: a single-center retrospective study.

BACKGROUND: Due to its occult position, complex anatomical structure, and spatial relationships, the causative tumor of Tumor-Induced Osteomalacia (TIO) in the hip region is quite difficult to detect and qualitatively diagnose in clinical practice. In this regard, clinicians often lack sufficient knowledge about such tumors, leading to frequent missed diagnoses, misdiagnoses, and unreasonable treatment. OBJECTIVE: This study aimed to investigate the clinical characteristics of TIO patients with culprit soft tissue tumors in the hip region and evaluate the effect of surgical treatment on these

Extraskeletal myxoid chondrosarcoma: Clinical features and overall survival.

INTRODUCTION: Extraskeletal myxoid chondrosarcoma is a rare form of soft tissue sarcoma characterized by a unique chromosomal translocation involving the NR4A3 gene on chromosome 9. It is most frequently diagnosed in the proximal extremities of older adult males and is notable for its insidious growth with predilection for local recurrence and metastasis. Currently, extraskeletal myxoid chondrosarcoma is managed with wide resection, with recent investigations supporting the utility of adjuvant radiation and novel chemotherapeutic strategies. METHODOLOGY: A retrospective study was performed wi

Soft Tissue Sarcoma of Extremities: Descriptive Epidemiological Analysis According to National Population-based Study.

BACKGROUND: Soft-Tissue Sarcoma (STS) is a heterogeneous group of neoplasms of mesenchymal origin, occurring in connective tissues. According to previously conducted studies, STS accounts for approximately 1% and 7-%15% of adult and pediatric malignancies, respectively. Almost 50%-60% of sarcomas arise from extremities and usually present as a large painless or rarely painful soft-tissue mass. The present study aimed to describe the epidemiology of soft-tissue sarcomas, especially in the Iranian population. METHODS: This epidemiological study of limb soft-tissue sarcoma was conducted based on

Is the Addition of Anaerobic Coverage to Perioperative Antibiotic Prophylaxis During Soft Tissue Sarcoma Resection Associated With a Reduction in the Proportion of Wound Complications?

BACKGROUND: Wound complications are common after resection of soft tissue sarcomas, with published infection rates ranging from 10% to 35%. Multiple studies have reported on the atypical flora comprising these infections, which are often polymicrobial and contain anaerobic bacteria, and recent studies have noted the high prevalence of anaerobic bacterial infections after soft tissue sarcoma resection [ 26, 35 ]. Based on this, our institution changed clinical practice to include an antibiotic with anaerobic coverage in addition to the standard first-generation cephalosporin for prophylaxis dur

Editorial Commentary: The Pelvis is the Lowest Vertebral Level: Diagnostic Approach to Hip-Spine Syndrome.

The human pelvis represents a wonderful example of apparent idealistic simplicity overwhelmed by realistic complexity. Traditionally, the pelvis has been termed a "ring" linking the lower extremity to the spine via the sacroiliac joint. In essence, the pelvis is the lowest vertebral level-"the hip bone's connected to the spine bone." Thus, the law of parsimony seemingly applies in the diagnosis and management of both arthritic and nonarthritic hip and spine disorders in isolation or combination. However, an inverse Occam's razor is much more likely. The layered theory of hip disorders illustra

Does the L5 spinal nerve move? Anatomical evaluation with implications for postoperative L5 nerve palsy.

PURPOSE: While palsy of the L5 nerve root due to stretch injury is a known complication in complex lumbosacral spine surgery, the underlying pathophysiology remains unclear. The goal of this cadaveric study was to quantify movement of the L5 nerve root during flexion/extension of the hip and lower lumbar spine. METHODS: Five fresh-frozen human cadavers were dissected on both sides to expose the lumbar vertebral bodies and the L5 nerve roots. Movement of the L5 nerve root was tested during flexion and extension of the hip and lower lumbar spine. Four steps were undertaken to characterize these

Minimally invasive anterior muscle-sparing versus a transgluteal approach for hemiarthroplasty in femoral neck fractures-a prospective randomised controlled trial including 190 elderly patients.

BACKGROUND: The relevance of femoral neck fractures (FNFs) increases with the ageing of numerous societies, injury-related decline is observed in many patients. Treatment strategies have evolved towards primary joint replacement, but the impact of different approaches remains a matter of debate. The aim of this trial was to evaluate the benefit of an anterior minimally-invasive (AMIS) compared to a lateral Hardinge (LAT) approach for hemiarthroplasty in these oftentimes frail patients. METHODS: Four hundred thirty-nine patients were screened during the 44-months trial, aiming at the evaluatio

Magnetic resonance imaging of pelvis and hips in infants, children, and adolescents: a pictorial review.

Due to its superior tissue resolution, magnetic resonance imaging has become the modality of choice for assessing pathologies of soft tissues, joints, cartilage, and bone marrow. This is also true in the pediatric population. This article reviews magnetic resonance imaging appearances and techniques for a variety of congenital abnormalities and acquired conditions including posttraumatic, infectious, inflammatory, ischemic, metabolic, benign, and malignant processes.

Strength around the hip and flexibility of soft tissues in individuals with and without patellofemoral pain syndrome.

STUDY DESIGN: Case control design. OBJECTIVES: To investigate whether differences exist in lower extremity muscle strength and soft tissue length between patients with patellofemoral pain syndrome (PFPS) and age- and gender-matched control subjects. BACKGROUND: Based on our clinical experience and emerging data, impairments such as muscular weakness surrounding the hip and limited flexibility of key lower extremity muscles may be important impairments to consider in the conservative management of PFPS. METHODS AND MEASURES: Thirty patients with PFPS and 30 age- and gender-matched controls w
