Volumetric phase-specific three-dimensional radiomics and machine learning for differentiation of non-mass enhancement in breast magnetic resonance imaging.

PURPOSE: Non-mass enhancement (NME) in breast magnetic resonance imaging (MRI) is a diagnostically challenging entity due to overlapping benign and malignant features, observer variability, and high false-positive rates. This study evaluated the diagnostic performance of three-dimensional (3D) volumetric radiomics and multiple machine learning (ML) algorithms, using early (2nd) and late (7th) post-contrast phases to differentiate benign from malignant NMEs. METHODS: A total of 110 NMEs (86 benign and 24 malignant) from 108 patients were analyzed. Radiological features were recorded. Radiomics

Rare overlap of subtype and phenotype: HER2-positive pleomorphic invasive lobular carcinoma presenting with an inflammatory breast cancer phenotype: a case report and targeted treatment strategy.

Pleomorphic invasive lobular carcinoma (PLC) is a rare and aggressive subtype of invasive lobular carcinoma. When it presents with an inflammatory breast cancer (IBC) phenotype, due to its rarity, this can lead to substantial diagnostic and therapeutic challenges. Herein, we report the case of a 53-year-old woman presenting with a rapidly enlarging left breast mass, diffuse edema, erythema, and pain; these symptoms have affected the entire breast over the course of one month. Imaging revealed diffuse parenchymal abnormalities and nodal involvement, while biopsy indicated PLC, histologic grade

Concurrent therapy-related acute myeloid leukemia and lymph node tuberculosis following treatment for lung squamous cell carcinoma: a case report and literature review.

Concurrent development of therapy-related acute myeloid leukemia (t-AML) and lymph node tuberculosis (LNTB) following comprehensive anti-tumor therapy for locally advanced lung squamous cell carcinoma (LSCC) is extremely rare in clinical practice. This is not a new biological concept but represents a rarely documented clinical scenario in the setting of neoadjuvant chemoimmunotherapy, surgery, and anti-PD-1 maintenance therapy. Herein, we systematically summarize the clinical features, pathogenesis and individualized therapeutic strategies of these two concurrent rare complications based on a

A multi-modal deep learning framework for enhanced breast cancer diagnosis using mammograms and clinical data.

The fatality due to breast cancer is one of the most universal causes among women worldwide. The limited interpretation is due to tissue overlapping, high breast density and delicate lesion characteristics even though mammography is the standard screening tool. To overcome this universal challenges, we propose a multi-model deep learning framework that integrates mammographic image with structured clinical data (note: synthetic clinical variables were employed based on the known distributions of the risk factors due to the lack of real paired data) to achieve the accuracy in the diagnostics. A

Progress in Ultrasound Research on Non-Mass Breast Lesions: Definition, Classification, and Differential Diagnosis.

The objective of this review is to deepen the understanding and mastery of non-mass breast pathologies, enabling ultrasonographers to enhance diagnostic accuracy and improve their capabilities in image analysis and clinical interpretation. Narrative means have been used to synthesize evidence in this review. Currently, "non-mass breast lesions" are not included in ultrasound terminology of the 5th Edition Breast Imaging Reporting and Data System (BI-RADS). Although multiple classification systems have been proposed in the literature, there remains no standardized ultrasound definition or malig

MassSeg-Framework: A Breast Mass Detection and Segmentation Framework Based on Deep Learning and an Active Contour Model.

This work introduces the MassSeg-Framework, a fully automatic two-stage pipeline for breast mass analysis in mammography that integrates YOLOv11-based detection with Chan-Vese ACM refinement to achieve accurate mass localization and segmentation with a lightweight computational footprint. The framework was trained and evaluated on two publicly available datasets using consistent experimental protocols. In the detection stage, YOLOv11-nano was the most effective architecture, with a confidence threshold of 0.4, achieving statistically significant mAP50 values of 0.862 and 0.709 on the dINbreast

Mucinous carcinoma of the breast: morphological spectrum, diagnostic pitfalls and classification challenges.

BackgroundMucinous breast carcinoma (MBC) is a well-recognised special type of invasive breast carcinoma (BC) characterised by abundant extracellular mucin and, in its classical form, a favourable clinical behaviour. However, extracellular mucin production is not unique to classical MBC and extracellular mucin is associated with a spectrum of benign and malignant entities with varying and overlapping morphological patterns but with very different clinical outcomes. Failure to recognise this diversity risks diagnostic imprecision and inappropriate management decisions. This review adopts a clini

Decoding object categories from electroencephalography during free viewing reveals early information evolution compared to passive viewing.

Object processing is central to visual perception, and its neural substrates inform cognitive and computational models of vision. Electroencephalographic (EEG) research has increasingly adopted multivariate pattern analysis (MVPA) to decode distributed neural representations, moving beyond average signal changes and predefined electrodes. Yet, most MVPA studies use passive viewing paradigms that constrain eye movements and evoke brain activity by external stimuli presentation, creating an artificial dissociation between perception and action. Here, we examine object category representations du

Discriminating inflammation from malignancy with short-dynamic patlak parametric 18 F-FDG PET/CT.

PURPOSE: Differentiating malignant from inflammatory uptake on 18F-FDG PET/CT remains a major diagnostic challenge, as standardised uptake value (SUV) lacks specificity. This study evaluated whether parametric imaging from short-duration dynamic FDG PET/CT provides complementary information beyond SUV for distinguishing malignancy from inflammation. METHODS: Twenty-eight patients undergoing oncologic PET/CT (breast, lung, lymphatic or gastrointestinal cancer) were included, yielding 68 lesions (43 malignant, 25 inflammatory). Short dynamic acquisitions (20 min) were motion-corrected and used

Breast core needle biopsy diagnosis and personalized treatment: navigating pitfalls and challenging lesions.

Most breast lesions can be diagnosed accurately on core needle biopsy (CNB); however, certain entities remain diagnostically challenging due to limited tissue sampling, morphological overlap with other lesions, unusual immunoprofiles or lack of reliable ancillary markers. These challenges are compounded by modern clinical requirements, particularly the increasing shift towards neoadjuvant therapy (NAT) where a definitive diagnosis is required before surgical excision. Misclassification may have clinically significant consequences, including inappropriate initiation of NAT or undertreatment of

Deep learning-based spatiotemporal estimation of lesion changes for patient-level assessment of breast cancer lung metastases on longitudinal CT.

Clinical management of breast cancer lung metastasis is challenging because of the complexity of dynamic lesion assessment. Traditional methods based on RECIST1.1 rely on size measurement, and existing studies require image registration and are limited to lesion-level assessment. In this study, we proposed a patient-level spatiotemporal assessment framework without registration to comprehensively analyze multiple lesions evolvement based on longitudinal CT images. Our method considers metastatic lesions that vary in size and often overlap with complex structures such as blood vessels and bones

Metastatic brachial plexopathy presenting as radiation-induced plexopathy in metastatic lung cancer: a case report and literature review.

BACKGROUND: Metastatic brachial plexopathy (MBP) is a rare but debilitating complication of advanced malignancies, most commonly associated with lung and breast carcinomas. Distinguishing MBP from radiation-induced brachial plexopathy (RIBP) represents a significant clinical challenge due to their overlapping clinical presentations and similar appearances on structural imaging. While magnetic resonance imaging (MRI) is considered the gold standard for evaluating the brachial plexus (BP), it often lacks the sensitivity required to definitively differentiate between malignant infiltration and po

Clinical significance of lesion conspicuity on contrast-enhanced mammography.

OBJECTIVE: To determine if lesion conspicuity on contrast-enhanced mammography (CEM) is independently associated with malignancy. MATERIALS AND METHODS: This retrospective, single-institution study identified consecutive abnormal screening and diagnostic CEMs between January 2019 and December 2021. The conspicuity of enhancing lesions on CEM recombined images was graded (low, moderate, or high) by one or both breast radiologists assigned as readers for the study, blinded to two-year imaging follow-up or biopsy pathology results. Primary breast leiomyosarcoma-pathological challenges and immunohistochemical insights into the diagnosis of a rare tumor: a case report.

Primary leiomyosarcoma of the breast is an extremely rare malignancy, accounting for less than 1% of all breast tumors. Diagnosis is challenging because its morphology overlaps with that of other spindle cell lesions, and standardized treatment guidelines are currently unavailable. A 44-year-old woman presented with a rapidly enlarging, firm, 15×15 cm mass in the left breast. She had previously undergone surgery elsewhere, with diagnoses of leiomyoma and desmoid-type fibromatosis. Imaging demonstrated a lobulated mass without evidence of metastasis. Excision revealed pleomorphic spindle cells

Bilateral primary diffuse large B-cell lymphoma of the breast: diagnostic and surgical management-a case report.

BACKGROUND: Primary breast lymphoma (PBL) is a rare extranodal lymphoma of which diagnosis is challenging due to overlapping clinical and imaging features with breast carcinoma. While the existence of PBL is known, the diagnostic significance of specific sonographic patterns like the "grid-like echo" and the critical imperative for exhaustive systemic staging to differentiate primary from systemic lymphoma warrant emphasis. This case is unique as it demonstrates a diagnostic journey from a bilateral "grid-like echo" on ultrasound to a revised diagnosis of a systemic, high-grade lymphoma, highl
