Hyperkeratosis of the nipple and areola has been classically divided into three categories: a part of an epidermal nevus, a type associated with ichthyosis, and a nevoid form seen in young women. A case of hyperkeratosis of the nipples in a patient with adenocarcinoma of the prostate treated with diethylstilbestrol is described herein. A possible correlation between estrogens and an acquired form of this rare skin disorder is suggested.

This report provides a histological review of 264 female primary breast cancers analyzed for estrogen receptor protein (ER). We also describe 5 cases of male breast cancer all of which bound estradiol specifically. Nuclear diameters of breast cancer cells obtained by aspiration cytology from 245 patients were measured by ocular micrometry. The nuclei of 50 benign lesions were also measured. Cancers with 80% or more of their nuclei larger than 12 mu were classified as "large nuclear type" (LN). Those with nuclei of 12 mu or less were considered "small nuclear type" (SN). In 40 breasts with primary carcinoma, the nipple and areola were cut horizontally in order to investigate the frequency of intraductal and invasive cancer. In 20 of these breasts the nipple and/or areola were found to be involved at a depth of 1 cm (50%; 95% confidence limits: 33.8-66.2%). Fourteen primary melanomas arising in the nipple and areola of the breast were treated by mastectomy and axillary dissection. Four patients had axillary lymph node metastases and all were dead within 3 years of their operation, while the 10 patients with no axillary node involvement were free from recurrent disease 5 years after their operation. On the basis of clinical and anatomical studies, it is suggested that a wide local excision without mastectomy is adequate for the treatment of nipple and areola melanomas.

Thirty-one consecutive cases of mammographic isolated, clustered microcalcifications which prompted breast biopsy were studied by radiologic and histopathologic methods. Isolated, clustered microcalcifications were sensitive, but nonspecific cancer markers; 35% of the lesions were malignant and half of these were noninvasive. Radiologic and histopathologic similarities, and overlap between benign and malignant disease, prevented accurate differentiation. Breast biopsy is indicated when isolated, clustered microcalcifications are discovered by mammography.

A method to reconstruct the breast, nipple, and areola after mastectomy is described. The importance of symmetry is emphasized. A split-skin graft is used to reconstruct both (not one) areolae; this provides almost complete symmetry in terms of size, texture, and color. Ultraviolet light is used postoperatively to hyperpigment the skin-graft areolae, and the long-term appearance has been excellent. The method is quite simple, technically.

Hyperkeratosis of the nipple and areola is a rare condition that may occur in association with various forms of ichthyosis or as the result of extension of an epidermal nevus. The nevoid form is extremely rare and appears most commonly in women in their second or third decade of life. It is characterized by verrucous thickening and brownish discoloration of both nipples and areolae. Thirteen cases of this type have been reported. We are describing two additional cases to bring the total to 15 instances.

Seventeen women who had had a mastectomy for cancer of the breast underwent reconstructions. Alloplastic implants were used in all. Preservation of the nipple and areola was possible in some of these patients. The normal (or uninvolved) breast sometimes required reduction in size or reshaping, to match as nearly as possible the reconstructed breast. The conditions suitable and unsuitable for mammary reconstruction, after mastectomy for cancer, are discussed.

We present a method for reconstructing the nipple and areola bilaterally in one stage, one which produces a corrugated, elevated nipple and a glabrous pigmented areola. A satisfactory appearance has been maintained in 4 cases during a follow-up period of up to two years.
