Reducing postoperative psychological morbidity is an important goal in breast cancer patient care; a multidisciplinary approach is crucial in order to select candidates for immediate breast reconstruction. There is no oncologic reason for a delay after a modified radical mastectomy. Our procedure allows immediate breast reconstruction without significantly prolonging operative time and hospitalization. Complete muscular coverage of the implant and shifting of the redundant skin to the mastectomy site minimize the risk of postoperative complications and provide a natural looking reconstructed b

Seventy-nine patients of 100 desired a nipple-areola reconstruction. In nipple-areola reconstruction, skin from the upper inner thigh gives the best results as areola replacement, except perhaps in women with very large contralateral areola. Nipple-sharing was the most favourable method for reconstruction of the nipple. Mushroom nipple plasty could give satisfactory results when nipple sharing was not feasible. There were no early complications of nipple-areola reconstruction, which often can be done on an out-patient basis. Most of a series of women who underwent breast reconstruction thought

Tuberous breasts have been attributed to hypoplasia of the breast with herniation of breast tissue into the nipple and areola. Several procedures have been used to correct the condition. A new method has been designed that reduces breast tissue herniation and areolar size. The surgery is done after subpectoral augmentation, by deepithelialization of a doughnut-shaped piece of skin around the periphery of the areola and excision of four wedges of breast tissue from underneath the new areola. Closure of the gaps results in reduction of both the herniated breast tissue and areola size.

A modification of the original inferior flap reduction mammaplasty is presented using a dermal pedicle based on the inferior flap to support the nipple and areola complex. The technique is extremely versatile, and the breast contour is excellent. The nipple shows excellent projection and generally improved sensation over the preoperative state.

Usually hyperkeratosis of the nipple and areola is associated with ichthyosis and epidermal nevus. The patient's lesions will resemble acanthosis nigricans but histologically suggest a completely different entity. Presented is a case of hyperkeratosis of the nipples and areolae with no association with ichthyosis or epidermal nevus.

The problem connected with reduction-mammoplasty consists of preserving a sufficient circulation of nipple and areola. A pedicled cutis fatty flap in the direction of the submammary fold prevents this complication. The surgical technique is described in detail. This pedicled flap is also in use for subcutaneous mastectomy in cases of macromastia with a surplus of skin to be removed. The pedicled flap prevents the dislocation or turning of the silicone-prosthesis.

In the authors' opinion there is no physical reason for a waiting period after total mastectomy before the breast is reconstructed. If the duration of the patient's survival is limited, the sooner the breast is reconstructed the better. The authors do not use silicone implants that may be blamed for the appearance of recurrent tumour but reconstruct the breast with a vertical rectus abdominis myodermal flap immediately after the mastectomy. The flap is an island of skin, fat and muscle extending from the xiphoid to the pubis; it receives its blood supply from the perforators of the superior-in

In a series of 80 consecutive mastectomies it was investigated histologically, by a horizontal sectioning technique, how often the nipple and/or areola were involved in breast cancer. Such involvement was found in 35 cases (43.8%). In the same series the value of the clinical findings as to involvement of the nipple and/or areola was assessed. Only 6 of the 35 cases had shown clinical changes that could give rise to a suspicion of spread to the nipple and/or areola.

Considering the female breast to be a secondary sexual organ the sensibility of nipple and areola gains essentially in functional significance. This sensory function is impaired as well by reduction mammaplasty. Thus all different operation methods propagated in recent years have to be rated as to functional and esthetic results with regard to resection weight. The optimal operation method presupposes: 1. Satisfactory esthetic result, 2. optional reductional possibility up to subcutaneous mastectomy, 3. minor sensory impairment of nipple and areola.

Despite careful preoperative planning and accurate surgery, nipple necrosis may follow reduction mammaplasty. Impending nipple necrosis developed following translocation of the nipple and areola on a dermal pedicle in two patients. An excellent result was obtained by converting the nipple-areola complex to a full-thickness graft and removing the avascular portion of the breast. We recommend this technique to the surgeon whose patients develop impending nipple necrosis after reduction mammaplasty.

Forty-five surgical mastectomy specimens obtained during resection of carcinoma of the breast were evaluated for involvement of the nipple and/or areola by cancer. Such involvement was found in 11 specimens (25%). Tumor size, relative distance of the tumor from the nipple, and tumor histologic type were investigated and compared with nipple-areolar involvement. The high incidence of nipple involvement found here indicates a definite risk of recurrence for surgical procedures that preserve the nipple.

The technique for the application of areola and nipple in the mastectomy for the mammary carcinoma is proposed. It is pointed out that the preservation of the nipple with its areola is psychologically important for the patient. It is also showed how the application of a silicone prosthesis, volumetrically restoring the extirpated tissue, is afterwards possible. During the operation, a disk of tissue is drawn under nipple and areola and histologically tested; if this tissue certainly results without neoplastics cells, the reapplication is achieved.

Benign lymphocytic infiltrations may often occur in the skin of nipple and areola: 55 cases have been described in literature especially in Scandinavia and in Germany, sometimes in correlation with tick bites or erythema chronicum migrans, but until now no cases have been reported in France. In a retrospective study of 30 cutaneous lymphocytomas, we found 8 cases localized on the breast-3 children and 5 older patients-, associated with a erythema chronicum migrans in 3 patients and following a tick bite in another patient. The course of the disease is always benign and the healing can be accel

The authors present 8 different methods for reconstruction in order to determine a technique that best creates a nipple-areola complex after reconstruction of the breast mound. The main principles are: 1. preservation of the nipple-areola complex is possible, 2. sharing of the remaining nipple and areola, 3. Use of full thickness graft from other parts of the body and tattooing the areola. Our experience from more than 200 patients is discussed.

In a prospective analysis of 291 specimens taken at mastectomy for carcinoma, 48 contained tumor in the nipple-areola complex. The characteristics of greater tumor size, higher frequency of lymph node involvement, midbreast tumor location and multifocality of tumor correlated well with the probability of involvement of the nipple. The findings emphasize the importance of close clinical and histopathologic examination of the nipple and areola when preservation of the nipple is being considered.

Hyperkeratosis of the nipple and areola has been classically divided into three categories: a part of an epidermal nevus, a type associated with ichthyosis, and a nevoid form seen in young women. A case of hyperkeratosis of the nipples in a patient with adenocarcinoma of the prostate treated with diethylstilbestrol is described herein. A possible correlation between estrogens and an acquired form of this rare skin disorder is suggested.

The prospect of surgical body contouring is a rather recent innovation for the surgeon. For underdeveloped or atrophic breasts, augmentation mammaplasty is reasonably successful. For ptosis and/or hypertrophy of the breast, reduction and elevation on the chest wall by making the breasts more conical and more projecting is a common procedure for the plastic surgeon. Breast reconstruction is now done after either simple or radical mastectomy, and subcutaneous mastectomy procedures, preserving the nipple and areola, are also commonly done for disease of the breast which is not yet malignant. The

Once reconstruction of the breast mound has been accomplished following an ablative procedure, reconstruction of the nipple and areola provides added dimension to the psychological well-being of the patient. A number of techniques are available to accomplish this satisfactorily. An areola can be created by tattooing; areolar sharing; grafting from any skin area, especially the medial thigh, labium minora or minora-majora, and the scrotum in men; and by banking the areola and nipple at the time of breast removal. The nipple can be constructed from grafts from the opposite nipple, toe tip, ear l

In 40 breasts with primary carcinoma, the nipple and areola were cut horizontally in order to investigate the frequency of intraductal and invasive cancer. In 20 of these breasts the nipple and/or areola were found to be involved at a depth of 1 cm (50%; 95% confidence limits: 33.8-66.2%). Eleven neoplasms were purely intraductal, eight intraductal as well as stromal, and only one purely stromal. By means of clinical or physical findings, it was not possible to select the breasts in which the nipple and/or areola were not involved. It is concluded that the general use of surgical methods prese

Fourteen primary melanomas arising in the nipple and areola of the breast were treated by mastectomy and axillary dissection. Four patients had axillary lymph node metastases and all were dead within 3 years of their operation, while the 10 patients with no axillary node involvement were free from recurrent disease 5 years after their operation. On the basis of clinical and anatomical studies, it is suggested that a wide local excision without mastectomy is adequate for the treatment of nipple and areola melanomas.
