Pigmented basal cell carcinoma of the nipple-areola complex mimicking malignant melanoma in a 61-year-old Thai woman: a case report and literature review.

BACKGROUND: Pigmented lesions of the nipple-areola complex (NAC) are uncommon and may pose a diagnostic challenge because they can mimic malignant melanoma and other NAC conditions. Pigmented basal cell carcinoma (BCC) arising in this sun-protected site is particularly rare and may be overlooked, especially in Asian patients. This case highlights the diagnostic overlap between melanoma-like clinical features and pigmented BCC in the NAC, which can create uncertainty during initial evaluation. CASE DESCRIPTION: A 61-year-old Thai woman presented with a 1-year history of a progressively enlargi

Synchronous breast carcinoma and diffuse large B-cell lymphoma: a case report and literature review on diagnostic challenges and management implications.

BACKGROUND: Synchronous breast carcinoma and diffuse large B-cell lymphoma occurring as independent primary malignancies is exceptionally rare. Because these tumors differ markedly in histological origin, biological behavior, and treatment strategies, their coexistence can easily lead to diagnostic pitfalls and therapeutic dilemmas. Reporting such a case provides new insights into the clinical recognition and management of rare dual primaries. CASE PRESENTATION: A 54-year-old woman presented with a painless left cervical (neck) mass. Imaging revealed a left breast lesion with multi-station lym

A case of giant juvenile fibroadenoma in an adolescent girl: a case report.

BACKGROUND: Breast masses in adolescents are predominantly benign and are most commonly associated with fibroepithelial tumors. These tumors represent a heterogeneous group of biphasic lesions, ranging from benign fibroadenomatoid changes to malignant phyllodes tumors. Differentiating between fibroadenomas and phyllodes tumors is clinically significant owing to their differing malignant potential and the consequent implications for treatment strategies. However, this distinction is often challenging, as the two entities share considerable overlap in both clinical presentation and imaging chara

A Rare Case of Adenoid Cystic Breast Cancer: A Case Report and Literature Review.

INTRODUCTION: Adenoid cystic carcinoma (AdCC) of the breast is an exceptionally rare malignancy, representing less than 0.1% of all breast cancers. Despite its triple-negative receptor profile, it typically follows an indolent course with excellent prognosis, distinct from the aggressive behavior usually seen in triple-negative breast cancers (TNBCs). CASE PRESENTATION: We report the case of a 48-year-old woman with a significant familial history of cancer who was diagnosed with breast AdCC. Imaging revealed a suspicious lesion in the left breast, and biopsy confirmed AdCC. Surgical resection

Overlap syndrome between sarcoidosis and Sjögren's disease presenting as hypopituitarism: case report and review of literature.

Sarcoidosis is a granulomatous disease that affects multiple organs; however, pituitary involvement is extremely rare. Although autoimmune diseases, such as Sjögren's disease (SjD), may coexist, overlapping cases presenting with hypopituitarism are exceedingly rare. A 76-year-old man was admitted with hyponatremia and altered consciousness as chief complaints. Endocrine testing revealed panhypopituitarism, and magnetic resonance imaging showed pituitary gland and pituitary stalk enlargement with homogeneous enhancement. Whole-body imaging revealed extensive lymphadenopathy. Histological examin

Inflammatory Myofibroblastic Tumor of the Breast Mimicking Complex Fibrocystic Changes, With a Novel CSTF3::ALK Fusion.

Inflammatory myofibroblastic tumor (IMT) of the breast is an uncommon neoplasm that presents significant diagnostic challenges due to its overlap with more common breast lesions. Clinical and imaging features are often nonspecific, leading to potential misdiagnosis. We present a breast IMT in a 38-year-old woman, initially detected as a 7-mm lesion classified as BI-RADS 3 during routine screening and suspected to represent fibrocystic changes. Follow-up ultrasound demonstrated growth to 10 mm, prompting a core needle biopsy that revealed a spindle cell lesion with prominent inflammatory infilt

A Mystery in the Male Breast: Immunomorphological Confirmation of Invasive Papillary Carcinoma in an Elderly.

Male invasive papillary carcinoma (IPC) of the breast is an uncommon malignancy, accounting for a small proportion of male breast cancers, and poses significant diagnostic challenges because of its rarity and morphological overlap with other papillary lesions. We report the case of an 85-year-old man who presented with a recurrent painless right breast mass following previous excision without histological evaluation. He underwent simple mastectomy with sentinel lymph node biopsy. Histopathological examination demonstrated an invasive papillary carcinoma comprising over 90% papillary architectu

Automated breast ultrasound (ABUS) versus handheld ultrasound (HHUS) for the preoperative assessment of tumor size in invasive breast cancer.

PURPOSE: Accurate preoperative assessment of tumor size (T parameter) is crucial for surgical planning in breast cancer. This study aimed to compare the diagnostic performance of automated breast ultrasound (ABUS) and conventional handheld ultrasound (HHUS) in evaluating tumor size, using histopathology as the reference standard. METHODS: Between September 2025 and March 2026, 43 patients with 46 invasive breast carcinoma lesions underwent preoperative evaluation with both HHUS and ABUS. Tumor diameters were measured by two experienced radiologists. Measurements obtained with each imaging mod

Myxoid Fibroadenoma of the Breast With Radiologic-Pathologic Correlation: A Case Report.

Myxoid fibroadenoma is a rare histologic variant of conventional fibroadenoma characterized by prominent hypocellular myxoid stromal change. Owing to its overlapping morphologic features with other myxoid breast lesions, including mucinous carcinoma and phyllodes tumor, it may pose a significant diagnostic challenge, particularly in limited biopsy specimens. We report the case of a 47-year-old woman presenting with a non-palpable breast mass, initially evaluated with imaging and core needle biopsy. Preoperative histopathologic assessment demonstrated a fibroepithelial lesion; however, definiti

Metaplastic carcinoma masquerading as fibromatosis: A diagnostic dilemma.

Metaplastic breast carcinoma (MBC) is a rare, heterogeneous group of neoplasm of breast, accounting for about 0.2-1% of all invasive breast carcinomas. Its variants include low grade adenosquamous carcinoma, fibromatosis like metaplastic carcinoma, spindle cell carcinoma, squamous cell carcinoma and mixed metaplastic carcinoma. We report a case of a 48 year old female, who presented with painful lump in lower inner quadrant of right breast since one and half month, gradually increasing in size. Trucut biopsy was reported as Fibromatosis of breast. Excisional biopsy was consistent with malignan

LENS: A mammography-specific hybrid CNN-Transformer with lesion-aware evidence modeling.

BACKGROUND: Artificial Intelligence (AI) models for mammography classification is prone to shortcut learning because diagnostically relevant evidence is typically sparse, localized, and easily dominated by non-lesion background context. This study aimed to develop a mammography-specific framework that integrates lesion-focused evidence with global image representations to improve classification performance and provide more clinically interpretable decision support. METHODS: We propose LENS, a hybrid CNN-Transformer family designed specifically for mammography. LENS combines a lightweight mult

Primary adenoid cystic carcinoma of the breast: a clinicopathological case series.

Adenoid cystic carcinoma of the breast (ACCB) is a rare special-type carcinoma, accounting for <0.1% of breast malignancies. Despite its frequent triple-negative phenotype, ACCB typically demonstrates indolent behavior and a favorable prognosis. Owing to its rarity and overlapping radiologic and histopathological features, accurate diagnosis can be challenging. We retrospectively analyzed three cases of histologically confirmed primary ACCB diagnosed at a tertiary care center. Clinical presentation, imaging findings, histomorphology, immunohistochemistry, treatment, and follow-up data were rev

Primary Tumor Epigenetic and Transcriptomic Alterations Associated with Nodal Burden and Metastatic Risk in ER+/HER2- Breast Cancer.

De-escalation of axillary surgery has resulted in the loss of pathologic nodal information, yet the extent of lymph node involvement remains an important determinant of treatment decisions in estrogen receptor-positive (ER+)/HER2- disease. We examined whether primary tumors differed molecularly according to the extent of this regional dissemination. Genome-wide DNA methylation profiling of primary ER+/HER2- tumors from 47 patients with pN1 (n = 29) vs. >pN1 (n = 18) disease showed differences concentrated at promoters of developmental and cell-adhesion genes. By integrating methylomes with tra

A segmentation-guided CNN-Vision transformer feature fusion framework for multi-class breast ultrasound image classification.

Breast ultrasound (BU) imaging is widely used for detecting breast abnormalities because it is cost-effective, non-invasive, and suitable for dense breast tissue. However, multi-class classification of BU images is considered a challenging task due to low contrast, speckle noise, and overlapping visual patterns between benign and malignant tumours. To address this issue, we develop a segmentation-guided CNN and Vision Transformer based feature fusion framework for efficient multi-class BU image classification. The framework first applies a lesion segmentation model to identify the region of in

Primary breast diffuse large B-cell lymphoma mimicking breast carcinoma: A case report and literature review.

RATIONALE: Primary breast diffuse large B-cell lymphoma (PB-DLBCL) is a rare extranodal manifestation of non-Hodgkin lymphoma (NHL) that can closely resemble breast carcinoma clinically and radiologically. This overlap may delay accurate diagnosis and appropriate treatment. We report a case of PB-DLBCL in a postmenopausal woman whose initial multimodal imaging findings were suspicious for breast carcinoma, but whose final diagnosis was established by core needle biopsy with immunohistochemical and molecular evaluation. PATIENT CONCERNS: A 59-year-old postmenopausal woman presented with a pain

Radiation-Induced Morphea of the Breast in a Patient with Pre-Existing Mycosis Fungoides: A Diagnostic Challenge.

Radiation-induced morphea (RIM) is a rare, immune-mediated late complication of breast radiotherapy that may clinically mimic radiation fibrosis, malignancy, or other inflammatory dermatoses. Diagnosis is particularly challenging in patients with pre-existing cutaneous T-cell lymphoma (CTCL), in whom new post-radiation skin lesions may raise concern for lymphoma involvement or progression. To the best of our knowledge, based on a non-systematic search of the available literature, this is the first reported case of radiation-induced morphea occurring in a patient with pre-existing mycosis fungo

The role and potential of digital breast tomosynthesis in neoadjuvant systemic therapy evaluation for optimising breast cancer management: a pictorial essay.

Neoadjuvant therapy (NT) has become the gold standard for treating locally advanced breast cancer (BC). The assessment of pathological response (pR) post-NT plays a crucial role in predicting long-term survival, with contrast-enhanced MRI currently recognised as the preferred imaging modality for its evaluation. Traditional imaging techniques, such as digital mammography (DM) and ultrasonography (US), encounter difficulties in post-NT assessments due to breast density, lesion changes, fibrosis, and molecular patterns. Digital breast tomosynthesis (DBT) offers solutions to prevalent challenges

Adaptive ensemble loss and multi-scale attention in breast ultrasound segmentation with UMA-Net.

The generalization of deep learning (DL) models is critical for accurate lesion segmentation in breast ultrasound (BUS) images. Traditional DL models often struggle to generalize well due to the high frequency and scale variations inherent in BUS images. Moreover, conventional loss functions used in these models frequently result in imbalanced optimization, either prioritizing region overlap or boundary accuracy, which leads to suboptimal segmentation performance. To address these issues, we propose UMA-Net, an enhanced UNet architecture specifically designed for BUS image segmentation. UMA-Ne
