Self-collected cervical samples for detection of high-risk human papillomavirus: A community outreach approach in a remote region.

Background and objectives Self collection of cervical samples is evolving as a potential screening approach to overcome cultural, geographical and logistical barriers that contribute to poor screening uptake. We conducted a study to compare performance of self and physician-collected cervical samples for high-risk human papillomavirus (hr-HPV) detection through community outreach approach in the hilly terrains of Sikkim. Methods A prospective longitudinal study was conducted for 2 y, sampling 210 sexually active women aged 18-65 y attending Central Referral Hospital and 3 district hospitals in

Unintended consequences of vaginal misoprostol use in a patient with hydatidiform mole: A case report.

RATIONALE: Hydatidiform mole (HM) is usually managed by suction curettage, while misoprostol is not recommended as a method of medical evacuation for molar pregnancy because of concerns regarding incomplete evacuation and postmolar gestational trophoblastic neoplasia. The use of misoprostol for cervical ripening before hysterectomy in patients with known molar pregnancy is also not supported by established evidence. This case highlights the potential risks of unintended misoprostol use in gynecologic procedures involving molar tissue. PATIENT CONCERNS: A 49-year-old woman presented with vagin

Integrating behavioural science into HPV self-sampling and cervical cancer elimination strategies.

Cervical cancer is highly preventable through regular screening and vaccination. The World Health Organization and governments worldwide have identified the elimination of cervical cancer as a public health problem, as an attainable goal, representing a major advancement in women's health. While screening has traditionally relied on the Papanicolaou test (Pap test), evidence shows that HPV DNA testing is more accurate in detecting cervical precancers. HPV testing can be performed using clinician-collected or self-collected samples, with self-sampling shown to improve screening access and uptak

Vaginal natural orifice transluminal endoscopic myomectomy.

BACKGROUND: Vaginal natural orifice transluminal endoscopic surgery (vNOTES) avoids abdominal incisions and is increasingly used for benign gynecologic surgery, but evidence for fertility-sparing myomectomy remains limited. METHODS: We searched PubMed, MEDLINE, EMBASE and Scopus (January 1, 2000 - July 31, 2025) for comparative studies (vNOTES vs. multi-port or laparoendoscopic single-site [LESS] myomectomy) and single-arm vNOTES cohorts. Two reviewers screened/extracted data. Risk of bias was assessed using Newcastle-Ottawa (comparative) and Joanna Briggs Institute (JBI; case series). Random

Retrospective study of inflammatory markers in high-grade cervical lesions.

OBJECTIVE: The aim of this study was to evaluate the predictive value of systemic inflammation markers for abnormal cervical screening results and high-grade cervical lesions in women participating in a human papillomavirus- and cytology-based screening program, thereby determining colposcopy indications more accurately to reduce unnecessary colposcopy requests and prevent overtreatment. METHODS: A total of 2,758 women who underwent cervical cancer screening between 2020 and 2025 were included in this retrospective cross-sectional study. Abnormal screening results were defined as abnormal cyt

Divergent immune ecosystems in HPV-associated and HPV-independent lower anogenital tract malignancies.

Lower anogenital tract malignancies comprise anatomically adjacent but biologically heterogeneous epithelial cancers of the cervix, vulva, vagina, anal canal and penis. This narrative and conceptual Review used a transparent literature search to examine how human papillomavirus (HPV) etiology intersects with tumor lineage, somatic alterations, antigen-presentation competence, immune-cell state and spatial organization. Evidence quality is heterogeneous: cervical cancer is supported by the largest genomic, single-cell, spatial and randomized-trial literature, whereas vaginal, vulvar and penile

Impact of Human Papillomavirus Primary Cervical Screening on Referral Rates and Outcomes for People Referred to Colposcopy for Clinical Reasons.

UNLABELLED: Colposcopy is most commonly indicated for people with screen-positive cervical screening tests. However, clinical reasons such as post-coital bleeding (PCB) or an abnormal appearing cervix can also prompt referral. Aotearoa New Zealand (AoNZ) transitioned from a cytology-based to an HPV-based cervical screening programme in September 2023. AIM: To assess the incidence of high-grade or malignant pathology in people seen in Capital & Coast colposcopy clinic for clinical reasons and to evaluate the impact of HPV-based primary screening on these referral patterns and outcomes. METHOD

Cost-effectiveness of self-sampling strategies for cervical cancer screening: a commentary.

Tackling cervical cancer represents a global health priority and relies on preventative approaches such as screening. To increase uptake, self-sampling strategies used to detect human papillomaviruses have shown promise of cost-effectiveness. This commentary critically appraises an economic evaluation comparing first-void urine and vaginal swab self-sampling strategies against clinician-collected cervical samples in the UK. While the self-sampling strategies could improve screening uptake among non-attenders and health outcomes, the impacts on the subsequent incidence of cervical cancer and ty

INTRODUCTION:  Cervical cancer is a major public health issue in Algeria, where a national screening program based on the Pap smear was launched in 2001. However, its effectiveness has never been rigorously evaluated. This study aimed to assess the impact of the screening program on cervical cancer incidence in the province of Oran between 1996 and 2020. METHODS:  Data were extracted from the Oran Cancer Registry and included all cervical cancer cases diagnosed between 1996 and 2020. Age-standardized incidence rates based on the world standard population were calculated, and

Vaginal Laser Treatment for Genitourinary Syndrome of the Menopause: A Dose-Determining Study in Breast Cancer Survivors.

OBJECTIVES: Breast cancer survivors (BCSs) receiving endocrine therapy frequently develop genitourinary syndrome of menopause, characterized by vaginal dryness, pain, and urinary symptoms. Fractional CO2 vaginal laser therapy has been proposed as a non-hormonal management option, but the optimal number of treatment sessions remains unclear. As preparation for a forthcoming randomized, blinded, sham-controlled trial, this dose-determining study evaluated how the number of fractional CO2 laser sessions influences urogenital symptoms and objective vaginal parameters to identify session-dependent

Adjuvant therapy in early uterine serous carcinoma.

Uterine serous carcinoma (USC) is a rare diagnosis but is associated with high mortality. There are limited data to guide adjuvant treatment decisions in early-stage disease. The purpose of this study is to evaluate the impact of adjuvant therapy on recurrence-free survival (RFS) and overall survival (OS) in early-stage USC. Patients with stage I and II USC treated at a single institution from January 2006 to December 2019 were identified. Demographic, clinicopathologic, treatment, and outcome data were collected. Data were compared using descriptive statistics. Survival analyses were performe

Menopausal hormone therapy for breast cancer patients: what is the current evidence?

IMPORTANCE AND OBJECTIVES: Many breast cancer survivors struggle with menopausal symptoms due to oncological treatment-induced hormone deficiency, or because they experience menopause some years after completing treatment, but have limited menopause treatment options. Estrogen replacement therapy is the most effective treatment for menopausal symptoms, but is not recommended after breast cancer because it can increase the risk of relapse. Our objectives were to review the evidence and develop a consensus statement to define the role of menopausal hormone therapy after breast cancer, and to hig
