Three-Dimensional Microscopic Characteristics of the Human Uterine Cervix Evaluated by Microtomography.

Objectives: To analyze the microscopic anatomy of the human uterine cervix in two-dimensional (2D) and three-dimensional (3D) images obtained by microtomography (microCT). Methods: Human uterine cervixes surgically removed for benign gynecologic conditions were immersed in formalin and iodine solution for more than 72 h and images were acquired by microtomography. Results: In total, 10 cervical specimens were evaluated. The images provided by microCT allowed the study of the vaginal squamous epithelium, demonstrated microscopic 3D images of the metaplastic process between the exo and endocervi

Counseling and Educating About Vasa Previa Using Intraoperative Photographs of Vessel Rupture: A Case Report.

Vasa previa describes vessels that are unprotected by Wharton's jelly and traverse over or near the endocervix. Rupture of membranes or labor increases the risk of vessel rupture, followed by rapid fetal exsanguination. Learners are often taught to manage this condition by case scenarios and simulations. We present a case of type I vasa previa where intraoperative photographs demonstrate the potentially catastrophic rupture of membranes into the unprotected fetal vessels during a scheduled cesarean delivery. We recommend using intraoperative photographs to educate learners and counsel patients

Role of Reserve Cells in Metaplasia and the Development of Human Papillomavirus-Associated High-Grade Squamous Intraepithelial Lesions at the Cervical Transformation Zone.

Squamous cervical cancers generally arise as a result of persistent infection with high-risk human papillomaviruses (hrHPVs) and occur near the squamocolumnar junction (SCJ) and within the transformation zone (TZ). The susceptibility of the TZ to HPV-related carcinogenesis appears linked to epithelial cell plasticity, with squamous metaplasia originating from a specialized stem cell population at this site. Two alternative cell populations have been implicated: keratin (K)7+ve cuboidal cells located at the SCJ vs a more broadly distributed K17+ve cervical reserve cell population. To distinguis

Massive vaginal bleeding caused by ruptured cervical varices associated with fetal loss in the second trimester of pregnancy: A report of a rare case.

Cervical varices during pregnancy are a rare clinical entity. The rupture of cervical varices in pregnant women may be associated with fetal loss following the termination of the pregnancy or pre-term delivery, in the case that it occurs in the presence of a viable fetus. The present study describes a case of pregnancy termination by caesarean section due to massive vaginal bleeding following the rupture of cervical varices in the second trimester. A 26-year-old primigravida, with a history of placenta previa, presented to the hospital at 21 weeks of gestation, reporting significant painless v

The art of cold knife conization: a demonstration of technique in live patients and a low-fidelity simulation model.

Cervical excision via cold knife conization is recommended for conditions, such as adenocarcinoma in situ or predominantly endocervical high-grade dysplasia. A successful cold knife conization involves visualization, targeting of the pathology, obtaining an unfragmented specimen, and consideration of postexcision endocervical curettage. This article presents a video that demonstrates cold knife conization techniques to facilitate pathologic analysis and a low-fidelity simulation model for learners. The video depicts 2 cold knife conizations in nulliparous patients with (1) high-grade lesion on

Oxidative Stress, Inflammation, and Antioxidant Strategies in Cervical Cancer-A Narrative Review.

Cervical cancer ranks third among malignant diseases of the female reproductive system and progressively develops through a series of pathological changes known as cervical intraepithelial neoplasia (CIN). Despite being extremely aggressive and causing increased mortality, the main treatment options include surgery or a combination of chemotherapy and radiotherapy, often based on cisplatin-based chemotherapy and external beam radiotherapy or brachytherapy. Cervical dysplasia is an abnormal growth of cells on the surface of the cervix that could lead to cervical cancer. CIN most commonly occurs

Metastatic carcinoma of the cervix with unexpected ovarian involvement: A rare case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Ovarian metastasis from cervical carcinoma (CC) is rare. As adenocarcinomas tend to develop within the endocervix, they may be hidden in Pap tests and have a normal Colposcopic appearance. Only a limited number of Ovarian metastases from CC are reported in the literature, and various aspects of this involvement still need to be clarified. PRESENTATION: In this study, we reported a case of unexpected ovarian metastatic cervical cancer in a 57-year-old female. The uniqueness of the present case was the large size of the metastatic ovarian mass, which led to ureter c

Beyond the transformation zone: Outcomes in the management of CGIN over five years.

BACKGROUND: Cervical Glandular Intraepithelial Neoplasia (CGIN) is the term used to describe endocervical glandular lesions that predispose to cervical adenocarcinoma. Colposcopy is an important step in the investigation and management of suspected glandular disease. Excisional biopsy is necessary given the nature of the condition, which may reside in the endocervix with no visible lesions at colposcopy. Once diagnosed, a cylindrical excision is recommended, the depth of which is dependent on age and type of transformation zone. Our aim was to analyse the management and outcomes in this cohort

Optimized segmentation of overlapping cervical cells based on Mask2Former and denoising.

Cell instances segmentation in cervical cytology images have crucial importance for cancer screening and automatic biomedical image analysis. However, that the highly overlapping cells cause ambiguous boundaries makes accurate cell segmentation still a challenging task. This paper proposes a novel transformer-based neural network to segment overlapping cervical cells, by using the technique of denoising with Mask2Former. The model optimization includes two aspects. One is that class embeddings of ground truth categories are used as extra content queries to transformer decoder. The other is tha

Highly viable gastrointestinal Chlamydia trachomatis in women abstaining from receptive anal intercourse.

Chlamydia trachomatis (CT) may employ persistence to evade antimicrobial clearance, possibly residing in the gastrointestinal tract. This study assessed the reliability of droplet digital PCR (ddPCR) in CT detection, its functionality in viability assessment, and predictions on CT transmission dynamics by combining viability PCR (vPCR) and clinical data from 52 infected women. The ddPCR showed 94% positive and 100% negative agreement with Abbott Alinity STI-M for endocervical samples, and 92% positive and 87% negative agreement in rectal samples. Viability was higher in endocervical samples (8

Effect of Azithromycin treatment on the microbial composition, functional dynamics and resistomes of endocervical, vaginal and rectal microbiomes of women in Fiji with Chlamydia trachomatis infection.

Antibiotics disrupt mucosal microbial communities, yet the effects on microbiomes with Chlamydia trachomatis (Ct) infection remain poorly understood. Some data exist on vaginal microbiomes pre- and post-treatment, but none are available for the endocervix or rectum that are primary sites of infection. We applied metagenomic shotgun sequencing to vaginal, endocervical and rectal samples from women who, overtime, had Ct persistence, clearance, or no infection to evaluate azithromycin-induced changes in microbial composition, function, and the resistome. Our results show a shift in composition an

Human Papillomavirus 42: A Novel Sublineage Identification, and Comparative Analysis of HPV42 Genetic Variability Present in Cervical Cell and Digital Papillary Adenocarcinoma.

Although HPV 42 is classified as a low-risk HPV, it has been identified as an oncogenic driver of digital papillary adenocarcinoma (DPA) and is associated with high-grade squamous intraepithelial lesion (HSIL) in the mucosal genital region. This study aim to identify novel HPV42 sublineage, characterize its genetic variability, and explore the differences or associations of genetic variation between HPV42 variants derived from cervical and DPA samples. We successfully amplified 17 complete HPV42 genomes from cervical cells, including 5 A1, 11 A2, and 1 novel A4 sublineages. The A4 sublineage (

Endocervix exhibits greater susceptibility to HIV-1 infection compared to ectocervix following ex vivo exposure to Transmitted/Founder HIV-1 variants.

The kinetics and identification of targets of Human Immunodeficiency Virus (HIV) infection within mucosae is a valuable tool for the development of new HIV-prevention strategies. Human tissue explants offer an informative model for studying HIV-1 pathogenesis and can support the development of novel HIV prevention interventions. Here, we infected cervical explants from HIV-1-uninfected women undergoing routine surgery with HIVBaL, a lab-adapted virus, and isolates HIV4790 and HIV4791, transmitted/founder (T/F) HIV-1 variants, and monitored the subsequent viral infection and replication using r

Laminaria tent use for dilation, extraction, and curettage leading to septic shock: a case report.

BACKGROUND: Laminaria tents are commonly used to aid cervical dilation in gynecological procedures, but their potential risks are not well documented in the literature. This case report describes a severe adverse outcome following their use. CASE PRESENTATION: A 27-year-old Caucasian woman G2P1 with a 14 + 1 weeks arrested pregnancy was consented for a dilation, extraction, and curettage. Two laminaria tents were inserted into the endocervix, and the following day the dilation and curettage was performed without immediate complications, although laminaria tents were in an unusual state of dis

Genome, HLA and polygenic risk score analyses for prevalent and persistent cervical human papillomavirus (HPV) infections.

Genetic variants that underlie susceptibility to cervical high-risk human papillomavirus (hrHPV) infections are largely unknown. We conducted discovery genome-wide association studies (GWAS), replication, meta-analysis and colocalization, generated polygenic risk scores (PRS) and examined the association of classical HLA alleles and cervical hrHPV infections in a cohort of over 10,000 women. We identified genome-wide significant variants for prevalent hrHPV around LDB2 and for persistent hrHPV near TPTE2, SMAD2, and CDH12, which code for proteins that are significantly expressed in the human e
