Monitoring Chlamydia trachomatis Development in Polarized Endocervical Epithelial Cells.

Understanding Chlamydia trachomatis infection requires physiologically relevant models tailored to various research questions. One of the main sites of C. trachomatis infection is the columnar epithelium of the endocervix; therefore, a model that accurately mimics the environment of endocervical epithelium is essential. Polarized cell models provide a valuable solution, offering a cost-effective yet biologically relevant alternative to conventional 2D culture systems and more complex and costly 3D or animal models. Immortalized human endocervical epithelial cells (A2EN cells) form a polarized

Chlamydia trachomatis Infection in a Microphysiologic Cell Culture Model.

Chlamydia trachomatis is an obligate intracellular bacterium that infects the columnar epithelium of the human endocervix. While conventional two-dimensional cell cultures and animal models have been instrumental in advancing our understanding of C. trachomatis biology, they are limited in capturing the multicellularity, architecture, and physiological microenvironment of the human cervix. This chapter describes the use of a three-dimensional (3D) microphysiologic model to study Chlamydia trachomatis infection. The model is inexpensively made without specialized equipment and is designed to re

Endometrial tuberculosis mimicking gynecological malignancy in a postmenopausal woman.

BACKGROUND: Female genital tuberculosis is a rare form of extrapulmonary tuberculosis that may mimic gynecologic malignancy, particularly when clinical and imaging findings are nonspecific. Diagnosis can be especially challenging in postmenopausal women. CASE PRESENTATION: A 78-year-old postmenopausal woman presented with pelvic pain and was found to have marked endometrial cavity distention, hematometra, and a cervical lesion concerning for malignancy. Severe cervical stenosis prevented adequate endometrial sampling despite examination under anesthesia, attempted hysteroscopy, and dilation a

Unraveling the Cross-Tissue Neuroimmune-Vascular Genetic Architecture of Migraine Using Integrated Multi-Omics, Single-Cell, and Spatial Transcriptomics: Prioritizing T-Cell Regulatory Networks and Peripheral Targets.

Migraine is a complex neurovascular disorder in which immune signaling intersects with vascular and neural circuits, yet the tissue and cell-type context of common genetic risk remains incompletely defined. We integrated large-scale migraine genome-wide association study (GWAS) summary statistics with Genotype-Tissue Expression (GTEx) v8 expression and splicing quantitative trait loci (eQTLs and sQTLs), Bayesian co-localization, single-cell RNA sequencing of peripheral blood mononuclear cells (PBMCs) from migraine cases and controls, a healthy single-cell multi-omics atlas (assay for transposa

Enumeration, Phenotyping, and Clinical Associations of Tissue-Resident T Cells in the Ecto- and Endocervix of Women Attending a Colposcopy Clinic.

PROBLEM: Tissue-resident memory (TRM) cells represent important immune sentinels that mount rapid recall responses to pathogens and cancers. However, there are limited data in humans on genital tract TRM collected by clinically feasible sampling methods, limiting a full understanding of their role in immunity and clinical disease. METHOD OF STUDY: We used flow cytometry and single cell RNA sequencing (scRNAseq) to characterize T cells isolated from ectocervical biopsies and endocervical cytobrushes collected from women attending a colposcopy clinic in Winnipeg, Canada. RESULTS: The ectocervi

Cervical mucus viscoelasticity and sperm velocity are correlated and concentration-dependent in Vitro.

BACKGROUND: Mucus in the endocervix serves as fertility gatekeeper in the reproductive tract through hormonally regulated changes in biophysical properties. Cervical mucus can thicken to prevent ascension of sperm into the upper reproductive tract or thin to permit fertilization. Current reproductive studies of mucus viscoelastic properties rely on subjective visual appraisal of cervical mucus changes. Our goal was to use particle tracking microrheology (PTMR) to objectively assess cervical mucus viscoelastic properties and associate these measurements with in vitro measures of sperm velocity.

Endometrial Gastrointestinal Mucinous Metaplasia With Goblet Cells and KRAS Mutation-Report of Two Patients and Review of Literature.

Mucinous metaplasia in benign and malignant endometrial entities is well recognized. The mucinous epithelium may resemble the endocervix, coined as "usual mucinous metaplasia," or may be gastric type epithelium consisting of bland epithelium with abundant pale cytoplasm, with or without goblet cells, so-called "gastrointestinal metaplasia." Mostly described in premalignant and malignant endometrial entities (endometrial intraepithelial neoplasia/atypical hyperplasia and carcinoma), rare examples of gastrointestinal metaplasia arising in benign endometrial polyps have been reported. In this pap

Olfactomedin4 marks luminal progenitor cells that give rise to secretory cell lineage in the mouse cervix.

Development of the female reproductive tract in mice occurs in early postnatal life. The current model identifies Trp63 as the master regulator that initiates differentiation of simple columnar Keratin 8+ epithelium in the cervix and vagina into a stratified squamous epithelium. Thereafter Trp63+ basal progenitors maintain cervicovaginal epithelial cell homeostasis and in the adult serve as the progenitor for hormone-regulated shifts in stratified squamous and secretory luminal cells. This model differs from the human in which two progenitors, one columnar and the other basal gives rise to sec

Clinical manifestations and ultrasonographic features of lobular endocervical glandular hyperplasia: a retrospective study of 135 patients.

BACKGROUND: Lobular endocervical glandular hyperplasia (LEGH) is a precursor to malignant lesions of the endocervix. However, precise histopathological sampling for early detection remains a challenge. This study aimed to review the clinical features of LEGH and particularly focus on its ultrasonographic characteristics. PATIENTS: A retrospective analysis was conducted on 135 patients with LEGH (including those with or without concurrent malignancy). Clinical data, preoperative diagnostic evaluations, and histopathological findings were systematically collected and analyzed. Concurrently, we

Brushing versus curettage: Which is the best sampling mode in potmenopausal women with suspected endocervical pathology?

INTRODUCTION: Assessment of the endocervical canal is essential when cervical dysplasia or neoplasia is suspected. In postmenopausal women, anatomical and histological changes associated with hypoestrogenism may hinder diagnostic evaluation. This study aimed to compare the diagnostic performance of endocervical brushing and endocervical curettage in this population. METHODS: We conducted a prospective study including 270 postmenopausal women with suspected endocervical preneoplastic or neoplastic lesions. Each patient underwent both endocervical brushing and curettage in a randomized sequence

Uterine Müllerianosis.

Introduction: Müllerianosis is a rare pathological entity characterized by ectopic Müllerian-derived tissues, including endometrium, endocervix, and/or endosalpinx. Most reported cases involve the urinary bladder, while uterine involvement is extremely uncommon. We present a case of müllerianosis presenting as a cystic lesion of the anterior uterine wall, and we provide a review of the available literature. Case Presentation: We report a 37-year-old woman with an asymptomatic anterior uterine wall lesion detected 11 years ago on routine ultrasound. Initially measuring 60 × 35 mm, the lesion wa

In-vivo detection of cervical cancer lesions using hyperspectral colposcopy.

Cervical cancer originates from precursor lesions in the cervix, where early diagnosis is essential to avoid progression to invasive carcinoma. Today, lesion detection relies mainly on colposcopy, a highly operator-dependent procedure with considerable variability in accuracy, particularly among less-experienced clinicians. In this study, we present a hyperspectral imaging-based approach for assisted colposcopy. We developed a custom hyperspectral colposcope covering 470-900 nm and conducted a 32-month clinical study including 116 patients and 245 hyperspectral images acquired during routine e

Pharmacological evaluation of an ex vivo cervicovaginal HIV prevention model.

BACKGROUND: The female genital tract (FGT) is a unique compartment with physiologically distinct properties complicating the extrapolation of drug efficacy; critical gaps remain in understanding regional variability within the FGT itself. We performed an in-depth investigation across endo- and ectocervical tissues on the utility of the cervical explant model to evaluate pre-exposure prophylaxis (PrEP) efficacy. METHODS: Using normal cervical tissues, we evaluated gene expression of relevant drug metabolizing enzymes and transporters (DMETs) via qRT-PCR and compared ecto- and endocervix. To de

Efficacy and Safety of Acellular Dermal Matrix for Prevention of Epidural Fibrosis after Lumbar Fusion Surgery: A Propensity Score-Matched Study.

PURPOSE: Resistance exercise (RE) has been shown to have a positive impact on cardiometabolic health, independent of moderate-vigorous physical activity (MVPA). However, previous studies primarily used self-reported MVPA data, which are prone to bias. The aim of this study was to examine if the association between RE frequency and cardiometabolic health was independent of device-measured MVPA. METHODS: Cross-sectional analyses were conducted using participants from the 1970 British Cohort Study. This included up to 4,881 participants (age 46-48) with outcome-specific analytical samples. Enhancing plasmonic superconductivity in layered materials via dynamical Coulomb engineering.

PURPOSE: The prevalence of sleep disturbances in children is recognized as far higher than previously estimated. Furthermore, data previously collected in Türkiye have not been thoroughly consolidated, and there are notable variations across research reporting prevalence rates. This meta-analysis seeks to assess the general prevalence of sleep disturbances among children and adolescents in Türkiye and to identify factors that may contribute to potential heterogeneity. METHODS: The meta-analysis was performed as a systematic review adhering to PRISMA guidelines and PROSPERO registration. Studi

Increases in the susceptibility of human endometrial CD4+ T cells to HIV-1 infection post-menopause are not dependent on greater viral receptor expression frequency.

Epidemiological evidence suggests that post-menopausal women are more susceptible to HIV infection following sexual intercourse than are younger cohorts for reasons that remain unclear. Here, we evaluated how menopause-associated changes in CD4+ T cell numbers and subsets as well as HIV coreceptor expression, particularly CCR5, in the endometrium (EM), endocervix (CX), and ectocervix (ECX) may alter HIV infection susceptibility. Using a tissue-specific mixed cell infection model, we demonstrate that while no changes in CD14+ macrophage infection susceptibility were observed, CD4+ T cell HIV-1

Exploring the Role of Lower Genital Tract Microbiota and Cervical-Endometrial Immune Metabolome in Unknown Genesis of Recurrent Pregnancy Loss.

Recurrent pregnancy loss (RPL) of unknown genesis is a complex condition with multifactorial origins, including genetic, hormonal, and immunological factors. However, the specific mechanisms underlying endocervical cell proliferation disorders in women with RPL remain inadequately understood, particularly concerning the role of microbiota and viral infections. The aim of this study was to investigate the mechanisms of endocervical cell proliferation disorders in women with RPL of unknown genesis by examining microbiota, human papillomavirus (HPV) typing, and the expression levels of key molecu

Counseling and Educating About Vasa Previa Using Intraoperative Photographs of Vessel Rupture: A Case Report.

Vasa previa describes vessels that are unprotected by Wharton's jelly and traverse over or near the endocervix. Rupture of membranes or labor increases the risk of vessel rupture, followed by rapid fetal exsanguination. Learners are often taught to manage this condition by case scenarios and simulations. We present a case of type I vasa previa where intraoperative photographs demonstrate the potentially catastrophic rupture of membranes into the unprotected fetal vessels during a scheduled cesarean delivery. We recommend using intraoperative photographs to educate learners and counsel patients

Role of Reserve Cells in Metaplasia and the Development of Human Papillomavirus-Associated High-Grade Squamous Intraepithelial Lesions at the Cervical Transformation Zone.

Squamous cervical cancers generally arise as a result of persistent infection with high-risk human papillomaviruses (hrHPVs) and occur near the squamocolumnar junction (SCJ) and within the transformation zone (TZ). The susceptibility of the TZ to HPV-related carcinogenesis appears linked to epithelial cell plasticity, with squamous metaplasia originating from a specialized stem cell population at this site. Two alternative cell populations have been implicated: keratin (K)7+ve cuboidal cells located at the SCJ vs a more broadly distributed K17+ve cervical reserve cell population. To distinguis
