Trophoblast derived extracellular vesicles trigger secondary vesicle release from endometrial epithelial cells.

Extracellular vesicles (EVs) are emerging mediators of cellular communication, carrying bioactive molecules that can alter the function of recipient cells. Yet, how these signals are recognized and interpreted by recipient cells remains unclear. For communication to be effective, signals must reach the right cells at the right time, suggesting that EVs likely target specific cells, an aspect often overlooked in mechanistic studies. In this study, we used an in vitro embryo implantation model consisting of JAr (trophoblast) and RL95-2 (endometrial) cells to explore targeted communication in a ph

Pathophysiological Mechanisms and Clinical Implications of Lower Urinary Tract Symptoms in Patients with Deep Endometriosis: A Narrative Review.

INTRODUCTION AND HYPOTHESIS: Endometriosis is a chronic inflammatory condition characterized by endometrium-like tissue outside the uterus. Deep endometriosis (DE) frequently involves the posterior pelvic compartment and can affect the urinary tract directly or indirectly, leading to a wide spectrum of lower urinary tract symptoms (LUTS). This narrative review aimed to synthesize current evidence on the mechanisms underlying LUTS in women with deep endometriosis, including direct urinary tract involvement, extra-urinary mechanisms, and postoperative dysfunction, and to discuss their diagnostic

Malady in motion: Effects of chronic wasting disease on host movement.

Pathogens can have significant impacts on host movement and behavior with consequences for ongoing transmission. For example, chronic wasting disease (CWD) causes severe nutritional deficiencies and neurologic changes, which may alter white-tailed deer (Odocoileus virginianus) movement and subsequent transmission risk. Our objectives were to test for changes in movement and habitat selection in deer affected by CWD, and determine the timing of any changes we observed. We leveraged comprehensive movement and mortality data from a large study of free-ranging deer in southwest Wisconsin, a region

Adenomyosis and association between ultrasonographic features and pregnancy outcomes in women with infertility or pregnancy loss: a systematic review and meta-analysis.

BACKGROUND: Adenomyosis has been shown to reduce live birth rates and increase the risk of pregnancy loss. The diagnosis is often established by ultrasonographic assessment using the revised Morphological Uterus Sonographic Assessment (MUSA) criteria. However, the reproductive impact of specific sonographic features remains unclear. A systematic review and meta-analysis was conducted to investigate whether individual ultrasonographic adenomyosis features influence pregnancy outcomes in women with infertility or pregnancy loss. METHODS: We searched MEDLINE (PubMed), Embase, Cochrane Library (C

Case Report: Sequential blastocyst transfer for women with unexplained recurrent implantation failure: a case series.

BACKGROUND: Repeated implantation failure (RIF) is a frustrating situation for patients and medical physicians. In spite of extensive research, the underlying mechanisms of RIF are not fully understood. Endometrial receptivity is considered a critical factor for embryo implantation, particularly in patients with unexplained RIF who have usable embryos. Sequential embryo transfer (ET) has been demonstrated to improve clinical pregnancy rates in RIF patients by optimizing embryo-endometrium synchronization and interaction. In this study, we propose a strategy of sequential blastocyst transfer wi

Increased Endometrial CD38 and CD138 Positivity in Women with Ectopic Pregnancy History: A Retrospective Study.

Ectopic pregnancy (EP) represents the most common gynecological emergency. However, its specific etiology and pathogenesis remain incompletely understood. This retrospective study investigated the correlation between chronic endometritis (CE) markers (CD38 and CD138) in the endometrium and EP. Patients were categorized as follows: women with a history of EP but no prior intrauterine operations (EP Group A; n = 98), women with a history of EP and at least one intrauterine operation (EP Group B; n = 244), and a control group of women with recurrent spontaneous abortion (RSA; n = 267). The preval

Comparative analysis of small extracellular vesicles in peritoneal fluid from endometriosis and non-endometriosis patients: a pilot case-control study.

Aim: Endometriosis is a chronic inflammatory disorder requiring clinical examination, imaging, and surgical intervention for accurate assessment. The aim of the present study was to provide a detailed biophysical and biochemical characterization of small extracellular vesicles (sEVs) derived from peritoneal fluid, and to explore disease-associated molecular differences that may inform future investigations. Methods: sEVs were isolated from the peritoneal fluid of women with endometriosis (n = 7), women without endometriosis who underwent surgery for benign, noninflammatory conditions (n = 7),

Establishment of Rat Endometriosis Model by Autologous Endometrial Transplantation and Proof-of-Concept Pharmacological Validation with Elagolix.

Endometriosis (EM) is a common chronic gynecological disorder in women of reproductive age, yet its pathogenesis remains unclear, and current therapeutic options are limited by significant side effects. Endometriosis itself increases the risk of premature ovarian failure. Long-term lack of standardized treatment may accelerate the decline of ovarian function and prematurely trigger the aging process of the reproductive system. Reliable animal models are therefore essential for investigating disease mechanisms and evaluating novel treatments. This study presents a detailed protocol for establis

Laparoscopic Induction of Endometriosis in a Preclinical Swine Model by Autologous Endometrial Transplantation.

Pigs are increasingly used as models of human diseases, largely because their anatomy, physiology, and genetics more closely resemble those of humans. Given its relatively high prevalence and incompletely understood pathogenesis, endometriosis is an important condition to model in a large-animal model. This protocol describes the laparoscopic induction of endometriosis in a preclinical swine model via autologous transplantation of endometrial tissue into the peritoneal cavity. Endometrial fragments are isolated shortly after standing estrus, minced into uniform pieces, and optionally labeled e

RAC1-YAP1-PIEZO1 signaling axis links intrauterine inflammation to uterine hypercontractility and preterm labor.

Preterm birth is a leading cause of neonatal morbidity and mortality, yet its underlying mechanisms remain incompletely understood. Our previous studies have demonstrated that the mechanosensitive ion channel PIEZO1 plays a critical role in regulating intracellular calcium levels and amplifying inflammation in uterine smooth muscle (USM) cells, thereby promoting USM contractility and preterm labor. However, the upstream regulatory mechanisms driving PIEZO1 expression during infection-induced preterm labor remain unclear. Here, we established an infection-induced preterm birth mouse model using

Administration of injectable platelet-rich fibrin improved repair of thin endometrium via mitochondria in a rat model.

BACKGROUND: Thin endometrium(TE) has been widely recognized as a critical factor contributing to reduced success rates of assisted reproductive technology (ART). In recent years, platelet-rich plasma (PRP) has been extensively employed in the treatment of thin endometrium; however, its underlying mechanisms remain poorly understood, and its clinical efficacy remains controversial. Given the superior therapeutic outcomes demonstrated by platelet concentrates (PCs) in regenerative medicine, the present study aimed to investigate the therapeutic potential and mechanistic basis of injectable plate

AKNA serves as a prognostic biomarker and restrains endometrial cancer progression via STING activation and macrophage reprogramming.

BACKGROUND: AKNA, an AT-hook transcription factor, plays a critical role in cancer development. However, its prognostic value and biological function in endometrial cancer (EC) remain unclear. METHODS: To systematically investigate AKNA in EC, we integrated multi-omics data (The Cancer Genome Atlas (TCGA), Genotype-Tissue Expression (GTEx), and single-cell transcriptomics), dual independent clinical cohorts (TCGA-UCEC dataset and two institutional cohorts), and in vitro/in vivo experimental models. Immunohistochemistry, RT-qPCR and Western blotting were used for expression validation. Functio

Beyond the Silent Endometrium: Dysbiosis, Immune Subversion and Antimicrobial Resistance in Chronic Endometritis: A Narrative Review.

Chronic endometritis (CE) represents an insidious and frequently underdiagnosed inflammatory condition of the endometrial stroma, increasingly recognized as a principal contributor to unexplained infertility, recurrent implantation failure and recurrent pregnancy loss. Historically relegated to a histological curiosity, CE has been reframed as a biologically complex disorder characterized by the convergence of endometrial microbial dysbiosis, immunological subversion, and, increasingly, antimicrobial resistance (AMR). This narrative review synthesizes contemporary evidence examining the mechan

Modern hysteroscopic management of congenital uterine anomalies: current techniques, emerging technologies, and future directions.

BACKGROUND: Congenital uterine anomalies are clinically relevant in reproductive medicine, particularly among women with infertility, recurrent pregnancy loss, adverse obstetric outcomes, or obstructive symptoms. Hysteroscopy has transformed the management of correctable intracavitary anomalies by enabling conservative, image-guided reconstruction of the uterine cavity while avoiding the morbidity of historical abdominal metroplasty. SCOPE: This narrative review critically examines contemporary hysteroscopic management of septate uterus, dysmorphic uterus, bicorporeal septate uterus and rare

Beyond the embryo: reframing recurrent implantation failure through immunopriming and endometrial immune dysregulation.

Recurrent implantation failure (RIF) remains one of the most debated and emotionally challenging conditions in assisted reproductive technology (ART). Despite remarkable advances in embryology, including blastocyst culture, time-lapse imaging and preimplantation genetic testing for aneuploidy (PGTA), a significant proportion of patients continue to fail implantation even after transfer of morphologically good-quality or euploid embryos. Contemporary reproductive medicine has largely adopted an embryocentric model of implantation, often defining RIF based upon repeated failed embryo transfers.

Sprayable metal-phenolic network tailored antioxidant powder remodels endometrial oxidative-inflammatory homeostasis for chronic endometritis therapy and fertility restoration.

Chronic endometritis (CE) is a persistent inflammatory disorder of the endometrium characterized by aberrant plasma cell infiltration and strongly associated with female infertility. CE pathogenesis is driven by persistent microbial stimulation, excessive reactive oxygen species (ROS) accumulation, and local immune dysregulation, whereas antibiotics and hysteroscopic surgery do not sufficiently resolve the sustained oxidative-inflammatory microenvironment. A sprayable antioxidant powder, RuO2@GelMA@TA-Fe3+ (RGTF), is developed for localized remodeling of endometrial oxidative-inflammatory home
