Integrating residual myometrial thickness and Adler vascularity index to predict suction curettage outcomes in cesarean scar pregnancy: A retrospective cohort study.

OBJECTIVE: This study identifies quantitative ultrasound predictors for the failure of ultrasound-guided suction curettage (USG-SC) in cesarean scar pregnancy (CSP) and develops a risk stratification model integrating residual myometrial thickness (RMT) and local vascularity (Adler grade). METHODS: This retrospective cohort study included 415 patients with CSP who underwent primary USG-SC. Baseline morphological categorization adhered to the international Delphi consensus. Procedural failure was defined as hemorrhage (≥500 mL), requirement for secondary interventions, or incomplete evacuation

Anti-Müllerian hormone ameliorates uterine DNA damage response and prevents pregnancy complications in doxorubicin-treated mice†.

Anti-Müllerian hormone is a promising fertoprotective agent, demonstrating particularly strong efficacy against doxorubicin-induced ovarian toxicity. However, the impact of chemotherapy on the uterus, and the potential benefits of anti-Müllerian hormone in this context, remain poorly understood. In this study, we characterized doxorubicin-induced uterine damage and assessed the fertoprotective effect of anti-Müllerian hormone co-treatment in mice. Acutely, doxorubicin treatment caused the accumulation of DNA damage in multiple uterine cell-types, including the myometrium, as evidenced by both

miR-483-5p promotes uterine leiomyoma progression by targeting TIMP2.

INTRODUCTION AND OBJECTIVE: Uterine leiomyoma (UL) is a prevalent tumour of the female reproductive system which is poorly understood in terms of pathogenesis, and treatment options are limited. MicroRNAs (miRNAs) have been identified as being associated with tumorigenesis. The aim of the study is to elucide the function of miR-483-5p in promoting UL progression by targeting TIMP metallopeptidase inhibitor 2 (TIMP2) and its underlying molecular mechanisms. MATERIAL AND METHODS: The study included 340 patients with UL. The expression level of miR-483-5p was measured using quantitative real-tim

Establishment and characterization of a new immortalized human adenomyosis epithelial-like cell line, tAEC21†.

Adenomyosis occurs when endometrial glands and stroma grow within the uterine myometrium. As a clinically significant disease, adenomyosis causes substantial pelvic pain and heavy menstrual bleeding. It remains understudied due to a lack of translational research tools and model systems. This study aimed to develop a telomerase-transformed, epithelial-like cell line derived from the eutopic endometrium of a subject with focal adenomyosis. De-identified endometrial tissue was processed through mechanical and enzymatic digestion. Epithelial and stromal populations were separated by selective adh

Endometrium-free closure technique for hysterotomy incision at cesarean delivery.

Cesarean deliveries are associated with uterine scar defects that significantly impact women's health and future pregnancy outcomes. The methods used for hysterotomy closure following cesarean delivery have undergone significant evolution. Commonly used techniques, which are relatively rapid and maintain hemostasis, include the endometrium in a single-layer or "bulk" closure. Such endometrium-inclusive cesarean closure has been linked to an increased risk of scar defects and long-term complications. However, the available data show no difference in outcomes related to cesarean closure techniqu

What is placenta accreta?

Accreta placentation is a clinicopathologic diagnosis at delivery when 1 or more placental lobules are abnormally attached into a myometrial scar or congenital defect requiring surgical removal. Over 90% of cases of placenta accreta spectrum are found in patients with a prior history of cesarean delivery presenting with a low-lying placenta or a placenta previa developing inside a lower uterine segment scar. Placenta previa accreta is a complex obstetric condition, and management strategies and clinical outcomes are directly linked to the quality of epidemiology data. We have recently question

Navigating complexity: review and updates of endometriosis and adenomyosis imaging.

Endometriosis and adenomyosis are prevalent chronic inflammatory conditions that significantly impact reproductive-age women, leading to debilitating symptoms such as pelvic pain, dysmenorrhea and infertility. Endometriosis is characterized by ectopic endometrial tissue outside the uterus, while adenomyosis involves endometrial deposits within the myometrium, often causing overlapping symptoms. Accurate diagnosis is essential for effective management, yet challenges persist due to variability in lesion location and presentation. Imaging plays a crucial role in the diagnosis, staging, and manag

Beyond VEGF and TGF-β: A Comprehensive Review of Growth Factor Pathways in the Pathophysiology of Uterine Leiomyomas.

Uterine leiomyomas or fibroids, are non-cancerous smooth muscle proliferations of the uterus, occurring mostly in women of reproductive age. Their pathogenesis involves complex growth factor interactions that regulate cellular proliferation, extracellular matrix (ECM) remodeling, and angiogenesis in myometrium. Women affected by fibroids often have a range of consequences such as infertility, endometriosis, and dysmenorrhea. Several growth factors such as vascular endothelial growth factor (VEGF), transforming growth factor (TGF-β), and platelet-derived growth factors (PDGF) have long been des

Impact of placenta accreta spectrum on sexual and psychological health: a comparative analysis using FSFI and SF-36.

BACKGROUND: Placenta accreta spectrum (PAS) is a potentially life-threatening obstetric condition characterized by abnormal placental adherence to the myometrium. While hysterectomy remains the standard definitive management, conservative strategies such as wedge resection or delayed hysterectomy have been increasingly adopted to preserve fertility. The comparative impact of these approaches on women's quality of life (QoL), particularly social and sexual functioning, has not been well investigated. METHOD: A cross-sectional study included 185 women who underwent caesarean section (CS) for hi

Comparative study of transvaginal ultrasound and magnetic resonance imaging in the diagnosis and classification of cesarean scar pregnancy.

OBJECTIVE: Cesarean scar pregnancy is a rare form of ectopic pregnancy in which the fertilized egg implants within the scar of a previous cesarean section. With the rising global cesarean delivery rate, the incidence of cesarean scar pregnancy has increased, posing serious risks such as massive hemorrhage and uterine rupture. To evaluate and compare the diagnostic and classification accuracy of transvaginal sonography and magnetic resonance imaging in the detection of cesarean scar pregnancy. METHODS: This retrospective study included 102 patients clinically suspected of cesarean scar pregnan

First-trimester risk stratification algorithm for placenta accreta spectrum.

BACKGROUND: Antenatal diagnosis of placenta accreta spectrum is imperfect, with cases undiagnosed before delivery, resulting in suboptimal outcomes. Currently, there is no agreed-upon standardized protocol for risk stratification of placenta accreta spectrum. Most patients are identified in the second or third trimester of pregnancy, although findings on first-trimester ultrasound may be associated with the development of placenta accreta spectrum. To date, there is no recommended risk stratification algorithm that applies first-trimester findings in a systematic manner with outcome data. OBJ

Two Giant Cystic Uterine Adenomyomas in a Premenopausal Woman: The Largest Case to Date With Immunohistochemical Findings.

BACKGROUND AND AIMS: Cystic adenomyoma is a rare focal cystic variant of adenomyosis, and giant lesions are particularly uncommon. Malignant transformation has been reported in endometriosis-related disease, but the molecular features of cystic adenomyoma, especially in adults, remain unclear. We are aimed at describing a premenopausal patient with two giant cystic adenomyomas, including the largest lesion reported to date, and to explore a possible pathogenic mechanism using immunohistochemistry. METHODS: A 47-year-old nulliparous premenopausal woman presented with progressive abdominal dist

Simultaneous high-resolution electrophysiology measurements and motion tracking of ex vivo rat uterine smooth muscle activity.

Uterine smooth muscle undergoes electrical, chemical, and mechanical transients during phasic contraction. The spread of electrical activity precedes the mechanical contraction of muscle fibers, and the initiation and coordination of these events is critical to uterine function, in both pregnancy and non-pregnancy. Characterization of non-pregnant activity is scarce, and understanding of the relationship between electrical and mechanical events is not well understood. Isolated uterine horns from virgin female Wistar rats were used in this study. Electrical and mechanical activity were assessed

Enhanced myometrial vascularity on MRI in a patient with retained products of conception.

Secondary postpartum hemorrhage (PPH), sometimes referred to as secondary postpregnancy hemorrhage, is a potentially serious complication that arises between 24 hours and 12 weeks after delivery. Retained products of conception (RPOC) and subinvolution of the placental site are common causes, but uterine vascular anomalies such as arteriovenous malformations (AVMs) can present with similar clinical imaging findings, creating diagnostic uncertainty. Enhanced myometrial vascularity (EMV) has recently been recognized as a postpartum vascular finding that may mimic AVMs on ultrasound and magnetic

Time-of-Day Impacts Uterine Circadian Rhythms and Response to Oxytocin: Comparison of Uterine Function in Melatonin-Deficient C57BL/6 Versus Melatonin Proficient CBA/B6 Hybrid Mice.

Reaching term gestation requires a complex interplay between the uterus and hormonal signals regulating its contractile profile. Most pregnancy-associated hormones vary in their overall level of release throughout pregnancy, but also have a circadian release pattern, including progesterone, oxytocin, and melatonin. It remains poorly understood how the circadian release of hormones impacts uterine function. To determine how time-of-day, mouse strain, and melatonin proficiency were associated with the uterotonic efficacy of oxytocin, the primary hormone promoting uterine contractions, we used me

Enhanced Myometrial Vascularity: Is It an Arteriovenous Malformation? Review of Definitions, Imaging Findings, and Management.

Enhanced myometrial vascularity (EMV) is a common postpregnancy (ie, postpartum, postpregnancy termination, and postpregnancy loss) sonographic finding that represents involuting myometrial hypervascularity of the placental bed in the early puerperium and may persist if retained products of conception (RPOC) are present. On color Doppler US images, EMV is characterized by tortuous and dilated myometrial vessels with high-velocity and turbulent flow. These features can be easily mistaken for an arteriovenous malformation (AVM). However, despite sharing overlapping flow patterns, AVM and EMV are

Massive retained products of conception with arteriovenous fistula successfully treated with arterial balloon occlusion and laparoscopic hysterectomy.

OBJECTIVE: We report the case of retained products of conception (RPOC) with massive uterine arteriovenous fistula (AVF). CASE REPORT: A 42-year-old woman was referred to our hospital for having abnormal pregnancy at 9 weeks. Ultrasonography showed a deformed gestational sac. Serum hCG levels were initially markedly elevated but demonstrated a progressive decline over time. At 20 weeks gestation, a uterine mass grew with an abundant blood flow to replace the myometrium. The 3-dimensional blood vessel image constructed from the contrast enhanced CT showed a formation of massive AVF in the uter
