Safeguarding the Ureter: Strategies to Prevent Intraoperative Ureteric Injury in Gynaecological Surgery.

Iatrogenic ureteric injury is an uncommon but serious complication of gynaecological surgery, with significant potential for renal impairment, fistula formation, prolonged hospitalisation, and medico-legal consequences. The ureter is at risk not only in complex pelvic procedures but also during apparently straightforward operations, and injuries may go unrecognised at the time of surgery, contributing to delayed diagnosis and worse clinical outcomes. We present a case series of five female patients aged 33, 59, 45, 46, and 47 years, respectively, who underwent complex gynaecological surgery fo

Broad Ligament Hernia Causing Small Bowel Obstruction: When Gynecologic Symptoms Conceal Surgical Emergencies-A Case Report.

Internal hernias are an uncommon cause of small bowel obstruction (SBO), accounting for approximately 1% of cases. Among these, broad ligament hernias represent a rare subtype, comprising approximately 4% of internal hernias. Despite their rarity, they carry a significant risk of bowel strangulation and ischemia, necessitating prompt recognition and surgical intervention. We report the case of a 35-year-old G5P3023 woman with no prior abdominal surgeries who presented with a 2-day history of sharp abdominal pain, nausea, and vomiting. Initial imaging findings were suggestive of a tubo-ovarian

Post-hysterectomy leiomyoma: an everlasting enigma.

Broad ligament leiomyomas are rare extra-uterine smooth muscle tumours that may present a diagnostic challenge, particularly in patients with a prior hysterectomy where they may closely mimic malignancy. We report a case of a woman in her early 60s, with a history of laparotomy and hysterectomy, who presented with abdominal pain and a pelvic mass detected on imaging. Intraoperatively, a large, solid, cystic pelvic mass was identified, adherent to the adjacent pelvic structures and deriving vascular support from the surrounding tissues. The mass was excised and sent for histopathological examin

When Benign Masquerades as Malignant: A Case of Expansively Growing Ovarian Leiomyoma Challenging Preoperative and Intraoperative Diagnosis.

Ovarian leiomyoma (OL) is an exceedingly rare benign tumor that originates from ovarian smooth muscle cells and accounts for approximately 0.5%-1% of all benign ovarian neoplasms. To date, around 100 cases have been documented worldwide. OL is often asymptomatic and is typically discovered incidentally, presenting as a small, well-circumscribed, noninvasive solid or cystic mass within the ovary. We report a case of a 22-year-old woman who presented with Abnormal Uterine Bleeding (AUB). Imaging identified a solid left adnexal mass. Laparoscopic exploration revealed features suggestive of malign

Primary pelvic extraskeletal Ewing sarcoma: a case report and comprehensive literature review of molecularly confirmed cases arising within the female genital tract.

INTRODUCTION: Primary extraskeletal pelvic Ewing sarcoma within gynecologic organs is extremely rare and current literature is limited to case reports or series. Definitive diagnosis requires molecular confirmation of the EWSR1 fusion gene which drives cancer proliferation. Current treatment is guided by the National Comprehensive Cancer Network (NCCN) guidelines for Ewing sarcoma of the bone. CASE DESCRIPTION: A previously healthy 21-year-old woman presented to her primary care provider with two weeks of malaise and pelvic symptoms. Imaging revealed a 10 cm heterogenous cystic and solid mass

Situs inversus totalis with bilateral broad ligament defects identified during laparoscopic surgery: a case report.

BACKGROUND: Peritoneal defects of the broad ligament of the uterus are uncommon anatomical abnormalities that may be congenital or acquired. Situs inversus totalis is a congenital condition characterized by a complete mirror-image arrangement of the thoracic and abdominal organs. We report what appears to be the first case in which bilateral peritoneal defects of the broad ligament were incidentally identified in a patient with situs inversus totalis during laparoscopic surgery performed for suspected endometrial cancer. CASE PRESENTATION: A 71-year-old Japanese woman with a known history of

Ectopic ureter opening into the vagina in an adult with duplex kidney: a case report and literature review.

INTRODUCTION: Duplex kidney with an ectopic ureter typically presents with continuous urinary incontinence in childhood. Adult females presenting with renal colic and intermittent vaginal discharge are rare, especially when initial ureteroscopy is normal. METHODS: We report a 26-year-old woman with a one-week history of severe right flank pain, each episode accompanied by a small amount of clear, odourless vaginal discharge. She had undergone caesarean section four months earlier. Non-contrast computed tomography (CT), magnetic resonance imaging (MRI) and magnetic resonance urography (MRU) we

A rare case of multiple papillary cystadenomas found in a female patient with von Hippel-Lindau syndrome.

Papillary cystadenoma (PC) is a benign epithelial tumor believed to be of mesonephric origin and is recognized as one of von Hippel-Lindau syndrome (VHL)-related neoplasms. PC tends to develop frequently in male VHL patients but rarely in females and arises from the mesosalpinx or broad ligament in females. Having experienced a case of multiple PCs in a female patient, we here report it with literature review. The patient was a 34-year-old woman. In her past medical history, VHL including multiple hemangioblastomas of the central nervous system, pancreatic neuroendocrine tumor, and multiple cl

Vaginal Cuff Closure with a Figure-of-Eight Suture in Total Laparoscopic Hysterectomy: Outcome from 173 Consecutive Cases.

Background and Objectives: Hysterectomy is one of the most frequently performed gynecological surgical procedures worldwide. It is well established that the laparoscopic approach offers better outcomes for patients compared to abdominal hysterectomy. However, the technique for vaginal cuff closure during laparoscopic hysterectomy (TLH) remains insufficiently standardized in the international literature. Based on our results, we aim to demonstrate that closure of the vaginal cuff using a figure-of-eight suture is a sufficient, reproducible, time-efficient and safe method during total laparoscop

Magnetic resonance imaging-guided surgical strategy for secondary abdominal pregnancy implanted in the broad ligament: A case report highlighting hemorrhage-control techniques.

Secondary abdominal pregnancy implanted in the broad ligament poses a high risk of catastrophic hemorrhage because of complex parametrial vascularization. Optimal surgical planning is therefore critical. A 28-year-old primigravida at 13 weeks' gestation was diagnosed with secondary abdominal pregnancy at Gansu Provincial Maternity and Child Care Hospital in October 2023. Pelvic magnetic resonance imaging precisely delineated placental attachment to the posterior leaf of the left broad ligament and identified adjacent hypervascular parametrial structures. Based on the imaging findings, a hemorr

Case Report: Diagnosis and treatment report and literature review of 2 cases of VHL-deficient renal cell carcinoma.

Von Hippel-Lindau (VHL) disease is a rare familial autosomal dominant disorder with an incidence rate of approximately 1 in 36,000. It primarily results from mutations or inactivation of the VHL tumor suppressor gene located on chromosome 3p25-p26. The hallmark of this disease is hereditary hemangioblastoma, which can affect multiple organs and systems, including the brain (commonly infratentorial), spinal cord, retina, and internal organs such as the kidneys, adrenal glands, and pancreas. Less commonly, lesions may include papillary cystadenomas and endolymphatic sac tumors (ELST), which can

Ureteric Recovery Without Stenting After Intraoperative De-ligation During Cesarean Section: A Case Report.

Urinary tract injury during cesarean section is uncommon, but the risk is much higher in the case of multiple sections due to dense adhesions. Bladder injury is the most frequent. Accidental ureteric injury is also rare; however, the combination of bladder rupture, ureteric ligation, and massive uterine artery hemorrhage in a single operation is what happened in our case. Current guidelines recommend the placement of a ureteric stent after de-ligation, but this is a situation in which stenting was not technically achievable. We report a case of a 36-year-old woman, gravida 4 para 2, with two p

Pseudo-Broad Ligament Leiomyoma: A Rare Cause of Diagnostic Uncertainty.

Broad ligament fibroids are rare extrauterine leiomyomas that present diagnostic and surgical challenges due to their atypical location and distortion of pelvic anatomy. Large tumors may closely mimic adnexal masses, making accurate preoperative diagnosis difficult. A 44-year-old multiparous woman presented with progressive abdominal distension and a sensation of heaviness and pain in the lower abdomen for one year. Clinical examination and ultrasonography suggested a uterine fibroid, but the precise site of origin could not be determined. Intraoperatively, a large abdominopelvic mass measurin

Post-operative umbilical cellulitis probably related to incidental colonic puncture by Veress needle entry during insufflation.

OBJECTIVE: Laparoscopic surgery has become increasingly prevalent in modern gynecologic practice. However, this approach is not without risks. The most dangerous step is insufflation of the abdomen using a Veress needle or the blind creation of the camera port at the beginning of the surgery. Here, we report an interesting case of an umbilical camera port abscess, probably resulting from an incidental puncture of the colon by a Veress needle during initial insufflation. CASE REPORT: A 61-year-old Taiwanese postmenopausal woman underwent laparoscopic surgery for bilateral adnexal solid masses,

Economic evaluation of tirzepatide, semaglutide, and liraglutide in the treatment of obese and overweight patients without diabetes: a short-term cost-effectiveness analysis in Iran.

Hakura Hashimoto described Hashimoto's Thyroiditis (HT) in 1912. In HT, there occurs formation of antibodies against the self (thyroid), which causes enlargement of the thyroid gland as a whole and decreased thyroid hormone levels. Most of the patients with HT are given medical line of management (hypo/hyperthyroidism). The indications for surgical line of management for HT are due to pressure symptoms over the trachea and oesophagus, pain in the thyroid gland and for chance of malignancy. Surgical treatment gives complete relief from pain and pressure symptoms and also prevents the risk of ma

PLAG1-rearranged uterine mesenchymal tumours: A study of 11 cases showing distinct histological features beyond myxoid morphology.

AIMS: PLAG1 gene fusions have been identified in a subset of uterine sarcomas and were historically associated with myxoid morphology. However, recent evidence shows that not all cases demonstrate myxoid features or conventional smooth muscle immunophenotype. Herein, we present 11 cases of PLAG1-rearranged uterine mesenchymal tumours to further characterize their clinicopathologic, immunohistochemical and molecular profiles. METHODS: Eleven cases of PLAG1-rearranged uterine mesenchymal tumours were included from Fudan University Shanghai Cancer Center, comprising nine consultation referrals a

Uterine Leiomyomas with Intravenous Leiomyomatosis: A Report of Two Cases.

PURPOSE: Intravenous leiomyomatosis (IVL) is a rare histological variant of uterine leiomyoma characterized by benign smooth muscle cells proliferating within the venous system. Despite its benign histology, IVL exhibits "quasi-malignant" behavior through intravascular extension, often leading to misdiagnosis as venous thrombosis or malignancy. CASE PRESENTATION: We report two cases of uterine leiomyoma complicated by IVL. In Case 1, a 42-year-old woman with initially suspected iliac vein and inferior vena cava (IVC) thrombosis underwent contrast-enhanced computed tomography (CT), which demon
