When a Lower Abdominal Lump Turned Out to Be a Stromal Tumor of Unknown Malignant Potential.

Abdominopelvic masses usually do not occur in young females and should be evaluated carefully to rule out rare smooth muscle tumors. Here, we present the case of a 23-year-old female who complained of pain in the lower abdomen, urinary frequency, constipation, and dysuria for the last two months. The examination showed a solid, immobile lower abdominal mass measuring approximately 20 × 25 cm. MRI revealed a clear-cut abdominopelvic lesion measuring 7.9 × 19.7 × 30.4 cm, and fluorodeoxyglucose-positron emission tomography depicted subtle hypermetabolic activity, indicative of a low-grade stroma

A primary fistula connecting a broad ligament leiomyoma to the cervix.

The endocervico-myomatous fistula is a rare condition that has been reported as a complication following uterine artery embolization. To date, no primary case of this nature has been documented in the PubMed database. The objective of this study was to present the diagnostic and therapeutic approaches for a primary fistula connecting a broad ligament leiomyoma to the cervix. A 35-year-old woman with vaginal bleeding, transvaginal ultrasound revealed features initially interpreted as a bicornuate unicollis uterus according to the ASRM 2021 classification, with 2 distinct uterine cavities commun

Uterine-sparing Robot-assisted Sacrocolpopexy for Pelvic Organ Prolapse in Women Aiming for Pregnancy.

Pelvic organ prolapse (POP) is frequently encountered in elderly women. However, it can also affect relatively young women of reproductive age. Here, we present three cases of uterine-sparing robot-assisted sacrocolpopexy (RSC) in women aiming for pregnancy. All cases were diagnosed with POP-Q stage III. Utilizing the da Vinci X or Xi robotic surgical system, they underwent uterine-sparing RSC with dual mesh anterior and posterior walls. The anterior wall mesh was not a Y-shaped mesh encircling the cervix; The anterior wall mesh was not a Y-shaped mesh encircling the cervix; rather, it was pla

Laparoendoscopic single site surgery for cervical myomectomy: surgical technique.

Cervical fibroid is a rare type of fibroid which occurs in a special location. Due to the low position, it tends to grow into the broad ligament or even extend into the pelvic floor, compressing the ureter or uterine artery. Surgical treatments for cervical myoma are divided into three categories: open transabdominal cervical myomectomy, laparoscopic cervical myomectomy, and transvaginal cervical myomectomy. Among these, laparoscopic cervical myomectomy is the most common. Traditional multiport laparoscopy (MPL) often encounters difficulties in specimen retrieval after cervical myomectomy, fre

Labium Majus Ectopic Pregnancy: Diagnostic and Therapeutic Implications of a Rare Obstetric Phenomenon-A Case Report.

Ectopic pregnancy occurs when a fertilized ovum implants outside the uterine cavity, most commonly in the fallopian tube. While rare, ectopic pregnancies can also occur in atypical locations such as the ovary, cervix, abdominal cavity, or broad ligament, often posing significant diagnostic and therapeutic challenges. We report a highly unusual case of ectopic pregnancy implanted in the left labium majus, a site not previously documented in the literature. A 20-year-old woman presented with abdominal pain, vulvar swelling, and amenorrhea of approximately 4 months. Initial imaging and clinical e

Analysis of Effect of Melatonin on Postoperative Sensory Nerve Recovery Following Open Reduction and Internal Fixation of Anterior Mandible Fractures: A Prospective Randomized Control Trial.

INTRODUCTION: HbA1c is a biochemical marker to evaluate hyperglycemia status in past 2-3 months. HbA1c ≥ 6.5% is diagnostic criteria for diabetes mellitus according to the American Diabetes Association. Recent studies suggest that anemia alters HbA1c levels, but results are conflicting. This study was planned to understand utility of HbA1c in relation to hemoglobin (Hb) for the diagnosis of gestational diabetes mellitus (GDM) and also to understand if HbA1c can be used as a screening modality for diagnosis of GDM in countries with higher anemia prevalence. MATERIALS AND METHODS: An observatio

Case report: Emergency management of unilateral herniation of bowel and a bilateral defect of the broad ligament in a resource limited setting.

INTRODUCTION: Broad ligament herniation is a rare type of internal hernia, presenting as a diagnostic challenge. An exploratory laparotomy can be performed to definitively diagnose and treat the defect. CASE PRESENTATION: A 36-year-old female, with no known comorbids and no significant past medical and surgical history presented with acute abdominal pain, multiple episodes of vomiting, and absolute constipation. After clinical and radiologic investigations, the diagnosis of an internal herniation of small bowel to the broad ligament was suspected. An emergent exploratory laparotomy was then p

Holes can be perilous: A rare presentation of intestinal obstruction - Herniation through the broad ligament.

A 57-year-old lady presented with symptoms of intestinal obstruction preceded by a 2-day history of lower abdomen pain. Clinically, she had a distended abdomen with tenderness in her lower abdomen. Laboratory investigations were unremarkable. An abdominal radiograph showed a dilated small bowel with no extensive bowel gas. A computed tomography (CT) scan showed suspected intestinal obstruction secondary to herniation into the right broad ligament. The decision was made to proceed with surgery, and the intraoperative results confirmed the CT results. The literature review is outlined here, and

Computed tomography of a canine omental torsion secondary to a chronic abdominal gossypiboma and concurrent incidental broad ligament torsion.

A 5-year-old female spayed Dogue de Bordeaux was referred for concerns of an abdominal mass and peritoneal effusion. Abdominal radiographs identified a mid-ventral abdominal soft tissue opaque mass containing a radiopaque marker consistent with a gossypiboma. Contrast-enhanced abdominal CT identified two whirl signs associated with the abdominal gossypiboma. Exploratory laparotomy confirmed an omental torsion with encapsulated gossypiboma and concurrent incidental torsion of the remnant of the right broad ligament. Based on a literature review, omental torsions are an unreported complication o

Perivascular Epithelioid Cell Tumor with Uncertain Malignant Potential Arising in the Round Ligament.

A 12-year-old adolescent was diagnosed with a right-sided solid mass in the round ligament of the uterus. The chief complaints were abdominal pain and pelvic discomfort. She underwent laparoscopic tumor resection. Histological examination demonstrated a trabecular growth pattern of epithelioid cells with mitotic activity (3 per 50 HPF), which expressed melanocytic and myoid markers. Due to aforementioned findings, a final diagnosis of perivascular epithelioid cell tumor (PEComa) with uncertain malignant potential was made. To the best of our knowledge, this localization of PEComa is considered

Ewing Sarcoma of the Female Genital Tract: Clinicopathologic Analysis of 21 Cases With an Emphasis on the Differential Diagnosis of Gynecologic Round Cell, Spindle, and Epithelioid Neoplasms.

Ewing sarcoma is an uncommon neoplasm considered in the differential diagnosis of tumors with "small round cell" morphology, but its occurrence in the gynecologic tract has only been sporadically documented. Herein, we describe the largest cohort of Ewing sarcoma localized to the female genital tract to date, and emphasize their clinicopathologic resemblance to more common gynecologic neoplasms. Ewing sarcoma (n=21) was retrospectively identified from 5 institutions. The average patient age was 35 (range 6-61) years. Tumor sites included uterus (n=8), cervix (n=4), vulva (n=5), vagina (n=1), b

Don't be thrown for a loop: a review of internal hernias for the abdominal imager.

Internal hernias are herniations of abdominal viscera, usually small bowel, through congenital or acquired openings of the peritoneum or mesentery. Congenital hernias may involve anatomic fossae such as the epiploic foramen as well as those related to abnormal peritoneal or mesenteric defects; these include left and right paraduodenal, transomental, transmesenteric, pericecal and broad ligament hernias. Acquired hernias are due to defects in the mesentery or peritoneum, usually resulting from prior surgeries, and include those associated with Roux-en-Y surgery as well as colorectal cancer rese
