The objective of this study was provide an overview of current practices on hormonal therapy (HT) in children with undescended testes (UDT). An online questionnaire about HT, referral patterns, and orchidopexy was spread via several (social media) networks involved in pediatric urology. A total of 283 individuals responded, with 54 countries sampled. The majority (84%) did not prescribe HT for UDT, predominantly due to a paucity of scientific evidence (76%). Among those who use HT, main reason was bilateral UDT (62%). There was no clear preference in administration type (50% intramuscular inje

Spigelian-cryptorchidism syndrome is a rare and diagnostically challenging condition in neonatology that is characterized by the association of an undescended testis with a Spigelian hernia, the latter usually without radiologic evidence. We report the case of a full-term male neonate delivered at 39 weeks and two days of gestation by kiwi-assisted vaginal delivery after a spell of fetal bradycardia. Antenatal imaging had shown persistent bilateral pelviectasis of the kidneys and an undescended right testis. Postnatally, the initial course was normal; however, clinical evaluation showed partia

RATIONALE: Undescended testis (cryptorchidism) is the most common congenital anomaly of the urogenital system. Testicular tissue exhibits variable fluorodeoxyglucose (FDG) uptake and may demonstrate a physiologically increased standardized uptake value. In cases of undescended testis with FDG uptake, immediate recognition can be challenging, increasing the risk of misinterpretation as a pathological lymph node.
PATIENT CONCERNS: A 45-year-old male inpatient, was referred for ultrasound-guided biopsy of a right inguinal structure suspected to be a metastatic lymph node. Three weeks earlier, hi

OBJECTIVE: This study aimed to evaluate the functional status of the urethra using uroflowmetry before surgery, as well as three and six months postoperatively in cases of distal hypospadias.
MATERIAL AND METHODS: Thirty-nine consecutive patients who underwent surgery for distal hypospadias (hypospadias group) between 2016 and 2019 were prospectively included as part of this study. The control group consisted of 40 patients with a normal urethra who underwent surgery due to conditions other than hypospadias (phimosis, undescended testis, hernia). Uroflowmetry was performed preoperatively in t

Disorders of sex development or hermaphroditism are a condition associated with a discrepancy between genetic, gonadal, and phenotypic sex. Persistent Müllerian duct syndrome (PMDS) is a rare form of male pseudohermaphroditism. Typical features are cryptorchidism and the presence of underdeveloped fallopian tubes, uterus or upper vagina in a male with karyotype 46, XY. Over the past 50 years, about 200 cases of persistent Müllerian duct syndrome have been reported. The article describes an observation of a 33-year-old patient with bilateral cryptorchidism, testicular neoplasm, and false hermap

Spigelian hernia (SH) is an infrequent aponeurotic defect in Spiegel's semilunar line. The literature on pediatric SH is scarce. A comprehensive review of the previous literature was conducted. Eligible studies were identified by searching primary medical bibliography databases, and a pooled analysis of published case-level data was performed. Medians and interquartile ranges were used to describe the quantitative variables and proportions for categorical variables. The Kruskal-Wallis, Mann-Whitney U, and Fisher's exact tests were used to compare group variables. Spearman's and Pearson's corre

Background: Undescended testis (UDT) is the most frequent pediatric anomaly of the male genitals, with a high incidence in premature male neonates. Due to the risk of long-term complications such as infertility, testicular malignancy, and psychological distress, special attention on the accuracy of management is needed. Despite the existence of well-established guidelines recommending early surgical intervention, significant delays in diagnosis, referral, and treatment are still observed in practice. Objectives: This study aims to evaluate the clinical management practices of undescended testi

BACKGROUND: Torsion of undescended testis (UDT) is a rare urological emergency requiring immediate intervention to salvage the affected testis. Ultrasonography is used for identifying torsion of UDT, but it is mostly performed by radiologists or urologists. Point-of-care ultrasound (POCUS) is currently used in pediatric emergency care but reports of its use for diagnosing torsion of UDT are scarce.
CASE REPORT: A previously healthy 11-year-old male patient presented to our emergency department (ED) complaining of swelling of his left inguinal area with mild pain. He denied having any fever, v

BACKGROUND: Bilateral testicular tumors in infants are extremely rare. This case report describes synchronous bilateral mature teratomas complicated by torsion of an intra-abdominal undescended testis (IAT), and underscores the clinical importance of early diagnosis, timely surgical intervention, and fertility-preserving management, providing valuable reference for future cases.
CASE PRESENTATION: A 3-month-old boy presented with an empty right hemiscrotum. Imaging revealed a right intra-abdominal mass (22.8 × 15.9 × 21.3 mm) and left testicular lesion (7.1 × 3.9 × 7.0 mm). Serum alpha-fetopr

BACKGROUND/OBJECTIVES: Undescended testis (UT) is a common congenital urological condition in boys, with an incidence of 2-8%. Orchiopexy is the recommended surgical treatment for UT, ideally performed before 12 months of age, with a latest window of 18 months. Testicular torsion (TT) is a pediatric urological emergency, occurring in 3.8 per 100,000 boys. While both UT and TT are common conditions individually, their co-occurrence in children under 18 months is rare and represents a challenging clinical scenario, especially when diagnosis and treatment are delayed, increasing the risk of testi

Torsion of an intra-abdominal undescended testis is an exceptionally rare cause of acute abdomen in adults and may mimic more common conditions such as appendicitis or pelvic abscess. We present a 28-year-old man with fever and right lower quadrant pain, leukocytosis, and an empty right hemiscrotum. CT revealed a large nonenhancing hypogastric mass with a twisted pedicle, and laparotomy confirmed torsion of a necrotic intra-abdominal testis. Histopathology showed hemorrhagic necrotizing orchitis without malignancy. This case highlights the importance of considering intra-abdominal testicular t

BACKGROUND: The unique altered biological processes in testicular germ cell tumor (TGCT) patients with cryptorchidism history require further investigation and the accumulation of tumor-related features in cryptorchidism remains unclear.
MATERIALS AND METHODS: We analyzed transcriptome and DNA methylation (DNAme) data from normal, undescended testis (UDT), and TGCT samples. Clustering, pseudo temporal trajectory analysis, and functional annotation of differentially methylated probes (DMPs) and differentially expressed genes (DEGs) were performed. Protein-protein interaction (PPI) networks wer

PURPOSE: Boys with genetic syndromes have an increased risk of undescended testes (UDT), but laparoscopic findings and outcomes after two-stage Fowler-Stephens orchiopexy (FS2) are rarely reported. This study aims to compare laparoscopic findings and results after FS2 between syndromic and non-syndromic boys.
METHODS: A retrospective cohort study on boys with UDT who underwent laparoscopy between 2014 and 2024. Syndromic and non-syndromic patients were compared regarding age at surgery, bilaterality, type of UDT, and atrophy rate after FS2.
RESULTS: In total, 190 boys with 298 testicles unde

A 1-year-old boy with a known left undescended testis presented with 7 days of fever, irritability, and progressive lower-abdominal distension. He was febrile and tachycardic with a firm lower-abdominal mass and an empty left hemiscrotum; white blood cells 17 × 109/L, C-reactive protein 200 mg/L, platelets 680 × 109/L, hemoglobin 6.8 g/dL. Ultrasound showed a heterogeneous avascular intra-abdominal mass; computed tomography showed a large cystic lesion containing fat and calcifications with a left pedicle "whirlpool sign". Urgent laparotomy with detorsion and excision revealed prepubertal-type

Complete testicular-epididymal non-fusion (cTENF) is a rare and underestimated congenital anomaly, commonly associated with an undescended testis. Dissociated epididymis is always located distally to the testis and frequently into the scrotum, resembling a testicular nubbin. We report the case of an 18-month-old boy with left cryptorchidism and cTENF initially regarded as a testicular remnant due to testicular atrophy. We highlight the distinct anatomical features of this condition to overcome the diagnostic challenge and avoid pitfalls that may lead to significant consequences.
