Detection of insulin-like growth factor receptor-1 in the human cremaster muscle and its role in the etiology of the undescended testis.

PURPOSE: Previous studies of undescended testis (UT) has focused on insulin-like hormone 3 (INSL3), the genitofemoral nerve, and androgens in the testicular descent. Leydig cells, which are under the control of insulin-like growth factor-1 (IGF1), produce both androgens and INSL3. We aimed to investigate whether insulin-like growth factor receptor-1(IGFR1) exists in the cremaster muscle (CM) complex and is associated with normally descended testis as well as UT cases in humans. METHODS: We studied 30 CM from 15 patients who comprised the UT group (UTG), and 15 patients with unilateral testicu

45,X/46,XY Mosaicism Presenting With Isolated Unilateral Cryptorchidism and a Normal Blood Karyotype.

CONTEXT: 45,X/46,XY mosaicism is a disorder of sex development leading to abnormal gonadal development and to unpredictable genital phenotype, growth, and pubertal development. CASE DESCRIPTION: A 2-year-old male presented with a right impalpable testis. Blood karyotype was 46,XY. A laparoscopy performed for right orchidopexy revealed a right streak gonad with Mullerian structures, whereas on the left side, a normal descended testis was present. The karyotype of the removed gonad was 45,X/46,XY. The child grew along the second centile, within the midparental height (MPH) range, until the time

Histopathology of Unilateral Cryptorchidism.

Defective mini-puberty inducing insufficient gonadotropin secretion is one of the most common causes of nonobstructive azoospermia in men suffering from congenital isolated unilateral or bilateral cryptorchidism. The aim of our study was to determine the risk for azoospermia by histologic criteria in a cohort of unilateral cryptorchid boys undergoing orchidopexy and bilateral testicular biopsy. We performed a retrospective analysis of data available in the library of the Cryptorchidism Research Institute, Liestal, Switzerland. Complete histological evaluations were available for 319 boys opera

Acquired undescended testis and possibly associated testicular torsion in children with cerebral palsy or neuromuscular disease.

INTRODUCTION: Torsion of an undescended testis (UDT) associated with cerebral palsy (CP) and neuromuscular disease (NMD) is an uncommon condition that is not well recognized by primary care physicians or healthcare providers. OBJECTIVE: The objective of this study was to highlight the clinical importance of torsion of a UDT in children with CP and NMD. MATERIALS AND METHODS: Eleven children with testicular torsion of a UDT operated on at the study institute between 1991 and 2015 were identified. The records of seven children (63.6%) associated with CP or NMD were retrospectively reviewed. Cl

A Rare Variation of Transverse Testicular Ectopia (TTE) in a Young Adult as an Incidental Finding during Investigation for Testicular Pain.

Transverse testicular ectopia (TTE) with fused vas deferens is an extremely rare clinical entity. Herein, we present a case of a 19-year-old patient with persistent left testicular pain lasting for a week. Clinical examination revealed an empty right hemiscrotum, a normal left-sided descended testis, and in close proximity a mass-like structure resembling testicular parenchyma. Laboratory tests were significant for elevated follicle-stimulating hormone (FSH), while sperm count revealed azoospermia. Ultrasound imaging (US) of the scrotum demonstrated the presence of both testes in the same left

To 'Pex or Not to 'Pex: What to Do for the Contralateral Testis When a Nubbin Is Discovered.

PURPOSE OF REVIEW: Testicular remnants or nubbins are commonly found in the evaluation and treatment of cryptorchidism. While much debate focuses on the management of the nubbin itself, there is also great uncertainty and variation in the management of the contralateral descended testis. Herein, we review the relevant literature informing the decision to perform a contralateral orchiopexy. RECENT FINDINGS: Although there is very little recent literature directly addressing the question, some studies have better characterized differences in practice, the risk of intravaginal torsion in the con

Castration in the standing horse combining laparoscopic and conventional techniques.

BACKGROUND: Standing laparoscopic castration without orchidectomy has been shown to be a reliable method of castration in animals with abdominal cryptorchidism, but it cannot be recommended as a method of castration in those with inguinal cryptorchidism or normal males. An alternative laparoscopic method for the castration of horses with descended testes was developed. OBJECTIVES: To report the results of laparoscopy-assisted abdominal transection of the spermatic cord and a modified open method in the standing horse for castration of descended testes in unilateral abdominal cryptorchids. Contralateral metachronous undescended testis: Is it predictable?

INTRODUCTION: Metachronous undescended testis (mcUDT), an acquired UDT after contralateral orchiopexy, can occur in some boys. If one were able to predict its occurrence, one might consider a proactive approach or at least one would be able to counsel the parents accordingly. Our hypothesis was there may be characteristics evident at the time of initial orchiopexy which could predict the development of contralateral mcUDT. OBJECTIVE: The aim was to Identify factors present at initial orchiopexy that predict development of subsequent mcUDT. STUDY DESIGN: Subjects were identified using the Cur

Smoking and its association with cryptorchidism in Down syndrome.

INTRODUCTION: Cryptorchidism is a common and prevalent condition in patients with Down syndrome. Environmental factors, such as smoking, can be associated with malformations during fetal development. The study of the prevalence of cryptorchidism and its association with parental tobacco use in Down syndrome can contribute to alert health care professionals, patients and family members regarding the prevention of the harms caused by cryptorchidism and its possible predisposing factors. OBJECTIVE: To evaluate the prevalence of cryptorchidism in Down syndrome and its association with maternal an

Morcellation for testes extraction in horses undergoing standing laparoscopic cryptorchidectomy.

The major objective of this study was to describe the use of morcellation for standing laparoscopic cryptorchidectomy in 30 client-owned horses. A second objective was to describe a laparoscopic-assisted standing scrotal technique for removal of a descended testis in unilateral cryptorchids. Morcellation for extraction of abdominal testes is effective, efficient, and well-tolerated. Morcellation enabled safe and straightforward extraction of testes from the abdomen through an approximately 13- to 20-mm incision using a 2-portal technique. No incisional or morcellator-related complications occu

Advantage of early orchiopexy for undescended testis: Analysis of testicular growth percentage ratio in patients with unilateral undescended testicle.

This study reports the experience of our tertiary referral center and proposes a new indicator, the growth percentage ratio (GPR), for determining the optimal timing of surgical intervention. A retrospective review of boys who underwent orchiopexy for undescended testis from 2001 to 2013 was conducted. We analyzed testicular volumes in different age groups using the UDT to normally descended testis ratio and testicular GPR. A total of 134 boys with unilateral undescended testicle underwent regular ultrasonography follow-up examinations for more than a mean of 3.9 years. Forty-five (33.4%) of t

Macroscopic Sertoli cell nodule of the testis containing numerous spermatogonia.

Sertoli cell nodule consists of clusters of seminiferous tubules composed of immature Sertoli cells and, in some cases, spermatogonia. We report a case of macroscopic Sertoli cell nodule that occurred in the descended left testis of a 26-year-old man. The nodule was centrally located and measured 1.7cm in greatest dimension, the largest such lesion reported to date. Spermatogonia were prominent, a feature that is particularly common in the larger nodules. Macroscopic Sertoli cell nodules (greater than 0.5cm) typically occur in a descended testis. The age range of patients in cases reported sin

A caecal pseoudotumour with an incidental adenomatoid testicular tumour in a man with right undescended testis: a case report.

BACKGROUND: Inflammatory pseudotumour refers to a non-malignant tumour-like mass resulting from an inflammatory reaction that is composed of granulation tissue with leukocyte infiltration that commonly occurs in the paediatric or young adult population. These tumours occur more commonly in the lungs and the orbit but rarely does it affect the gastrointestinal tract. It poses a clinical diagnostic challenge since it is a benign condition than can mimic the malignant counterpart. Our case is a rare presentation of the caecal pseudotumour in the presence of a right undescended abdominal testis ev

Influence of naturally unilateral cryptorchidism on the histomorphometry of the testes and daily sperm production in West African Dwarf goats.

Quantitative histology of the descended testis of unilateral cryptorchid bucks was compared with testis of normal bucks to evaluate the reproductive potentials of the scrotal testis in unilateral cryptorchids, using light microscopy techniques. The contralateral scrotal testes of the unilateral cryptorchids and the testes of the normal bucks contained profiles of seminiferous epithelium and each showed histological evidence of normal activity. The mean heights, lengths, lumen diameter, diameter of the seminiferous tubules were significantly higher in the contralateral scrotal testes when compa

Malignant seminoma in two unilaterally cryptorchid stallions.

Two unilateral cryptorchid stallions were referred to the clinic because of chronic debilitating condition with emaciation. Rectal examination, and ultrasound and gross examination revealed in both animals an abdominal mass, caudally of the kidney, and multiple nodules spread over the abdomen. Histologic analysis revealed an intra-abdominal malignant seminoma with intraperitoneal and renal metastasis. Interestingly, a seminoma was also present in the descended testis of the draught horse.

Scrotal ultrasound findings in previously congenital and acquired unilateral undescended testes and their contralateral normally descended testis.

The aim of this study was to report on different anomalies found by physical examination and scrotal ultrasound in men with previously unilateral congenital undescended testes (UDT; N = 50), acquired UDT (N = 49), their contralateral normally descended testis (CNDT) and control testes (N = 53). Acquired UDT significantly more often had a testicular volume being <15 mL than congenital UDT (88% vs. 68%). In the congenital group, significant differences were found between UDT and CNDT for soft consistency (UDT 36% vs. CNDT 14%), epididymal diameter (UDT 7.6 mm vs. CNDT 8.9 mm), testicular volume

Characteristics of Sertoli cells in the ectopic and scrotal testes of unilateral cryptorchid West African dwarf goats.

Sertoli cell population and histology were studied in testes of unilateral cryptorchid goats. Histomorphological and electron microscopic techniques were employed. The results demonstrated that the number of Sertoli cell per testis was significantly (p < 0.05) higher in the descended scrotal testes of unilateral cryptorchid bucks compared with the right and left testes of the normal bucks, respectively. However there was no significant difference (p > 0.05) on comparing the number of Sertoli cells in the scrotal testes of the unilateral cryptorchid bucks with the paired testes of the normal bu
