BACKGROUND: Inguinoscrotal herniation of the urinary bladder (ISHB) is a rare but underdiagnosed condition, accounting for 1-4% of inguinal hernias and discovered intraoperatively in more than 80% of cases. Recognised predisposing factors include advanced age, obesity, and chronic infravesical obstruction, of which benign prostatic hyperplasia (BPH) is the most common. Failure to identify ISHB preoperatively exposes the surgeon to the risk of inadvertent vesical injury, reported in up to 12% of unprepared herniorrhaphies.

BACKGROUND: Scrotal abscesses requiring hospitalization are uncommon, with progression to sepsis being even rarer. A feared complication of a scrotal abscess is Fournier gangrene, an uncommon type of necrotizing fasciitis that starts in the scrotum or perineum. Fournier gangrene is associated with a mortality rate of about 40% and accounts for less than 0.02% of all hospital admissions. The patient in this report was hospitalized for a complex scrotal abscess and, during repeat incision and drainage (I&D), was found to have Fournier gangrene.

This report describes two unique gonadal serous carcinomas, one low-grade serous carcinoma (LGSC) of the ovary and one serous carcinoma of the scrotum each harboring novel PAK gene fusions. The first patient with ovarian LGSC was identified to have a PTPRM::PAK1 fusion, identifying PTPRM as a novel fusion partner for PAK1. The case is presented of a 69-year-old male patient who underwent surgery for a giant inguinoscrotal hernia on the right side. Intraoperatively, a giant liposarcoma in the right scrotum was discovered. A complete (R0) resection of the tumour was performed. Two years postoperatively, the patient remains asymptomatic and without any signs of tumour recurrence. This case represents the very rare differential diagnosis of a giant inguinoscrotal hernia.

We studied the malaria transmission dynamics in Kassena Nankana district (KND), a site in northern Ghana proposed for testing malaria vaccines.
