Sparganosis mansoni rarely occurs in the inguinal and perineal regions in Japan. A case of Sparganosis mansoni with a painless mass in the left inguinal region is presented. A 67-year-old male visited our hospital with a complaint of a painless mass in the left inguinal region in May, 1995, and another mass appeared in the scrotum two days after the first visit. From March 1988 to May 1994, 15 patients underwent the treatment protocol of superficial photodynamic therapy (PDT) in dermatological localized malignancies. Two tumours (one M. Queyrat of the penis, one basalioma) were treated primarily; the other 13 patients experienced relapses of underlying disease after treatment by surgery, radiotherapy or chemotherapy. Two unrelated patients with small distal deletions of the long arm of chromosome 13 are described, with shawl scrotum and penoscrotal transposition, penoscrotal hypospadias, a reduced perineum, and anal atresia. The patients have small deletions of 13(q32.2qter) and 13(q32q34), respectively. This report and the literature present evidence for one or possibly more gene(s) within region 13q32.2q34 which regulate the development of the ano-genital structures. The clinical spectrum includes bifid or shawl scrotum, hypospadias, biseptate uterus, malplaced and imperforate anus, and common cloaca.

Forty-three men underwent varicocele scintigraphy to exclude underlying varicocele as a possible cause of decreased semen quality. Fifty-one patients underwent surgery for symptomatic epididymal cysts or hydroceles, using local anaesthesia with no sedation and no diathermy. Of the patients surveyed, 91% found the procedure comfortable or uncomfortable, 9% finding it painful. Most patients (89%) were fully recovered after 3 weeks. None of the patients had to have operative intervention for complications. Treatment of these conditions under local anaesthesia is cost-effective, and can be performed with the minimum of special equipment on patients who might otherwise be unfit for a general anaesthetic.

In connection with a typical case, clinical features of genitaly extended Fournier gangrene--its pathogenic factors and the standpoints of treatment--are demonstrated. This infection of foudroyant nature, going on with significant mortality is held to be a characteristic form of necrotizing fasciitis, constituting main manifestation of toxic shock-like syndrome. Attention is drawn to the fact, that in case of internal diseases, predisposing factors respectively-even by adequate therapy-outcome of the infection may be lethal nowadays as well.
