Are we missing an important point? Risk factors for intraoperative complications and additional procedures when performing post-chemotherapy RPLND for testicular tumors.

PURPOSE: This study aims to investigate the risk factors associated with additional intraoperative procedures during post-chemotherapy retroperitoneal lymph node dissection (PC-RPLND) in patients with advanced germ-cell tumors. MATERIALS AND METHODS: We retrospectively analyzed the data of 206 patients who underwent PC-RPLND for advanced germ-cell tumors between January 1990 and December 2022. Additional intraoperative procedures were defined as surgical interventions necessitated by injuries to intestinal, ureteral, major vascular, or renal vascular structures, as well as planned en-bloc res

Antibiotic prophylaxis in percutaneous nephrostomy placements and replacements for malignant urinary tract obstruction. Retrospective cohort study with systematic review and meta-analysis.

INTRODUCTION: Percutaneous nephrostomy (PN) can lead to urinary tract infections (UTIs), but these can be prevented by administering antibiotics. However, the effectiveness of this approach is debated. METHODS: To evaluate the association between PN-related UTIs and antibiotic prophylaxis in cancer patients. This is a hybrid study was designed. A retrospective cohort study was conducted on cancer patients requiring PN, comparing antibiotic prophylaxis with no prophylaxis. The outcome, UTI, was measured up to seven days after the procedure. This cohort study was included in a systematic review

Diagnostic Value of Routine Delayed Pelvic Imaging for Patients with Biochemical Recurrence of Prostate Cancer Undergoing 68Ga-PSMA PET/CT.

The detection of biochemical recurrence (BCR) of prostate cancer remains challenging, as small-volume disease may fall below the sensitivity of standard imaging. Prostate-specific membrane antigen (PSMA) PET/CT is currently the most sensitive imaging modality for tumor detection at low prostate-specific antigen (PSA) levels, but recurrence continues to be difficult to detect. We evaluated whether routine delayed pelvic imaging at 90 min postinjection improves diagnosis of sites of recurrence in patients referred for BCR. Methods: We retrospectively analyzed 201 patients with prostate cancer un

Success rate of Double J stent insertion in patients with advanced cervical cancer and hydronephrosis at tertiary hospital.

INTRODUCTION: Obstructive uropathy is a common complication in advanced cervical cancer, occurring because of ureteral compression by the tumour mass and often leading to hydronephrosis and impaired renal function. Double-J (DJ) stent insertion is frequently used as the first-line decompression method to restore ureteral patency; however, its technical success is influenced by anatomical distortion and severity of obstruction. Understanding the factors associated with successful stent placement is essential to guide appropriate urinary diversion strategies. MATERIALS AND METHODS: This retrosp

Development and multicohort validation of an interpretable postoperative model for intravesical recurrence after radical nephroureterectomy in upper tract urothelial carcinoma.

PURPOSE: Intravesical recurrence (IVR) after radical nephroureterectomy (RNU) for upper tract urothelial carcinoma (UTUC) is frequent, but postoperative recurrence risk is heterogeneous. We developed and multicohort-validated an interpretable postoperative model for IVR risk stratification after RNU. METHODS: We included 813 patients with pathologically confirmed UTUC treated with RNU across four predefined cohorts: retrospective development (n = 400), retrospective external validation (n = 173), internal prospective validation (n = 166), and external prospective validation (n = 74). Long-term outcomes of upper tract urothelial carcinoma in kidney transplant recipients: efficacy of bilateral radical nephroureterectomy and prognostic comparison of unilateral nephroureterectomy with non-transplant patients.

BACKGROUND: The incidence of upper tract urothelial carcinoma (UTUC) is significantly elevated in kidney transplant (KT) recipients, yet the optimal surgical strategy-unilateral radical nephroureterectomy (URNU) versus bilateral radical nephroureterectomy (BRNU)-remains debated, and prognostic comparisons between KT recipients undergoing URNU and non-transplant (NKT) patients are lacking. METHODS: This retrospective study analyzed KT recipients with native-system UTUC treated at a single institution (2001-2023). Patients were stratified into BRNU or URNU groups. Propensity score matching (PSM

ELF-3 and TSC-1 as potential markers of recurrence in low-risk non-muscle-invasive bladder cancer.

OBJECTIVE: Tumor recurrence is a major clinical challenge in low-risk non-muscle invasive bladder cancer (NMIBC). Molecular studies have identified two main subtypes with distinct genetic pathways: papillary, low-grade tumors, often with FGFR3 mutations and favorable outcomes, and nonpapillary, high-grade tumors, which are linked to higher rates of progression and muscle invasion. This study evaluates the prognostic value of immunohistochemical (IHC) markers- cluster of differentiation 44 (CD44), cytokeratin 5/6 (CK5/6), cytokeratin 20 (CK20), GATA-binding protein 3 (GATA3), human epidermal gr

Nephroureterectomy with ileal cuff for upper tract recurrence after radical cystectomy: oncologic and survival outcomes.

PURPOSE: Upper tract urothelial carcinoma developing after radical cystectomy presents a technical challenge because complete ureteral removal to the ileal anastomosis is often hindered by postoperative adhesions. This study evaluated the prognostic impact of the completeness of ureteral excision including the ileal cuff in patients undergoing nephroureterectomy for upper tract recurrence after radical cystectomy. METHODS: We retrospectively analyzed 30 patients who underwent nephroureterectomy for recurrent upper tract urothelial carcinoma after radical cystectomy between 2001 and 2024. Pati

Durable complete metabolic response to enfortumab vedotin plus pembrolizumab in metastatic upper tract urothelial carcinoma: a case report.

BACKGROUND: Upper tract urothelial carcinoma (UTUC) accounts for approximately 5-10% of urothelial malignancies and is frequently associated with advanced stage and aggressive clinical behavior. In metastatic disease, platinum-based chemotherapy has historically represented standard first-line therapy; however, long-term outcomes remain limited. The combination of enfortumab vedotin and pembrolizumab has recently demonstrated superior survival outcomes compared with platinum-based chemotherapy in advanced urothelial carcinoma. Nevertheless, the depth and durability of response following early

Stage-Specific Prognostic Impact of Tumor Location in Upper Tract Urothelial Carcinoma: A Propensity Score-Matched Analysis.

BACKGROUND: The prognostic relevance of tumor location in upper tract urothelial carcinoma (UTUC), particularly the difference between renal pelvic and ureteral tumors, remains controversial. This study aimed to clarify the impact of tumor location on stage-specific survival and recurrence patterns following radical nephroureterectomy (RNU), using a large multi-institutional cohort. METHODS: Patients diagnosed with UTUC who underwent RNU between 2003 and 2023 were retrospectively reviewed and categorized into the renal pelvic urothelial carcinoma (RPUC) group or the ureteral urothelial carcin

Incidence, risk factors and prognostic impact of pyelonephritis in cancer patients with percutaneous nephrostomy for malignant ureteral obstruction.

INTRODUCTION AND OBJECTIVES: Percutaneous nephrostomy (PCN) is commonly used for malignant ureteral obstruction but carries significant infection risk in immunocompromised cancer patients. Data on pyelonephritis incidence, risk factors, and prognostic impact in this population remain limited. This study aimed to determine the incidence, risk factors, and prognostic impact of nephrostomy-related pyelonephritis and identify modifiable targets for infection prevention. MATERIALS AND METHODS: In this prospective observational study, 115 adult patients with malignancy-associated grade ≥2 hydroneph

Robotic ileal ureteral reconstruction for radiation-induced ureteral strictures in pelvic malignancy survivors: experience from a multi-institutional database.

BACKGROUND: Chronic double-J stenting or nephrostomy dependence remains the default management for radiation-induced ureteral strictures (RIUS). This study aimed to evaluate the feasibility and clinical efficacy of robotic ileal ureter replacement (IUR) in managing RIUS with prior pelvic malignancy radiotherapy. METHODS: In our prospective multi-institutional RECUTTER database, 35 patients with a diagnosis of RIUS underwent robotic IUR between March 2019 and April 2024. Unilateral and bilateral procedures were performed with or without augmentation ileocystoplasty. Baseline characteristics, p

Retroperitoneal Robot-Assisted Nephroureterectomy: Technical Description and Single Center Experience.

INTRODUCTION: Robot-assisted radical nephroureterectomy (RANU) is increasingly used for upper tract urothelial carcinoma (UTUC). The retroperitoneal approach avoids bowel mobilization, but its adoption in robotic surgery remains limited because of the confined working space and technical complexity. We described our step by step retroperitoneal technique and evaluated perioperative outcomes in an initial single center series. METHODS: We retrospectively reviewed consecutive patients who underwent intended fully robot assisted retroperitoneal RANU at our institution between July 2022 and Septe

Robot-Assisted Sigmoid Colon Conduit for Urinary Diversion: Avoiding Bowel Anasto-mosis in a Patient with Prior Loop Colostomy and Pelvic Radiation.

PURPOSE: Late radiation-induced genitourinary and gastrointestinal complications after multimodal treatment for pelvic malignancies frequently require complex reconstructive surgery, often necessitating both urinary and fecal diversion (1). In this setting, a colonic segment may provide an effective option for urinary diversion while avoiding bowel anastomosis (2, 3). Minimally invasive robotic approaches have increasingly expanded the feasibility of these complex reconstructions and may reduce morbidity, particularly in frail or previously irradiated patients (4-7). We describe a single-stage

Anatomical refinement matters: ureteropelvic junction location confers survival benefit for patients with upper tract urothelial carcinoma after radical nephroureterectomy.

OBJECTIVE: Whether primary tumor location influences prognosis in upper tract urothelial carcinoma (UTUC) remains contentious. We aim to clarify the prognostic impact of tumor location in upper tract urothelial carcinoma (UTUC) by anatomically subsegmenting ureteral tumors, with specific focus on ureteropelvic junction (UPJ) carcinomas. METHODS: This retrospective cohort study evaluated 2,315 patients with non-metastatic, unifocal UTUC, including tumors localized to the renal pelvis or ureter, who underwent radical nephroureterectomy. Ureteral tumors were rigorously subsegmented into UPJ, pro
