OBJECTIVE: To evaluate the technique of three-dimensional (3-D) ultrasound imaging of the urethra and its application in both research and clinical practice.
PATIENTS AND TECHNIQUE: The study involved 23 patients: 10 with benign prostatic hyperplasia, four with urethral strictures, three post-insertion of prostatic stents, one with bladder neck dyssynergia, three post-transurethral resection of the prostate, and two with non-urological conditions. Ten patients with post-prostatectomy urinary incontinence underwent collagen implantation via the transrectal ultrasonographic guided method. Mean followup was 14 months (range 9 to 18 months). The procedure was performed through the perineum with the patient under local or epidural block anesthesia. The mean amount of collagen implantation was 20.0 ml. A series of 109 female patients with urinary incontinence were treated by pelvic floor stimulation and biofeedback. Eighty-one patients received the full course of treatment and 28 patients dropped out. Among the 81 patients who completed the treatment, the improvement and cure rates (completely dry) shortly after treatment were 70.4% and 12.3%, respectively. A 58-year-old male complaining of pollakisuria, miction pain and back pain visited us Dec. 26, 1979. Rectal examination revealed the prostate enlarged by 5 digital width, stony hard and irregular. Transrectal needle biopsy revealed moderately differentiated adenocarcinoma of the prostate. Experience had with suprapubic cystoscopy and endovesical surgery, performed in a series of twenty-two patients presenting various diseases of the bladder--tumors, mainly of the anterior wall, calculi, foreign bodies, bladder neck sclerosis, defective Foley catheter--is shared. As a sole complication a case of penetrating the abdominal cavity without inflicting lesion to the intestine, is described. The suprapubic approach significantly broadens the scope of transurethral surgery, preserves its advantages, and is furthermore free of the shortcomings associated with open operations.
