Primary Adenocarcinoma of the Urinary Bladder: A Case Report on a Rare Malignancy.

Of all primary bladder cancers, primary adenocarcinoma is an uncommon tumor. When considering all tumor origin areas, secondary bladder involvement from carcinoma, whether by direct extension or metastasis, is actually more prevalent than primary adenocarcinoma, despite its rarity. The most common source of subsequent bladder tumors is endometrial, lung, colon, prostate, breast, or other organ adenocarcinomas. Primary bladder adenocarcinoma is thought to result from urothelial metaplasia, which is frequently linked to persistent irritation or inflammation. Bladder exstrophy, recurrent urinary

Xanthogranulomatous cystitis with malakoplakia leading to recurrent spontaneous bladder perforation in a young girl.

Recurrent bladder perforation due to xanthogranulomatous cystitis with malakoplakia is rare entity and can lead to spontaneous bladder perforation. A 15 years girl presented with sudden pain abdomen with reduced urine output. Her exploratory laparotomy revealed, perforation of 2 cm at the dome of bladder with unhealthy margins. Excisional bladder biopsy and repair of bladder perforation by 3-0 polyglactin suture was done. The histopathology showed xanthogranulomatous cystitis with malakoplakia. Her records revealed the same histopathology in bladder perforation at age of 9 with lost follow-up

Intraperitoneal urinary bladder rupture diagnosed with ultrasound: An uncommon image.

We present a case of intraperitoneal bladder rupture after fall. The patient presented with lower abdominal pain, difficulty to void urine, and mild hematuria. Catheterization revealed blood-tinged urine. Ultrasound showed a well-defined defect in the dome of bladder with flow of saline through the defect into the peritoneum, on flushing the Foley's catheter. On laparotomy, a 5 cm × 5 cm defect was found in the bladder dome that was repaired.

[Unfixed Mesh Plug Migration from Inguinal Ring to Urinary Bladder].

A 65-year-old man presented to a clinic with a chief complaint of macrohematuria and frequent urination. The computed tomographic scan and cystoscopy revealed a dome of bladder tumor. He was referred to our hospital with the diagnosis of bladder tumor. He had undergone bilateral inguinal hernia repair and magnetic resonance imaging suggested mesh plug migration on the urinary bladder inserted into the right inguinal lesion 11 years previously. Under the diagnosis of mesh plug migration, partial cystectomy with extraction of the foreign body was performed. After the surgery he was well and symp

Index of suspicion with appropriate timing of CT scan helps prevent missed diagnosis in urologic trauma.

Ureteric and bladder injuries are uncommon, difficult to diagnose and rarely occur in isolation. Diagnosis is often delayed or missed at presentation. Therefore, high clinical suspicion and appropriate timing of computed tomography (CT) are of paramount importance. We report two cases (ureteropelvic junction avulsion and ruptured dome of bladder) whereby the presentations were subtle and would have been missed if not for high clinical suspicion. This article discusses the problems associated with these urologic injuries, as well as how to develop a high index of suspicion based on the pattern

A Late Presentation of Spontaneous Bladder Rupture During Labor.

Spontaneous bladder rupture is usually due to bladder diseases. Bladder rupture during labor or postpartum is extremely rare. Acute abdomen is the usual presentation of spontaneous bladder rupture. Patients may complain of suprapubic pain, anuria and hematuria. Some patients with intraperitoneal bladder rupture may have no abdominal pain and can pass urine without any symptoms so the diagnosis of intraperitoneal rupture may be difficult in these situations. We report a nulliparous woman with abdominal pain and distension about 20 days after normal vaginal delivery. There was intraperitoneal ru

A Modified Technique for Performing Transurethral Resection of the Prostate Combined with Percutaneous Cystolithotripsy.

INTRODUCTION: This study aimed to determine the safety and efficacy of a modified technique for performing transurethral resection of the prostate (TURP) combined with percutaneous cystolithotripsy (PCCL). METHODS: Clinical data from 24 patients with benign prostate hyperplasia (BPH) and aggregate stone sizes ≥3 cm were assessed retrospectively between June 2011 and January 2014. All stones fragmented via pneumatic lithotripter were removed. An 18-F Foley catheter was inserted into the Amplatz sheath, which was then removed from the bladder. The Foley catheter balloon was inflated with 15 ml

Endoscopic removal of intravesical polypropylene suture with plasmakinetic resection after abdominal hysterectomy.

INTRODUCTION: Intravesical foreign substances such as mesh or suture are among the rare reasons of recurrent urinary tract infections. Anti-incontinence and prolapsus procedures are associated with mesh/suture extrusion into the bladder, however, this complication is uncommon with abdominal hysterectomy. PRESENTATION OF CASE: A 61-year-old female, obese patient admitted to our clinic with recurrent urinary tract infections and voiding symptoms which were worsened after abdominal hysterectomy. Radiological evaluation revealed an intravesical foreign material within the bladder. The cytoscopy w

Peroxisome proliferator-activated receptor agonists and bladder cancer: lessons from animal studies.

This article reviews available animal studies on the possible link between the use of peroxisome proliferator-activated receptor (PPAR) agonists and bladder cancer, with further discussion on the possible implications to humans. Carcinogenicity studies suggest that the PPARγ agonist pioglitazone and dual PPARα/γ agonists such as ragaglitazar, muraglitazar, and naveglitazar may increase the risk of bladder cancer in a dose-responsive pattern in rats. It is interesting that bladder cancer related to PPAR agonists shows remarkable species- and sex-specificity and has a predilection to occur in th

Urachal urothelial carcinoma diagnosed at a radical prostatectomy operation: a case report.

BACKGROUND: Urachal carcinomas are rare tumors, and the majority of them are adenocarcinomas. Up to now, only 21 urachal urothelial carcinomas (UCas) have been reported. Here, we describe a case of high grade UCa arising from the urachal remnants. CASE: A 66-year-old man presented with voiding difficulties. Prostate specific antigen (PSA) was 5.46 ng/mL. Prostatic adenocarcinoma (PCa) (Gleason score 6) was diagnosed by needle biopsies. After the diagnosis of high grade, muscle invasive UCa with intact mucosa on frozen examination of the dome of bladder wall during the radical prostatectomy op

Adenocarcinoma in ectopic prostatic tissue at dome of bladder: a case report of a patient with urothelial carcinoma of the bladder and adenocarcinoma of the prostate.

We report a case of ectopic prostate tissue with an associated prostatic adenocarcinoma occurring in the dome of the urinary bladder. A 62-year-old man presented with a 4-month history of persistent microscopic hematuria following a urinary tract infection. Other complaints included frequent urination, but there was neither dysuria nor gross hematuria. Digital rectal examination revealed a smooth prostate of normal size. Cystoscopic examination revealed a sessile lesion of the anterior bladder neck and multiple smaller papillary lesions throughout the bladder. Following a transurethral resecti

Holmium laser treatment of a vesical calculus secondary to TVT procedure.

A case of vesical calculus is being presented as a complication of TVT procedure done for severe stress incontinence in an elderly lady. The patient, who presented with obstructive and irritative lower urinary tract symptoms, needed an indwelling catheter. Non-enhanced CT scan of abdomen confirmed an intraluminal calcific density in the anterior wall of urinary bladder just left of midline. On cystoscopy, a blue-colored polypropylene mesh of the TVT was visible at the ends of this calculus, holding it to the anterior vesical wall. Holmium laser was used to disintegrate the ends of the mesh dee

A rare cause of hemorrhagic shock in children: bladder hemangioma.

We report the first case of urinary bladder hemangioma that caused hemorrhagic shock consequent to massive hematuria. A 2-year-old girl was presented with hemorrhagic shock and macroscopic hematuria. After rapid resuscitation was performed, radiologic investigations demonstrated a solid enhancing mass originating from the dome of bladder. Partial cystectomy was made. Histopathologic examination of resected specimen revealed cavernous hemangioma. Although bladder hemangiomas are rare benign tumors, it should be kept in mind that they can lead to life-threatening complications when the diagnosis

Laparoscopic partial cystectomy for a vesical leiomyoma.

A 47-year-old woman presented with gross hematuria and urinary frequency for 6 months. The tumor located at the dome of bladder was resected by transurethral resectoscope. The pathology revealed leiomyoma; however, her symptoms did not improve after the endoscopic procedure. We performed transurethral mucosal incision for circumscribing the margin of tumor and transperitoneoscopic partial cystectomy to treat the patient successfully.

Assessment of pigmented skin lesions in terms of blood perfusion estimates.

The rising prevalence of vision-threatening eye diseases poses a major global health and economic burden, yet timely diagnosis remains limited by workforce shortages, diagnostic delays, and restricted access to specialized care. Artificial intelligence (AI) offers potential solutions. In particular, recent progress in foundation models and large language models-especially multimodal large language models (MLLMs)-has shown promise in medical image interpretation and automated clinical documentation. However, advancing MLLMs for ophthalmology is hindered by the lack of unified, comprehensive ben

[Two cases of successful treatments with steroid for local and systemic hypersensitivity reaction following intravesical instillation of Bacillus Calmette-Guerin].

We have found steroid pulse therapy to be effective and safe for local and systemic adverse reactions of BCG therapy. Two cases are reported. Case 1: A 57-year-old woman with initial recurrence of urinary bladder carcinoma was treated with transurethral resection. The histopathological findings were transitional cell carcinoma (TCC), G2 > G1, pT1a. To prevent a second recurrence, she was administered Bacillus Calmette-Guerin (BCG) instillation therapy: 80 mg of BCG, (Tokyo strain) suspended in 40 ml of normal saline, instilled into her bladder weekly. After the fifth week of instillation, she

[Transitional cell carcinoma of urachus: a case report].

We report a case of transitional cell carcinoma of urachus in a 72-year-old man. At follow up cystoscopy for past history of bladder cancer, we found a papillary tumor in the right orifice that came out to the bladder cavity intermittently. Although there was no cancerous lesion on the surface of the bladder mucosa, a submucosal eminence at the dome of bladder was observed. Sagittal magnetic resonance imaging (MRI) revealed an extravesical tumor (2 cm) at the position of urachus. Under the diagnosis of right ureteral cancer and urachal cancer, we performed right distal ureterectomy, ureteral r
