Comprehensive efficacy of different prostate resection volumes for patients with benign prostatic hyperplasia: a systematic review and meta-analysis.

Benign prostatic hyperplasia is one of the most common urological diseases in middle-aged and elderly men worldwide. The most effective treatment is surgery, and multiple surgical approaches, including traditional electroprostatectomy, laser vaporization, or steam ablation, have already been widely applied in clinical practice. However, few studies summarizing and reporting the comprehensive outcomes of whether different prostate resection volumes affect the efficacy of benign prostatic hyperplasia surgery exist. Thus, we conducted a systematic review and meta-analysis involving cohort studies

Surgical Outcomes of Transurethral Bladder Neck Incision and Resection in Female Primary Bladder Neck Obstruction: A Systematic Review.

BACKGROUND: Primary bladder neck obstruction (PBNO) in women is an underrecognized cause of lower urinary tract symptoms. This systematic review evaluates surgical outcomes following transurethral bladder neck incision (TUBNI) and resection (TURBN). METHODS: A systematic search of PubMed, Embase, Scopus, and Cochrane databases identified studies from 2000 to 2025 evaluating surgical treatment of female PBNO. Primary outcomes included functional improvements and major complications. Quality assessment used the ROBINS-I tool. RESULTS: A total of 16 studies encompassing 612 patients were includ

Robot-Assisted Radical Prostatectomy After Rezūm: A Case Report and Literature Review.

Minimally invasive surgical therapies (MISTs), such as Rezūm™ Water Vapor Therapy, are emerging treatment options for benign prostatic obstruction (BPO). When prostate cancer is subsequently diagnosed, radical prostatectomy may still be indicated. However, evidence regarding intraoperative challenges and the surgical and functional outcomes of robot-assisted radical prostatectomy (RARP) following Rezūm remains limited. We report the first documented case of RARP following Rezūm in a 68-year-old man. He initially underwent Rezūm for symptomatic BPO. Due to rising PSA, a suspicious lesion on MRI

The influence of neonatal birth weight on postpartum pelvic floor function in primiparas.

This study aims to investigate the effect of birth weight on the incidence of pelvic floor dysfunction (PFD) in primiparas at 42 days and 1-year postpartum, using pelvic floor ultrasound and surface electromyography (sEMG). A retrospective analysis was conducted on 200 primiparas women who had a full-term vaginal delivery. All participants underwent pelvic floor evaluation at 6 to 8 weeks postpartum, with a follow-up at 1 year. The primiparas were categorized into 2 groups based on infant birth weight: a study group (n = 100, 3500 g ≤ birth weight <4000 g) and a control group (n = 100, 2500 g

Transvesical Clipless Robot-Assisted Radical Prostatectomy Using a Multiport Plat-form: Technical Feasibility and Early Outcomes.

INTRODUCTION: Robot-assisted radical prostatectomy (RARP) continues to evolve with surgical approaches aimed at preserving continence-related anatomy and optimizing postoperative recovery (1). Transvesical RARP has gained increasing interest, particularly with the advent of single-port platforms (2, 3). Building on our prior experience with clipless techniques and functional outcome optimization (4), we evaluated a multiport transvesical approach. OBJECTIVE: To describe, step-by-step, a clipless transvesical RARP technique and to report its feasibility and early functional and oncologic outco

A mechanistic, imaging-based classification of complete retrograde ejaculation: insights from dynamic transrectal ultrasonography.

BACKGROUND: Complete retrograde ejaculation (CRE), defined by total semen reflux into the bladder, currently lacks a standardized classification based on underlying pathophysiological mechanisms. Conventional diagnostic approaches are indirect and unable to visualize the dynamic, real-time dysfunction of the bladder neck (BN) and external urethral sphincter (EUS) during ejaculation. OBJECTIVES: To directly characterize the ejaculatory process in CRE using dynamic transrectal ultrasound (TRUS) and establish an etiology-specific, mechanism-driven classification system. METHODS: In this single-

Failed Trial Without Catheter Post Rezūm in Non-Catheter Dependent Patients: Risk Factors From the Canadian Rezūm Registry.

INTRODUCTION: Rezūm has become an increasingly popular surgical approach for treating bladder outlet obstruction. Despite the practice of routinely placing a Foley catheter, successful TWOC (Trial Without Catheter) remains variable. This study seeks to identify the TWOC failure incidence and risk factors to inform clinical decisions and enhance patient counseling. METHODS: We conducted a retrospective review of non-catheter dependent patients who underwent Rezūm therapy between April 2019 and June 2023 in two high-volume Canadian centers. All patients received a urinary catheter post-treatmen

The risk factors of anatomical cystocele recurrence: the hiatal area and the lowest vaginal mesh location observed by pelvic floor ultrasound.

OBJECTIVES: To explore the risk factors for the anatomical cystocele recurrence (ACR) after transvaginal mesh (TVM) implantation. METHODS: Patients with a TVM between 2015 and 2023 were enrolled. ACR is determined as the bladder descended ≥ 10 mm below the pubic symphysis or a cystocele stage II or higher. The lowest mesh position (LMP) determined by ultrasound corresponded to the caudal mesh end relative to the bladder neck at maximum descent. Positions below the bladder neck were noted as negative values. The hiatal area (HA) was measured using four-dimensional pelvic floor ultrasound. The

Longitudinal voxel-based analysis identifies a bladder neck subregion predicting genitourinary toxicity after prostate cancer reirradiation.

BACKGROUND AND PURPOSE: In the context of prostate cancer reirradiation, stereotactic body radiation therapy (SBRT) may allow to spare anatomical volumes potentially implicated in genitourinary (GU) toxicities. This work aimed to perform a longitudinal voxel-based analysis to explore urethrovesical subregions where a higher planned dose was associated with an increased risk of late GU toxicity. MATERIAL AND METHODS: A single-centre cohort of 108 local prostate cancer recurrence patients treated with salvage SBRT (36 Gy/6 fractions) and followed-up for a median period of 50 months was consider

Low Chimney Cutaneous Ureterostomy (LCCU): a novel technique for supra-vesical urinary diversion.

BACKGROUND: Supra-vesical urinary diversion is rarely needed in current pediatric urology practice. Developments in reconstructive surgery, pediatric endoscopy, and anesthesia have promoted early definitive management of majority of indicated congenital obstructive uropathies. Ureterostomy is an effective alternative for decompressing upper urinary tract; however, none of current techniques provide an optimal diversion. End ureterostomy (EU) and low-loop ureterostomy (LLU) do not allow urinary bladder cycling especially in bilateral cases and those with solitary kidneys. EU cannot drain the ur

IR-780 improves urination function and complications in rats with partial bladder outlet obstruction by protecting bladder smooth muscle cell mitochondria from oxidative stress.

INTRODUCTION: Partial bladder outlet obstruction (pBOO) is the most common cause of lower urinary tract symptoms (LUTS). Prolonged BOO induces bladder remodeling, which can lead to severe bladder dysfunction and refractory LUTS in some patients, even after obstruction resolution. This condition significantly impairs patients' quality of life, and no effective treatment is currently available. This study investigated a pBOO rat model using IR-780, a novel near-infrared lipophilic dye with potential targeted antioxidant effects. METHODS: A partial ligation of the rat bladder neck was performed

Addressing complicated vesicovaginal fistula post-intercourse in MRKH: the critical imperative for comprehensive sex education and awareness.

INTRODUCTION: Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome, a rare congenital disorder, affects approximately 1 in 4500 to 5000 females, leading to the underdevelopment or absence of the uterus and upper vaginal segment. While the physical manifestations of MRKH are substantial, the associated psychological and social repercussions, exacerbated by societal norms, can be profound. Inadequate sex education further compounds the challenges faced by these women, particularly in postmarital contexts. This report highlights an unprecedented case of a young woman with MRKH type II who developed a c

Ejaculation-preserving holmium laser enucleation of the prostate: a systematic review of techniques, functional outcomes, and safety.

CONTEXT: Holmium laser enucleation of the prostate (HoLEP) is a guideline-endorsed, size-independent surgical standard for benign prostatic hyperplasia (BPH). However, conventional HoLEP is frequently associated with loss of antegrade ejaculation (AE), an outcome of increasing importance for sexually active patients. Anatomically informed modifications aimed at preserving peri-ejaculatory structures have been proposed, yet the evidence supporting antegrade ejaculation-preserving HoLEP (EP-HoLEP) remains fragmented. OBJECTIVE: To systematically review and synthesize the contemporary evidence on

The INSIDE-OUT longitudinal technique for bladder neck closure in children: A simplified and innovative approach.

INTRODUCTION: The article presents the INSIDE-OUT longitudinal technique for bladder neck closure in children, a simplified approach aimed at improving outcomes in pediatric patients, particularly those with myelomeningocele. Traditionally, these patients undergo bladder neck plasty or sling procedures, which, while potentially preserving bladder function, achieve continence rates of approximately 50 %. In contrast, the new INSIDE-OUT technique boasts a success rate exceeding 90 %. METHODS: The method begins with a midline incision of the bladder, allowing for internal exposure of the bladder

Bladder sarcomatoid carcinoma and giant cell carcinoma without recurrence after robot-assisted radical cystectomy: A case report.

Bladder sarcomatoid carcinoma is a rare, aggressive tumor with epithelial and mesenchymal features. We report a 69-year-old man with muscle-invasive bladder neck urothelial carcinoma with giant cell subtype. After two cycles of gemcitabine and cisplatin, imaging revealed tumor shrinkage. Robot-assisted radical cystectomy with pelvic lymphadenectomy and intracorporeal ileal conduit was performed. Pathology confirmed sarcomatoid carcinoma with lymphovascular invasion but negative margins. The urothelial component responded to chemotherapy, whereas the sarcomatoid component did not. No recurrence
