I.v. urography is often insufficient for visualization of the intramural and juxtavesical portion of the ureter. From April to September 1992, systematic screening for iatrogenic vesicoureteric reflux was performed in a series of 30 patients treated by rigid ureterorenoscopy. The authors describe a modification of the classical double J stent designed to improve its clinical acceptability. It consists of cutting the lower J so that the catheter, inserted in the ureter, does not protrude into the bladder. A loop of nonresorbable suture attached to the lower extremity of the stent is passed through the ureteric orifice, allowing its easy extraction. This procedure has been used in eight patients with no complications.

The paper reports the anatomy of the ureterovesical junction in pigs without urinary tract disease and the changes that occur with ageing. In comparison with other mammals the pig has a long intravesical ureter. Its length increases with age, from a mean length of 5 mm. at birth to 36 mm. at maturity. Based on an incidental case of ectopic ureteric orifice in the seminal tract associated with homolateral renal agenesis, the authors demonstrate the value of ultrasonography, particular transrectal, both for diagnosis and follow-up. Digital rectal examination is the key to the diagnosis, combined with ultrasonography and urography. The indications for surgery depend on the clinical symptoms and signs.

We recently observed 3 cases of ectopic ureteric orifice in the seminal tract in patients between the ages of 16 and 42 years. The first case was discovered incidentally, but the other two presented with scrotal symptoms. Intravenous urography, ultrasonography and computed tomography contributed to the diagnosis, but magnetic resonance imaging was the most conclusive, avoiding the need for deferentography in one case. These 3 recent cases allow an evaluation of the respective value of these various imaging modalities.

The ureterovesical junction of pigs with cystitis and pyelonephritis was examined. There was significant shortening of the intravesical portion of the ureter in sows with pyelonephritis. In cases associated with acute pyelonephritis the ureteric orifice was found to be significantly wider than that found in the normal population. The ratio of the intravesical length to the ureteric orifice diameter was also significantly reduced. A 74-year-old female presented with a two years history of malaise, weakness and intermittent gross hematuria. The effect of balloon dilatation of the ureteric orifice and the intramural ureter was evaluated histologically and radiologically in eight piglets. The distal part of the ureter was dilated from its normal calibre of 4F to 12F. A 44-year-old female was admitted because of gross hematuria six years after beginning hemodialysis for chronic renal failure. There was a past history of hematuria one year before admission. A total of 618 children with primary vesicoureteric reflux (VUR) were examined preoperatively and 6, 9 and 24 months after an antireflux ureterocystostomy of the vertex of the bladder (AUVB); 206 children were operated on bilaterally, so that 824 AUVBs were performed. During a 32-month period, 215 ureterorenoscopies were attempted in 171 ureterorenal units. The success rates for stone retrieval were 87% in the lower third of the ureter, 67% in the middle third, 75% in the upper third, and 55% in the pelvis. Seventeen prostatic-type polyps of the lower urinary tract are reported.
