[Results of treatment following 474 Leadbetter-Politano ureterocystoneostomies in vesicorenal reflux].

This is a report of our experience with 474 operations for vesico-renal reflux according to the Leadbetter-Politano technique at the Department of Paediatric Surgery, Medical University of Lübeck. Diagnosis of vesico-renal reflux is mostly based on urinary tract infection or enuresis. Indication for primary surgery was low pressure reflux or anomalies and ectopies of the ureteric orifice, found on cystoscopy. Ureterocystoneostomy using a modification of the Leadbetter-Politano technique was the method of surgery used, resecting the mostly fibrous distal ureteric segment. In our patients, the r

Modified nephroureterectomy: a risk of tumour implantation.

The standard surgical management of patients presenting with transitional cell carcinoma of the upper urinary tract is nephroureterectomy with excision of a cuff of bladder around the ureteric orifice. Recently a modified technique of resecting the lower ureter endoscopically and completing the nephroureterectomy through a single loin incision has been advocated as a safe and simple procedure. We consider that this technique may have a risk of tumour implantation at the site of the resected lower ureter. We report our experience of this operation in five patients, two of whom developed invasiv

[Ectopic ureter without urinary incontinence despite ureteric orifice in the vestibulum: report of one case].

UNLABELLED: This is one case of ectopic ureter which lacked urinary incontinence despite ureteric orifice in the vestibulum. CASE: A 20-year-old woman complained of miction pain. On examination, we found a small orifice in the vestibulum. DIP revealed slight hydronephrosis in the left contracted kidney and normal pyelogram in the right kidney. On cystoscopy, the bilateral ureteral orifices were normal, but we found the head of a catheter placed in the orifice of the vestibulum was passing under the mucosa of the bladder. The abnormal lumen was recognized by introducing an opaque medium throug

[A case of congenital vesicovaginal fistula].

Congenital vesicovaginal fistula is very rare and only five cases have been reported so far. This is a report on a case of congenital vesicovaginal fistula in a three-year-old girl. Vesicovaginal fistula was confirmed by cystoscopy and colposcopy. Roentgen examinations, including IVP, CG and CT revealed a left hypoplastic kidney, left ectopic ureteric orifice and left vesicoureteral reflux. Under general anesthesia, the fistula was resected and closed transvesically, and ureterocystoneostomy was performed. Five out of the six cases of congenital vesicovaginal fistula reported worldwide had oth

New techniques for stenting severely strictured or occluded ureters.

Percutaneous stenting of the severely strictured ureter is facilitated by catheterization through a rigid curved tube leading through the kidney. Alternatively, bidirectional traction tensioning of a guide wire pulled through the entire urinary tract permits antegrade dilatation and stent drainage. Complete occlusion near the ureteric orifice may be bypassed by transureteral puncture of the bladder followed by stent insertion.

Histochemical changes after ureterosigmoidostomies and colonic diversion.

Forty patients with colonic urinary diversions were studied for morphological and histochemical changes in the colon using haematoxylin and eosin, and high iron diamine alcian blue stains. Eighty-nine per cent of ureterosigmoidostomies biopsied at the ureteric orifice and 44% biopsied distal to the orifice showed "transitional" changes thought to represent a pre-malignant state. Thirty-eight per cent of patients who had had ureterosigmoidostomies in the past and 100% of the colon conduits also demonstrated this change. The earliest "transitional" change occurred 1 year after the diversion. Thi

Vesicoureteric reflux in patients with bladder tumours.

In the course of treatment of 224 patients with bladder neoplasms over an 18-year period, 44 patients were noted to have associated vesicoureteric reflux, bilateral in 12 and unilateral in 32. Reflux was suspected in 8 patients because of associated ureterotrigonal abnormalities such as paraureteric diverticulum, complete duplication of the ureter and gaping ureteric orifice, and it was diagnosed by micturating cystography obtained before the bladder tumour was treated. In the remaining 36 patients reflux developed after treatment of the bladder tumours. In most patients with reflux the clinic

Ureteric orifice deformation causing secondary reflux.

In two boys with posterior urethral valve and dilated ureters ureteroneocystostomy according to Politano-Leadbetter was performed at 4 and 6 months of age. Postoperatively both children showed vesicoureteric reflux. Endoscopic examination revealed in both cases a mucosal bridge between the medial borders of the ureteric orifices. The bridge was transected with transurethral endoscopic diathermy whereupon the reflux disappeared permanently. The fixation and deformation of the orifices were probably part of the mechanism causing the reflux. Endoscopy is recommended in case of postoperative reflu

[Complex vesicovaginal fistulas].

The author reviews the therapeutic indications (it being necessary to abstain from repair of certain fistulae) in vesico-vaginal fistulae. Fistulae following pelvic irradiation, african fistulae of obstetric origin and the various difficulties (and their possible solutions) which may arise when the fistula passes through an extensive zone of fibrous tissue, is close to a ureteric orifice, at the neck of the bladder or associated with a loss of the urethra or with a recto-vaginal fistula, are studied. In the opinion of the author, classification between simple and complex vesico-vaginal fistula

The single ectopic ureter and the dysplastic kidney.

Eight children, 3 girls and 5 boys, with single ectopic ureters arising from non-concentrating kidneys are described. Two children, both girls, complained of persistent wetting with a pattern suggestive of ectopic ureter: 2 of the boys presented in the neonatal period with massive abdominal distension: the remaining children developed a urinary infection or chronic urinary retention. Diagnosis depends on an appreciation of the possibility that a non-concentrating kidney on intravenous urography may be giving rise to an ectopic ureter and, in the boys, on a correct interpretation of cystourethr

The pathogenesis of renal dysplasia. I. Quantification of hypoplasia and dysplasia.

In order to assess the relative effects of abnormal ureteric orifice position and abnormal urodynamics on the morphogenesis of hypoplasia and dysplasia in kidneys obtained from infants, we devised a method of quantifying the renal structures. The method was based on radial glomerular counts which ranged from zero to normal (seven to nine), a score for dysplastic structures, and the ratio of normal to abnormal tissues present. These three values, when plotted against each other, correlated closely. The glomerular count, with occasional minor adjustment for inconsistencies, was the best paramete

The pathogenesis of renal dysplasia. II. The significance of lateral and medial ectopy of the ureteric orifice.

Renal hypoplasia and dysplasia may be primary malformations linked to a panureteric bud deformity or result from damage to the developing nephrons caused by abnormal urodynamic pressures, Kidneys with misplaced ureteric orifices were graded, according to histologic criteria, on the hypodysplasia scale. With lateral ectopy of the ureteric orifices with and without congenital urethral obstruction, the grades correlated with specific orifice positions. The grades of kidneys with caudal ectopy of the ureters indicated a more general correlation. Dysgenesis of the bud and nephrogenic mesenchyme may

"Stasis"--a test for vesico-ureteric orifice competence in children with reflux.

Intravenous radionuclide cystography (IVRNC) with one injection on 99m-Tc-DTPA measures renal function and detects vesico-renal reflux [1, 2, 3]. This paper describes a possible means of detecting incompetent ureteric orifices during IVRNC examinations. In some patients a hold-up of material in the renal areas (stasis) was observed which suddenly cleared at micturition. A prospective study of 58 patients who had IVRNC and cystoscopy within 28 days of each other revealed that 76.3% had anatomically abnormal ureteric orifices on the same side as the stasis. This contrasted with only 12.8% of abn

The interrelationship between paraureteric diverticula, vesicoureteric reflux and duplication of the pelvicaliceal collecting system: a family study.

The incidence of a bifid pelvicaliceal system, vesicoureteric reflux, its sequelae and paraureteric diverticulum was determined in the patients and siblings of 22 patients with a paraureteric diverticulum. The results support the hypothesis that there is a direct genetic relationship between a paraureteric diverticulum, duplication of the pelvicaliceal system and vesicoureteric reflux. This suggests that the vesicoureteric reflux associated with a parauretic diverticulum is primary and related to lateral ectopia of the ureteric orifice. This syndrome complex may be inherited by multiple genes

The site of recurrence of non-infiltrating bladder tumours.

A study of 56 cases of non-infiltrating bladder tumour showed a striking difference in the distribution within the bladder of the primary and the recurrent tumours. More than three-quarters of the primary tumours were confined to a restricted area near a ureteric orifice whereas only one-fifth of recurrent tumours were found in this site. By far the commonest site for recurrent tumours was the postero-superior wall of the bladder. Forty-five recurrent tumours formed in the air bubble region, a site in which no primary tumour was found in the whole series. The postero-superior wall of the bladd

Reimplantation of the wide ureter.

A modification of the technique of Politano and Leadbetter for reimplantation of wide ureters is described. Nine patients with obstructive megaureter (10 ureters) and 6 with gross vesicoureteric reflux (9 ureters) were treated. A satisfactory result was obtained in 6 of the obstructed patients (7 ureters) and ultimately, after re-operation, in 1 of the remaining ureters. Reflux was corrected in all 6 patients. This method requires less extravesical dissection than others involving extensive remodelling of the wide ureter. There is therefore less risk of damage to the distal ureteric blood supp

Paraureteral diverticula. Associated renal morphology and embryogenesis.

A study was made of paraureteric diverticula, the associated ureteric orifices, and the accompanying kidneys. It was shown that the kidney was more abnormal as the ureteric orifice position lay more laterally and into the diverticulum. The fascial arrangements of the terminal ureter and the related diverticulum were examined. When the ureter opened totally within the diverticulum, both structures showed a single investment of fascia, suggesting a common embryologic orgin of the two structures. When the ureter opened either separately into the bladder or on the verge of the diverticulum, the ur

The interrelationship between vesico-ureteric reflux, trigonal abnormalities and a bifid pelvicalyceal collecting system: a family study.

3 investigations have been undertaken to study the interrelationship between a bifid pelvicalyceal collecting system, vesico-ureteric reflux and lateral ectopia of the ureteric orifice. Firstly, in 30 families investigated to confirm the familial incidence of bifid and double ureters there were 3 families in which siblings were found with primary reflux. Secondly, reflux was found in 62 of 110 patients investigated with a duplex pelvicalyceal system. Reflux was to the ipsilateral kidney in 48 patients but occurred to the contralateral kidney, unaffected by duplicity, in 14. This high incidence

[The physiopathology of vesicoureteral reflux (author's transl)].

The tightness of the ureterovesical junction depends on all the structures composing the terminal and intra-mural ureter. The muscular, collagenic, and elastic fibers of the ureter constitute a mesh net which is stretched during bladder distention and closes the ureteric orifice as a valve. Congenital vesico-ureteric reflux results from a primary structural insufficiency of the terminal ureter or insufficiency of the bladder wall backing. As embryology shows primary reflux can be caused by a high ectopic implantation of the ureter. It results from the development of an ureteric bud appearing i
