Phenotyping Interstitial Cystitis/Bladder Pain Syndrome: Clinical and Mechanistic Evidence, Candidate Biomarkers, and Implications for Management.

Interstitial cystitis and bladder pain syndrome are clinically overlapping conditions that should be distinguished according to Hunner lesion status. IC with Hunner lesions is a distinct inflammatory bladder disease characterized by urothelial denudation, lymphoplasmacytic infiltration, adaptive immune activation, and progressive structural impairment. The intensity and distribution of these inflammatory and structural abnormalities may vary among patients with IC, but the underlying pathological pattern is comparatively coherent. BPS without Hunner lesions is substantially more heterogeneous.

Postoperative Fibro-Inflammatory Pseudocyst of the Vesicouterine Space Mimicking a Chronic Hematoma: A Case Report.

Chronic pelvic pain remains a challenging clinical problem due to its diverse underlying causes and often nonspecific presentation. Although common conditions account for most cases, rare postoperative or inflammatory cystic lesions within the vesicouterine space may mimic more common pelvic pathologies and create significant diagnostic uncertainty. These lesions may demonstrate imaging features that overlap with those of chronic hematomas and endometriotic cysts, further complicating the preoperative diagnosis. A woman in her mid-30s, gravida 3, para 1, with two previous miscarriages, present

State of the art imaging for endometriosis and adenomyosis.

Endometriosis is a chronic oestrogen-dependent condition characterised by the presence of endometrial-like tissue outside the uterus. Although benign, the associated chronic inflammation and fibrosis can result in substantial symptomatic burden, including chronic pelvic pain, menorrhagia, dysmenorrhoea and dyspareunia. Adenomyosis, which commonly co-exists with endometriosis, is defined by endometrial glands within the myometrium, causing chronic inflammation and fibrosis. Symptoms overlap with endometriosis, including heavy menstrual bleeding, dysmenorrhoea and chronic pelvic pain. Historical

Bladder tumors in patients initially diagnosed with interstitial cystitis: a ten-year retrospective study.

BACKGROUND: Interstitial cystitis/bladder pain syndrome (IC/BPS) is a chronic inflammatory condition characterized by bladder pain and lower urinary tract symptoms. As a diagnosis of exclusion, IC/BPS shares overlapping features with bladder tumors, which may lead to misdiagnosis or delayed diagnosis. Clinical evidence regarding the relationship between IC/BPS and bladder tumors remains limited. METHODS: In this single-center retrospective study, 863 patients initially diagnosed with IC/BPS who underwent cystoscopic hydrodistension with random multi-site bladder biopsies between January 2015

Application of transformer-enhanced convolutional neural network: multicenter MRI assessment of muscle invasion in bladder cancer.

OBJECTIVE: Accurate preoperative assessment of muscle invasion in bladder cancer (BCa) guides therapy selection. However, MRI interpretation varies across readers and lesion morphologies. Therefore, we aimed to overcome the morphology-associated diagnostic bias through a deep learning method. MATERIALS AND METHODS: This multicenter study included 1374 patients with BCa. An nnU-Net was fine-tuned to assist in lesion segmentation on T2-weighted images, providing inputs for a 2.5D ConvNeXt-tiny model to assess muscle invasion. The performance of the model was compared between pedunculated and se

5-Aminolevulinic Acid-Based Fluorescence Guidance in Urologic Oncology: Current Status, Pitfalls, and Future Directions.

5-Aminolevulinic acid (5-ALA) induces tumor-selective accumulation of protoporphyrin IX (PpIX), enabling fluorescence-guided visualization of malignant tissue. In urologic oncology, the most established application is photodynamic diagnosis (PDD) during transurethral resection of non-muscle-invasive bladder cancer, in which fluorescence can identify occult carcinoma in situ and additional papillary lesions; however, specificity may decline in the presence of inflammation, recent instrumentation, or intravesical therapy. Renal applications are emerging: oral 5-ALA before partial nephrectomy can

Precursor flat urothelial lesions: Molecular landscape, pathogenesis, and biological insights.

Flat urothelial lesions represent a diagnostically challenging and clinically significant spectrum in bladder pathology. Distinguishing urothelial carcinoma in situ (CIS) from precursor lesions such as dysplasia remains problematic due to overlapping morphologic features, marked interobserver variability, and unresolved questions about biological behavior and progression risk. These challenges pose a substantial risk of underdiagnosis of neoplastic lesions-misclassified as reactive or benign-or overtreatment of precursors overcalled as CIS. As a result, intermediary lesions-particularly dyspla

Development and multi-observer validation of a histopathological scoring system for interstitial cystitis/bladder pain syndrome diagnosis.

PURPOSE: To develop and validate a pragmatic histopathological scoring system for interstitial cystitis/bladder pain syndrome (IC/BPS) that distinguishes Hunner-type interstitial cystitis (HIC) from bladder pain syndrome (BPS) and inflammatory non-HIC chronic cystitis mimicking Hunner lesions ("other cystitis"). METHODS: Bladder biopsies from 96 patients were analyzed: HIC (n = 32), BPS (n = 42), and other cystitis (n = 22; bacillus Calmette-Guérin-related, follicular, and chronic bacterial cystitis). Four pathologists (A-D; A as index/model observer) independently scored hematoxylin-eosin se

Optical Spectroscopy and Spectral Imaging for Bladder Cancer Detection and Characterisation: A Scoping Review of Diagnostic Evidence and Translational Readiness.

Background/Objectives: White-light cystoscopy remains central to bladder cancer detection and surveillance, but diagnostic uncertainty persists in detecting flat lesions, particularly carcinoma in situ, distinguishing inflammatory mimics, and targeting biopsies. Optical spectroscopy and spectral imaging may provide objective tissue information beyond visual inspection. This scoping review mapped the diagnostic evidence and translational readiness of these technologies. Methods: A scoping review was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analy

Urothelial carcinoma of the bladder with prostatic stromal invasion and early postoperative bone metastases: a case report.

BACKGROUND: Prostatic involvement by bladder urothelial carcinoma is uncommon, and prostatic stromal invasion usually indicates aggressive tumor biology and poor prognosis. Such patients may present with prostatic enlargement or a mass at the bladder neck-prostate junction, while serum prostate-specific antigen (PSA) may remain within the normal range. This diagnostic overlap may lead to clinical confusion with benign prostatic hyperplasia or primary prostatic tumors. CASE PRESENTATION: A 54-year-old man was admitted with urinary frequency, urgency, dysuria, and intermittent gross hematuria.

Zhenwu Decoction ameliorates experimental benign prostatic hyperplasia: association with mitochondrial quality control, oxidative stress and apoptosis regulation.

ETHNOPHARMACOLOGICAL RELEVANCE: Zhenwu Decoction (ZWD), first recorded in the Shang Han Lun, is a classical formula used to warm Yang and promote water metabolism. It has traditionally been used for Yang deficiency with water retention, impaired Qi transformation and dysuria. Benign prostatic hyperplasia (BPH) can be classified under the traditional categories of "Longbi" and "dysuria", and its pathogenesis is closely related to kidney Yang deficiency, impaired bladder Qi transformation and water retention in the lower Jiao. Given that BPH is not merely a proliferative lesion but a chronic pro

Case Report: Multiple sclerosis and Neurofibromatosis type 1: a rare comorbidity.

INTRODUCTION: Neurofibromatosis type 1 (NF1) and Multiple Sclerosis (MS) are distinct neurological conditions with different underlying pathogenetic mechanisms. Their co-occurrence is exceedingly rare, with approximately 40 cases reported in the literature. Due to overlapping clinical and neuroimaging features, diagnosis is often challenging. We describe two cases of NF1-MS comorbidity highlighting the importance of a careful neuro-ophthalmological and neuroradiological evaluation for this rare association. PATIENT 1: A 33-year-old female presented with acute bilateral visual loss and bladder
