Clinical and Molecular Characterization of Clear Cell Adenocarcinoma of the Urinary Tract: A Multi-Institutional Study.

Clear cell adenocarcinoma of the urinary tract (CCA-UT) is a rare, potentially aggressive tumor with very limited information regarding its clinicopathologic characteristics and molecular alterations. This study aimed to elucidate the clinicopathologic features and molecular landscape of one of the largest cohorts (35 cases) of this tumor, to identify genomic alterations and potential therapeutic targets. Seventy-nine percent of the patients were women, with a median age of 61 years. The urethra was the most common site (18; 51%), and all cases were ≥pT2 (pT2:15; pT3:11; pT4:8). Twenty-nine pe

Suprapubic transvesical adenoma resection of the prostate (STAR-P): step-by-step technique.

OVERVIEW: Surgical management of benign prostatic hyperplasia (BPH) has evolved significantly, incorporating various minimally invasive procedures aimed at reducing morbidity and optimizing patient outcomes. Despite advancements, transurethral approaches continue to pose risks such as urethral strictures and urinary incontinence due to mechanical and thermal stress. To address these limitations, the Suprapubic Transvesical Adenoma Resection of the Prostate (STAR-P) was developed, offering a direct suprapubic route that bypasses the urethra entirely. Recent studies have validated STAR-P as both

Non-ablative transurethral laser treatment for collagen remodeling with functional recovery in an in vivo model of stress urinary incontinence.

Stress urinary incontinence (SUI) is a common condition characterized by involuntary urine leakage under increased intra-abdominal pressure, such as coughing and physical exertion. The current treatments, including pelvic floor muscle training and surgical interventions such as mid-urethral sling procedures, remain limited by variable efficacy, invasiveness, and potential complications. This study aimed to develop and evaluate a novel, non-ablative transurethral 980 nm laser treatment as a minimally invasive, non-implantable alternative using a vaginal distension (VD)-induced SUI guinea pig mo

Laparoscopic extraperitoneal excision of a symptomatic anterior midline periprostatic cyst: A case report.

Anterior midline periprostatic cysts are rare and may cause lower urinary tract symptoms due to compression of the membranous urethra. We report a 44-year-old man with progressive voiding symptoms. Perineal ultrasound and pelvic MRI revealed a 12 × 25 mm anterior midline periprostatic cyst clearly separated from the prostatic parenchyma and compressing the urethra. Because of its deep anterior location and the risk of sphincter injury with transurethral approaches, laparoscopic extraperitoneal excision was performed. The cyst was completely removed without complications, and symptoms improved

Anatomical, functional, and patient-reported outcomes following anterior urethroplasty. Can we predict when and why are patients with anatomical recurrences requiring reinterventions?

PURPOSE: Despite modern advances in urethroplasty, little consensus exists on defining treatment success. The absence of reintervention was the main benchmark. However, recent efforts incorporate anatomical, functional and patient-reported criteria. Our aim is to evaluate urethroplasty outcomes using these four criteria, and to assess the progression of asymptomatic anatomical recurrences to predict the need for future treatments. METHODS: An ambispective study was conducted on patients > 18 years undergoing anterior urethroplasty between 2020 and 2023. Minimum follow-up of 2 years. Techniques

A rare high anorectal malformation with recto-prostatic urethral fistula in a 1-year-old male: diagnostic and surgical management challenges in a resource-constrained setting: a case report.

BACKGROUND: High anorectal malformations (ARMs) with recto-urinary fistulas constitute some of the most complex congenital anomalies in pediatric surgery. The recto-prostatic variant is particularly rare and poses significant diagnostic and reconstructive challenges, especially in resource-limited settings. Early and accurate identification of the fistula level and preservation of the sphincteric complex are crucial for optimal functional outcomes. CASE PRESENTATION: We report a 1-year-old male with a high ARM associated with a recto-prostatic urethral fistula. The patient initially underwent

Association Between Preoperative Membranous Urethral Length and Urinary Continence Recovery After Retzius Sparing Robotic Assisted Radical Prostatectomy: Results From a Retrospective Single-Center Study.

BACKGROUND: Urinary incontinence (UI) remains a common complication after radical prostatectomy, even with advancements such as robotic-assisted radical prostatectomy (RARP). Preoperative membranous urethral length (MUL) has been suggested as a predictor of early continence recovery, but its role in Retzius Sparing RARP (RSRARP) remains unclear. This study evaluated the association between preoperative MUL and early postoperative UI in patients undergoing RSRARP. METHODS: We retrospectively analyzed 157 patients who underwent RSRARP between March 2024 and February 2025 at a high-volume center

Postoperative spindle cell nodule of the urethra and bladder in a 90-year-old male: A diagnostic challenge.

Postoperative spindle cell nodule is a rare benign reactive lesion of the genitourinary tract that can closely mimic malignant spindle cell neoplasms. We report a 90-year-old male with recurrent urethral obstructions and bladder lesions over four years following repeated instrumentation for urethral strictures. Multiple cystoscopies revealed recurrent masses (2-5 cm) throughout the urethra and urinary bladder. Initial biopsies were nondiagnostic. The final histopathological examination demonstrated fibrosis, edema, inflammation, and atypical spindle cells without mitosis or necrosis. Immunohis

Effect of ASP5633, a novel peripherally restricted MT1/MT2 receptor agonist, on urethral function in female rats.

Melatonin is a hormone that regulates numerous physiological processes through activation of specific G protein-coupled receptors, termed MT1 and MT2 receptors. We previously reported that stimulation of MT1 receptors in female rat urethra activated intracellular Ca2+ mobilization via a Gq protein-dependent pathway, resulting in urethral smooth muscle contraction. This study aimed to investigate the potential of MT1 receptor-related urethral contraction in the treatment of urological disorders using ASP5633 (3-(2-acetamidoethyl)-6-fluoro-5-methoxy-N-methyl-1H-indole-2-carboxamide), a novel per

Minimally Invasive Mitrofanoff in Children: Versatile Laparoscopic Strategies-From Low-Resource to Non-Robotic High-Cost Settings in an Exploratory Case Series.

Background/Objectives: The Mitrofanoff appendicovesicostomy (MAV) is the gold standard for creating a continent catheterizable channel in children unable to perform clean intermittent catheterization (CIC) through the native urethra. Minimally invasive surgery has progressively replaced open techniques in pediatric urology, offering improved recovery and favorable cosmetic outcomes, and robotic-assisted Mitrofanoff has gained popularity in recent years. However, the high costs and limited availability of robotic systems create disparities in access to pediatric urologic reconstruction, particu

Physiological medium and 3-hydroxybutyrate modulate autophagy-linked organelle remodeling in human external urethral sphincter myoblasts.

Autophagy-linked organelle remodeling is essential for skeletal muscle differentiation and is closely linked to the metabolic environment. The ketone body 3-hydroxybutyrate (3HB) serves as an alternative energy substrate and signaling molecule that modulates organelle function and myogenic programs. This study investigated how extracellular metabolic conditions and 3HB regulate autophagy-linked organelle remodeling in human external urethral sphincter (hEUS) myoblasts. Immortalized hEUS myoblasts (US2-KD) were differentiated under four conditions combining high-glucose Dulbecco’s modified Eagl
